Diagnostic and management challenge of concurrent tongue squamous cell carcinoma with an unknown parapharyngeal mass

  • There are very few case reports of the diagnosis and management of concurrent oral cavity and parapharyngeal space tumors. We present a case involving a 49-year-old female who presented with oral cavity squamous cell carcinoma confirmed by biopsy. Initial diagnostic workup revealed a concurrent parapharyngeal mass. Diagnostic studies and surgical therapy were tailored to account for both pathological entities. The patient was treated with a combination of surgery and adjuvant therapy. The surgical strategy was designed to address both lesions simultaneously. One year post-surgery, the patient had good response to therapy with no evidence of persistent or recurrent disease. This report discusses the outcome and treatment of a rare case of concurrent squamous cell carcinoma with a complicating parapharyngeal space tumor. It explores the diagnostic process, comprehensive workup, and the surgical management.

Anti-proliferative and Anti-telomerase Activity of Curcuma Rhizome Extract on Oral Squamous Cell Carcinoma and Osteosarcoma Cells

  • Anti-proliferation of methanol extract of Curcuma rhizome on oral squamous cell carcinoma (KB) and osteosarcoma (HOS) cells were investigated. In order to elucidate the involvement of telomerase inhibitory activity as a part of anti-proliferative effect of Curcuma rhizome on cancer cells, we measured telomerase activity in Curcuma rhizome extract-treated cancer cells. The concentration inhibited cell proliferation to 50% $(IC_{50})$ of the methanol extract of Curcuma rhizome against oral squamous cell carcinoma (KB) cells and osteosarcoma (HOS) cells were 21.30 ${\mu}g/ml$ and 39.3${\mu}g/ml$, respectively. The methanol extract of Curcuma rhizome showed inhibitory telomerase inhibitory effect which is required for cancer cell immortality. Therefore, it seems that the anticancer effect of methanol extract of Curcuma rhizome is at least partially due to telomerase inhibitory effect. Five fraction samples were prepared according to its polarity differences and analyzed anti-proliferative effects of each fraction samples on oral squamous cell carcinoma and osteosarcoma cells. Anticancer effect was observed in dichloromethane, and ethylacetate fractions. The highest anticancer effect was found in dichloromethane fraction which had $IC_{50}$ value of 23.3 ${\mu}g/ml$ and 10.5${\mu}g/ml$ against oral squamous cell carcinoma (KB) cells and osteosarcoma (HOS) cells, respectively.

  • Malignant tumor have hypoxic cell fraction, which makes radio-resistant and hypoxia in tumor is a result from the blood flow decrease caused by increase in blood flow resistance. Blood viscosity increase is major factor of increased blood flow resistance and it could be attributed to the decrease in blood deformability index. For the evaluation of the change of blood viscosity and blood deformability in oral squamous cell carcinoma, we perform the test of the change of those factors between the normal control group and oral squamous cell carcinoma cell patient group. Relative viscosity measured against distilled water was $5.25{\pm}0.14$ for normal control group, and $5.78{\pm}0.26$ for the SCC patient group and there was statistical significance between the groups. However, there was no significant difference between the groups in blood viscosity between the groups by tumor size (T1+T2 vs T3+T4). Also, there was no significant difference between the normal control group and SCC patient group in blood deformability index and between the groups by tumor size (T1+T2 vs T3+T4). Increase in blood viscosity was confirmed with this study and it can be postulated that modification blood viscosity might contribute to decrease of hypoxia fraction in oral squamous cell carcinoma, thus improve the effect of radiotherapy and it can be assumed that the main factor of blood viscosity increase is not decrease of blood deformability in oral squamous cell carcinoma.

구강 편평세포암종의 경부 림프절전이에 대한 임상통계학적 연구 (A CLINICO-STATISTICAL STUDY ON CERVICAL LYMPH NODE METASTASIS OF ORAL SQUAMOUS CELL CARCINOMA)

  • Cervical lymph node metastasis is one of the most important predicting factors that influence the prognosis of oral squamous cell carcinoma. Correct diagnosis on cervical lymph node metastasis is essential in determining the extent of operation and treatment modality. So we investigated a clinico-statistical evaluation on cervical lymph node metastasis in 183 patients who were diagnosed with oral squamous cell carcinoma at the Department of Oral and Maxillofacial Surgery in Kyungpook National University Hospital, from January 1st, 1999 to December 31st, 2007. The following results were obtained : 1. Among 183 patients who were diagnosed with oral squamous cell carcinoma, 149 were male and 49 were female. The average age of the patients was 61.8 years old. 2. Patients with advanced T classification showed higher incidence of cervical lymph node metastasis than those with lower T classification. 3. Patients with less differentiated tumors had higher tendency of manifesting cervical lymph node metastasis than those with more differentiated tumors. 4. Sensitivity and specificity on PET/CT was 87.5% and 58.3% respectively. PET/CT showed higher sensitivity and lower false-negative values than those of CT or USG. 5. The 5 - year survival rate of all the oral squamous cell carcinoma patients appeared to be 63.2% By N classification, patients in N0 stage showed a higher survival rate than patients in N1 or N2. 5 - year survival rates according to the modality of neck dissection were as follows in increasing order: no neck dissection group, MRND group, SND group, and RND group.

소목 추출물의 구강암 및 골육종 세포주에 대한 항암작용에 관한 연구 (ANTICANCER EFFECTS OF CAESALPINIA SAPPAN EXTRACTS ON ORAL CARCINOMA AND OSTEOSARCOMA CELLS)

  • Anticancer effect of methanol extract of Caesalpinia sappan L. on oral carcinoma(KB) and osteosarcoma(HOS) cells were investigated in this study. In order to elucidate the anticancer mechanism of Caesalpinia sappan L, we analyzed telomerase inhibitory effect of the methanol extract of Caesalpinia sappan L. In addition, we prepared 5 fraction samples according to its polarity differences and analyzed anticancer effects on oral carcinoma and osteosarcoma cells. Following results are obtained in this study. 1. 50% cell proliferation inhibitory value($IC_{50}$) of the methanol extract of Caesalpinia sappan L. against oral carcinoma(KB) cells and osteosarco ma(HOS) cells were $9.0{\mu}g/ml\;and\;10.9{\mu}g/ml$, respectively. 2. The methanol extract of Caesalpinia sappan L. showed inhibitory effect of telomerase which is required for cancer cell immortality. Therefore, it seems that the anticancer effect of methanol extract of Caesalpinia sappan is at least partially due to telomerase inhibitory effect. 3. Five fraction samples were prepared according to its polarity and 88.7% of ingredient of total methanol extract was transferred to ethylacetate fraction. Thin layer chromatography analysis showed that dichloromethane fraction contained ingredient with relatively high polarity and ethylacetate fraction contained similar ingredient found in total methanol extract. 4. Anticancer effect was observed in n-hexane, dichloromethane, and ethylacetate fractions. The highest anticancer effect was found in dichloromethane fraction which had $IC_{50}$ value of $4.4{\mu}g/ml$ and > $4.0{\mu}g/ml$ against oral carcinoma(KB) cells and osteosarcoma(HOS) cells, respectively.

  • Carcinoma ex pleomorphic adenoma is transformed at the incidence of 1-20% in pleomorphic adenoma and frequently recurred. It accounts for 10% of all malignant salivary tumors and its average age of occurrence is 60s. It will present in a large, long-standing pleomorphic adenoma or in one that was previously treated but has recurred. According to cell composition in malignant cell carcinoma, and clear cell adenocarcinoma. Most (75%) occur in parotid gland, while about 20% occur in the minor gland of the oral mucosa. The metastasis rate to regional lymph node is about 25%, and to distant organs about 33% and the 5-year survival rates are 40%. Though the treatment of the carcinoma ex pleomorphic adenoma is not established, it is treated ideally with and extensive resection, neck dissection, postoperative radiotherapy, and chemotherapy. When occurred in parotid gland, facial paralysis is reported. With a review of literatures, we report a case of carcinoma ex pleomorphic adenoma which operated with total parotidectomy and supraomohyoid neck dissection.

Differential Expression of Cytokeratin 13 in Non-Neoplastic, Dysplastic and Neoplastic Oral Mucosa in a High Risk Pakistani Population

  • Background: Gradual loss of cytokeratin 13 (CK13) may be linked with the severity of dysplastic changes and transformation to malignancy. In this study we assessed the differential expression of CK13 in normal, hyperplastic, dysplastic and cancerous oral mucosa. Materials and Methods: A total of 93 oral biopsies were collected during the 2011-2014 period. The biopsies were characterized as normal (19), hyperplastic (21), severely dysplastic/carcinoma in situ (16) and invasive oral squamous cell carcinoma (OSCC) (37) after morphological assessment. Formalin fixed paraffin embedded sections were stained with a monoclonal antibody against CK13 using the Envision technique. Immunohistochemically stained slides were then analyzed for CK13 expression. Results: CK13 was consistently and diffusely expressed in all normal and hyperplastic tissue biopsies from oral mucosa. Severely dysplastic/carcinoma in situ biopsies showed complete loss in 50% of cases, while in the remaining 50% expression was very focal and weak. OSCC cases showed complete or near complete loss of CK13 in all cases. Few cases showed weak expression in keratin pearls only. Conclusions: This study validates the utility of CK13 IHC as a useful immunohistochemical marker in routine diagnostic practice to make distinction between non-neoplastic from dysplastic and neoplastic (malignant) oral lesions.

Myiasis Associated with Oral Squamous Cell Carcinoma - A Literature Review

  • Advanced oral squamous cell carcinoma (SCC) is a possible risk factor for myiasis, a parasitic infestation of vital tissue of humans or other mammals by dipterous larvae (maggots). Oral myiasis is a rare entity, and is mostly associated with various medical and anatomical conditions, such as neglected mandibular fracture, lip incompetence, cerebral palsy, poor oral hygiene, suppurative lesions, and cancerous wounds. Larvae cause itching and irritation due to their crawling movements and can destroy vital tissues, inducing serious or even life-threating hemorrhage. The aim of the present article was to highlight the occurrence of oral myiasis in association with squamous cell carcinoma and also to highlight the treatment and preventive approaches for such cases. A literature search was performed using MEDLINE for articles published in English relating to the occurrence of oral myiasis in oral SCC. Our search revealed 6 reports on myiasis associated with oral SCC. The surgical debridement of infected tissue with the removal of maggots is the treatment of choice in most cases of oral myiasis.

  • Basaloid squamous cell carcinoma(BSCC) is uncommon and distinct variant of squamous cell carcinoma that arises mostly in the upper aerodigestive tract and aggressive, high grade tumor with an increased tendency to be deeply invasive, multifocal, and metastatic even at the initial presentation. The typical microscopic features of carcinoma with a basaloid pattern in intimate association with a squamous component helps in diagnosis of this tumour. Since Wain's report in 1986, BSCC of oral cavity, the palate, floor of the mouth, nasopharynx, oropharynx and mastoid region have been reported. However, BSCC in the nasal cavity or in the paranasal sinuses is rare and there are few reports in the Korean literature. We had experienced a case of basaloid squamous cell carcinoma that occurred in the left maxillary sinus of 72-year-old woman and reported with review of the clinical and pathologic features from the literature.

Cytotoxic Effect of Syringic Acid on Human Oral Epithelioid Carcinoma Cells

  • This study was undertaken to clerify the cytotoxic effect of syringic acid by colorimetric assay on human cancer cells. For the evaluation of cytotoxicity of syringic acid, the cell viability and cell adhesion activity of syringic acid on cancer cells, human oral epithelioid carcinoma cells were determined using by colorimetric assays such as MTT (3-[4,5­dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide) assay and XTT (2,3-bis-[2-methoxy-4-nitro-5-sulfophenyl]­2H-tetrazolium-5-caboxanilide) assay, respectively after human oral epithelioid carcinoma cells were treated with syringic acid for 48 hours. In this study, the cell viability of syringic acid on human oral epithelioid carcinoma cells showed a significant decrease by MTT assay compared with control, and also, the cell adhesion activity by XTT assay was decreased significantly in these cells after cells were treated with various concentrations of syringic acid for 48 hours. $MTT_{50}\;and\;XTT_{50}\;were\;282.3\;{\mu}M\;and\;418.8{\mu}M$ syringic acid, respectively. These results suggest that syringic acid shows midcytotoxic effect on human oral epithelioid carcinoma cells by the decreasement of the cell viability and the cell adehision activity assessed by colorimetric assay in these cultures.
