작업환경측정 자료를 활용한 특별관리물질 노출 현황 및 고위험 업종 분석 (Analysis of Exposure Status and High-risk Industries of Special Management Substances using Working Environment Measurement Data)

  • Objectives: This study aims to assess the current status of work environment measurement (WEM) for carcinogenic, mutagenic, reproductive toxic substances and to identify their high-risk industries for hazard surveillance and risk assessment. Methods: WEM Data from 37 specially managed CMR substances (2018-2022), the high-risk industries were identified by using estimated 95th percentile levels (X0.95) of measurements compared with the occupational exposure limit (OEL). Results: The substances most frequently measured were nickel (insoluble), followed by sulfuric acid, lead, chromium (VI), formaldehyde, phenol, dimethylformamide and benzene. The industries with highest number of measurement samples for special management substances were plating of metals, followed by hospitals, general repair services of motor vehicles, building of ships, manufacture of synthetic resin and other plastic materials, manufacture of finished medicaments. Out of the 37 special management substances studied, 22 had a non-detection rate of over 90%. The rate of samples exceeding the OEL was less than 1% for all substances. The substance with the highest number of samples exceeding the OEL was trichloroethylene, which also had the highest average concentration compared to exposure limit. The substances with the highest percentage of industries which (X0.95) exceeding OEL was formaldehyde followed by sulfuric acid, trichloroethylene, lead, I-bromopropane etc. Conclusions: This study identified high-risk industries of CMR sunstances, highlighting the need for prioritizing these industries in hazards surveillance and risk assessment.

  • The study subjects included the workers who were estimated to have been exposed in a low density of lead-laden glaze from pottery manufacturers. They were evaluated in terms of PbB and PbU as indices of lead concentration in the body, and Hb, Hct, ALAD, and FEP as indices of biochemical lead poisoning. Consequently, the study results revealed the relationship between each of indices and its effectiveness to indicate the degree of occupational lead exposure. 1. The mean of PbB was $22.85{\;}{\mu}g/d{\ell}$(range; $4.2~44.7{\;}{\mu}g/d{\ell}$) and PbU $45.69{\mu}g/d{\ell}$(range; $6.7~87.5{\mu}g/d{\ell}$), to indicate the level of lead exposure in the subjects. 2. While there was a high, linear correlation between PbB, PbU and FEP (r=0.835, r=0.855, respectively), a low correlation existed between PbB and ALAD(r=-0.240). 3. While there was a high, linear correlation between PbU and FEP(r=0.825), a low correlation existed between PbU and ALAD(r=-0.352). The above results suggest that FEP can be used as a highly useful biochemical index of lead poisoning, based on its high correlation with PbB and PbU indicating lead concentration in the body.

Association of Blood Pressure with Blood Lead and Cadmium Levels in Korean Adolescents: Analysis of Data from the 2010-2016 Korean National Health and Nutrition Examination Survey

  • Background: We evaluated the association of blood pressure (BP) with blood levels of cadmium, lead, and cadmium and lead together (cadmium + lead) in a representative sample of adolescents from Korea. Methods: We used 2010-2016 data from the Korean National Health and Nutrition Examination Survey. This cross-sectional study enrolled adolescents aged at 10-18 years-old who completed a health examination survey and had blood measurements of lead and cadmium. The association of adjusted mean differences in diastolic and systolic BP with doubling of blood lead and cadmium were estimated by regression of BP against ${\log}_2$-transformed blood metals and their quartiles after covariate adjustment. Adjusted odds ratio for prehypertension were calculated for ${\log}_2$-transformed blood levels of lead and cadmium and their quartiles. Results: Our analysis of adolescents in Korea indicated that blood levels of lead and cadmium were not significantly associated with increased BP or risk of prehypertension. However, the cadmium + lead level was associated with prehypertension. Previous studies showed that blood levels of lead and cadmium were associated with increased BP and risk of hypertension in adult populations. We found no such association in Korean adolescents. Conclusion: We found that the cadmium + lead level was associated with prehypertension. The differences between adults and adolescents are because adolescents generally have lower levels of these blood metals or because adolescents only rarely have hypertension.

A Study on High Current Rectifier Systems with Mitigated Time-Varying Magnetic Field Generation

  • This paper investigates occupational exposure to time-varying magnetic field generation in high power rectifier systems. Two different kinds of high power rectifier systems of 25kA are modeled and analyzed. The performance is compared and evaluated on the basis of exposure guidelines from ICNIRP. In order to focus on the qualitative effect of rectifier operation, the mechanical structure of current carrying conductors is simplified as infinite long bus-bar model and low frequency harmonic contents up to 65kHz are considered. Thyristor rectifier generates a significant amount of low frequency magnetic field harmonic contents both at ac and dc side of rectifier infringing the limit from ICNIRP. The multilevel rectifier-IGCT type has almost negligible field generation from ac input side and smaller harmonic contents in dc load side complying with ICNIRP guideline. This remarkable advantage of multilevel rectifier-IGCT type can lead to very simple site layout design for installation and cost-effective compliance to guideline of occupational exposure against magnetic field.

일부 공단지역주민의 환경노출수준 평가 연구 (A study on environmental exposure levels of residents in an industrial complex area)

  • As a follow-up survey of A Study of Monitoring Method on Exposure Level and Biomarkers of Environmental Pollutants-Focused on Ulsan Industrial Comlpex Area, published in 2005, a close examination of the health status was conducted for selected Ulsan residents. Based on the previous study, a total number of 129 subjects were divided into two groups: the exposure group consisted of 39 residents whose blood levels of lead, mercury and/or cadmium were higher than international reference values, and 90 residents of control group with normal levels. Environmental exposure level and its association with health condition were examined by various methods such as questionnaire, analyses of lead, mercury and cadmium in blood and medical examinations. The geometric mean concentrations of lead, mercury and cadmium in blood were respectively $2.07{\mu}g/d{\ell}$ (exposure $2.54{\mu}g/d{\ell}$, control $1.90{\mu}g/d{\ell}$), $5.94{\mu}g/L$ (exposure $8.57{\mu}g/L$, control $5.07{\mu}g/L$), $1.32{\mu}g/L$ (exposure $1.30{\mu}g/L$, control $1.33{\mu}g/L$). The concentrations of the three heavy metals in blood showed lower levels than internationally recommended values except for blood mercury. Also, any abnormal or, peculiar disease, or target tissue damage related to the heavy metals was not observed among the all subjects. Therefore, it can be said that no significant difference of heavy metal concentrations and health conditions was found between the two groups.

옥외 근로자들의 카드뮴과 납 노출 영향요인 (High Influential Factor of Cadmium and Lead Exposure in Outdoor Workers)

  • Objectives: The study was evaluated exposure variation and daily absorption level of cadmium, lead concentration of ambient air of monthly data from 1999 to 2017 for main exposure factor in outdoor workers. Methods: Based on the monthly data from 'The annual report of air quality in Korea from 1999 to 2018' in 'Air Korea' website in the Korean Ministry of Environment. The monthly data of PM2.5, PM10, cadmium, lead concentration of ambient air were recalculated to average, minimum, and maximum. And these data were combined to Asian-dust exposure data from 'The annual report of Asian-dust·smog in 2017' of National Institute of Meteorological Sciences in Korea. Results: Geometric mean(minimum-maximum) concentration in ambient air of monthly data were 0.0017 (ND-0.2015) mg/㎥ in cadmium and 0.0467(ND-0.8554) mg/㎥ in Pb from 1999 to 2017. Both of Cd and Pb concentration in ambient air showed the highest concentration in January and the lowest in August among annual variation from 1999 to 2017. PM10 and PM2.5 level showed the highest in March(PM10) and February (PM2.5) the lowest in August both of PM10 and PM2.5. Discussion: Based on exposure data and prior reports, daily Cd absorption was estimated to 0.013(ND-1.511) mg/day from respiration and 1.89 mg/day from daily food(25.2 mg/day of daily Cd intake). In case of Pb, daily absorption was estimated to 0.350(ND-6.416) mg/day from respiration and 1.38-1.71 mg/day from daily food intake. Conclusion: Cd and Pb with Asian-dust have high influential factor to increase the Cd and Pb exposure at Winter and Spring season in outdoor workers.

과거의 직업적 납 노출에 의한 혈중납 및 골중납의 증가가 남성 골밀도 변화에 미치는 영향 (Effect of increased blood and tibia lead on the change of bone mineral density in retired male lead workers)

  • This study was designed to investigate the effect of increased blood and tibia lead on the change of bone mineral density in retired male lead workers. One hundred nine retired male lead workers who worked in 4 different lead industries and 51 nonoccupationally lead exposed male subjects were recruited from March 2004 to October 2004. Bone mineral density(BMD) was measured by broadband ultrasound attenuation(BUA) at left calcaneous bone area with broadband ultrasound attenuation method of QUS-2(Metra Biosystems Inc, USA). Tibia bone lead was measured for skeletal bone lead with K-xray fluorescence(K-XRF) and blood lead was analyzed with flameless atomic spectrophotometer. Hemoglobin, hematocrit, serum calcium and iron were also analyzed. In addition, information for smoking and drinking status and basic personal data such as age and lead exposure were also collected using questionnaire inquiry. Blood lead was correlated with tibia lead (r=0.711) and these two variables were negatively correlated with BUA in bivariate analysis. BUA and tibia lead showed significant main effects on the change of blood lead after adjusting covariates. The effect modification by the level of BMD (low: lower than the median of BUA and high: higher than the median of BUA) was observed between the association of tibia lead and blood lead after adjustment of covariates. The subjects who had higher BMD seemed to have lower blood lead by the increase of tibia lead than those of lower BMD. In the multiple regression analysis of blood lead and tibia lead on BUA after adjustment of covariates, only blood lead showed statistically significant effect on BUA. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.

저 농도의 망간과 납 노출이 면역기능에 미치는 영향 (Effects of low-level exposure to manganese and lead on immune function)

  • 이 연구는 Mn과 Pb의 노출이 면역체계에 미치는 영향을 파악하고자 하였다. 연구대상자는 남성 근로자 42 명으로, 대상자 중 13명은 사무직 근로자(실험군 I), 21 명은 제조업 사업장 근로자(실험군 II), 8명은 용접 작업자(실험군 III)였다. 대상자의 혈액 중 Mn과 Pb 농도는 실험군별로 유의한 차이를 보였다. 실험군 I 대상자의 CD19+와 total lymphocytes 농도는 기타의 실험군 대상자에서보다 유의하게 높았으나 다른 T lymphocytes subpopulation의 농도는 차이가 없었다. Mn과 Pb 농도는 T lymphocytes subpopulation과 음의 상관관계를 보였으나, Mn은 CD4+CD45RO+와 자연살해세포를 제외한 T lymphocytes subpopulation과 유의한 음의 상관관계를 보였다. Pb 농도는 단지 total lymphocytes 농도와 유의한 음의 상관관계를 보였다. 이 연구에서 Mn과 Pb의 직업적인 노출은 세포성면역계에 영향을 미칠 수 있음을 제시하였다.

남성 근로자의 생식보건 역학연구 (Epidemiologic Studies of Reproductive Health in Male Workers)

  • Objectives: The reproductive health of female workers has been extensively investigated in South Korea and other countries worldwide. However, few studies have discussed the reproductive health of male workers. In this study, we reviewed the recent literature that reports on the effects of occupational exposure on the reproductive health of male workers and the health of their children. Methods: In May 2020 we used the PubMed search engine to search the literature over the last 10 years and chose case-control, cohort, and cross-sectional studies and reviews. We selected epidemiological studies that investigated the association between pre-pregnant occupational exposure and the reproductive health of male workers and the health of their children. We excluded case reports, non-epidemiological studies (animal experiments, cellular-level experiments, and similar articles), and studies that described postnatal occupational exposure. Results: We eventually selected 23 studies. The studies that included exclusively male workers reported that those employed in the agricultural sector or those exposed to pesticides showed lower blood levels of reproductive hormones and a high risk of lympho-hematopoietic system cancer in their children. Male workers exposed to complex organic solvents and organic compounds showed a high risk of poor semen quality, increased time to pregnancy, decreased blood levels of reproductive hormones, and a high risk of lympho-hematopoietic system cancer in their children. Male workers employed in occupations that involved significant social contact, or in the leather and livestock industries, and in occupations with high levels of exposure to lead and organic solvents showed a high risk of malignancies, including lympho-hematopoietic system cancer, neuroblastoma, and central nervous system tumors in their children. Studies that investigated both male and female workers reported that children of male smelters showed a high risk of premature birth, and children of male workers exposed to metals showed a high risk of hypospadias and cryptorchidism. Children of male welders and workers employed in the glass, ceramic, and tile industries showed a high risk of premature birth. Conclusion: The findings of this study will serve as basic data for further research on male workers' reproductive health and provide a scientific basis for the development of strategies to protect the reproductive health of males employed in high-risk occupations. Moreover, the results of this study may provide guidelines to improve the understanding of and knowledge on male workers' reproductive health.

Banding the World Together; The Global Growth of Control Banding and Qualitative Occupational Risk Management

  • Control Banding (CB) strategies to prevent work-related illness and injury for 2.5 billion workers without access to health and safety professionals has grown exponentially this last decade. CB originates from the pharmaceutical industry to control active pharmaceutical ingredients without a complete toxicological basis and therefore no occupational exposure limits. CB applications have broadened into chemicals in general - including new emerging risks like nanomaterials and recently into ergonomics and injury prevention. CB is an action-oriented qualitative risk assessment strategy offering solutions and control measures to users through "toolkits". Chemical CB toolkits are user-friendly approaches used to achieve workplace controls in the absence of firm toxicological and quantitative exposure information. The model (technical) validation of these toolkits is well described, however firm operational analyses (implementation aspects) are lacking. Consequentially, it is often not known if toolkit use leads to successful interventions at individual workplaces. This might lead to virtual safe workplaces without knowing if workers are truly protected. Upcoming international strategies from the World Health Organization Collaborating Centers request assistance in developing and evaluating action-oriented procedures for workplace risk assessment and control. It is expected that to fulfill this strategy's goals, CB approaches will continue its important growth in protecting workers.
