  • Obesity is a major nutritional problem in the developed countries. The prevalence of obesity may range from 10 to 50 per rent or mort of adult population and it may be increasing tendency. Many efforts have been made to understand the pathogenesis of obesity, but except a few metabolic obesities in the most of obese patients, the mechanisms are not understood. The treatment modalities of obesity, ranging from dietary and pubilc health intervention through the pharmacological and surgical therapy, have been developed and tested. In the obese patients mortalities and mobilities are significantly increased than non obese subjects due to hypertension, diabetics, and other problems. There are four possible mechanisms by which energy balance might be altered to enhance metabolic efficiency. futile metabolic pathway, alteration of protein rum over, alteration in sodium-potassium ATPase and alteration in uncoupled oxidation in brown adipose tissue are considered as possible mechanisms. Low calory and very low calory diets are recommended as a dietary program. Several pharmacological agent such as benzphetamine, fenfluramine, mazindol and fluoxetin are currently popular drugs for the treatment of obesity.

  • Background: Serum concentration of cystatin C, a marker of glomerular filtration has been associated with cardiovascular disease (CVD). The aim of this study was to evaluate cystatin C as a marker of obese patients without chronic kidney disease (CKD). Materials and Methods: The study population consisted of 36 subjects with metabolic syndrome and 32 subjects free of metabolic syndrome (the control group). HDL-C, LDL-C, blood urea, triglycerides, glucose, HbA1c, serum cystatin C and serum creatinine were measured in both groups. GFR was calculated in both groups using Cockroft-Gault equation. Results: Obese patients showed higher cystatin C levels than normal samples ($1.28{\pm}0.29$, P < 0.05). In the binary logistic regression, obese patients were significantly associated with elevated cystatin C levels. Conclusion: Our results suggest that cystatin C may be a marker for obese patients and may identify a certain degree of renal dysfunction even when serum creatinine does not exceed the normal level. In this study, we demonstrated that serum creatinineand GFR did not differ significantly between the diabetic and the control groups. Serum concentration of cystatin C was significantly higher in the diabetic group compared with the control group. The strengths of this study are the evaluation of reliability and sensivity in comparison with a 'routine test of GFR'. The methodology used allows an appropriate statistical comparison of reliability in contrast to most other previous evaluations of GFR.

  • Purpose: We performed to reveal the association between the Helicobacter pylori infection and body weight among children. Methods: Out retrospective study included patients who underwent the H. pylori immunoglobulin G testing at Konyang University Hospital between March 2011 and June 2014. These patients were classified as seropositive (28 boys, 27 girls; mean age: $9.89{\pm}3.28years$) or seronegative (55 boys, 54 girls; mean age: $9.84{\pm}3.02years$). Next, we compared various characteristics between the seropositive and negative groups, as well as between obese children (body weight ${\geq}90th$ percentile) and non-obese children (body weight <90th percentile). Furthermore, we compared the change in body weight after 2 months of treatment with amoxicillin, clarithromycin and omeprazole among the 55 seropositive children (14 treated children and 41 non-treated children). Results: There were no differences in the weights and laboratory data for the 55 seropositive children and 109 seronegative children (weight; $40.96{\pm}18.11kg$ vs. $36.85{\pm}13.72kg$, respectively; p=0.14). And, there was no difference in the prevalence of H. pylori infection among the 29 obese and 135 non-obese children (p=0.581). However, after 2 months of eradication, the 14 treated patients exhibited a significant weight gain ($+0.91{\pm}0.52kg$), compared to the 41 non-treated patients ($-0.29{\pm}1.16kg$, p=0.025). Conclusion: Our findings present that obesity was not associated with the H. pylori infection, although H. pylori eradication led to significant increase in body weight.

  • Objectives: Obesity is known to influence physical and mental health as well as the general quality of life. The aim of this study was to evaluate the effect of obesity related quality of life on selecting a goal for weight management in overweight and obese female patients. Methods: A total of 140 overweight or obese (Body mass index $23kg/m^2$) female outpatients aged ${\geq}20$ and ${\leq}60$ years from one clinic participated in this study. Patients' desired weight (goal weight, ideal weight, satisfactory weight, acceptable weight and disappointed weight) and obesity related quality of life measures were evaluated. Univariate and multivariate analysis were performed to evaluate the effect of obesity related quality of life on goal weight reduction (%) and goal body mass index (BMI). Results: Mean BMI of overweight group, mild obesity group and severe obesity group were $62.0{\pm}4.8kg$, $68.5{\pm}5.5kg$ and $83.5{\pm}9.6kg$, respectively. Mean weight loss expectations of the three groups were $16.4{\pm}4.7%$, $19.5{\pm}5.3%$ and $30.2{\pm}6.8%$, respectively and goal weight was significantly different among the three groups. Severe obesity group had a lower total quality of life score including physical, work-related, daily living domains than overweight or mild obesity groups. In univariate and multivariate regression analysis, psychosocial domain of quality of life had an effect on goal weight reduction (%) and goal BMI. Conclusions: The results of this study demonstrated that the obese patients with poorer quality of life and psychosocial health tended to choose higher goal weight reduction and lower goal BMI.

  • Purpose: Obesity is risk factor for nonalcoholic fatty liver disease (NAFLD). However, nonobese patients are also increasingly susceptible to NAFLD. The aim of this study was to compare the clinical characteristics of obese and nonobese pediatric patients with NAFLD. Methods: We retrospectively studied 68 patients who were diagnosed with NAFLD between January 2010 and October 2016 at 10-18 years of age. Body mass index ${\geq}95th$ percentile for age and sex was defined as obesity. Abdominal ultrasonography and laboratory, anthropometrics measurements were evaluated. Results: Among the 68, 26 (38.2%) were nonobese patients. The ratio of male to female was 5.8:1, and the median age at diagnosis was 13 years (range, 10-17 years). Significant higher triglyceride (223.0 mg/dL vs. 145.9 mg/dL, P=0.047) and total cholesterol levels (211.6 mg/dL vs. 173.2 mg/dL, P=0.011) were shown in nonobese than obese patients. High-density lipoprotein cholesterol level <40 mg/dL (hazard ratio [HR], 6.5; 95% confidence interval [CI], 2.13-7.10; P=0.048), total cholesterol level >200 mg/dL (HR, 5.6; 95% CI, 1.23-15.31; P=0.038) and abdominal obesity (HR, 2.53; 95% CI, 1.22-4.68; P=0.013) were significant risk factors for NAFLD in nonobese patients. Conclusion: Nonobese patients present a substantial proportion of pediatric NAFLD cases. Significant abnormal lipid concentrations were found in nonobese and abdominal obesity was important risk factor for nonobese NAFLD.

  • Aim: To evaluate precancerous lesions such as hyperplasia and endometrial polyps in obese postmenopausal women. Materials and Methods: Women who were referred with abnormal uterine bleeding in postmenopausal period or the presence of endometrial cells on cervical cytology in our department were investigated. Anthropometric measurements such as height, weight, body mass index, waist/hip ratio and endometrial thickness were compared between a precancerous lesion (hyperplasia and endometrial polyp) group and a pathologically normal group. Results: We detected statistically significant thickening of endometrium in patients with precancerous lesions. Moreover patients with precancerous lesions had higher body mass index than the pathologically normal group. Conclusions: We found elevated precancerous lesion rates in overweight and obese women in the postmenopausal period, of interest given that the prevalence of obesity is increasing in most parts of the world. Although screening for endometrial cancer is not recommended for the general population, in high-risk populations like obese postmenopausal women, it may be very important.

  • The effects of Proprioceptive Neuromuscular Facilitation (PNF) and elastic band exercise on the physical functions and blood lipids of obese elderly women were investigated. The experimental group ($n_1=16$) patients underwent PNF for 12 weeks, and the control group ($n_2=15$) patients performed elastic band exercises. SPSS 21.0 was used to compute the means and standard deviations. After the 12-week PNF, both the experimental and control groups showed statistically significant differences in the physical functions (cardiovascular endurance, strength of the lower extremity, muscular endurance, flexibility, balance, and agility) (p<.05), but the difference in the experimental group was more significant than that in the control group (p<.05). In terms of the changes in the blood lipid levels (total cholesterol, triglycerides, high-density lipoprotein, and low-density lipoprotein), the experimental group showed significant changes (p<.05). In conclusion, PNF was confirmed as more effective than elastic band exercise in improving the physical functions and blood lipid levels of obese elderly women.

  • Background: Correlation between colorectal cancer (CRC) and abdominal obesity has been established, but there is a paucity of data on non-obese CRC patients. The aim of this study was to establish the characteristics of CRCs that occur in such patients. Materials and Methods: Consecutive CRC patients without cachexia were included. Unintended body weight loss, T4- or M1-staged CRCs, extensive lymph node involvement, or synchronous malignancy were classified as cachectic conditions. Abdominal fat volumes were measured using a multidetector CT unit with a software (Rapidia, INFINITT, Seoul, Korea). Results: Of the newly-diagnosed CRC patients, 258 non-cachectic and 88 cachectic patients were analyzed. The cancer size (p<0.001) and T stage (p<0.001) were inversely correlated with body mass index (BMI), visceral fat and subcutaneous fat volumes. Cancer size was the only independent factor related to BMI (p=0.016), visceral fat volume (p=0.002), and subcutaneous fat volume (p=0.027). In non-cachectic patients, a significant inverse correlation was found only between the cancer size and visceral fat volume (p=0.017). Conclusions: Non-obese CRC patients tend to have larger CRC lesions than their obese counterparts even under non-cachectic conditions. Such an inverse correlation between cancer size and visceral fat volume suggests that considerable CRCs are not correlated with abdominal obesity.
