A Changes of Electromyogram Activities in Deltoideus and Supraspinatus During Shoulder Abduction Using TheraBand (TheraBand를 이용한 견관절 외전시 삼각근과 극상근의 근전도 변화)

  • The purpose of this study was to compare the changes of muscle activities in deltoid with those in supraspinatus using electromyography(EMG) while subjects abducted their shoulder to different angles with different resistance. Methods : 20 volunteers who were comprised of 10 males(mean age: $21.2{\pm}2.0$) and 10 females(mean age: $20.2{\pm}1.6$) were collected. Surface electrodes were attached on deltoid and supraspinatus muscle for recording. Each reference electrode was located 3 cm to the each recording electrode. Muscle action potentials were recorded with changing the angle of shoulder abduction, $30^{\circ}$, $60^{\circ}$, $90^{\circ}$. This procedure was repeated with different resistance 0 pound, 2 pounds, 4.5pounds. The angle of shoulder abduction was determined by clinical goniometer. SPSS(Statistical Program for Social Science)/WIN 10.0 was used for statistics. Analysis included ANOVA, T-test. Results : The following results were obtained in this study. 1. There was significant differences during isometric shoulder abduction $30^{\circ}$, $60^{\circ}$, $90^{\circ}$ comparing muscle activity in deltoid and supraspinatus muscles at 0 pound, 2pound, 4.5pound resistance(p<0.05). 2. There was no useful significant in the deltoid and supraspinatus muscles activity EMG compared by physical condition. 3. Their was significant in the deltoid EMG compared by gender t=-5.41, P<0.01, but no difference in the supraspinatus EMG(P=0.333 >a=0.05). Conclusions: There are many previous studies on influence of shoulder angles and speeds of muscle activity. However most of them placed the focus on isotonic or isokinetic exercise, or shoulder function. But this study was done during isometric exercise, which is better for early assessment and treatment for injured patients. There is strong relationship between the shoulder angle with resistance in deltoid supraspinatus muscle activity. We conclude that it is important to apply isometric shoulder abduction exercise with correct angle and resistance especially in early stage.

Evaluation of the Probiotic Potential of Bacillus polyfermenticus CJ6 Isolated from Meju, a Korean Soybean Fermentation Starter

  • To evaluate the probiotic potential of Bacillus polyfermenticus CJ6 isolated from meju, a Korean traditional soybean fermentation starter, its functionality and safety were investigated. B. polyfermenticus CJ6 was sensitive to all antibiotics listed by the European Food Safety Authority. The strain was also non-hemolytic, carried no emetic toxin or enterotoxin genes, and produced no enterotoxins. The resistance of B. polyfermenticus CJ6 vegetative cells and spores to simulated gastrointestinal conditions was high (60-100% survival rate). B. polyfermenticus CJ6 produced high amounts (0.36 g as a purified lyophilized form) of ${\gamma}$-polyglutamic acid (PGA). We speculate that the improved cell viability and the production of ${\gamma}$-PGA have a significant correlation. Adhesion of the strain to Caco-2 and HT-29 cells was weaker than that of the reference strain (Lb. rhamnosus GG), but it was comparable to or stronger than those of reported Bacillus spp. When B. polyfermenticus CJ6 spores were given orally to mice, the number of cells excreted in the feces was 4-fold higher than the original inocula. This suggests the inoculated spores propagated within the intestinal tract of the mice. This idea was confirmed by field emission scanning electron microscopy, which revealed directly that B. polyfermenticus CJ6 cells germinated and adhered within the gastrointestinal tract of mice. Taken together, these findings suggest that B. polyfermenticus CJ6 has probiotic potential for both human consumption and use in animal feeds.

Development of Dissolution Test Method for Acebrophylline Capsules and Bromhexine Hydrochloride Tablets in Korean Pharmaceutical Codex (고시수재 의약품 중 아세브로필린 캡슐 및 브롬헥신염산염 정의 용출시험법 개발)

  • Although the dissolution test can serve as an effective tool for quality control and predictor of in vivo performance, there are a number of drugs with no established dissolution specification in Korean Pharmaceutical Codex (KPC). So, with each reference and test drugs, the dissolution test method and an analytical procedure by HPLC were developed and validated to establish dissolution specification for acebrophylline capsules and bromhexine hydrochloride tablets. The dissolution condition was determined based on the "Guidelines on Specifications of Dissolution tests for Oral dosage forms" of Ministry of Food and Drug Safety (MFDS). The analytical method of HPLC was validated in specificity, linearity, precision and accuracy. Final dissolution test was performed with commercially available samples of 3 lots to establish specification. In addition, no difference was observed by the inter-laboratory evaluation. Dissolution specifications and conditions will be used for revising the monograph of acebrophylline capsules and bromhexine hydrochloride tablets in next supplement of KPC.

Effect of High Dose Insulin/Euglycemia Therapy for Acute Calcium Channel Blocker Intoxication: A Systematic Review (급성 칼슘통로차단제 중독에서 고용량 인슐린/정상혈당치료의 효과: 체계적 고찰)

  • Purpose: The purpose of this study is to evaluate the effectiveness and the adverse events of high dose insulin/euglycemia therapy in acute calcium channel blocker (CCB) poisoning. Methods: We developed a systematic search strategy and applied it to 4 electronic reference databases. We searched medical journals as well as the bibliographies of relevant articles. All forms of literature relevant to human use of high dose insulin for acute CCB poisoning were included. The literature search was conducted by two investigators in August, 2015 with publication language restricted to English and Korean. Case reports were divided between CCB overdose alone and multi-drug overdose including CCB. The effect and adverse event of high dose insulin and clinical outcome of each case were analyzed. Results: Among 55 searched studies, 20 studies were included. A prospective study, a retrospective study, a systematic review study, and 17 case reports were identified. Case reports consisted of 11 CCB alone and 12 multidrug overdose cases including CCB. Although most cases described significant clinical improvements, one of them showed no beneficial effect. Several adverse events including hypoglycemia and hypokalemia were reported. No significant sequalae from adverse events was reported. Conclusion: Although there were many case reports demonstrating successful use of high dose insulin for CCB poisoning, the effect cannot be estimated due to a possibility of publication bias. Therefore, high dose insulin/euglycemia therapy might be considered adjunctive therapy in cases of CCB intoxication refractory for standard therapy.

Risk Factors of Breast Cancer in the Eastern Mediterranean Region: A Systematic Review and Meta-Analysis

  • Background: Identifying risk factors of breast cancer is a key point for preventive strategies to reduce the incidence. The aim of current study was to determine most important risk factors for breast cancer in the Eastern Mediterranean Region (EMR) using a systematic review. Materials and Methods: PubMed, Scopus, Web of Science till August 24, 2012 and the reference lists of all included studies were searched. Analytic studies which had reported odds ratios (OR), relative risk (RR) or required data to calculate them were included. A total of 343 studies were critically appraised and finally 30 studies were meta-analyzed. Heterogeneity between the studies was assessed by $I^2$ and Cochran's Q. Egger's test was used to assess publication bias. Results: Twenty five casecontrol studies, one nested case-control and four cohort studies were included. The largest ORs were obtained for history of no live birth (2.25; 95%CI: 1.58-3.18), body mass index (BMI) more than 30 (2.21; 95%CI: 1.71-2.36), age at first pregnancy more than 30 years old (1.52; 95%CI: 1.30-1.77) and meat consumption more than three times per week (1.39; 95%CI: 1.03-1.87). The other important predictors were higher education and smoking as risk factors, physical activity and ovulatory stimulating medication as protective factors. Conclusions: The most important predictors of breast cancer in EMR were history of no live birth, BMI more than 30, age at first pregnancy more than 30 years old, physical inactivity and smoking. Almost all these risk factors are consistent with known risk factors for this cancer in other parts of the world.

Effect of Diabetic Camp Program on the Depression, Self-Efficacy and Self-Esteem in Diabetic Children and Adolescent. (당뇨캠프 프로그램이 당뇨병 환아의 우울, 자기효능감, 자기존중감에 미치는 영향)

  • The purpose of this study was to explore the effect of diabetic camp program on depression, Seoul National University Hospital self-efficacy and self-esteem of the juvenile diabetic patient who had participated with diabetic camp program at Seoul and Kyung In area from August 5 to August 9, 1995. Nursing staffs explained them how to respond to the questionnaire of depression, self-efficacy and self-esteem prior to beginning the program and following the program. The questionnaire of depression, self-efficacy and self-esteem was developed by both diabetes mellitus nurse specialist and pediatric nursing professors in reference with the previous research. Response items of the questions were structured as yes or no for the elementary school students and that of the questions were structured as 5 likert scale for the both middle and high school students. Paired t-test was used for the significance of the difference between values before and after the diabetic camp program. Depression decreased following the program, while self- esteem increased significantly following the program in elementary school students. There was no change in self-esteem, while self-efficacy increased significantly following the program in middle and high school students. Self-efficacy following the program was high as the frequency of exercise increased in the elementary school students. Self-efficacy prior to the program was high as the experience of diabetic education increased in the middle and high school students. self-efficacy and self-esteem prior to the emu was highly correlated with that following the camp in middle. and high school students. The result suggests that diabetic camp program could be one of ways to decrease depression and to increase self-efficacy of the diabetic children.

The Effect of Head Posture Change on Initial Occlusal Contacts (두부의 자세 변화가 초기 교합접촉에 미치는 영향)

  • The purpose of this study was to evaluate effect of head posture change on initial occlusal contacts through measuring the distances between initial occlusal contacts and maximum intercuspal position at different head posture. Two special devices were designed and constructed. Mandibular movement replicator was used to assess reliability of the K6 diagnostic system(MKG; Myo-tronic Inc, Seatle, USA) and head posture calibrator was used to maintain the constant head posture during experiment. We measured difference of distance between initial occlusal contact and maximum intercuspal position with MKG in upright, supine, 45 degrees extension, 30 degrees flexion, 30 degrees right and left bending postion of the head. The Frankfurt horizontal plane was used as a reference plane. 21 adults aged from 23 to 25 were selected, who have normal or class I molar relationship, and have no symptoms on TMJ and masticatory muscles, and have restorations less than 3 surfaces on each tooth, and have no other prosthetic restoration. The obtained results were as follows : The mean absolute distances between initial occlusal contact and maximum intercuspal postion were 0.39(0.18mm in the upright position, 0.65(0.37mm in the supine position, 0.59(0.33mm in the 45 degree extension, 0.70(0.53mm in the 30 degrees flexion, 1.12(1.10mm in the 30 degrees right bending and 1.94(0.67mm in the 30 degrees left bending of the head. The positions of the initial occlusal contacts have a tendency to locate anterior, left and inferior to maximal intercuspal position in upright position, posterior and inferior in supine position and 45 degrees extension, anterior and inferior in 30 degrees flexion, right and inferior in 30 degrees right bending, and left and inferior in 30 degrees left bending of the head. There were significant differences among the initial occlusal contacts in each head postures(P<0.0001). Therefore, we need to check initial occlusal contacts in the altered head posture during occlusal analysis and adjustment of occlusal appliance and dental occlusion for diagnosis and treatment of temporomandibular disorder.

The Actual Wearing Conditions and Preferences for New Silver Women's Brassiere (뉴실버 여성을 위한 브래지어 착용실태 및 선호도 조사)

  • This study provides basic reference data for brassiere wearing conditions, design preferences of new silver women (50s-60s) and development of brassiere products. We compiled and compared 163 pieces of brassiere (considering of 105 domestic general brassieres and 58 foreign silver brassieres) analyzed using SPSS Statistics 21 program. A survey was then conducted on the actual wearing, purchasing conditions, design preferences for 176 females (50s-60s). The result of this study are as follows: First, comparing actual product conditions (domestic general products and foreign silver products), the ratio of full cup in cup height, V-shape type in neckline shape, long type in front-end length, wide type in wing width, U-shape type in shoulder strap form, wide type in shoulder strap width, no-wire brassieres in breast wire type, all-in-one type in shoulder strap separation, back type in closure type appeared higher than other types of brassiere in domestic general product. Second, a study also showed that 60s women's wearing time is lower than 50s women's; however, 60s women expressed a higher figure and preference for the purchasing ratio in the department store, full cup in cup height, short type in length of brassieres, wide type in wing width, U-shape in shoulder strap form, small type in shoulder strap width, back type in closure type and no-wire brassieres than those in their 50s. It was found they prefer fabric with a high natural content, nude tone color without decoration & pattern and camisole brassieres.

Case Report of Breast Reconstruction with SIEA Flap (얕은 하복벽동맥 천공지 피판을 이용한 유방 재건술 - 2예 보고 -)

  • Purpose: Breast reconstruction with abdominal flap has many advantages. However, it might cause abdominal complications such as bulging or hernia. SIEA (Superficial inferior epigastric artery) flap is the most advanced form of abdominal flap which has no adverse effect on abdominal fascia. We report 2 cases of breast reconstruction with SIEA flap with reference review. Methods: From Jun 2006 to Jan 2009, 110 patients underwent breast reconstruction with free abdominal flap. We tried to find the SIEA in every cases and adopted SIEP flap if the diameter was larger than 1mm and it had visible pulsation. 2 patients underwent breast reconstruction with SIEA flap. After design of abdominal flap SIEA was investigated with portable Doppler. SIEA was usually found 4-5 cm lateral to the midline. Flap was elevated with SIEA and SIEV to their maximal length at hiatus. Results: There were no complications, such as infection, hematoma, and necrosis of flap. In both cases, flaps survived completely with excellent vascularity and breast reconstruction was successful. In one case, there was skin necrosis of mastectomy flap, and it was healed by conservative management. Conclusion: With the SIEA flap, donor site morbidity can be minimized along with reduction of operation time. If there is reliable SIEA, SIEA flap would be the preferred skills. However, the limitation of the SIEA flap is difficulty in identifying the SIEA. The reason for less availability of the flap in Korea might include high prevalence of the Caesarian section scar and relatively higher level of the lower margin of the flap.

Distinct Postsurgical Management in Young and Elderly Breast Cancer Patients Results in Equal Survival Rates

  • Background: Although breast cancer (BC) is one of the most common malignant diseases in women, the majority of the studies describing the characteristics of BC in elderly patients have been limited to survival assessments or tumor features, without using younger BC patients as a reference group. The aim of our study was to describe and compare tumor characteristics and management patterns in elderly versus younger breast cancer patients in Turkey. Materials and Methods: We retrospectively analyzed 152 patients with invasive breast cancer who underwent surgery in our institution between 2002 and 2012. Patients were divided into 2 groups according to age at the time of diagnosis. Results: There were 62 patients in the elderly group (${\geq}65$ years) and 90 patients in the younger group (<65 years). Compared to the younger group, tumors in the elderly group were more likely to be larger (p=0.018), of lower grade (p=0.005), and hormone receptor-positive (p>0.001). There were no significant differences regarding histology, localization, lymph node involvement, or types of surgical procedures between the 2 groups. Comorbidities were more common in elderly patients (p<0.001). In addition, elderly patients were more likely to receive hormonal therapy (p<0.001) and less likely to receive radiotherapy (p=0.08) and chemotherapy (p=0.003). There was no difference in survival and locoregional recurrence rates between the groups. Conclusions: The results of this study demonstrate that breast cancer in elderly patients has more favorable tumor features, warranting less aggressive treatment regimens after surgery.
