전문간호사의 전문직업성, 임상적 의사결정능력 및 간호업무성과의 관계 (The Relationship between Nursing Professionalism, Clinical Decision Making Abilities, and Job Performance in Advanced Practice Nurses)

  • Purpose: To identify the extents of nursing professionalism, clinical decision making abilities and job performance of advanced practice nurses and investigate the relationship among the variables Methods: Participants, selected by a convenience sampling method, were 135 advanced practice nurses working in 4 tertiary care general hospitals in 'B' Metropolitan City and in 1 tertiary care general hospital in 'J' city. Data collection was done from January 15 to February 28, 2013 using self-reporting questionnaires. Results: The average score for nursing professionalism of study participants was $81.19{\pm}7.56$. for clinical decision making abilities, $140.42{\pm}9.62$ and for job performance, $102.54{\pm}10.30$. These averages are relatively high. The relationship between the extent of nursing professionalism and the extent of clinical decision making abilities showed an intermediate level positive correlation (r=.45, p<.001). The relationship between the extent of nursing professionalism and the extent of job performance was also an intermediate level positive correlation (r=.42, p<.001). The extent of clinical decision making abilities and the extent of job performance was an intermediate level positive correlation (r=.41, p<.001). Conclusion: Developing a program, which can improve nursing professionalism and clinical decision making abilities of nurse, is required to enhance their job performance.

간호사의 특성분노 및 분노표현이 직무만족, 조직몰입, 조직성과에 미치는 영향 (The Relationships among Trait Anger, Anger Expression, Jab Satisfaction, Job Commitment, and Organizational Performance in Clinical Nurses)

  • Purpose: The purpose of this study was to identify the relationships among trait anger, anger expression, job satisfaction, job commitment, and organizational performance, and especially to verify the effect of anger experience on organizational performance. Method: A descriptive study was conducted using a convenient sample. Data were collected by a self-report questionnaire from 671 clinical nurses in three university hospitals and analyzed by using SPSS 14.0 Win program with descriptive statistics, t-test, ANOVA, pearson correlation coefficient, and stepwise multiple regression. Result: Organizational performance was significantly correlated with trait anger, anger expression-in, anger expression-out, anger control, job satisfaction, and job commitment. Multiple regression analysis showed that organizational performance was significantly predicted by job commitment, job satisfaction, educational level (university graduation), anger control, and working place (operative room), and 34.7% was explained by these variables. Conclusions: Out of anger expression, anger control significantly affected on organizational performance together with job commitment and job satisfaction. From this result, it could be concluded that nursing intervention for anger control have a positive effect on improving organizational performance. Further studies are needed to develop and test the effect of nursing intervention programs for anger control.

임상간호사의 셀프리더십과 간호조직문화가 간호업무성과에 미치는 영향 (Effects of Clinical Nurses' Self-leadership and Nursing Organizational Culture on Nursing Performance)

  • 본 연구의 목적은 임상간호사의 셀프리더십, 간호조직문화 및 간호업무성과의 관계를 파악하고 간호업무성과에 미치는 영향요인을 분석하고자 함이다. A광역시 H병원에서 근무 중인 242명의 간호사를 대상으로 자료수집을 시행하였다. 자료수집 기간은 2016년 10월 17일부터 23일까지였다. 자료분석 방법은 SPSS 21.0 프로그램을 이용하여 분석하였다. 분석결과 간호업무성과와 통계적으로 유의한 차이가 있는 인구사회학적 특성은 연령, 결혼상태, 교육수준, 직위, 임상경력, 현 부서 임상경력이었다. 위계적 다중회귀분석결과, 간호업무성과에 미치는 영향요인으로 현 부서 임상경력과 셀프리더십 중 자기기대요인으로 나타났으며 모형의 설명력은 35.7%였다. 간호업무성과를 향상시키기 위해서는 현 부서 임상경력을 높이도록 하고 간호사의 자기기대를 향상시키기 위한 교육프로그램이 필요하겠다.

간호 대학생의 의료관련감염 관리에 대한 표준주의 인지도와 표준주의 수행도: 내적 건강통제위의 매개효과 (Awareness and Performance on Standard Precautions of Hospital-acquired Infection Control in Nursing Students: The Mediating Effect of Internal Health Locus of Control)

  • Purpose: The aim of this study was to identify the status of performance on standard precautions among nursing students and to examine the mediating effects of internal health locus of control on the relationship between awareness and performance on standard precautions of hospital-acquired infection control. Methods: The participants in this study were 134 nursing students. The measurements included a standard precautions awareness and performance scale, and a multidimensional health locus of control scale. Data were analyzed using independent t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficient, and simple and multiple regression techniques. Mediation analysis was performed by the Baron and Kenny's method and Sobel test. Results: The mean score of standard awareness, standard performance, and internal health locus of control about standard precaution were $174.30{\pm}9.08$; $169.48{\pm}12.04$; and $20.43{\pm}2.82$; respectively. There was a positive correlation between standard awareness and performance (r=.414, p<.001). Also, standard awareness was significantly correlated with internal health locus of control (r=.413, p=.014). Internal health locus of control showed partial mediating effects in the relationship between awareness and performance. Conclusion: The results indicate a need to improve the internal health locus of control of nursing students. Therefore, an internal health locus of control improvement program should improve performance on standard precautions for patients and themselves.

COVID-19로 인한 온라인 중심 교육에서 간호대학생의 핵심 기본 간호술 수행 자신감에 영향을 미치는 요인 (Factors Affecting the Confidence of Nursing Students in the On-line-Based Education by COVID-19)

  • 본 연구는 COVID-19로 인한 온라인 중심 교육에서 간호대학생의 핵심 기본 간호술 수행자신감 향상을 위한 교수법 개발을 위한 기초자료를 제공하고자 시도되었다. C시에 소재한 N대학 간호학과 146명이 연구에 참여하였다. 수집된 자료는 SPSS 23.0 통계 프로그램을 사용하여 기술통계, t-test, ANOVA 및 Duncan test를 활용한 차이 분석, 상관관계분석 및 다중회귀분석을 시행하였다. 연구 결과, 대상자의 핵심 기본 간호술 수행 자신감은 자기 주도 학습준비(r=.368, p<.001), 학습 동기의 하위영역인 내재적 동기(r=.232, p=.005), 외재적 동기(r=.344, p<.001), 과제 가치(r=.237, p=<.001), 학습통제신념(r=.262, p=<.001), 자기효능감(r=.443, p<.001)과 유의한 양의 상관관계가 있었다. 간호대학생의 핵심 기본 간호술 수행자신감 영향 요인으로는, 4학년(β=0.413, p<.001), 학습 동기의 하위 영역 중 외재적 동기(β=0.307, p<.001), 자기효능감(β=0.316, p=.005)이었으며, 설명력은 35.8%였다(F=8.354, p<.001). 본 연구결과를 토대로 핵심 기본 간호술 수행자신감 증진을 위해서 간호대학생의 외재적 동기, 자기효능감을 높일 수 있는 다양한 교수법의 개발 및 적용이 필요하며, 이는 핵심 기본 간호술 수행자신감 향상을 위한 효율적인 온라인 중심의 간호교육 전략 마련에 기초자료가 될 것이다.

재가간호서비스 제공자의 업무 수행 현황과 장애요인 (Current Status of Home Visit Programs: Activities and Barriers of Home Care Nursing Services)

  • Purpose: The purpose of this study was to examine the current status of home care nursing services provided by community health nurses and to identify barriers to the services. Methods: A cross-sectional survey was conducted with three types of community health care nurses. Participants were 257 nurses, 46 of whom were hospital based home care nurses, 176 were community based visiting nurses, and 35 were long term care insurance based visiting nurses. A structured questionnaire on 7 domains of home care nursing services with a 4-point Likert scale was used to measure activities and barriers to care. Data were analyzed using SPSS WIN 21.0 program. Results: Hospital based home care nurses showed a high level of service performance activity in the domain of clinical laboratory tests, medications and injections, therapeutic nursing, and education. Community based visiting nurses had a high level of service performance in the reference domain. Long term care insurance based visiting nurses showed a high level of performance in the service domains of fundamental nursing and counseling. Conclusion: The results show that although health care service provided by the three types of community health nurse overlapped, the focus of the service is differentiated. Therefore, these results suggest that existing home care services will need to be utilized efficiently in the development of a new nursing care service for patients living in the community after hospital discharge.

한국형 간호그룹파워 도구의 타당성 재검증 및 직위별 간호그룹파워와 직무만족 조직몰입, 조직성과와의 관계 (Validity Test of K-SKAGOAO for Korean Nursing Groups and the Relationship of Nursing Group Power and Job Satisfaction, Organizational Commitment, Organizational Performance)

  • Purpose: The purpose of this study was to test validity and reliability of the Korean-Sieloff King Assessment of Group Outcome Attainment within organization in Korea (K-SKAGOAO), and also to explore the relationship of Korean nursing group power and job satisfaction, organizational commitment, organizational performance. Methods: Participants were 481 nurse managers, nurse unit managers or staff nurses with over 7 years experience. Data were collected by post mail and analyzed using descriptive statistics, ANOVA, Kruskal-Wallis test, Tukey test, and Spearman's correlation. Results: Content Validity Index was over .82 and Cronbach's ${\alpha}$ coefficient for the K-SKAGOAO was .97. The mean score for Korean nursing group power was 136.77 among the total participants, 139.89 among nurse managers, 136.74 among nurse unit managers and 132.47 among staff nurses. These results showed high nursing group power in Korea nursing organization in hospitals. There was also a significant difference according to nurse position (p=.003). Korean nursing group power was positively related to job satisfaction, organizational commitment, and organizational performance. Conclusion: Findings show that the K-SKAGOAO has validity and reliability. Nursing groups can use the K-SKAGOAO to evaluate the nursing group power of each nursing organization and to develop strategies to improve nursing power and nursing outcomes.

균형성과표를 이용한 중환자실 간호부서의 핵심성과지표 개발 (Development of Nursing Key Performance Indicators for an Intensive Care Unit by using a Balanced Scorecard)

  • Purpose: The purpose of this study was to develop visions of nursing service, nursing strategies and key performance indicators (KPIs) for an intensive care unit (ICU) based on a Balanced Scorecard (BSC). Methods: This study was undertaken by using methodological research. The development process consisted of four phases; the first phase was to develop the vision of nursing in ICUs. The second phase was to develop strategies according to 4 perspectives of a BSC. The third phase was to develop KPIs according to the 4 perspectives of BSC and the final phase was to combine the nursing visions, strategies and KPIs of ICUs. Results: Two main visions of nursing service for ICUs were established. These were 'realization of harmonized professional nursing with human respect' and 'recovery of health through specialized nursing' respectively. In order to reach the aim of developing nursing visions, thirteen practical strategies and nineteen KPIs were developed by four perspectives of the BSC. Conclusion: The results will be used as objective fundamental data to attain business outcomes for the achievement of nursing visions and strategies of ICUs.

A Practical Performance Comparison of Nurse between Comprehensive Nursing Care Service Ward and General Service Ward

  • 본 연구는 간호간병통합병동과 일반병동에 근무하는 간호사의 업무수행능력 차이를 확인하기 위한 서술적 조사연구이다. D광역시의 S병원에 근무하는 간호사 185명을 대상으로 자료를 수집하여 간호전문직관은 윤은자 등(2005)이 개발한 간호전문직관으로, 업무수행능력 정도는 이원희 등(1990)이 개발한 임상수행능력 측정도구를 이용하여 측정하였다. 자료는 SPSS 20.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Duncan test, Pearson's correlation coefficients, multiple regression을 이용하여 분석하였다. 본 연구결과 간호간병통합병동 간호사가 일반병동 간호사보다 간호전문직관이 높게 나타났고, 간호간병통합병동과 일반병동의 간호사 모두 간호전문직관이 업무수행능력 향상에 영향을 미치고 있음을 확인할 수 있었다. 그러므로 향후 간호사의 간호전문직관을 정립하는 방안을 모색하여 간호업무의 수행능력을 향상시키는 방안으로 활용할 수 있을 것이다.

신생아중환자실 간호사가 인식한 간호업무의 중요도와 수행 자신감 (Analysis of the importance of nursing care and performance confidence perceived by nurses in the neonatal intensive care unit)

  • Purpose: Neonatal nurses are expected to have clinical competency to provide qualified and safe care for high-risk infants. An educational intervention to enhance nurses' clinical competence is often a priority in the nursing field. This study was conducted to explore nurses' perceived importance and performance confidence of nursing care activities in neonatal intensive care units. Methods: One hundred forty-one neonatal nurses from seven hospitals across South Korea participated in the online survey study. The scale of neonatal nursing care activity consisted of 8 subdomains including professional practice (assessment, diagnosis, planning, intervention, evaluation, education, research, and leadership). The Importance-Performance Matrix was used to analyze the importance of and confident performance in each of the nursing subdomains. Results: Both importance and performance confidence increased as nurses' age (p=.042 and p<.001) and clinical experience (p=.004 and p<.001). Participants scored relatively higher in importance and performance confidence in the professional practice subdomains (assessment, intervention, evaluation), but scored lower in the education and research subdomains. Conclusion: To provide evidence-based nursing care for high-risk infants in neonatal intensive care units, educational interventions should be developed to support nurses based on the findings of the research.
