Women's Cancer Screening According to Body Mass Index in a Cohort of Rural Korean Women (강원지역 농촌코호트 여성의 체질량 지수에 따른 여성암 검진행위)

  • Purpose: This study was done to examine the difference in cancer screening with mammography and Papanicolaou smear according to Body Mass Index (BMI). Methods: The participants in this study were 5,912 women ages 40 to 69 yr, selected from the Korean Genomic Regional Cohort in Kangwon province. Mammography and Papanicolaou smear were assessed by questionnaire and body weight (kg) and height (m) measured to calculate BMI. Results: The distribution of BMI was as follows: low weight (1.5%), normal weight (31.1%), over weight (24.6%), mildly obese (36.4%) and severely obese (6.3%). After adjusting for age, education and monthly income, compared with normal weight women, overweight women (odds ratio [OR]=1.283, 95% confidence interval [CI]=1.089-1.513) and mildly obese women (OR=1.214, 95% CI=1.048-1.406) were less likely to have had mammography. In contrast to mammography, cancer screening with Papanicolaou smear was not significantly different by BMI. Conclusion: Obese women in rural areas are less likely to screen for breast cancer by using mammography than non obese women. To ensure regular screening for breast cancer, health care providers need to give scrupulous care to obese women and remove barriers originated from obesity. Also, educational and clinical implications are considered to increase the Papanicolaou smear rate.

Effects of the 12 Weeks Weight Control Program on Improvement of BMI, Nutrition Knowledge, Physical Fitness and Stress Levels of Adult Obese Women (12주 체중조절 프로그램이 성인 여성의 BMI, 영양지식, 체력 및 스트레스 개선에 미치는 효과)

  • This study was conducted to investigate the effects of a weight control programs on BMI, nutrition knowledge, physical fitness and stress level of adult obese women. The subjects of this study were 36 adult obese women aged 30-65 years whose BMI were above $23\;kg/m^2$. The weight control program for obese women included nutritional education, exercise and cognitive behavioral therapy for 12 weeks. The BMI of the subjects was significantly decreased (p < 0.001) from $27.1\;kg/m^2$ to $26.4\;kg/m^2$ after the 12 weeks of the weight control program. In addition, the weight of body fat was significantly decreased (p < 0.001) and % body fat was significantly decreased (p < 0.05) from 35.4% to 34.9% after completion of the weight control program. In addition, nutritional knowledge (p < 0.001) and eating behavior scores (p < 0.01) were significantly increased. However, the food intake scores were not changed after the program. Stress levels of the adult obese women were significantly decreased (p < 0.001) from 85.63 to 66.88 and also obesity stress levels were significantly decreased (p < 0.01) after the weight control program. In the physical fitness levels of the subjects, muscle endurance (p < 0.05) and ability of balance (p < 0.01) were significantly improved after completion of the weight control program. There were positive significant correlations between general stress and obesity stress level (p < 0.05), and negative correlations between stress level and food intake score (p < 0.05). These results showed that the weight control program for adult obese women may be effectively used to improve weight reduction, physical fitness and stress levels.

A Study on Hematological Index and Health-Related Habit according to the BMI of Middle-Aged Men

  • This research analyzed the hematological index and health related habits such as alcohol drinking, smoking and exercise, by classifying 539 middle-aged men (age: 40 - 59) into a normal weight group, an overweight group and an obese group, according to BMI (body mass index). As a result, 33.6% (n = 181) of subjects had a BMI of 23 or less and 30.2% (n = 163) were overweight and 36.2% (n = 195) were obese (BMI > 25). The average systolic/diastolic blood pressures of total subjects were 130.96mmHg and 80.38mmHg, respectively. And the average pressure of normal subjects was 126.85/77.57mmHg, which was significantly lower than in overweight and obese subjects. Diastolic blood pressure shows significant differences among the three groups. The overall average cholesterol level of normal subjects was 193.81mg/dl, which was significantly lower than those in overweight subjects. Those in obese subjects were significantly higher than in overweight subjects. The GOT (glutamic oxaloacetic transaminase), GPT (glutamic pyrutic transaminase), ${\gamma}$-GTP (gamma-glutamyl transpeptidase) levels of obese subjects were significantly higher than in the overweight subjects. Frequency of alcohol drinking showed no significant difference among the groups. The smoking ratio in the obese group was higher than in the normal and overweight groups. The ratio of smokers was 53.1 %, and the alcohol drinking ratio was 81.0% of the total subjects. The frequency of drinking showed significant correlations with systolic blood pressure (p < 0.05) and diastolic blood pressures (p < 0.01) and ${\gamma}$-GTP (p < 0.01). The duration of smoking showed a negative correlation (p < 0.01) with diastolic blood pressure, total cholesterol and ${\gamma}$-GTP. On the other hand, exercising frequency showed negative correlations with blood glucose, ${\gamma}$-GTP and GPT(p < 0.01). These hematological indices were correlated with health related habits such as alcohol drinking, smoking and lack of exercise, and to be the cause of chronic diseases. Therefore, proper nutritional education is needed to establish advisable health related habits to maintain proper body weight. (J Community Nutrition 8(2): 63-68, 2006)

A Study on Body Mass Index and Associated Factors of the Middle Aged Women in Small City (중소도시 중년기 여성의 비만도 및 비만 관련변인 연구)

  • This study examined the state of health and weight maintenance according to the body mass index (BMI) , and explored demographic variables, diet variables, the degree of stress etc. The results presented were based on data collected from 428 women who are mothers of the middle school children in Jeongeup city. Using SPSS WIN (Ver 9.0) , the frequency, percentage, mean, standard deviation, chi-square, ANOVA, and correlation analysis were provided. The mean of BMI for the subjects was 22.6, the ratio of obese women (BMI $\geq$ 25) was 22.9%. The frequency of the reported circulation or respiratory related diseases was greater in the obese group compared to the normal weight ($20 \leq BMI \leq 25$) or the lean (BMI < 20) groups. The obese group had less awareness of obesity. Those who regard themselves obese had lower rates of satisfaction with their body shape and higher rates of interest in weight control. Approximately 65% of the subjects attempted weight reduction more than once. Subjects who were obese had the most undesirable dietary behavior. In particular, this group had the higher rates of rushed meals, overeating, meals while watching TV or newspapers, and snacking, and eating out. In analyzing the correlation of variables influencing obesity, there were positive correlations between obesity and overeating, obesity and the degree of stress. Correlations between obesity and satisfaction for life found to be negative. Moreover, stress had a positive correlation with obesity and overeating, and showed negative correlation with eating behavior. Consequently, stress seemed to induce undesirable eating behaviors and increase obesity. Of the demographic background variables, subjects who were obese tended to be older, had lower levels of education, higher rates of employment, longer period of marriage, the higher number of children, preschool children or children preparing for highschool or college, lower satisfaction with children and household life.

Dietary Behavior and Nutrition Quotient (NQ) Scores According to the Weight Status of Preschool Children in Gwangju Metropolitan City (광주광역시 유아의 체중별 식행동과 영양지수)

  • This study aimed to assess the association between the weight status and dietary behavior of preschool children. The survey included 412 preschool children aged 2-5 years in 31 childcare facilities in Gwangju. General characteristics of children and their mothers, weight, height, feeding type, complementary feeding, food allergy, and dietary behavior of children based on the Nutrition Quotient for preschoolers (NQ-P) questionnaire were considered. Children were classified into 4 groups by weight status; underweight, normal, overweight, and obese, based on the 2017 Korean National Growth Charts. The percentage of children in the respective groups was 5.3, 70.9, 10.7, 13.1%. Weight status was not related to breast feeding duration nor the starting time of complementary feeding. The NQ-P scores of underweight, normal, overweight, and obese groups were 55.4, 60.0, 60.8, 60.0. Among the three factors covered by the NQ-P, 'balance' and 'environment' scores of the underweight group were lower than that of other groups. Underweight children consumed beans, fish, and white milk less frequently. The 'moderation' scores of the overweight/obese group were lower than those of the normal or obese groups. Obese children more frequently ate processed meat and fast foods. Proper nutrition education is thus needed for not only obese children but also underweight children based on their age, along with their parents, and caregivers.

Effects of Resistance Exercise Training and High Protein Diet on Anabolic Factors of Skeletal Muscle in Sarcopenic Obese Rats (저항성 운동과 고단백식 혼합처치가 sarcopenic obese 쥐의 골격근 내 단백질 합성요인에 미치는 영향)

  • The purpose of this study was to investigate the effects of resistance exercise training and high-protein diet on anabolic factors and insulin resistance of skeletal muscle in sarcopenic obese rats. 50wks of male Sprague-Dawley rats were randomly assigned for 4 groups(Chow, HP, Ex, HPEx) after 6 weeks of high-rat diet induced obesity period. The 8-week of ladder climbing exercise significantly reduced body fat and insulin resistance, significantly increased mTOR activity. However hind limb muscles weight were not changed. When treat with exercise and high-protein diet, body fat and insulin resistance did not improve, but rather the effect of exercise training appeared to be inhibited. Therefore high protein diet for improving the sarcopenic obesity may be need more study about the amount and composition of protein.

Effects of Muscular Endurance Exercise and Supplement Intake on Appendicular Skeletal Muscle and Muscle Function-Related Physical Strength, Blood Lipids, and IGF-1 in Obese Older Adults with Sarcopenia (근감소성(sarcopenia) 비만 노인의 근지구력 운동과 보충제 섭취가 사지 근육량과 근기능 관련 체력과 혈중지질 및 IGF-1에 미치는 영향)

  • Purpose : This study aimed to introduce a 12-week muscular endurance exercise program and β-glucan consumption to obese elderly people with sarcopenia. We evaluated the program's impact on appendicular skeletal muscle strength and function, muscle function-related physical strength, blood lipids, and IGF-1. The results will serve as foundational data for preventing and improving sarcopenia. Methods : Forty elderly people aged 65 or older were recruited and underwent dual-energy X-ray absorptiometry. Based on criteria related to appendicular skeletal muscle mass (ASM/Height2: less than 5.4 kg/m2) and body fat percentage (at least 30% for women and 25% for men), we selected 24 obese elderly people with sarcopenia and excluded 56 who did not meet the criteria. Variables related to sarcopenia, blood lipids, IGF-1, and muscle function were measured before the 12-week muscular endurance exercise program. Results : In sarcopenic obese elderly participants, the 12-week muscular endurance exercise program significantly increased bone density and muscle mass while decreasing fat mass and percentage (p<.05). This program also improved grip strength, static balance, and SPPB in sarcopenic obese older adults. Furthermore, their 6-minute walk distance significantly increased (p<.05). Conclusion : Muscular endurance exercises combined with supplement intake can increase appendicular skeletal muscle and improve muscle strength and function, thereby improving the daily physical performance of the elderly.

Changes of Obesity Index, Serum Lipid Profiles and Nutrient Intakes in Obese Children after the Weight Control Program of Nutrition Education (체중조절 프로그램을 통한 비만 아동의 비만도, 혈중지질 및 영양소섭취량의 변화)

  • The purpose of this study was to evaluate the effect of a weight control program on anthropometric values, serum lipid profiles and nutrient intakes. The subjects of this study were 38 obese children (boy : 17, girl : 21) with obesity index over 120%. The weight control program for obese children included nutritional education (50 min) and exercise (50 min) for 10 weeks. The nutrition intakes of the children were surveyed before and after the weight control program by 24hr recall test. The BMI, R$\ddot{o}$hrer Index, Obesity Index, WHR (Waist - Hip ratio) and body fat(%) were significantly decreased after completion of the weight control program. The total cholesterol and HDL-cholesterol of subjects were significantly increased after the weight control program. Distribution of serum lipid profiles was slightly changed. The energy intakes were significantly decreased (p < 0.001). from 1760.8 kcal to 1435.2 kcal. In addition, the intakes of P, Zn, retinol, vitamin $B_1$, vitamin $B_2$, vitamin E and niacin were significantly decreased. while intakes of vitamin C and folate were increased. Calcium and folic acid were upper 25% of subjects, under EAR(Estimated Average Requirements) intake before the weight control program. The distribution of energy intakes was significantly changed into positive status; fat percentage was decreased 26.3% to 22.1% (p < 0.01). Carbohydrate was increased 58.6% to 61.2% (p < 0.05). Meal distribution of energy intakes was changed; calorie percentage from lunch significantly increased from 32.2% to 38.3%. Calorie percentage from snack significantly decreased from 17.7% to 13.5%. In conclusion, weight control program for 10 weeks is effective in obesity index and nutrient intakes although serum lipid values were a little changed.

Analysis of Anthropometric and Clinical Data in Obese Children in Changwon (창원지역 일부 비만아의 체격지수와 생화학적 지표에 관한 연구)

  • In this study, we investigated the effects of obesity index on the risk factors of chronic diseases in obese children. The subjects were male and female children aged 9 to 12. The average obesity index was 147.7$\pm$13.8% in boys and 147.6$\pm$16.5% in girls. The average percentages of body fat was significantly higher in boys(33.9$\pm$5.7%) than in girls(30.7$\pm$4.4%). It was found that 26% of subjects had abnormal serum GOT and GPT values. Percentages of subjects at risk of cardiovascular disease(CVD) based on corresponding criteria of TG, T chol, HDL chol, and LDL chol were 28.6%, 28.6%, 45.5% and 42.7%, respectively. The risk of CVD was increased with the degree of obesity, which was not significant. The AI(atherogenic index) was higher in boys(4.6$\pm$2.3) than in girls(3.6$\pm$1.2). The serum T chol and LDL chol levels of subjects whose mother are in thirties were significantly higher than those of subjects whose mother are in fourties. The T chol levels of subjects who had mother with job were significantly higher compaired to those of subjects who had housewife mother. The results indicated the urgent need of nu tritional management for the obese children to prevent further devel opment of childhood obesity and hyperlipidemia. Based upon this study, devising method and media for the mother nutritional education as well as for the children nutritional education is required for better growth and health promotion of primary school children.

Application of the Transtheoretical Model to Obese Adolescents' Exercise Behavior Change Stages (범이론적 모형을 적용한 비만 청소년의 운동행위변화단계 연구)

  • The Purpose of this study was to identify the process of change, decisional balance and self-efficacy corresponding to the stages of change in exercise behavior based on the Transtheoretical Model(TTM) in obese adolescents. A random sample of 517 obese adolescents was recruited in Pusan. The data were collected from June 15 to 30, 2004. The research instruments were Stages of Change for Exercise Behavior, Process of Change, Decisional Balance, and Self-Efficacy. The data were analyzed by descriptive statistics and ANOVA using SPSS WIN 10.0 program. The results of this study were as follows 1. The subjects were distributed in each stage of exercise behavior: There were 255 subjects($49.3\%$) in the contemplation stage, 131 subjects($25.3\%$) in the preparation stage, 52 subjects($10.1\%$) in the maintenance stage, 41 subjects($7.9\%$) in the action stage, and 38 subjects($7.4\%$) in the precontemplation stage. 2. Analysis of variance showed that cognitive process (F=13.57, p=.000), behavioral process(F=17.23, p=.000), decisional balance pros(F=11.95, p=.000), and self efficacy(F=12.52, p=.000) were significantly associated with the stages of exercise behavior change, but decisional balance cons(F=2.87, p=.023) was not significantly associated with the stages of exercise behavior change. This study can provide useful information for developing effective exercise behavior program considering obese adolescents' stages of change according t'.t the TTM.
