The Relationships of Pain cognition, Performance Status, and Hope with Health-related Quality of Life in Cancer Patients (암환자의 통증인지, 기능상태 및 희망과 건강관련 삶의 질의 관계)

  • Purpose: The purpose of this study was to determine the relationships of pain cognition, performance status, and hope with health-related quality of life. Methods: Patients(n=149) with various cancer diagnoses completed the SF-36 standard Korean Version and the Herth Hope Index. The Perceived Meanings of Cancer Pain Inventory was used to measure the cognition dimension of pain, whereas the Brief Pain Inventory Korean version was used to represent the sensory dimension of pain. Results: The patients in the pain group had significant differences in the three dimensions(loss, threat, spiritual awareness) of pain cognition. There were statistically significant negative correlations between the three dimensions(loss, threat, and spiritual awareness) of pain cognitions and SF-36 dimension, and the positive correlations between challenge dimension and SF-36 dimension. Hope had the positive correlation with SF-36 dimensions. Conclusion: Pain has a negative impact on health-related quality of life, especially on physical health. However, patients who ascribed more positive meaning to their pain, tended to have a higher quality of life. Therefore, nursing intervention to reinforce the positive aspects of pain cognition is to empower patients to create a sense of control and assume an active role in pain management and quality of life.

Content Analysis of Experiences of Chest Pain in Women Who Underwent the Treadmill Test with an Impression of Angina Pectoris (협심증이 의심되어 운동부하검사를 받은 여성들의 흉통에 관한 내용분석)

  • Purpose: Experience of chest pain may be different in women from that of men and this discrepancy may cause misdiagnosis of angina contribute to mortality by this disease in women. The purpose of this study was to analyze the characteristics of chest pain and responses to the symptoms in women. Method: A content analysis was conducted with interviewed data obtained from women who underwent the treadmill test Result: Seven major categories were identified from this content analysis: attributes of chest pain; accompanied symptoms; precipitating factors; relief strategies; family support; communication with physicians; and effects of chest pain on their lives. Characteristics of pain were described as heaviness, tightness, heating sensation, tearing, and others. Duration and intensity of pain varied in a wide range. Radiating pain presented in 9 patients, and the locations of radiation were throat, neck, shoulder arm and fingers. Women tended not to respond actively to their chest pain, and didn't get appropriate support either from their family or from their physicians when they reported chest pain. Conclusion: Women express non-typical as well as typical patterns of pain when they experience chest pain. Clinicians have to consider the variability of symptoms when they assess women with suspicions of angina.

The Effects of Aroma Inhalation on Sleep and Fatigue in Night Shift Nurses (아로마 흡입이 밤번근무 간호사의 수면과 피로에 미치는 효과)

  • Purpose: This study was conducted to describe the effects of aroma inhalation on sleep and fatigue in night shift nurses. Methods: The research design was one group pre-posttest design. The subjects were 17 night shift nurses who work at neurosurgery intensive care unit of K hospital of C university in Seoul. During three consequent night shifts, the participants were worn the Actigraph to measure the sleep pattern on their wrist since their first night shift work. The quality of sleep and fatigue was measured by survey when they awoke at 6 pm after second night shift work. And these measures were repeated during the night shifts in second and third week. Aroma inhalation was applied before going to bed after finishing each night shift work during second and third week. SPSS 12.0 program was used for data analysis. Results: There were no statistically significant differences in sleep latency and total sleeping time. However, significant differences were found in the sleep efficiency, awakening number, sleep quality, subjective symptom and intensity of fatigue. Conclusion: These results showed that aroma inhalation had positive effects on the pattern and quality of sleep and reducing fatigue in night shift nurses.

  • Purpose: The purpose of this study was to examine the effects of preoperative pain management education on postoperative pain control in patients with uterine tumor using patient controlled analgesia. Methods: This study used non-equivalent control group non-synchronized design. Data were collected from September, 2008 to March, 2009 at one university hospital in Daegu, Korea. There were 60 participants, 30 in both the experimental and control group. The experimental group was given preoperative pain education using videos, leaflets, and a PCA model. Postoperative pain intensity, frequency of the PCA button being pressed, and doses of additional analgesics were observed through 24 hours postoperative and knowledge of pain and attitude about the use of the pain medicine were measured at 3 days postoperative. Collected data were analyzed using t-test, ${\chi}^2$ test, repeat measured ANOVA, and Bonferroni methods. Results: There were no significant differences in the postoperative pain level between the experimental and control group. Postoperative frequency of the PCA button pressed, doses of additional analgesics, pain knowledge and attitude about the use of the pain medicine of the experimental group were significantly higher than those of the control group. Conclusion: Pain management education is an effective nursing intervention for pain control after surgery.

Effects of Physical Activity and Melatonin in a Rat Model of Depression Induced by Chronic Stress (자유로운 신체운동과 멜라토닌이 우울장애 동물모델에 미치는 효과)

  • Purpose: Stress, depending on its intensity and duration, results in either adaptive or maladaptive physiological and psychological changes in humans. Also, it was found that stressful experiences increase the signs of behavioral despair in rodents. On the other hand, exercise and melatonin treatment is believed to have many beneficial effects on health. Thus, this study was designed to evaluate the anti-depressant effects of physical activity and melatonin against chronic stress-induced depression in rats. Methods: Adult male Sprague-Dawley(SD) rats(200-250g, 7 weeks of age) were subjected to depression induced by chronic stress. Chronic depression was induced with forced-swim stress (FSS) and repeated change of light-dark cycle for 4 weeks. In the last 2 weeks, some rats were confined in a cage enriched with a running wheel, seesaw and chewed a ball from 19:00 to 07:00 every day. Melatonin was injected intra-peritoneally (I.P), and the rats received intraperitoneal injections of melatonin (15 mg/kg). The Forced Swim Test (FST) was performed to evaluate the immobility behaviors of rats for a 5 min test. Results: It was found that, the immobility time in FST was significantly (p<.05) lower in physical exercise ($M=58.83{\pm}22.73$) and melatonin ($M=67.33{\pm}37.73$) than in depressive rats ($M=145.93{\pm}63.16$) without physical activity. Also, TPH positive cell in dorsal raphe was significantly (p<.05) higher in exercise ($M=457.38{\pm}103.21$) and melatonin ($M=425.38{\pm}111.56$) than in depressive rats ($M=258.25{\pm}89.13$). Conclusion: This study suggests that physical activity and melatonin produces antidepressant-like effect on stress-induced depression in rats. So, physical exercise and melatonin may be a good intervention in depression patients.

Factors influencing health-related quality of life for young single-person households: the mediating effect of resilience (청년 1인 가구의 건강 관련 삶의 질 영향요인: 회복탄력성의 매개효과를 중심으로)

  • Purpose: To identify factors influencing health-related quality of life for young single-person households, this study investigated physical and mental health status, health behavior, depression, resilience, and health-related quality of life. Methods: An online survey was administered to members of young single-person households from March 22 to 30, 2022. The data were analyzed using the chi-square test, independent t-test, one-way analysis of variance, Pearson correlation coefficients, multiple regression, and a simple mediation model applying the PROCESS macro model 4 with 95% bias-corrected bootstrapped confidence intervals. Results: The participants were 229 members of young single-person households. Health-related quality of life showed significant relationships with residence (t = 2.80, p = .006), month (F = 3.70, p = .026), mental health status (F = 20.33, p < . 001), and high-intensity exercise (F = 7.35, p = .001) among general and health-related characteristics. Health-related quality of life had significant correlations with depression (r = -.72, p < .001) and resilience (r = .58, p < .001). Multiple regression analysis showed that depression (β = -.57, p < .001) and resilience (β = .21, p < .001) influenced health-related quality of life. Moreover, resilience had a mediating effect between depression and health-related quality of life (indirect effect = -0.002, 95% bias-corrected bootstrapped confidence interval = -0.003 to -0.001). Conclusion: Members of young single-person households tended to be more vulnerable to emergency situations, such as during the coronavirus disease 2019 pandemic, when lockdowns and quarantines were frequent. To improve health-related quality of life in young single-person households, people with high levels of depression or low levels of resilience need special attention and support to promote mental health.

Effectiveness of exercise for improving physical and renal function in older adults with pre-dialysis chronic kidney disease: A systematic review and meta-analysis (운동중재가 투석 전단계 만성 신질환 노인의 신체기능 및 신기능 향상에 미치는 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose : Exercise may prevent the worsening of chronic kidney disease (CKD) and progression of cardiovascular diseases in patients with CKD. This review aims to identify the best type of exercise modality and summarizes the beneficial effects of exercise on physical and renal function among older adults with pre-dialysis CKD. Methods : A systematic search of PubMed, Embase, CINAHL, Cochrane Library, Web of Science, SCOPUS, and domestic database was performed for randomized controlled trials (RCTs) assessing the effect of exercise intervention on older adults with pre-dialysis CKD published until February 2023. A random-effects meta-analysis was conducted. The risk of bias was assessed using a Cochrane tool for assessing the risk of bias in RCTs (RoB 2.0). Results : The systematic review included 11 RCTs (n = 591, average age 60.2-76), of which 8 could be included for meta-analysis. Exercise was significant in increasing peak oxygen consumption and knee muscle strength among physical functions, and also in improving glomerular filtration rate among kidney functions. Conclusion : Exercise has beneficial effects on physical and renal function among older adults with pre-dialysis CKD. In the future, it is necessary to verify the effectiveness of exercise by subdividing it by type, intensity, duration, and delivery.

A Study on the Obesity Index and the Physical Activities of Elementary School Children (초등학교 아동의 비만지수와 신체활동에 관한 조사연구)

  • This study researches an obesity index and the realities of physical activity of elementary school children and provide basic data to instruct correct physical activity for low weight and obese children. The subject was 813 Elementary School Children of grade 4-6 in Kyungju and data were collected with a Physical Activity Questionnaire from YHS, USA. The collected data were analyzed with real number, percentage, average and standard deviation techniques. The range was by SPSS WIN. Both of them were in the normal range, between -38.3 and 77.7, between low weight and obesity. The results are as follows : 1. An Obesity index according to grade and sex: the average obesity index of 423 boys was $7.56{\pm}15.7$, for girls it was $2.56{\pm}14.3$. Both of them were in the normal range, between -38.3 and 77.7. There were low weight($-38.85{\pm}0.9$) students as well as obese, including extremely low weight children ($-38.85{\pm}0.9$) and extremely obese children. 2. On the whole, in the Frequency of the 25 item physical activities breakdown, children reported to watch TV 1. ($2.7{\pm}0.6$), or do homework($2.1{\pm}0.9$), more than three-fourths of the week, male children, however, played more football($2.0{\pm}0.9$). As male obese children do less MET5, MET8(highly active activity) than male normal children, it was also discovered that low weight children do more severe activity in MET8(high activity). In female cases, it was revealed that neither do highly intense activity. 3. In physical activity, male children received higher points than female children with $92.94{\pm}37.95$ to $74.46{\pm}29.50$. On the whole, male and female obese children had lower score than normal children. 4. As for attitude toward physical activity, male children($31.96{\pm}4.37$) had a positive attitude than female children ($29.57{\pm}3.89$), but obese male children had a negative attitude regarding activity viewed as normal. Therefore, in preventing obesity in elementary school children the treating of under-weight children should be included in the obesity program so as to prevent mal-nutrition or deficiency. It is needed to instruct obese children to carry out intensive work-outs at least 3 times a week through systematic programs. It is also needed that parents and teachers educate elementary school children from an early age with information on the frequency and intensity of work-outs to prevent obesity. It is also desirable that children be educated to carry out physical activities that address their specific needs. As a result of this study, obesity management must be started from elementary school and children should be instructed to act in high-intensity actirities when they spend leisure time.

Nursing Method with Polypropylene Spunbonded Fabric in Rice

  • An extensive investigation has been made for the possible utilization of polypropylene spunbonded fabrics in rice seedling nursery. Considering the cropping systems available in southern part of Korea, sowing dates were fixed at April 5 and April 20 for single cropping and May 6 and May 26 for double cropping. Nursery period was fixed to 35 days for each sowing date. Four different thickness of polypropylene spunbonded fabrics, 40, 60, 80, 100 g/$m^2$, were tested in rice seedling nursery. The temperature and light intensity were not significantly different among the thickness of polypropylene spunbonded fabrics. Light intensity was significantly reduced in polypropylene spunbonded fabric (72.2 Klux) compared with polyethylene mm (85.5 Klux), however, the reduced light intensity was enough for seedling growth. The temperature in the polypropylene spunbonded fabric covering during low air temperature was higher than that in polyethylene mm tunnel. At transplanting, the rice seedlings grown in polypropylene spunbonded fabric condition was shorter (17cm) but healthier than those in polyethylene mm (23cm). The estimated possible nursery periods using the polypropylene spunbonded fabric covering may start from April 1 at Chinju (plain area in Southern Korea, 20m altitude) and April 15 at Susang (mountainous area, 430m altitude). Labour hours and cost were reduced by about 28% and 48%, respectively.

Improved Heat Lamp for Preventing Arterial Spasm after Microvascular Surgery (미세혈관 수술 후 혈관 연축의 예방을 위한 개선된 온열전등)

  • Purpose: Heat therapy by heat lamp after microvascular surgery is being used for preventing blood vessels's contraction and blood-flow's disturbance. As usually, incandescent lamp has been used. But there have been several problems and need for improvement in the existing heat lamp treatment. So we would like to introduce improved heat lamp to keep an appropriate temperature and intensity of illumination. Methods: The existing heat lamps are the ones of general light stands covered with newspaper, having 60 watt light bulb of incandescence and lampshade made of aluminum. We have tried to improve shortcomings of the existing heat lamps by enlarging the size of aluminum lampshade and attaching a curtain that can block heat and light. We conducted a comparative study between the existing and improved heat lamps. Under the assumption that there are several affected parts, we have also measured the distance from heat lamp to patients' eye region and then intensity of illumination. Result: The target temperature of surface was realized in 11 minutes with the maximum temperature reaching at 36.6 degrees C in 28 minutes at the existing heat lamp while the target temperature reached in 7 minutes with the maximum temperature reaching at 39.0 degrees C in 17 minutes at the improved heat lamp. The existing and improved heat lamp showed 38 lx and 0.1 lx of intensity of lumination, respectively. Conclusion: Using improved heat lamps, we can keep an appropriate temperature and we think we can make contribution to patients' treatment by making them and their neighbors able to sleep with minimized disturbance thanks to low intensity of illumination secured by blocking light.
