Validity and Reliability of ARQ-K (Korean Version of the Assault Response Questionnaire) for Emergency Department Nurses in Korea (한국형 폭력 반응 측정도구의 타당도 및 신뢰도 검증: 응급실 간호사 대상)

  • Purpose: To investigate the validity and reliability of the Korean Version of the Assault Response Questionnaire (ARQ-K) measuring the intensity of reaction to victimization of emergency nurses in Korea. Methods: An internal consistency reliability and construct validity using exploratory and confirmatory factor analysis were conducted using SPSS WIN (20.0) and AMOS (20.0). Survey data were collected from 321 nurses who worked in 3 levels - wide regional emergency centers, regional emergency centers, appointed emergency centers - of emergency care facilities in Busan, Korea. Results: The Cronbach's alpha values regarding internal consistency were .77~.93 for the subscales of ARQ-K. Factor loadings of the 26 items on the four subscales ranged from .59 to .84. The four-subscale model was validated by confirmatory factor analysis (${\chi}^2/df=3.85$, p<.001, RMR=.06, GFI=.80, NFI=.81, TLI=.83, CFI=.85, RMSEA=.09). Conclusion: This study shows that the Korean Version of the Assault Response Questionnaire is a valid and reliable instrument to assess nurses' reaction to victimization of emergency nurses in Korea.

Smoking Behavior and Predictors of Smoking Initiation in Childhood and Early Adolescence (학령기 및 청소년 초기 흡연행태와 흡연시작에 영향을 주는 요인)

  • Purpose: The purposes of this study were(a) to identify smoking behavior by following a cohort representative of the population of 4th grade elementary schoolers in South Korea over a four-year period(2004-2007), and(b) to explore predictors of smoking initiation among non-smokers in Wave 1. Methods: Secondary data, the Korea Youth Panel Study, was analyzed in this study. First, frequencies or percentages were calculated to identify smoking behavior(i.e., smoking initiation, smoking intensity, and smoking duration). Second, binary logistic regression analysis was performed to examine significant factors related to smoking initiation. Results: Smoking initiation and daily smoking were more pronounced when the participants entered middle school. In bivariate analysis, statistically significant predictors of smoking initiation were loneliness at school, self-control, delinquent behavior, depressive symptoms, and stress. However, after controlling for other factors, only a high level of risk-taking tendency and a greater number of delinquent behaviors remained statistically significant. Conclusion: Based on greater involvement in smoking among first-year middle schoolers, smoking prevention strategies should be provided to elementary schoolers rather than middle schoolers. A risk-taking tendency and delinquent behaviors should be considered as proxy measures to detect the high-risk group for smoking initiation.

The Effects of Aromatherapy Massage on Pain, Sleep, and Stride Length in the Elderly with Knee Osteoarthritis (아로마 마사지요법이 퇴행성 슬관절염 노인의 통증, 수면 및 활보장에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of aromatherapy massage in the elderly with knee osteoarthritis. Methods: Participants were assigned randomly to an intervention (n=21) group and a control (n=21) group. In order to increase a lasting effect of aromatherapy massage, an essential oil used in the form of cream. Each participant had aromatherapy massage on lower legs for 20 minutes each time twice a week for four weeks. Numerical rating scale was used for self-report of pain intensity. Sleep disturbance was measured by Korean Sleep Scale consisting of 15 items. Stride length had been obtained by measuring the distance from the heel of one foot to the heel of the other foot. Results: There was a significant difference between the pain scores of the experimental group and that of control group after the aromatherapy massage (p=.001). Whereas, no significant differences between sleep scores of the two groups (p=.592) was found. The experimental group's stride length significantly increased than those of the control group (p=.009). Conclusion: Aromatherapy massage could be recommended as an effective intervention to decease pain and to increase stride length in the elderly with knee osteoarthritis.

A Meta-Analysis of Intervention Studies on Nausea and Vomiting in Cancer Patients (국내 암환자의 오심구토에 적용한 비약물적 중재효과의 메타분석)

  • Purpose: This study was designed to analyze the characteristics and effect size of published intervention studies related to nausea & vomiting among cancer patients. Methods: A total of 1,083 studies were retrieved. From these studies, 20 studies met the inclusion criteria with a total of 698 participants. Two authors independently extracted data from the selected studies and assessed the methodological quality. The data were analyzed by the RevMan 5.0 program of Cochrane library. Results: The twenty studies utilized various non-pharmacological interventions: Nine studies on acupuncture (45%), Five studies utilized massage (25%), Two studies used oral cryotherapy (10%) and Four studies used other therapies. In the twenty studies the effect size of the intervention studies reported a higher effect sizes for massage (d=-1.62) and acupuncture (d=-0.89). Conclusion: This study suggests that non-drug therapy can reduce the levels of nausea and vomiting intensity. Massage and acupuncture interventions studies were more numerous and this may account for the higher effect rate.

Prevalence of Osteoarthritis and Its Affecting Factors among a Korean Population Aged 50 and Over (한국 50세 이상 성인의 골관절염 유병률과 관련요인)

  • Purpose: The purpose of this study was to identify the prevalence of osteoarthritis and its affecting factors among a community dwelling population aged 50 and over in Korea. Methods: A total of 2,640 subjects from the 2010 Korean National Health and Nutrition Examination Survey (KNHANES) were selected. Results: The prevalence of osteoarthritis was found to be 14.3%. Multivariate logistic regression analysis shows that osteoarthritis was more prevalent as people became older; in females; in people who graduated elementary school and had never educated as compared to those who graduated college; people without home ownership as compared to those with home ownership; in people with higher waist circumferences compared to those with normal waist circumference; in people who performed moderate intensity physical activity as compared to those who did not perform those physical activity. Conclusion: Our findings highlight the prevalence of osteoarthritis and its affecting factors, promoting the screening and prevention strategies for the aged 50 and over population in Korea.

Prevalence of Hypertension and Its Risk Factors among Aged 65 and over in Korea (한국노인의 고혈압 유병률과 위험요인)

  • Purpose: The purpose of this study was to identify the prevalence of hypertension and its risk factors among community dwelling aged 65 and over in South Korea. Methods: A total of 1,367 subjects from the 2010 Korean National Health and Nutrition Examination Survey (KNHANES) were selected. The data analysis was conducted by SPSS WIN 19.0 using descriptive statistics, ${\chi}^2$-test, and logistic regression analysis. Results: The prevalence of hypertension was found to be 47.70%. Logistic regression analysis shows that hypertension was more prevalent as people became older; in females; in elderly who were not employed as compared to those employed; elderly with diabetes, and obesity as compared to those without these health conditions; as elderly who performed more moderate intensity exercise. While, education, household income, high risk alcoholic drinking, smoking, stress perception, usual activity, and eating out behavior were not associated with prevalence of hypertension in this study. Conclusion: The finding of prevalence of hypertension and its risk factors is expected to promote the screening or prevention strategy for community dwelling aged 65 and over at risk of hypertension in Korea.

Development and Application of an Early Exercise Program for Open Heart Surgery Patients (개심술 환자를 위한 조기 운동프로그램의 개발 및 적용에 관한 연구)

  • Purpose: This study was to develop and evaluate stability and effects of an early exercise program for patients with open heart surgery. Methods: The subjects of this study were 30 patients who had either a coronary bypass surgery or a valvular heart surgery at a tertiary hospital in Seoul. The data was collected by observation and measurement from October 1, 2004 to November 15, 2004. Results: The early exercise program developed for this study consisted of range of motion exercise and walking. Intensity of walking was 1~3 METs and increased progressively to daily target distance. During exercise, the subjects were monitored heart rate, blood pressure and RPE (Rating of Perceived Exertion). The mean FIM (Functional Independent Measurements) score of subjects was significantly improved after the early exercise program. However, several complaints such as dizziness or pain were also reported. Most complaints were associated with chest tube and RPE. Conclusion: The early exercise program can help to recover patients' physical activities after surgery, and can be applied to most patients. Patients' RPE, dizziness and pain was possible limitations, therefore, active pain control and prevention of accidents for patients would be needed.

Pain Management Survey of Psychiatric Unit Nurses (정신과 근무하는 간호사의 통증관리 지식 및 태도에 대한 조사)

  • Purpose: This study was intended to measure the knowledge and attitudes of nurses working in a psychiatric ward regarding the subject of pain. Methods: In this descriptive study, data were collected in psychiatric wards(or hospitals) located in the Kyungnam and Kyungpook area from December 2013 to January 2014, using a modified version of the McCaffrey and Pasero Clinical Manual Survey Regarding Pain. The content of the survey follows a logical progression from common pain management problems through mechanisms of pain, assessment and pharmacology. Results: The average score of knowledge regarding pain was roughly $21.59{\pm}2.93$ among the 181 participants. The nurses selected Tylenol as the preferred choice (81.2%). Seventy-one percent of respondents didn't use Pain Intensity Instruments during the pain assessment. The subjects preferred the Numerical Rating Scale rather than Visual Analogue Scales. Pharmacology questions were the least likely to be answered correctly. No specific correlations were found between score and demographic variables of age and experience. No specific correlation between score and educational level was noted. Conclusion: Nurses require ongoing education on pain management, particularly regarding pharmaceutical agents.

The Effects of Hand Acupuncture Therapy on Dysmenorrhea (고려수지요법이 여고생의 월경곤란증에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of hand acupuncture therapy on dysmenorrhea(pain, related symptoms, ADL). Method: A quasi experimental pre-test and post-test design(experimental group 20, control group 20 by convenience assignment) was used. Data were collected from March to October 2004. The experimental group participated in the treatment(acupuncture & moxibustion on A1(haeum), A4(jaso), A5(samsin), A6(samcho), A8(sinje), A12(wijung), A16(simgyeok), F4(gongson), F6(eumryeom) and ALTENS which is an acupuncture like transcutaneous nerve stimulation), while the control group did not receive any treatment. Data were analyzed using the SAS programme with Fisher's exact χ2 test, paired t-test, and ANCOVA. Results: In the experimental group, pain intensity(f=33.31 p=0.00), related symptoms(t=4.06 p=0.00), ADL difficulty(t=3.85 p=0.00), and medication requirements(χ2=21.00 p=0.00) were significantly lower than those in the control group. Conclusion: These findings indicate that hand acupuncture therapy is effective for reduction of dysmenorrhea. Therefore, hand acupuncture therapy is considered as an independent nursing intervention for reducing dysmenorrhea.

Development of a Tool to Identify Severity of Illness Index in High Risk Newborns (고위험 신생아의 중증도 측정도구 개발)

  • Purpose: The purpose of this study was to develop a tool to assess the severity of illness in high risk newborns. Method: The research design was a methodological study. The tool was developed in 4 stages: first, preliminary items were developed based on a questionnaire about the severity of illness index that was given to 8 health professionals in Neonatal Intensity Care Units (NICU) second, a panel of specialists reduced the preliminary items using 3 validity tests; third, final items were selected from the results of a pre-test. Finally, from July 2005 to May 2006, reliability and validity were tested with a sample of 160 high risk newborns admitted to the NICU. Results: The final tool to identify the severity of illness index in high risk newborns consisted 39 items and Cronbach's alpha coefficient for internal consistency was .922. Using factor analysis, 4 factors were extracted and these factors explained 54.451% of the total variance. Conclusion: The instrument for assessing the severity of illness in high risk newborns developed in this study was identified as a tool with a high degree of reliability and validity. In this sense, this tool can be effectively utilized for assessing and implementing care for high risk newborns.
