Acupuncture Application Case Report for Symptom Relief of Oral Leukoplakia (구강 백반증의 증상 완화에 대한 침치료 적용 사례)

  • Oral leukoplakia is the most common premalignant lesion of the oral mucosa. This case report presents the effect of acupuncture on Oral leukoplakia. The 67 years old male patient who has suffered for 2 years was treated with Right side Saam acupucture Damjeonggyeok(BL66, GB43, LI1, GB44) without other treatment. Acupuncture was performed for 15 minutes. The effect of acupuncture was evaluated as photographic change and visual analog scale(VAS) of subjective symptoms in oral cavity. After 87th acupunture treatment during 226 days, Leukoplakias were decreased at the right side of ventral tongue and the left border of the tongue and the lower lip. Furthermore, ulcer, erosion at the left border of the tongue were disappeared and VAS of Burning pain in the tongue decreased from 8 to 2. Although acupuncture is unusual treatment for Oral Leukoplakia, this case report suggested the effectiveness of using Damjeonggyeok on Oral Leukoplakia according to Saam acupuncture method.

The Effect of Masticatory Muscle Fatigue on the Occlusal Contact Stability and Masticatory Muscle Actibities (저작근의 피로가 치아접촉안정성 및 저작근활성에 미치는 효과)

  • The authors performed a experimental study to evaluate the effects of masticatory muscle fatigue on tooth contact and masticatory muscle activity in 26 normal healthy women. The experimental masticatory muscle fatigue was induced by unilateral biting of 5kg force on mandibular first molar. The results were as follows : 1. The initial symptom related to muscle fatigue pain appeared in 85.19 seconds of isometric contraction and the endurance time of isometric contraction was 203.15 seconds. 2. The pain occurred more frequently in masseter region than in temporal region. In masseter pain the incidence was almost equal between both sides, whereas the temporal pain was more in contralateral side. 3. The spontaneity and the symmetry of tooth contact during maximum clenching were reduced after isometric unilateral biting. 4. After induction of experimental muscle fatigue, the EMG activities of masseter muscles of both sides and ipsilateral temporal muscle showed the tendency of decreasing activities. 5. The asymmetry indicies of masseter and temporal muscles were reduced after isometric bilateral biting.

Impact of physical stress symptoms and psycho-emotional stress symptoms on oral health in adults (성인의 신체적 스트레스 증상과 심리적 스트레스 증상이 구강건강에 미치는 영향)

  • Stress is closely associated with oral diseases, being considered to be one of important variables to affect the improvement of oral health and the quality of life. In this study, a survey was conducted on 550 adults from July 1, 2012, to February 28, 2013. A path analysis was carried out to determine the influence of physical stress symptoms and psycho-emotional stress symptoms on dry mouth, oral mucosal disease and halitosis symptoms. The findings of the study were as follows: Oral mucosa affected halitosis in a direct effect and dry mouth exerted a direct influence on that as well. When stress symptoms affected halitosis, dry mouth and oral mucosa had an indirect impact on that as parameters. Dry mouth exercised a direct influence on oral mucosa. The above-mentioned findings suggested that the physical stress symptoms and psycho-emotional stress symptoms of the adults affected their dry mouth, oral mucosal disease and halitosis both in direct and indirect effects. As the stress of adults is closely bound up with their oral diseases, how to properly cope with stress should carefully be considered in order for them to lead a better life.

Study on Oral Symptom Experiences and Oral Health Behaviors of Primary School Children (초등학생의 구강증상 경험 및 구강건강행위에 관한 연구)

  • This study carried out to provide basic educational material of primary school children to improve the children's oral health by oral health checkup and survey over 215 students in the first grade and the fourth grade at the primary school in Hwaseong city. The collected data were analyzed by SPSS Win 12.0 program 1. The rate of students who have decayed permanent teeth was 57.2%, and there seemed to be a significant difference between the two genders showing 61.6% for male students and 53.3% for female students. Also 1st grade showed 56.0% and 4th grade 58.1% respectively. 2. The male students who claimed 'bleeding gum' against questions about oral illness experiences were 15.7% whereas female students were 28.0%. There was a significant difference(p<0.05) by the gender. 3. The 4th grade students who said 'Toothache while eating cold food' about questions of oral symptoms appeared as 19.4% and 1st grade students were 8.8%. Also there was a significant difference(p<0.05) by the grade. 4. Regarding oral health improvement behavior, there were significant differences in frequency of toothbrushing a day, the number(p<0.05) of eating snacks a day and the number(p<0.01) of experience of visiting dental clinic(p<0.01). 5. About oral health behavior frequency of daily toothbrushing was found as twice(50.2%), twice of eating snacks a day(70.2%), using fluoride toothpaste 60.9% and visiting a dental clinic(60.0%), were shown as the highest numbers.

A Study of Personality Profile of Halitosis Patients by Symptom Checklist-90-Revision(SCL-90-R) (간이정신진단검사를 이용한 구취환자들의 인성평가에 대한 연구)

  • In order to evaluate the clinical features of the rcurred patients with temporomandibular disorders, 52 patients were subjected at the Dept. of Oral Medicine, PNUH from 1985 through 1996. As related to gender, duraton and diagnosis, Ll the relate factors-multiple diagnosis, emotinal problem, history of treatment, parafunctional habit, life distrubance, and trauma were analyzed. The obtained results were as follows : 1. The patients with multiple diagnosis were 83 percent and more often seen in women, chronic group and muscle-joint disorder group. 2. Duration of retreatment in chronic group and muscle-joint disorder was longer. 3. The patients with oral parafunctional habits were 89 percent and more often seen in women, chronic gorup, muscle-joint disorder group. 4. the Recurred patients showed 46 percent in history treatment, 42 percent in life distrubances, 39 percent in macrotrauma and 12 percent in emotional problem.

Treatment of dental implant-related maxillary sinusitis with functional endoscopic sinus surgery in combination with an intra-oral approach

  • The present report describes the case of a patient who underwent maxillary sinusitis right after dental implant installation with sinus lifting. Computed tomography scan revealed a dental implant (#16) was protruded inside the right maxillary sinus and confirmed the obstruction of ostium. A symptom remission was gained with the dual approaches combined by functional endoscopic sinus surgery and an intra-oral approach. Fully recovered function and healing of sinus were identified after 10 months follow-up. We report the case of sinusitis caused by protrusion of implants with sinus floor lift procedures and propose that practitioners should be aware of the possible its complications and management.

  • The sialolithiasis is one of the most common disease that is found in the submandibular gland and they can be usually observed in the extra glandular area. As the lumen of Wharton's duct is larger and more expandable than that of Stensen's duct, the symptom of them is initially painless. Obstruction of salivary secretion by a sialolith can result in sudden swelling and pain, as well as infection of the gland. It can be observed with clinical exam and the scintigraphy. After removal of sialoliths, the majority of them can get the recovery of function. Our department performed the transoral sialolithotomy. One of three cases was recurred sialolithiasis with sialadenitis and was performed by the re-sialolithotomy and extraoral sialoadenectomy.

Psychological Analysis of TMD Patients through the SCL-90-R (SCL-90-R을 이용한 TMD환자의 심리학적 분석)

  • Personality characteristics of TMD patients was studied through the SCL-90-R. III patients were subjected at the Orofacial Pain Clinic, Dept. of Oral Medicine, PNUH, from 1991 to 1996. All the TMD patients were divided and analyzied comparatively by the ways of duration of present illness, severity of symptoms, associated symptoms, traumatic contributing factor, history of treatment, treatment modality, response to therapy and treatment outcomes. The obtained results were as follows; 1. Mean value of T-scores on 9 basic scales in all the groups was within normal range. 2. There showed higher levels of T-scores in the chronic group, the unimproved group and the discontinued group. 3. T-scores in the patients with severe symptom, associated symptoms, and history of treatment revealed higher.

Acute Oral Toxicity Study of Kami-honghwa-tang (가미홍화탕의 단회 경구 투여 독성 연구)

  • Kami-honghwa-tang(KH-19) is a prescription for reducing the side effect of radiotherapy. In this study, safety of KH-19 was evaluated by GLP guideline of Korea Food and Drug Administration. In acute oral toxicity study on rat, transient inhibition of weight increase was shown, but change in general symptom was not detected. No dead animal was observed up to 5,000 mg/kg in both male and female animals. In acute oral toxicity study on Beagle dog, transient vomiting, diarrhea, anorexia, and weight reduction was observed. However, no dead animal was observed up to 2,000 mg/kg in both male and female animals.

Ameloblastic carcinoma of the mandible: A case report

  • Ameloblastic carcinoma is a rare odontogenic malignant tumor with the histologic features of both ameloblastoma and carcinoma. It occurs more frequently in the mandible than in the maxilla and it may appear de novo or develop from a preexisting ameloblastoma or odontogenic cyst. Rapidly progressing, painful swelling is the most common symptom, and radiographically, it shows significant bone resorption and cortical perforation. This report described a case of ameloblastic carcinoma in a 45-year-old man who presented with swelling in the left mandible. The lesion showed combined features of benign findings, such as an expansile cortex with a distinct border, and malignant findings, such as a large exophytic mass with frank bone resorption. Excisional biopsy was performed and a final diagnosis of ameloblastic carcinoma was made.
