Xanthoma disseminatum (XD) is a benign, non-Langerhans cell histiocytosis characterized by disseminated xanthomatous lesions with face, flexures, and mucosa. Most of XD develops in mucocutaneous lesions including skin, oral cavity and pharynx, however laryngeal involvement is uncommon. While the natural course of XD is usually benign and often self-limiting, but XD develop in critical anatomical locations may result in morbidity and mortality. Localized mucous lesions in oropharynx and larynx lead to dysphagia, dyspnea and air way obstruction. The diagnosis of XD was based on clinical, histological and immunohistochemical findings. The treatment is complex and non-consensual. Local treatment with cryotherapy, radiotherapy, surgery, and carbon dioxide lasers have been attempted with various results. Systemic medication with peroxisome proliferator-activated gamma receptors, statins, fenofibrate, chlorodeoxyadenosine, cyclophosphamide, doxycycline, and cyclosporine have also been reported, but none have proven particularly successful. A 59-year-old man presented with respiratory symptoms because of laryngeal involvement of XD. We had to remove the obstructive lesion for relieving the symptoms. We experienced XD in Larynx that was rare in otorhinolaryngology. So we report this case with review of literatures.

Frequently used for humans as a nonsteroidal anti-inflammatory drug, naproxen has been known to induce ulcerative gastric lesions. The present study was undertaken to investigate the in vivo therapeutic effect of astaxanthin, isolated from a Xanthophyllomyces dendrorhous mutant, against naproxen-induced gastric antral ulceration in rats. The rats were treated with three doses of astaxanthin [1, 5, and 25 mg/kg body weight (B.W.), respectively] once daily for 2 weeks after pretreatment of 80 mg of naproxen/kg B.W. twice daily for 3 days, while the control rats received only 80 mg of naproxen/kg B.W. twice daily for 3 days. The oral administration of astaxanthin (1,5, and 25 mg/kg B.W.) showed a curative effect against naproxen (80 mg/kg B.W.)-induced gastric antral ulcer and reduced the elevated lipid peroxide level in gastric mucosa. In addition, astaxanthin treatment resulted in significant increase in the activities of radical scavenging enzymes such as superoxide dismutase, catalase, and glutathione peroxidase. A histologic examination clearly proved that acute gastric mucosal lesion induced by naproxen nearly disappeared after the astaxanthin treatment. These results suggest that astaxanthin eliminated the lipid peroxides and free radicals induced by naproxen and may be a potential candidate for remedy of gastric ulceration.

Dentigerous cyst is a cyst arising by a seperation of follicles from around the anatomical crown of an unerupted tooth within the jaws. The dentigerous cyst is seen during routine radiographic examination. It is detected radiographically as a sharply delineated, round or oval, unilocular, homogeneous, and radiolucent area within which there is the crown of an unerupted tooth. The histological appearance of the dentigerous cyst is usually attached to the tooth at the cementoenamel junction and the sac lined by a thin, regular. stratified squamous epithelium. The dentigerous cyst can be treated by two surgical procedures: enucleation or marsupialization. The size and location of the lesion usually dictates the surgical procedures. The marsupialization consists of the removal of only a portion of the wall of the cyst to make the remaining cystic lining continous with the oral mucosa. The cyst, deprived of its supporting hydrostatic pressure, gradually shrinks until it is obliterated. The marsupialization is the best way to conserve the tooth affected by a dentigerous cyst and to permit its eruption. especially in a young person. Main advantages of the marsupialization are its relative simplicity and its conservation with respect to adjacent important structures. In marsupialization technique, a plugger can be constructed, if necessary, to maintain the opening and prevent food debris from entering the cystic cavity. However, the disadvantage is the potentiality of leaving pathologic tissues. Therefore frequent recall appointments are advisable.
Statement of Problem: The residual monomer of denture base materials causes hypersensitivity on oral mucosa and intereferes with the mechanical properties of the cured resin. The amount of residual monomer is influenced by materials, curing cycle, processing method, and etc. Purpose: The aim of this study was to investigate the residual methyl methacrylate(MMA) content of injection molded denture base polymer, and to compare this with the self-cured resin and the conventional compression molded heat-cured resin. Materials and Methods: Disc shaped test specimens (50mm in diameter and 3mm thick) were prepared in a conventional flasking technique with gypsum molding. One autopolymerized denture base resins (Vertex Sc. Dentimex. Netherlands) and two heat-cured denture base resins (Vertex RS. Dentimex. Netherlands, Ivocap. Ivoclar Vivadent, USA) were used. The three types of specimens were processed according to the manufacturer's instruction. After polymerization, all specimens were stored in the dark at room temperature for 7 days. There were 10 specimens in each of the test groups. 3-mm twist drills were used to obtain the resin samples and 650mg of the drilled sample were collected for each estimation. Gas chromatography (Agillent 6890 Plus Gas Chromatograph, Agillent Co, USA) was used to determine the residual MMA content of 10 test specimens of each three types of polymer. Results: The residual monomer content of injection molded denture base resins was $1.057{\pm}0.141%$. The residual monomer content of injection molded denture base resins was higher than that of compression molded heat cured resin ($0.867{\pm}0.169%$). However, there was no statistical significant difference between two groups (p>0.01). The level of residual monomer in self cured resin($3.675{\pm}0.791$) was higher than those of injection molded and compression molded heat cured resins (p<0.01). Conclusion: With respect to ISO specification pass / fail test (2.2% mass fraction) of residual monomer, injection molding technique($1.057{\pm}0.141%$) is a clinically useful and safe technique in terms of residual monomer.

Background &Objectives : Rhinitis is an inflammation of nasal mucosa and the major symtoms are watery rhinorrhea, sneezing, itchy nose, and nasal obstruction. Rhinitis is classified into allergic rhinitis and nonallergic rhinitis. Allergic rhinitis is an immune reaction by allergen, and vasomotor rhinitis which is nonallergic and noninfectious is hypersensitive reaction. The incidence of allergic rhinitis has increased and the rate of vasomotor rhinitis is high. However there have been no studies about vasomotor rhinitis compared with allergic rhinitis. And there have been no studies so far performed on the effect of Tongkwansan. Therefore this study is aimed to find out the effects of Tongkwansan on allergic rhinitis and vasomotor rhinitis. Materials and Methods : Fifteen BALC/c mouses were divided into three groups : normal group, control group and sample group. To induce the allergic rhinitis in control group and sample group, mouses were sensitized intrapertioneally 0.1% ovalbumin solution three times at intervals of 1 week. Then intranasal sensitization was performed by diffusing 0.1 % ovalbumin solution 3 times at intervals of 2 days. After that time, mouses in the sample group were oral administration treated by Tongkwansan for 28 days. We observed changes in the segment of IL-4, IL-5, $IFN-{\gamma}$, Total IgE, and ovalvumin specific IgE in blood. We used the statistical methods of ANOVA test(p<0.05). Results : There were no significant changes statistically in $IFN-{\gamma}$, IL-4, and IL-5 in blood(p<0.05). There were also no significant changes statistically in Total IgE, OVA-specific IgE in blood(p<0.05). Conclusion : According to above results, it is supposed that Tongkwansan has no significant effects on allergic rhinitis. But it is supposed that Tongkwansan has significant effects on vasomotor rhinitis which is nonallergic and noninfectious

A 3-year-old female Connemara pony was referred to Jeju National University Equine Hospital because of weight loss and prolonged anorexia. On admission, heart and respiratory rates were slightly elevated while body temperature was within the normal range. The color of the conjunctival and oral mucosa was yellowish pink. The blood chemistry results showed that total bilirubin, aspartate aminotransferase, and gamma-glutamyl transferase levels were remarkably elevated while blood urea nitrogen was within the reference range. Transcutaneous abdominal ultrasound revealed an enlarged right lobe of the liver with prominently increased parenchymal echogenicity, reduced hepatic vessels, and rounded caudal border. The pony was tentatively diagnosed with chronic hepatitis and icterus; rest and supportive treatments were provided. Clinical signs aggravated on day 2 with hind quarter paresis and cranial nerve signs such as circling, drooping, jerking, and head pressing against walls. Recumbency and generalized ataxia (grade 5/5) were shown on day 3. Plasma ammonia concentration on day 3 was as high as 656 µmol/L. Necropsy and histopathologic examinations strongly supported a diagnosis of hepatic encephalopathy. This case of hepatic encephalopathy exhibited rapid progress from low to terminal grade within 4 days in a Connemara pony. The results provide well-established clinical and pathological data for future application.
Background: Third molar extraction is the most commonly performed minor oral surgical procedure in outpatient settings and requires regional anesthesia for pain control. Extraction of the maxillary molars commonly requires both posterior superior alveolar nerve block (PSANB) and greater palatine nerve block (GPNB), depending on the nerve innervations of the subject teeth. We aimed to study the effectiveness of PSANB alone in maxillary third molar (MTM) extraction. Methods: A sample size comprising 100 erupted and semi-erupted MTM was selected and subjected to study for extraction. Under strict aseptic conditions, the patients were subjected to the classical local anesthesia technique of PSANB alone with 2% lignocaine hydrochloride and adrenaline 1:80,000. After a latency period of 10 min, objective assessment of the buccal and palatal mucosa was performed. A numerical rating scale and visual analog scale were used. Results: In the post-latency period of 10 min, the depth of anesthesia obtained in our sample on the buccal side extended from the maxillary tuberosity posteriorly to the mesial of the first premolar (15%), second premolar (41%), and first molar (44%). This inferred that anesthesia was effectively high until the first molars and was less effective further anteriorly due to nerve innervation. The depth of anesthesia on the palatal aspect was up to the first molar (33%), second molar (67%), and lateromedially; 6% of the patients received anesthesia only to the alveolar region, whereas 66% received up to 1.5 cm to the mid-palatal raphe. In 5% of the cases, regional anesthesia was re-administered. An additional 1.8 ml PSANB was required in four patients, and another patient was administered a GPNB in addition to the PSANB during the time of extraction and elevation. Conclusion: The results of our study emphasize that PSANB alone is sufficient for the extraction of MTM in most cases, thereby obviating the need for poorly tolerated palatal injections.

Maxillary bone defects may follow surgical treatment of benign and malignant tumors, trauma, and infection. Palatal defects often lead to problems with swallowing and pronunciation from the leakage of air into the nasal cavity and sinus. Obturators have been commonly used to solve these problems, but long-term use of the device may cause irritation of the oral mucosa or damage to the abutment teeth. Utilizing implants in the edentulous area for the fabrication of the obturators has gained attention. This case report describes a patient, who had undergone partial resection of the maxilla due to adenocarcinoma, in need of a new obturator after losing abutment teeth after long-term use of the previous obturator. Implants were placed in strategic locations, and an implant-retained maxillary obturator was fabricated, showing satisfactory results in the rehabilitation of multiple aspects, including palatal defect, masticatory function, swallowing, pronunciation, and aesthetics.
Eighty five patients of oral cavity cancer, treated with radiation at the Department of Therapeutic Radiology, Korea Cancer Center Hospital, during the period from March 1985 to September 1990 were analyzed retrospectively. Among 85 patients, 37 patients were treated with radiation only and 48 patients were treated with radiation following surgery. And 70 patients received external irradiation only by $^{60}Co$ with or without electron, the others were 7 patients for external irradiation plus interstitial implantation and 8 patients for external irradiation plus oral cone electron therapy. Primary sites were mobile tongue for 40 patients, mouth floor for 17 patients, palate for 12 patients, gingiva including retromolar trigone for 10 patients, buccal mucosa for 5 patients, and lip for 1 patient. According to pathologic classification, squamous cell carcinoma was the most common (77 patients). According to AJC TNM stage, stage I + II were 28 patients and stage III+IV were 57 patients. Acturial overall survival rate at 3 years was $43.9\%,$ 3 year survival rates were $60.9\%$ for stage I + II, and $23.1\%$ for stage III+IV, respectively. As a prognostic factor, primary T stage was a significant factor (p<0.01). The others, age, location, lymph node metastasis, surgery, radiation dose, and cell differentiation were not statistically significant. Among those factors, radiation plus surgery was more effective than radiation only in T3+T4 or in any N stage although it was not statistically sufficient (p<0.1). From those results, it was conclusive that definitive radiotherapy was more effective than surgery especially In the view of pertainig of anatomical integrity and function in early stage, and radiation plus surgery was considered to be better therapeutic tool in advanced stage.
The aim of this study was to analyze factors related to xerostomia in adults aged 30 years and over. The data were analyzed using the t-test, one-way ANOVA and multiple linear regression analysis in the SPSS version 12.0 program. The results were as follow. 1. The respondents who were older than 51 years old, unemployed and had less than 3 million won of average monthly income were more aware of xerostomia (p<0.05). 2. The respondents who answered poor and moderate for their general and oral health and the group with duplicate medication and comorbidity were more aware of xerostomia (p<0.05). 3. The respondents that had problems in chewing, communication, ordinary activities and complained of pain discomfort and suffered from anxiety depression were more aware of xerostomia (p<0.001). 4. The respondents that answered frequent dryness on their skin, eyes, lips, and nasal mucosa were more aware of xerostomia (p<0.001). 5. Xerostomia showed highest correlation with quality of life ($\beta$=0.436) followed by the number of medications ($\beta$=0.239), sense of entire body dryness ($\beta$=0.200), feeling of hopelessness ($\beta$=0.160) and number of oral mucosa disease symptoms ($\beta$=0.099) (p<0.05). According to the results of the study, xerostomia may cause deterioration in quality of life. Thus, it is advised to improve the patient management system among dental professions to prevent various complications caused by xerostomia and conduct regular health education on the cause and management method of xerostomia.
