지역사회에 거주하는 노인 뇌졸중 환자의 학대 관련 요인 (The Relating Factors of Elder Abuse among Community-dwelling Elderly with Stroke)

  • Purpose: The purpose of this secondary analysis study was to examine the incidence of abuse and the contributing factors for that abuse among the Korean elderly with stroke. Methods: Data were derived from the 2008 Korean National Survey on Older Adults. The participants were 934 community-dwelling elderly with stroke. Data were analyzed using the Rao-Scott $x^2$-test, simple logistic regression and multiple logistic regression with the PASW 18.0 complex samples analysis module. Results: The prevalence of elder abuse was 13.5%. Emotional abuse (10.0%) was the most frequently reported abuse, followed by financial neglect (3.8%), caring neglect (3.3%), financial abuse (2.1%), and physical abuse (1.9%). 44.6% of the perpetrators of emotional abuse were coresidential family members. Participants without spouses (odds ratio=2.05, 95% CI=1.14~3.68), those without diabetes (odds ratio=2.24, 95% CI=1.15~4.39), and depressed participants (odds ratio=2.72, 95% CI=1.34~5.52) were more likely to be abused. Conclusion: Emotional abuse was the most frequently reported type of abuse while physical abuse was least frequent among the elderly with stroke. Nurses caring for the elderly with stroke should be aware of the types of abuse and risk factors such as lack of spouse and incidents of depression.

Cross-sectional Study for Blood Metal Concentration in Patients with Herbal Medicine Intake

  • Evidences from various countries suggest that toxic heavy metals in herbal medicine may constitute a serious health problem. In order to evaluate whether the toxic heavy metals caused by herbal medicine intake, blood samples collected from 222 patients taking herbal medicine were analyzed. In average levels of analyzed metals, $0.4{\sim}33.9%$ of total samples for 8 metals such as Cd, Co, Cu, Hg, Mn, Ni, Pb and Zn except Cr and Fe exceeded the upper limit for WHO reference value. In analysis of regression coefficients indicating the levels of metals increased or decreased after taking herbal medicine for one month, however, there were different aspects by intake types for herbal medicine. For example, the metals increased by taking decoction in blood samples were as follows; Cd and Pb whether Mn, Ni and Pb as increased metals were identified in the group taking pill and decoction(combined intake group). The odds ratio showing values higher than 1 indicating that people who take herbal medicine would have possibility higher for metal accumulation in blood than that from people who do not take herbal medicine. The metals showing the odds ratio higher than 1 were Hg and Ni in decoction group, and Cd and Hg in combined intake group. However, eight of the total, 10 metals showed the odds ratios lower than 1 by taking herbal medicine. Thus, this may explain the possible role of herbal medicine as a chelator for heavy metals in body.

Psychological Distress and Occupational Injury: Findings from the National Health Interview Survey 2000-2003

  • Objectives : This study examined whether serious psychological distress (SPD) is associated with occupational injury among US employees. Methods : The employed population aged 18-64 years was examined (n=101,855) using data from the National Health Interview Survey (NHIS) 2000-2003. SPD was measured using the Kessler 6-item Psychological Distress Scale (K-6), a screening scale designed to identify persons with serious mental illness. The predicted marginal prevalence of psychological distress and occupational injury with the adjusted odds ratio were estimated using multiple logistic regression analyses. Results : The age-adjusted 3-month prevalence of occupational injury was $0.80{\pm}0.12%$ in workers with SPD, which was 37% greater than in workers without SPD ($0.58{\pm}0.03%$). The odds of occupational injury in workers with SPD were higher compared to workers without SPD (OR=1.34, 95% CI=0.93-1.92), after controlling for sex, age, race, education, occupation, and activity limitation by at least one medical condition. Male, service and blue collar occupation, and activity limiation by co-morbidity showed significantly higher odds of occupational injury for workers with SPD. Conclusions : The findings suggest that SPD accounts for an increased likelihood of occupational injury among US employees. A further longitudinal study is needed to differentiate the mechanism or causal pathways linking individual injury risk at the workplace, SPD, and socioeconomic factors.

계획되지 않은 재입원에 대한 위험요인분석 (A Study on the Identification of Risk Factors for unplanned Readmissions in a University Hospital)

  • This study was designed to identify the risk factors of unplanned readmission in a university hospital. The six-month discharge information from January to June, 2000 in a tertiary university hospital was used as a source of data through the medical record and hospital information system. To increase the effect of comparison. the data were collected by sampling 192 couples (384 patients) of unplanned readmission group through the matching by its disease groups, sex, and age. The accuracy of prediction for unplanned readmission was analyzed by constructing the predicted model of unplanned readmission through the logistic regression. The study results are as follows. The conditional logistic regression analysis was performed with nine variables at the significance level 0.05 through univariate analysis including residence, days after discharge, initial admission route, previous admission, transfer to special care unite, hospital stay days, medical care expenses, special cares, and laboratory and imaging services. As a result, the closer the patients live in Seoul and Gyeong-in area (Odds ratio=2.529, p=0.003), the shorter the days after discharge was (Odds ratio=0.600, p=0.000), and the more frequent admission rate was (Odds ratio=2.317, p=0.004), the more unplanned readmission was resulted. Also, the accuracy of prediction for data classification of this regression model showed $70.3\%$(032+83/306).

Risk Factors for Wound Dehiscence after Guided Bone Regeneration in Dental Implant Surgery

  • Purpose: The purpose of this study was to evaluate risks for wound dehiscence after guided bone regeneration (GBR) in dental implant surgery. Methods: Patients who received dental implant therapy with GBR procedure at Seoul National University Bundang Hospital (Seongnam, Korea) from June 2004 to May 2007 were included. The clinical outcome of interest was complications related to dental implant surgery. The factors influencing wound dehiscence, classified into patient-related factors, surgery-related factors and material-related factors, were evaluated. Results: One hundred and fifteen cases (202 implants) were included in this study. Wound dehiscence (19.1%) was considered a major complication. The risk of wound dehiscence was higher in males than in females (odds ratio=4.279, P =0.014). In the main graft, the allogenic group had the lowest risk of wound dehiscence (odds ratio=0.106, P =0.006). Though the external connection group had a higher risk of wound dehiscence than the internal connection group (odds ratio=2.381), the difference was not significant (P =0.100). Conclusion: In this study, male gender and main graft have the highest risk of wound dehiscence. To reduce wound dehiscence after GBR, instructions on postoperative care with supplementary procedure for the protection of the wound dehiscence is recommended, especially to male patients. A main graft with a gel base can reduce the risk of wound dehiscence.

한 농촌지역 주민들의 혈당장애에 따른 hs-CRP와 대사증후군 (The hs-CRP Levels and the Metabolic Syndrome according to Glucose Intolerance in Inhabitants of Rural Communities)

  • Purpose: This study was to investigate the relations and odds ratio between hs-CRP and the risk factors of metabolic syndrome according to glucose intolerance and diabetes among the residents of a rural community. Methods: The subjects include 1,968 adults aged from 40 to 70 who were divided into four groups and a diabetes group according to glucose intolerance to compare the relations and risk ratio between hs-CRP and the risk factors of metabolic syndrome. Results: The results reveal that the greater the subjects' glucose intolerance was, the higher their hs-CRP became and the more risk factors of metabolic syndrome they had. The impaired glucose tolerance group showed 1.7 times higher blood pressure than the control group. The diabetes group showed a high odds ratio with 2.3 times higher blood pressure, 2.2 times higher abdominal obesity, and 2.4 times higherW/Ht than the control group. And the odds ratio increased significantly by 1.7 times in the hs-CRP intermediate risk group and 2.5 times in the high risk group compared with the control group. Conclusion: Considering the study results, it is very important to monitor abdominal obesity, blood pressure and the intermediate and high risk group of hs-CRP in order to reduce the contraction of cardiovascular diseases.

제주지역 성인의 우울 유병률과 관련 요인 (The Prevalence of Depression and Related Factors among Adults)

  • Purpose: This study was to investigate depression prevalence and related factors of depression in Jeju Province. Methods: The study sample selected using randomized cluster sampling method, and the unit of cluster was 25 households. Data were collected from 1275 adults in 723 households through home visit interview. The response rate was 89.4%. The instrument used for measuring depression was CES-D, the cut-off score was 21. Data were analyzed using Chi-square test and logistic regression. Results: The prevalence of depression in Jeju was 18.1%. The significant differences were found with depression prevalence rate by the group of gender, age, inhabited area, marital status, educational status, monthly household income, life satisfaction, socioeconomic status, perceived health status, morbidity during last 2 weeks, perceived stress, perceived fatigue, participation in social activities, and family life satisfaction. The most predictive factors among those variables were perceived fatigue with 17.93 of odds ratio, family life satisfaction with 9.86 odds ratio, and perceived stress with 9.66 of odds ratio. Conclusion: The prevention and management of depression program development was suggested and mental health promotion program for the prevention of depression was suggested.

한국 노인의 음주 및 흡연 행태와 치주질환의 융합적 관련성 (Convergent Relationship between drinking and smoking behavior and periodontal disease in elderly Koreans)

  • 본 연구는 2013-2015년도 국민건강영양조사 원시자료를 활용하여 한국노인의 음주 및 흡연과 치주질환의 관련성을 평가하고자 하였다. 분석 결과, 비음주, 비흡연군에 비해 음주, 흡연군(odds ratio[OR] 1.50; 95% confidence interval[CI], 1.00-2.24)에서 치주질환 교차비가 유의하게 높았다. 치주질환 교차비는 음주와 흡연을 전혀 하지 않은 노인에 비해 음주와 흡연을 동시에 하는 노인에서 유의하게 높았다. 본 연구를 통해 음주 및 흡연은 치주질환과 유의한 관련성이 있음을 확인하였다. 따라서 노인의 구강건강유지를 위해 절주와 금연교육을 강화해야 할 것으로 판단된다.

Factors Associated With Quitting Smoking in Indonesia

  • Objectives: The aim of this study was to identify factors associated with quitting smoking in Indonesia Methods: Data on 11 115 individuals from the fifth wave of the Indonesia Family Life Survey were analyzed. Quitting smoking was the main outcome, defined as smoking status based on the answer to the question "do you still habitually (smoke cigarettes/smoke a pipe/use chewing tobacco) or have you totally quit?" Logistic regression was performed to identify factors associated with successful attempts to quit smoking. Results: The prevalence of quitting smoking was 12.3%. The odds of successfully quitting smoking were higher among smokers who were female (adjusted odds ratio [aOR], 2.69; 95% confidence interval [CI], 2.08 to 3.33), were divorced (aOR, 2.45; 95% CI, 1.82 to 3.29), did not chew tobacco (aOR, 3.01; 95% CI, 1.79 to 5.08), found it difficult to sacrifice smoking at other times than in the morning (aOR, 1.29; 95% CI, 1.14 to 1.46), and not smoke when sick (aOR, 1.32; 95% CI, 1.14 to 1.54). About 59% of variance in successful attempts to quit smoking could be explained using a model consisting of those variables. Conclusions: Female sex, being divorced, not chewing tobacco, and nicotine dependence increased the odds of quitting smoking and were associated with quitting smoking successfully. Regular and integrated attempts to quit smoking based on individuals' internal characteristics, tobacco use activity, and smoking behavior are needed to quit smoking.

Association between obesity and high-sensitivity C-reactive protein in Korean adults without cardiovascular disease

  • Purpose: This study aimed to investigate the association between obesity and high-sensitivity C-reactive protein (hs-CRP) levels in Korean adults without cardiovascular disease (CVD). Methods: The subjects were 3,634 adults, and data were extracted from the seventh Korea National Health and Nutrition Examination Survey (KNHANES VII-3). A complex sampling design analysis was applied to reflect the stratified and clustered weights. The data were analyzed using the complex sample Rao-Scott chi-square test and multiple logistic regression analysis (in SPSS for Windows version 26.0). Obesity, according to body mass index (BMI), was defined as obesity (BMI = 25-29.9 kg/m2), high obesity (BMI = 30-34.9 kg/m2), and super-high obesity (BMI ≥ 35 kg/m2), and abdominal obesity (AO) was defined as a waist circumference (WC) ≥ 90 cm in males and WC ≥ 85 cm in females. Results: The odds ratios for moderate CVD risk (hs-CRP; 1-3 mg/dL) were 2.21, 4.16, and 7.13 in the obesity, high obesity, and super-high obesity groups, respectively, compared to the normal BMI group. The odds ratio for moderate CVD risk was 2.18 in males with AO and 1.88 in females with AO. The odds ratios for high CVD risk (hs-CRP > 3 mg/L) were 4.40 and 17.55 in the high obesity and super-high obesity groups, respectively, compared to the normal BMI group. Conclusion: This study provides evidence that early detection and prevention programs for CVD should include obesity-related interventions aiming to modulate hs-CRP.
