Neuropathic Pain Behaviors and the Change of Spinal Neuropeptides following Peripheral Nerve Injury in Neonatal Rats

  • Objective : It has been suggested that the occurrence of persistent pain signal during the early postnatal period may alter an individual's response to pain later in life. The aim of this study is to assess whether neonatal nerve injury resulted in long-lasting consequences on nociceptive system in the rat. Methods : We examined whether neuropathic pain behaviors and the changes of spinal neuropeptides [SP, CGRP, VIP and VIP] induced by peripheral nerve injury within 1 day after birth [Neonate group] were different from those at 8 weeks after birth [Mature group]. Results : The Neonate group showed more robust and long-lasting pain behaviors than the Mature group. Immunohistochemical findings demonstrated that spinal SP- & CGRP-immunoreactivities[ir] of the ipsilateral to the contralateral side increased in the Neonate group, whereas those decreased in the Mature group. In addition, increase in spinal VIP- & NPY-ir of the ipsilateral to the contralateral side was more robust in the Mature group than in the Neonate group. Conclusion : These results suggest that peripheral nerve injury in the early postnatal period may result in long-lasting and potentially detrimental alterations in nociceptive pathways.

The Effects of Nerve Growth Factor Expression of Central Nerve System by Environmental Enrichment and Peripheral Nerve Electrical Stimulation in Brain Ischemia Model Rats (뇌졸중 유발 백서모델에서 환경강화와 말초신경전기자극이 중추신경계의 신경성장인자에 미치는 영향)

  • Purpose: To investigate environmental enrichment and nerve stimulation follows in application times with the change of BDNF & Trk-B receptor in the motor cortex and spinal cord. Methods: Experimental groups were divided into the five groups. Group I: normal control group, Group II: experiment control group, Group III: sciatic never electrical stimulation after MCAO, Group IV: application of only environmental enrichment after MCAO, Group V: never electrical stimulation with environmental enrichment after MCAO. Histologic observation and coronal sections were processed individually in goat polyclonal antibody phosphorylated BDNF and rabbit polyclonal antibody Trk-B receptor. Results: In immunohistochemistric response of BDNF and Trk-B, group II were showed that lower response effect at postischemic 1 days, 3 days, and 7 days. Group V were showed that increase response effect at postischemic 3 days, 7 days and 14 days. Specially showed that the most response effect at postischemic 14 days. In neurobehavioral assessment, group V were significantly difference from other groups on between-subject effects. Conclusion: The above results suggest that combined environmental enrichment with peripheral nerve electrical stimulation in focal ischemic brain injury were more improved that the change of BDNF & Trk-B receptor expression than non treatment.

  • The main cause of diabetic neuropathy, one of the most debilitating complications, is the chronic hyperglycemia, the increase sorbitol or the decrease of nerve growth factor(NGF). NGF, a protein, plays a major role in the development and maintenance of peripheral nervous system. Systemic administration of NGF prevents manifestations of neuropathy in rodent models of diabetic neuropathy. In the previous investigation, we report the hypoglycemia effect of Dioscorea rhizoma extract(DRE) in diabetic mice. The present study shows protective effect of DRE on diabetic neuropathy by induction of NGF protein. We investigated the NGF level in salivary gland and sciatic nerve of normal mouse and the effect of DRE on sciatic nerve conductivity and thermal hyperalgesia test in Type 2 db/db mouse. DRE increased endogenous NGF level in salivary gland and sciatic nerve of mouse. And sensory nerve conductivity velocity(SNCV), motor nerve conductivity velocity(MNCV) and thermal hyperalgesia increased in DRE treatment mice compared with control group. On the basis of our results, we conclude that DRE increase induction of endogenous NGF level and have protective effect on diabetic neuropathy by induction of NGF. Therefore, we propose that long-term use of DRE might help prevention of diabetes-associated complication; diabetic neuropathy.

Influence of Heat Stress Temperature on Sympathetic Nerve Activities (Heat-stress 온도 적용이 교감신경활동에 미치는 영향)

  • The purpose of this study were to investigate influence of heat stress temperature on sympathetic nerve activities. Subjects were 8 normal adults (4 men, 4 women, 21.36 years old). First sympathetic nerve activities were measured at the point that increase of core temperature stops at the state of applying normal thermic temperature (NIT; $34^{\circ}C$). After measurement, temperature of bathtub was increased to heat stress temperature (HST; $46^{\circ}C$) and sympathetic nerve activities were remeasured at the point that temperature increase stops. Sympathetic skin response (SSR) were analyzed using EMG, IR thermometer, and auto stethoscope. SSR latency showed significant differences at both palms by electrical stimulation to median nerve (p<.05). Electrical stimulation to forehead showed significant difference at left palm (p<.05) and electrical stimulation to navel showed significant difference at right palm (p<.05). Median nerve in changes of SSR amplitude showed significant differences at both palms in HST (p<.01). Electrical stimulation to navel showed significant difference at left palm (p<.05). Ts of forehead and xiphoid process showed significant differences (p<0.01). Tc of oral (p<0.05) and inner ear (p<0.01) showed significant differences. Pulse rate showed significant difference (<0.05). This study showed that immersion in HST had significant decrease of excitability in sympathetic nervous system compared to immersion in NTT.

  • Facail nerve palsy is a common disease in the face. This disease is treated by many methods. But it is not treated by Facial Acupunture. Therefore we performed to evaluate the effect of Facial Acupuncture with oriental dedical treatment in facail nerve palsy. This clinical study was carried out 2 caese of facail nerve palsy which had been admitted our hospital. In this study the patients were treated by Facail Acupuncture. Other treatment were herbal medication, regabilitative therapy. H-B scale(House-Brackmann facial nerve grading system) and Digital Infrared Thermal Imaging were measured from admission to discharge. After being treated by our methods, Patients' H-B scale and Inequality of Facial DITI were improved in cases compared to existing. In the study, The treatment of facial nerve palsy using Facial acupuncture was more effective, especially H-B scale, Digital Infrared Thermal Imaging and treatment period, compared to existing.

A Study of the Peripheral Neuropathy among the Workers Exposed to Carbon Disulfide (이황화탄소에 폭로된 근로자들의 말초신경병증에 관한 연구)

  • Neurotoxicity in the workplace may occur with exposure to scores of chemicals. Although large acute outbreaks of the occupational neurological disease are rare, the incidence of occupational neurotoxicity in its subtler aspects is unknown. A working knowledge of both the major occupational neurotoxic solvents and the tools used by cliniical neurologists and neurotoxicologists to evaluate neurotoxicity in working populations is a necessity fur the occupational physician. To investigate the effects of carbon disulfide($CS_2$) on the peripheral nerve system using the nervous conduction study, 105 male workers working in the spinning room of a viscose rayon factory were examined and compared with a sex and age matched, unexposed 105 male controls using t-test analysis. 72.4% of $CS_2$-exposed workers complained of neurological symptoms, and the abnormal cases in nerve conduction study were 48.6%. The abnormal cases of nerve conduction study increased in number according as the age and duration of exposure increased. In this study, asymptomatic workers were confirmed to have subclinical neuropathy by nerve conduction study. Also as there were abnormal cases even in its duration of exposure below 4 years, nerve conduction study turned out to be ways of discovering of early peripheral neuropathy. In nerve conduction study, the amplitude, velocity, F-wave latency and H-reflex of the motor and sensory nerves in both upper and lower extremities were significant different between $CS_2$-exposed workers and the controls. From the pathological viewpoint, both segmental and axonal degenerations were assumed in this study.

  • Neurolymphomatosis (NL) is defined as the involvement of the peripheral nervous system in lymphocytic invasion. It is a very rare form of lymphoma that may occur as an initial presentation or recurrence. It affects various peripheral nervous structures and can therefore mimic disc-related nerve root pathology or compressive mononeuropathy. NL often occurs in malignant B-cell non-Hodgkin lymphomas. Notwithstanding its aggressiveness or intractability, NL should be discriminated from other neurologic complications of lymphoma. Herein, we present a case of primary NL as the initial presentation of diffuse large B-cell lymphoma (DLBCL) of the sciatic nerve. The patient presented with weakness and pain in his left leg but had no obvious lesion explaining the neurologic deficit on initial lumbosacral and knee magnetic resonance imaging (MRI). NL of the left sciatic nerve at the greater sciatic foramen was diagnosed based on subsequent hip MRI, electrodiagnostic test, positron emission tomography/computed tomography, and nerve biopsy findings. Leg weakness slightly improved after chemotherapy and radiotherapy. We report a case wherein NL, a rare cause of leg weakness, manifested as the initial presentation of primary DLBCL involving the sciatic nerve at the greater sciatic foramen.

  • This research experimented on the change of the multiple colleague scan angle facing one scan object facet to many directions of the form of 3D about the visual angle nervous system forming the cubic distribution with the gradient magnetic field of the mri system and considered the existing basic angle oblique direction test coverage and comparison. MR system can freely select various pulse sequence and image slice. To oblique imaging for optic nerve viewing, we have studied the variation of scan angle between typical oblique scan method (sagittal-coronal plane) and multi directional angles oblique scan method (sagittal-coronal-axial plane) using gradient of MR system. In this study, the subjects of the experiment were normal adults in our country. As a result, we confirmed that multi directional angles oblique scan method can display anatomical information of more wider area than typical oblique scan method. In addition, to clearly display optic nerve, we also confirmed that image slice thickness and pulse sequence have effect on it.

Effects of Selection Method of Acupuncture between the Affected Part and the Unaffected Part on Peripheral Facial Nerve Paralysis in the Incipient Stage (말초안면신경마비 초기 환자에서 환측(患側) 및 건측(健側)취혈이 안면신경마비의 호전도에 미치는 영향)

  • Objectives : This study was to compare the effect of acupuncture on peripheral facial nerve paralysis between affected part, and unaffected part in the incipient stage. Methods : We investigated 32 cases of patient with peripheral facial nerve paralysis, and divided into two groups. One group was treated by acupuncture on the affected part, and the other was treated by acupuncture on the unaffected part. Both groups were evaluated by Yanagihara's unweighted grading system and House-Backmann grade before treatment, and in every one week and after treatment. Results & Conclusion : 1. The number of female was more than that of male and the number of left side was more than that of right side in distribution of lesion. Post ear pain was the highest related symptom at onset, Unknown was a major cause of peripheral facial nerve paralysis in distribution of cause. 2. Yanagihara's unweighted grading system and House-Backmann grade score between two groups were not statistically significant in every one week. 3. Yanagihara's unweighted grading system and House-Backmann grade score between two groups were not statistically significant in before treatment and final.

  • Objectives: The purpose of this study is to investigate the effectiveness of using Korean medical treatment for peripheral facial nerve palsy. Methods: We primarily treated the patient's peripheral facial nerve palsy during hospitalization (06/28/21~07/13/21) without any other steroids or antiviral drug treatments and additionally treated their spinal stenosis, using Korean medical treatments in both cases (herbal medicine, acupuncture, and herbal acupuncture therapy). The House-Brackmann grading system and Yanagihara's unweighted grading system were used to assess facial symptom changes, and the Numerical Rating Scale was used to evaluate the back pain caused by the spinal stenosis. Results: Following treatment, the patient's House-Brackmann and Numerical Rating Scale scores decreased, while their Yanagihara unweighted score improved. Conclusions: This study suggests that Korean medicine may be effective in the treatment of facial nerve palsy.
