Clinical Analysis on Primary Repair of Tetralogy of Fallot Under 10kg of Body Weight (10 kg 이하의 활로4징증 환자에서 완전교정술에 관한 임상적 고찰)

  • The surgical management of symptomatic tetralogy of Fallot in infants is debatable. From November 1986 to August 1990, 21 infants under 10 kg of the body weight with tetralogy of Fallot underwent primary repair. Mean body weight was 8.6$\pm$1.40kg. All the patient were clubbing and there were cyanotic except for 1 patient. Transannular patch was laid down in 8 patients. Right ventricular outflow patch was used with Goretex but pericardial patch was utilized in 3 patients at the initial period of operation. Incidence of the complications following total correction of tetralogy of Fallot was more frequent in the patients placed with transannular patch compared to the patients with right ventricular outflow tract patch. Two deaths occurred in the 21 patients, Hospital mortality was 9.4%, but there were no operative deaths in the patients who transannular patch was laid down. Causes of deaths were low cardiac output.

  • Clonidine, an antihypertensive drug, stimulates postsynaptic $\alpha_2$adrenergic receptors in the CNS and lowers arterial pressure by erects on both cardiac output and peripheral resistance. However, many patients experience that sedation and xerostomia occured upon oral administration of clonidine. These side effects are due to high plasma peak concentration and can be avoided when clonidine is given transdermally. In this study, we performed the skin irritation test for transdermal administration of clonidine patch on New Zealand white rabbits. Twelve New Zealand white rabbits were divided into two groups according to the dose levels, respectively. After transdermal administration of clonidine patch with two doses, clinical manifestations, body weight loss and postmortem findings were observed. We could not find any significant evidence of skin irritation by transdermal administration of clonidine patch.

  • Wearable medical device has been a resurgence of interest thanks to the development of technology and propagation of smart phone in recent years. Various types of wearable devices have been introduced and available in market. Capacitive coupled electrode which measures electrocardiogram over cloth is able to be applied wearable device. In previous approaches of capacitive electrode, they need proper pressure for stable contact of the electrode to body surface. However, wearable device that gives pressure on body surface is not suitable for long-term monitoring. In this study, we proposed adhesive polyurethane-based capacitive electrode for patch-type wearable electrocardiogram (ECG) monitoring device. Self-adhesive polyurethane make the electrode and whole system be adhered to the surface of skin without any pressure. The patch-type system is consisted of analog filter, analog-to-digital converter and wireless transmission module and designed to be attached on the body as a patch. To validate the feasibility of the developed system, we measured ECG signal in stable and active state and extracted heart rate. Therefore, we observed skin response after long-term attachment for biocompatibility of the adhesive polyurethane and adhesive strength of it. The result shows the possibility of applying the developed system for ECG monitoring in real-life.

Review, Assessment, and Learning Lesson on How to Design a Spectroelectrochemical Experiment for the Molten Salt System

  • This work provided a review of three techniques-(1) spectrochemical, (2) electrochemical, and (3) spectroelectrochemical-for molten salt medias. A spectroelectrochemical system was designed by utilizing this information. Here, we designed a spectroelectrochemical cell (SEC) and calibrated temperature controllers, and performed initial tests to explore the system's capability limit. There were several issues and a redesign of the cell was accomplished. The modification of the design allowed us to assemble, align the system with the light sources, and successfully transferred the setup inside a controlled environment. A preliminary run was executed to obtain transmission and absorption background of NaCl-CaCl2 salt at 600℃. It shows that the quartz cuvette has high transmittance effects across all wavelengths and there were lower transmittance effects at the lower wavelength in the molten salt media. Despite a successful initial run, the quartz vessel was mated to the inner cavity of the SEC body. Moreover, there was shearing in the patch cord which resulted in damage to the fiber optic cable, deterioration of the SEC, corrosion in the connection of the cell body, and fiber optic damage. The next generation of the SEC should attach a high temperature fiber optic patch cords without introducing internal mechanical stress to the patch cord body. In addition, MACOR should be used as the cell body materials to prevent corrosion of the surface and avoid the mating issue and a use of an adapter from a manufacturer that combines the free beam to a fiber optic cable should be incorporated in the future design.

  • In this paper, we analysis the electromagnetic scattering of an arbitrary shape dielectric cube subjected to plane wave incidence in three dimensions. MoM(Method of Moments)in which a surface of a body is divided with small triangular patches and equivalence principle are used to fuse the PMCHW(Poggio, Miller, Chang, Harrington, and Wu) Integral Equations with respect to equivalent currents on a dielectric body. Triangular patch and loop-patch basis functions that is robust in wide frequency ranges are used for MoM formulations. Proposed method is very useful to analysis the induced current of arbitrary dielectric bodies and numerical results for a dielectric cube are presented.

  • Typically, the vital signs that are representing the state of human body, are the body temperature, sphygmus, respiration and blood pressure. The body temperature is the result of metabolic regulation and human steady-state body temperature is maintained from 35.9 to $37.4^{\circ}C$ by heat regulatory center. The body temperature is indicative of infection and especially it should be monitored to requiring intensive care patients or after surgical patients. But, measuring of body temperature to a heavy workload on nursing staff has been recognized. And, the health service of nurse is limited by simple tasks such as the measurement and record of vital sign. In this paper, the body temperature monitoring telemetry system was proposed to prove the recoding and transmission of body temperature patch system according the standard(ISO TS11073-92001). We proposed the transmission protocol to suit the MFER(medical waveform format encoding rules). The telemetry patch system was implemented and it was verified by experiments.

Design of a TM31 Higher Order Mode Half Circular-Ring Microstrip Patch Antenna for On-Body Communications (인체 표면 통신을 위한 TM31 고차 모드 반원-링 인체 부착형 마이크로스트립 패치 안테나 설계)

  • In this paper, a $TM_{31}$ higher order mode half circular-ring microstrip antenna with monopole-like radiation characteristic for on-body communication is proposed. By using shorting vias, $TM_{31}$ resonance mode was excited, while achieving compact low-profile antenna with monopole-like radiation characteristics. To overcome the narrow bandwidth of a patch antenna, a C-shape half ring patch with shorting vias having $TM_{31}$ mode is closely located around a half circular patch. For size reduction, half mode is adopted. The proposed antenna has the overall dimensions of $0.25{\lambda}_0{\times}0.46{\lambda}_0{\times}0.025{\lambda}_0$ at the industrial, scientific, and medical(ISM) 2.45 GHz band(2.4~2.485 GHz) and the 10-dB return loss is 4.24 % ranging from 2.38 to 2.49 GHz. To verify body effect, two-thirds muscle equivalent semi solid phantom was fabricated and used to measure the antenna performance. A communication link is analysed to investigate the effect of human-body movements and antenna locations.
