In case of loss of many teeth due to dental caries or periodontal disease, improvement of masticatory function and aesthetics can be obtained through implant treatment. However, if the patient does not have a normal intercondylar relationship, it is difficult to achieve an ideal occlusal relationship with only prosthetic treatment. In particular, oral reconstruction with orthodontic treatment or orthognathic surgery is necessary for patients with mandibular prognathism. However, if the posterior occlusion collapses due to severe caries or periodontal disease, orthognathic surgery may be difficult. The occlusal vertical stop is very important for the stability of the mandibular position during occlusal reconstruction through orthognathic surgery. The patient in this case had posterior occlusion collapsed due to the caries of a large number of posterior teeth, and showed mandibular prognathism and long face. We planned a full arch restoration with orthognathic surgery and extracted the hopeless teeth. To secure the vertical stop required for orthognathic surgery, the implant was placed before surgery. After the orthognathic surgery and the final prosthesis application, the results were satisfactory for the improvement of the aesthetics and the restoration of the masticatory function.

Combining different procedures to reduce the number of surgical sessions and patient discomfort in implant placement and sinus floor elevation has been recommended, and evidence supports good outcomes. The aim of this study was to review the results of clinical studies on sinus floor elevation through extraction sockets and simultaneous immediate posterior implant placement. An electronic search was carried out in PubMed, Scopus, and Web of Science to find English articles published in or before August 2020. A manual search was also performed. Titles, abstracts, and the full-text of the retrieved articles were studied. Thirteen studies met our eligibility criteria: 6 retrospective case series, 3 case reports, 2 prospective cohort case-series, 1 prospective case series, and 1 randomized controlled trial. Overall, 306 implants were placed; 2 studies reported implant survival rates of 91.7% and 98.57%. The others either did not report the survival rate or reported 100% survival. Sinus floor elevation through a fresh extraction socket and simultaneous immediate implant placement appears to be a predictable modality with a high success rate. However, proper case selection and the expertise of the clinician play fundamental roles in the success of such complex procedures.
The aim of the study is to interpret the distribution of occlusal force by 3-dimensional finite element analysis of ISP(Implant Supported Prosthesis) supported by minimum number of implant to restore the edentulous patients. For this study, the Astra Tech implant system is used. Geometric modeling for 6 and 4 fixture ISP group is performed with respect to the bone, implant and one piece superstructure, respectively. Implants are arbitrarily placed according to the anatomical limit of lower jaw and for the favorable distribution of occlusal force, which is applied at the end of cantilever extension of ISP with 30mm. Element type is tetrahedral for finite element model and the typical mechanical properties, Young's modulus and Poisson's ratio of each material, cortical, cancellous bone and implant material are utilized for the finite element analysis. From this study, we can see the distribution of equivalent stress equal to real situation and speculate the difference in the stress distribution in the whole model and at each implant fixture, From the analysis, the area of maximum stress is distributed on distal contact area between bone and fixture in the crestal bone. The maximum stress is 53MPa at the 0.2mm area from the bone-implant interface in the maximum side for 300N load condition for 4 fixture case, which is slightly less than the stress calculated from allowable strain. This stress has not been deduced to directly cause the loss of crestal bone around implant fixture, but the stress can be much reduced as the old peoples may have lower chewing force. Thus, clinical trial may be performed with this treatment protocol to use 4 fixtured ISP for old patients.
목 적 : CT simulation 시 고 밀도 인공물에 의해 발생된 metal artifact는 주변조직의 관찰능력 감소 및 CT number 변화에 영향을 미치게 된다. 본 연구에서는 CT 촬영 시 Metal Artifact Reduction for Orthopedic implants(O-MAR)의 유용성을 평가해 보고자 한다. 대상 및 방법 : CT simulator를 이용하여 Gammex phantom과 Rando phantom의 original 영상, 고 밀도 인공물을 삽입 후 획득한 non O-MAR 영상과 O-MAR 적용 영상을 획득하였다. CT 영상 평가를 위해 Gammex phantom CT 영상 내 5개 조직과 artifact 영향을 직접 받는 3개 지점에 관심영역(region of interest, ROI)을 각각 설정하고, CT number와 노이즈(standard deviation, SD)를 평균값으로 비교, 분석하였다. 고 밀도 인공물 주변조직의 CT number 변화에 따른 선량변화 확인을 위해 전산화치료계획시스템 Eclipse를 이용하여 Rando phantom CT 영상 3곳에 원기둥 체적을 형성하였다. 치료계획은 6 MV 광자선, 7개 조사면, 조사야 $15{\time}15cm$, 1회당 처방선량 100 cGy를 적용하여 수립하고 평균선량을 비교, 분석하였다. 두 실험 모두 original 영상을 기준으로 분석하였다. 결 과 : Gammex phantom을 이용한 영상평가 실험에서 각 영상별 5개 조직의 CT number 차이는 작았으며, artifact 영향을 직접 받는 3개 지점은 O-MAR 적용 시 original 영상과 유사하게 나타났다. 그리고 O-MAR 적용 시 8개 지점의 SD는 non O-MAR 영상보다 모두 감소되었다. Rando phantom을 이용한 선량변화 실험 또한 O-MAR 적용 시 original 영상과 유사하게 나타났다. 결 론 : O-MAR 적용 시 고 밀도 인공물 artifact로 인한 영상 왜곡 및 SD 감소로 영상의 질이 향상되었으며 CT number 보정에 유용하였다. 이는 체표윤곽도(contour) 생성 및 치료계획 수립에 이점이 있을 것이라 사료된다.
연구 목적: 일체형의 o-ring type 미니 임플란트 고정체의 직경에 따른 파절강도의 차이를 비교하고자 한다. 연구 재료 및 방법: 길이 13mm의 one body o-ring type의 미니 임플란트(Dentis, Daegu, Korea)를 직경 2.0, 2.5, 3.0mm 각각 5개씩 준비하였다. Instron universal testing machine에 수직면에서 30도 각도로 샘플을 위치시키고 off-axis loading을 가하여 영구변형이 일어난 하중 값을 파절강도로 하고 5개의 시편의 평균을 구하여 각 직경에 따른 임플란트의 고정체의 파절강도를 비교하였다. 또한, 각 직경마다 3개의 시편을 준비하고 동적하중 피로 시험기를 이용하여, 파절이 발생할 때까지 파절강도의 80%, 60%, 40%의 loading을 가하여 파절되는 cycle수를 측정하여 각 직경의 피로 파절을 분석하였다. 추가적으로 총의치의 평균 저작력인 43 N의 하중을 가하여 파절되는 cycle 수를 측정하였다. 각 군간의 차이를 검증하기 위해서 일원분산분석(one-way ANOVA test)을 시행하였고, 통계처리는 SPSS ver.12 (SPSS Inc. Chicago, IL, USA) 을 이용하여 실시하였다. 결과: 직경 3.0mm의 미니 임플란트는 평균 $276.0{\pm}13.4N$의 압축력을 받았을 때 영구 변형이 일어났고 직경 2.5mm 미니 임플란트가 $149.0{\pm}6.1N$, 2.0mm 미니 임플란트가 $101.5{\pm}14.6N$일 때 영구 변형이 일어났다. 각 군간의 파절강도에는 유의한 차이가 있었다(P<.001). 총의치의 평균 저작력 하중에서 실시한 피로 파절 실험 결과, 세직경 모두 $5{\times}10^6cycle$까지 파절이 일어나지 않았다. 결론: 미니 임플란트의 정적 하중 하에서 최대 압축강도는 직경이 증가할수록 유의적으로 증가하였다. 최대 압축강도는 세 직경 모두 총의치의 평균 저작력 보다는 크나 최대 교합력보다는 직경 3.0mm에서만 크게 나타났다. 총의치의 평균 저작력 하중에서 실시한 피로 파절 실험 결과, 세 직경 모두 파절이 일어나지 않았다.
부분 무치악 환자에 있어 심한 골소실으로 인해 치관부 수직높이가 과도해질 우려가 있는 경우에는 임플란트 식립 후 고정성 보철물로 수복시 임플란트 주변 골의 응력 증가 및 나사 풀림이 발생할 가능성이 증가할 수 있다. 이러한 경우에 milled bar 및 가철성 보철물로 수복할 수 있는데, milled bar는 단일 임플란트 간의 일차적인 스플린팅 효과와 안정성을 부여할 수 있다. 또한 milled bar 사용 시 Advanced Dental Device-Treatment Of Choice(ADD-TOC), 자석 부착장치 같은 부가적인 부착장치 사용을 통해 임플란트 피개의치의 추가 유지력을 부여할 수 있다. 본 증례의 환자는 하악 좌측 구치부 평활근육종으로 인한 절제술 후 심한 골소실 및 다수 치아를 상실한 부분 무치악 환자로 장기간의 하악 구치부 상실로 인한 대합치는 정출된 상태였다. 상악 좌측 대구치는 교정적 압하를 통해 교합평면을 회복하였고, 하악 좌측 대구치 부위는 임플란트 식립 후 milled bar에 부가적인 부착장치인 ADD-TOC, 자석 부가장치를 이용하여 임플란트 피개의치로 수복하였다. 이를 통해 장기적으로 기능적, 심미적인 만족할 만한 결과를 얻었기에 보고하는 바이다.
부분 무치악 환자에서 구강 기능의 회복을 위한 보철 계획을 수립할 때, 잔존하는 치아에 따라 다양한 치료 방법을 선택할 수 있다. 특히 대합이 상실된 소수의 잔존치가 불안정한 교합을 야기하는 엇갈린 교합의 경우, 대합 되는 위치에 임플란트를 식립함으로써 교합지지를 회복할 수 있다. 이 증례에서는 발치 후 엇갈린 교합이 된 환자에서 의치의 하중을 견딜 수 있도록 상악 잔존치에 대응하는 하악 무치악 부위에 임플란트를 식립하고 자석유지장치(magnetic attachment)를 연결하는 국소의치를 제작하였다. 또한 상악 전치의 돌출과 하악 입술 지지도가 부족한 것을 개선하고 싶어한 환자의 요구사항에 따라 상악 전치부 보철적 수복 및 하악 의치를 통해 심미적 안모를 회복하였다. 5개월 정기검진 결과, 환자는 저작 기능 및 심미적인 면에서 만족하였고 의치의 안정성 또한 잘 유지되고 있기에 이를 보고하고자 한다.
오랜 기간 국소의치를 장착한 부분 무치악 환자에서, 치아가 결손된 부위는 점진적인 치조골의 흡수가 발생되는 반면, 잔존 지대치 부위는 치조골이 유지되어 있는 것을 볼 수 있다. 이는 차후 잔존 지대치를 상실한 후에도 치아 상실의 시점에 따라 수직적인 골 높이의 현저한 차이를 나타낸다. 이와 같이 전체 치아 발거 후 상하악 악간 거리가 좌우측 또는 전후방으로 다르게 나타나는 환자에서 임플란트를 이용한 고정성 보철물이나 임플란트 피개의치의 제작 시 여러 가지 보철적 문제가 야기될 수 있다. 본 논문은 구치부 결손으로 인해 오랫동안 국소의치를 사용했던 환자에서 치아 상실 후 잔존 치조골의 높이를 고려하여 임플란트 지지 국소의치를 제작한 증례로서 잔존 치조골 량이 많아 악간거리가 짧은 전치부는 임플란트를 식립하여 고정성 보철물로, 오랫동안 치조골의 흡수로 인해 악간거리가 긴 구치부는 국소의치로 수복하였다. 본 증례에서의 임플란트 지지 국소의치는 부가적인 수술이 없이 소수의 임플란트를 식립하고 환자에게 기존 보철물의 양식과 유사한 보철물을 제공함으로서 경제적이고 편안한 치료결과를 보여주었다.
