Anthropometric Index, Dietary Habits and Nutrient Intake of the Oldest-old Population Aged 95 and Over Living in Seoul (서울지역에 거주하는 95세 이상 남녀 초고령 노인의 체격지수와 식습관 및 영양섭취 실태)

  • The population aged 95 years and older in Seoul approximately increased to five-fold over the past 10 years, while nationwide rates increased to three-fold. In order to examine the dietary habit and nutritional status of oldest-old population living in Seoul, we recruited 87 subjects (25 males and 62 females) aged 95 years and older. The prevalence of underweight (BMI < 18.5 $kg/m^2$) and obesity (BMI ${\geq}25kg/m^2$) were 18.2% and 18.2% in males, and 20.8% and 9.4% in females, respectively. In self-assessment of health, only 25.3% answered to be unhealthy. More males exercised regularly and reported a wide range of activities than females. The average of %Kcal from carbohydrate, protein and fat (C : P : F) was 64.9 : 13.8 : 21.2 in males and 68.1 : 14.2 : 17.7 in females. The average daily energy intake was 1,307 kcal in males and 1,304 kcal in females. More than 75% of subjects were taking under estimated average requirements (EAR) for vitamin $B_1$, $B_2$ & C and Ca. The average of mean adequacy ratio (MAR) was 0.66 in males and 0.70 in females, and 28.8% of males and 12.9% of females were in MAR < 0.50. Based on MAR, 32.0% of males and 14.5% females were classified as normal and 16.0% of males and 25.8% of females were classified as malnourished. Our subjects were taking more animal food, especially milk and its products, compared to those living, in rural areas. However, a significant proportion did not meet the EAR for vitamin $B_1$, $B_2$ & C and Ca.

The Effects of Subjective Differences in Age and Self-Perceptions of Aging on Depression : Focusing on Moderating Effects of Gender and Age Groups (주관적 연령격차와 주관적 노후격차가 우울에 미치는 영향: 성별과 연령대의 조절효과를 중심으로)

  • The purpose of this study is to examine the influence of subjective differences in age (difference between subjective age and chronological age) and self-perceptions of aging (differences between chronological age and age in the elderly) on depression. Also, we confirm the moderating effect of gender and age groups(young-old, old-old, and oldest-old). The subjects of this study were 547 older adults aged 65 and over who use a senior welfare center or day-care center for the elderly in the Seoul metropolitan area. The results of this study showed that depression was higher when differences between subjective age and chronological age, and self-perceptions of aging were higher. Also, the age groups showed a moderating effect in relation to differences between subjective age and chronological age on depression. In addition, the moderating effect of gender on differences between self-perceptions of aging and depression was confirmed. In the young-old population, when the subjective age was higher than the chronological age, the level of depression rapidly increased, on the other hand, the level of depression of the oldest-old population decreased. When differences in self-perceptions of aging were higher, men showed more signs of depression than women. This study is significant in that it confirms the importance of subjective perception of old age rather than only chronological age.Based on the results of this study, we propose a practical intervention plan and social discourse on aging and the perception thereof in later life.

Comparison of Factors Associated With Agitation Among Youngest-Old, Middle-Old, and Oldest-Old Hospitalized Patients Referred to the Psychiatric Department (정신건강의학과에 협진의뢰된 연소노인, 고령노인, 초고령노인 입원환자의 초조의 연관요인 비교)

  • Objectives : In this study, elderly hospitalized patients aged 65 years or older who were admitted to a university hospital and referred to the department of psychiatry were classified into youngest-old, middle-old, and oldest-old. It was conducted to find out what factors contribute to the agitation pattern by age group, whether there is a difference in the factors, and whether there is a difference in the severity of the agitation pattern. Methods : From July 1, 2021 to December 31, 2021, the medical records of patients aged 65 years or older who were referred to the department of psychiatry were retrospectively reviewed. Age, gender, route of hospitalization, department of referral, reason for referral, treatment method, presence of internal and surgical diseases, and hematological test data were investigated. Results : There was a significant correlation with the RASS score in cases of surgery referral and high CRP levels for youngest-old aged 65 to 74 years, in cases of surgery referral, dementia and hyponatremia for middle-old aged 75 to 84 years, in case of dementia for oldest-old aged 85 years old or older. In addition, there were differences in the severity of agitation patterns between age groups. Conclusions : As the age group increases, the agitation of delirium patients becomes more severe, and various factors contributing to the agitation pattern also differ by age group. Therefore, when treating elderly patients with delirium, attention should be paid to factors that may affect agitation depending on age.

Total Anomalous Pulmonary Venous Connection in Adult -A Case Report- (성인 총폐정맥 환류이상 -1례 보고-)

  • We experienced one case of TAPVC patient who was 39 years old man with mild dyspnea on exertion and easy fatigability. He was the oldest patient among TAPVC which was reported in Korea. Preoperative echocardiogram, cardiac catheterization and angiogram revealed supracardiac type TAPVC drained through left innominate vein. An anastomosis between common pulmonary venous trunk and left atrium, pericardial patch closure of ASD and ligation of left vertical vein were performed with extracorporeal circulation. The postoperative course was uneventful and discharged with excellent general condition and has been well during 8 months follow-up.

Age-related Circulating Inflammatory Markers and Cardiovascular Disease Risk Factors in Korean Women (한국 성인 여성에서 연령에 따른 혈중 염증 표지자와 심혈관계 질환 위험 요인에 대한 연구)

  • The purpose of this study was to investigate the age-related changes of cardiovascular disease risk factors and inflammatory markers in non-obese Korean women. Subjects were 112 women over 20 years old with body mass index (BMI) less than $30 kg/m^2$ and were divided into 3 groups (< 40 years, $40{\sim}59$ years, ${\ge}60$ years). Mean weight and BMI in the oldest group were significantly higher than those in the other 2 younger groups (p < 0.05). Mean total cholesterol, triglyceride, LDL-cholesterol and apolipoprotein B/apolipoprotein A1 ratio (BAR) in the oldest group were significantly higher than those in the youngest group (p < 0.05), and mean HDL-cholesterol of the oldest group was significantly lower than that of the youngest group (p < 0.05). The older-aged group showed significantly higher mean values of atherogenic index (AI) and LDL/HDL ratio (p < 0.05) than the respective younger-aged group, and AI was significantly correlated with age, nitric oxide and thiobarbituric acid reactive substances (p < 0.01). In addition, mean vascular cell adhesion molecule-l (VCAM-1) tended to be higher in the older-aged group than the younger group. Tumor necrosis factor-${\alpha}$, a proinflammatory maker, was significantly positively correlated with serum homocysteine, a cardiovascular disease risk factor (p < 0.01). In addition, a significantly positive correlation was observed between C-reactive protein and BAR (p < 0.01). Overall results suggested that the aging might affect the increase of cardiovascular disease risk factors including the serum lipid profiles, weight and BMI, and age-related increases of weight and BMI might play a role in changes in certain biomarkers of inflammation. Dietary Behavior Factors for Desirable Nutrient Intakes of the Oldest-old Population in Sunchang County (순창군 장수인의 영양소 섭취수준에 영향을 미치는 식행동 인자)

  • In order to identify valuable factors for improving dietary quality of Korean rural elderly, the consumption frequency of food groups, dietary habits and health behaviors related to nutrient intakes of the elderly over 85 of Sunchang County were studied. The 171 subjects (61 males and 110 females) were chosen from Sunchang Province by stratified sampling. Among food groups, the consumption frequency of meat and fish groups influenced significantly on their nutrient intakes than the other groups. The subjects consuming more than 4 times per week of meat or fish had significantly higher nutrient intakes level than the other groups. The group consuming milk and milk products everday was higher in vitamin $B_2$ intake level than the other groups. Among dietary habits, the groups with good appetite and regular mealtime compared with the groups with low appetite and irregular mealtime had significantly higher nutrient intake levels. The family type was the determining factor of nutrient intakes. The subjects living with family members consumed higher nutrient intakes than the subjects living alone. The smoking and the alcohol consumption themselves did not affect nutrient intakes, but the subjects having alcohol consumption frequently more than once a day had lower nutrient intakes than the others. The group with dentures did not differ significantly from the without denture group in nutrient intakes; however, those who had chewing problems showed the lower nutrient intakes compared to those who did not have chewing problems. The subjects with more than two kinds of clinical symptoms had lower nutrient intakes than the subjects with less than two kinds of clinical symptoms. These results indicate that the identifying factors for desirable nutrient intakes of rural elderly were characterized as appropriate consumption frequencies of meat and fish, good appetite, living with family, regular mealtime, chewing without difficulty, and low incidence of chronic disease.

  • To understand the governing institutions' arrangement in an old city of the late Joseon dynasty, it is necessary to compare and weigh the detailed old map with regional geographical records Eupji. In particular, the investigation of 'Closure Land Registration Map' is indispensable, which was made in the period of the Japanese occupation. This study aims to restore the urban land lots arrangement of Cheonan-Gun's old center part in the initial period of the Japanese occupation, using the oldest 'Closure Land Registration Map(1940)' of Cheonan-Gun. The results of this study will be helpful in understanding the governing institutions' arrangement of Cheonan-Gun in the late Joseon dynasty.

A Study on the Restoration of the Russian Legation in Jeongdong Seoul in the Latter Era of Choseon Dynasty (구한말 서울 정동의 러시아공사관에 대한 복원적 연구)

  • Old Russian Legation(1890) has been attracted public attention in terms of politics and foreign affairs than architectural style because of conditions of location and historical incident. Though it was heavily damaged during the Korean war, and only the tower has been restored, it is very important in architectural history in Korea, because it is the oldest western style brick building which is existing with the old bishop's house in Myeong-Dong. But it has been impossible to reveal exactly the original form as there has been few reliable materials(picture, drawing, document) and it's remains were nearly destroyed owing to development of the surroundings. Recently the decisive document(site outline map) was found, and new research on the architect Sabatine was studied. So I can get into close with the original form by the results. In this study, I tried to conjecture the original form of the old Russian legation(the size, style, plan and indoor space, arrangement of attached facilities) and I checked the location and structure of the room where King Gojong stayed when he took refuge, and the use of the passage in the basement.
