Survival of Triple Negative versus Triple Positive Breast Cancers: Comparison and Contrast

  • Background: Triple negative (TN) and triple positive (TP) breast cancers both are aggressive types but TN generally has a shorter survival. Objectives: To compare the clinical characteristics and treatment outcomes for patients with TN versus TP breast cancer and to assess various prognostic factors affecting overall survival. Materials and Methods: A retrospective audit of 85 breast cancer patients was conducted in the Department of Radiation Oncology and Medical Oncology on patients from 2006 to 2013 for whom IHC for ER, PgR and Her-2 neu were available. The patients were stratified into: ER-, PR- and Her-2 neu- (Arm A, n=47) and ER+, PgR+ and Her-2 neu+ (Arm B, n=38). Results: TN subtype had higher numbers of premenopausal and advanced stage patients as compared to TP subtype. The locoregional recurrence (LRR) and distant metastatic rate was also higher in TN subtype but there was no definite pattern in both the arms. Among the prognostic factors, patients with premenopausal status and advanced stage in TN breast cancer had inferior survival (P=0.07) whereas for those with postmenopausal status and early stage there was no survival difference between the two arms. Conclusions: TN subtype tends to be more aggressive in terms of younger age and advanced stage at presentation, higher tumour grade, LRR and metastasis, suggesting need for future research efforts on providing aggressive treatment to these patients. We could attribute better outcome for TP subtype to receptor positivity enabling role of hormonal treatment and targeted therapy, although less number of patients received targeted therapy.

Arthritis in the Subacute Stage of Kawasaki Disease after Responding to Intravenous Immunoglobulin Treatment (가와사끼병에서 정맥용 면역글로불린에 반응 후 아급성기에 발생한 관절염)

  • We evaluated the clinical and laboratory characteristics of five children with Kawasaki disease who had showed arthritis after responding to intravenous immunoglobulin(IVIG) treatment. Age distribution was between 13 months and six years of age(mean $3.2{\pm}1.6$ years). There were two males and three females. Arthritis occurred when acute symptoms were subsiding, with the average onset on day $5.8{\pm}1.8$ after final IVIG treatment. Arthritis was pauciarticular in three, and polyarticular in two. Regarding laboratory findings, one child was positive in rhematoid factor and changed to negative after two months. Three patients were examined for HLA B27 and all showed negative results. High dose aspirin(two cases), anti-inflammatory drug(ibprofen, three cases), and corticosteroids(methyprednisolon pulse therapy, one case) were used for this type of arthritis. Symptoms and signs of arthritis in all patients were improved by these therapies. There was no relapse or complications within six months. Arthritis after responding to IVIG therapy was rarely observed in children with Kawasaki disease. This type of arthritis responded well to anti-inflammatory drugs including corticosteroids, and showed no relapses.

A Clinical Trial of Combined Treatment with Estrogen and Antipsychotics in Females with Chronic Schizophrenia (만성 여성 정신분열병 환자에서 Estrogen 병합투여의 임상시도)

  • Objective : This study was a open clinical trial aimed at exploring the effectiveness of combined treatment with estrogen and antipsychotics to the chronic female schizophrenics. Method : 40 female patients who met DSM-VI criteria for schizophrenia who were chronically ill were randomly assigned to estrogen group(EG) and control group(CG). EG patients were received estrogen 1.25mg for 8weeks in addition to their routine antipsychotic regimens. CG patients were received their routine antipsychotic regimens only. Both groups were evaluated by PANSS(Postive and Negative Syndrome Scale), CGI(Clinical Global Impression) at 0, 4, 8 week during the trial period and compaired with each other. Results : 40 female patients have completed the study during 8weeks. EG was significantly improved in PANSS and CGI scores than CG during the 8weeks. In EG patients, all symptom subtypes(positive symptoms, negative symptoms, general psychopathology symptoms) of PANSS were significantly improved and positive symptoms were most significantly improved at 8week. Conclusions : This results support the clinical value of combined estrogen therapy among chronic female schizophrenics.

Attitude and Recognition of Medical School Students on Korean Traditional Medicine Diagnosis and Treatment (의대생들의 한의학 진단 및 치료에 대한 인식 조사)

  • Objectives : This study was aimed to facilitate mutual comprehension between Western and Traditional Korean Medicine (TKM) by analyzing recognition of medical school students on traditional medical prescription and treatment such as herbal medicines and acupuncture. Methods : We conducted a survey targeting medical school students of four medical schools from April through May 2009, and analyzed 208 cases responded. Results : Survey results revealed that medical school students took knowledge of traditional medicine from TV programs and on the Web in general. They thought they had considerable traditional medical knowledge but did not almost understand for professional contents. Regardless of some positive recognition for traditional herbal medicine, negative perspectives were more common and its safety and efficacy on the drug reference were considered as a major reason. On the other hand, recognition on acupuncture was comparatively positive. Scientific data for both herbal medicine and acupuncture are lacking in reality but the attitude only for acupuncture was different. Of the survey respondents, 57.8% had negative attitude against diagnostic methods of TKM and 59.1% opposed to the usage of modern medical equipments by TKM doctors. With regard to the future position of TKM, they tacitly approved that TKM would be absorbed into the complementary and alternative medicine (CAM) and importance of TKM objectification, followed by Government's support. Conclusions : Medical school students neither affirm nor deny TKM as a whole but the state of awareness for each item was similar to the physician. In terms of TKM its internal and external innovation would be required to continuously establish objectification of TKM prescription and scientific research on treatment technology.

  • Graphite and carbonaceous materials intercalate and deintercalate Li-ion reversibly into their layered structures. These materials show an excellent capacity for using a negative electrode in Li-ion batteries, because the electrochemical potential of Li-ion intercalated carbon is almost identical with that of lithium metal. Carbon used in this study was obtained by the pyrolysis of petroleum pitch, and heat-treated at the several temperatures between $700^{\circ}C$ and $1300^{\circ}C$. XRD analysis revealed that crystallization of carbon increased with increasing the heat treatment temperature. Charge/discharge properties were studied by a constant-current step at the rate of 0.1C, and the interfacial reaction between the electrolyte and the surface of carbon electrode was studied by cyclic voltammetry. Cell capacities were investigated in terms of the heat treatment temperature and the cycle number. Reversible capacity increased with the heat treatment temperature up to $1000^{\circ}C$, thereafter decreased continuously. Also, charge capacity decreased with the cycle number, while the reversibility improved with it.

Thermotherapy and Dynamic Warm-up on the Kinetic Parameters during Drop-landing (드롭랜딩 시 국소부위 온열처치와 동적 준비운동이 하지의 운동역학적 변인에 미치는 영향)

  • Objective: The aim of this study was to analyze kinetic variables between thermotherapy and dynamic warm-up during drop-landing. Method: Twenty male healthy subjects (Age: 21.85 ± 1.90 years, Height: 1.81 ± 0.06 cm, Weight: 68.5 ± 7.06 kg) underwent three treatments applied on the thermotherapy of femoral muscles and a dynamic warm-up. The thermotherapy was performed for 15 minutes while sitting in a chair using an electric heating pad equipped with a temperature control device. Dynamic warm-up performed 14 exercise, a non-treatment was sitting in a chair for 15 minutes. Core temperature measurements of all subjects were performed before landing at a height of 50 cm. During drop-landing, core temperature, joint angle, moment, work of the sagittal plane was collected and analyzed. All analyses were performed with SPSS 21.0 and for repeated measured ANOVA and Post-hoc was Bonferroni. Results: Results indicated that Thermotherapy was increased temperature than other treatments (p = .000). During drop-landing, hip joint of dynamic warm-up was slower for angular velocity (p < .005), and left ankle joint was fastest than other treatments (p = .004). Maximum joint moment of dynamic warm-up was smaller for three joints (hip extension: p = .000; knee flexion/extension: p = .001/.000; ankle plantarflexion: p = .000). Negative work of dynamic warm-up was smaller than other treatments (p = .000). Conclusion: In conclusion, the thermotherapy in the local area doesn't affect the eccentric contraction of the thigh. The dynamic warm-up treatment minimized the joint moment and negative work of the lower joint during an eccentric contraction, it was confirmed that more active movement was performed than other treatment methods.

In Vitro Immune-Enhancing Activity of Ovotransferrin from Egg White via MAPK Signaling Pathways in RAW 264.7 Macrophages

  • Ovotransferrin (OTF) is a well-known protein of the transferrin family with strong iron chelating activity, resulting in its antimicrobial activity. Furthermore, OTF is known to have antioxidant, anticancer, and antihypertensive activities. However, there have been few studies about the immune-enhancing activity of OTF. In current study, we investigated the immune-enhancing activity of OTF using the murine macrophage cells in vitro. The effect of OTF on production of pro-inflammatory mediators and cytokines were determined using Griess assay and quantitative real-time PCR. Using Neutral Red uptake assay, we confirmed the effect of OTF on phagocytic activity of macrophages. Ovotransferrin significantly increased the production of nitric oxide (NO) and secretion of inducible nitric oxide synthase (iNOS) mRNA with no cytotoxic activity. Ovotransferrin (2 mg/mL) stimulated NO production up to $31.9{\pm}3.5{\mu}M$. Ovotransferrin significantly increased the mRNA expression levels of pro-inflammatory cytokines which are tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), Interleukin-$1{\beta}$ (IL-$1{\beta}$), and IL-6: OTF (2 mg/mL) treatment increased the secretion of mRNA for TNF-${\alpha}$, IL-$1{\beta}$, and IL-6 by 22.20-, 37.91-, and 6.17-fold of the negative control, respectively. The phagocytic activity of macrophages was also increased by OTF treatment significantly compared with negative control. Also, OTF treatment increased phosphorylation level of MAPK signaling pathways. These results indicated that OTF has immune-enhancing activity by activating RAW 264.7 macrophages via MAPK pathways.

Intraventricular Antimicrobial Therapy for Intractable Ventriculitis: Two Case Reports

  • It is challenging to treat ventriculitis with parenteral treatment alone in some cases because of the difficulty involved in maintaining an appropriate level of antibiotics in cerebrospinal fluid (CSF). We report two cases of ventriculitis who did not respond to intravenous (IV) antibiotics but were successfully treated with intraventricular antibiotics using IV agents. The first case was a four-month-old male patient with X-linked hydrocephalus. He showed ventriculitis due to Klebsiella pneumoniae not producing extended-spectrum β-lactamase and susceptible to third-generation cephalosporins and gentamicin, following ventriculoperitoneal (VP) shunt. His condition did not improve during the 47 days of treatment with IV cefotaxime and meropenem. We achieved improvement in clinical presentation and CSF profile after three times of intraventricular gentamicin injection. The patient was discharged from the hospital with antiepileptic drugs. The second case was a six-month-old female patient with a history of neonatal meningitis complicated with hydrocephalus at one month of age, VP shunt at two months of age, followed by a methicillin-resistant coagulase-negative staphylococci (CoNS) shunt infection with ventriculitis after the shunt operation. CoNS ventriculitis recurred four weeks later. We failed to treat intractable methicillin-resistant CoNS ventriculitis with IV vancomycin for ten days, and thus intraventricular antimicrobial treatment was considered. Five times of intraventricular vancomycin administration led to improvement in clinical parameters. There were only neurological sequelae of delayed language development but no other major complications. Patients in these two cases responded well to intraventricular antibiotics, with negative CSF culture results, and were successfully treated for ventriculitis without serious complications.

Surface Modification of PET Irradiated by Ultra-Violet (Part I) -Transformation of Chemical Structure and Surface Properties- (UV조사를 통한 PET의 표면개질 (제1보) -화학구조 변화 및 표면특성 변화-)

  • The irradiation of Ultra-Violet (UV) is an efficient treatment for polymer to improve hydrophilic properties. 4-Channel PET knit fabrics were treated with UVA and UVC to develop functional and environment-friendly fabric. The fabric was treated with various treatment times and distances from UV lamps having different wavelength. FT-IR and XPS investigated the chemical changes. To confirm the change of surface properties, contact angle, surface energy and SEM were examined. The study of UV as a treatment for PET knit fabric shows significant changes in chemical and surface properties, which is proved by analyses. FT-IR and XPS analyses prove the augmentation of carboxylic, Hydrophilic groups on the surfaces treated by UV. The increase of water contact angle and surface energy means more water wettable and surface energy of PET film was substantially increased by UV irradiation time. The ageing after surface treatment had little influence on the surface energy of the irradiated PET film. SEM proves the surface modification of PET such as etching, bubble and crack. The negative effects are increased in accordance with increasing treatment time.

A Study on Natural Dyeing (3) - Change of physical structure and Chinese gallotannin treatment on silk fabric - (천연염색에 관한 연구 (3) - 견에 대한 Chinese gallotannin 처리 및 구조변화 -)

  • As the basic study to investigate the mechanism of improvement in the dyeing fastness of natural dyes on silk fabrics the optical behavior of tannin solution, the optimal condition of tannin treatment and the changes of the physical structure of silk fabrics were studied in the treatment of tannin. Results obtained were as follows ; 1.The K/S values of tannin-treated silk fabrics were not changed by tannin content on silk fabrics but yellowing of tannin in treatment condition. 2. As the concentration of tannin was increased in its treating condition, the tannin content and K/S vague of tannin-treated silk fabrics were increased. 3. Because tannin gave rise to yellowing by hydrolysis and oxidation at high temperature, the higher tannin-treating temperature was, the lower the tannin content and K/S values of silk fabrics 4. The tannin content of silk fabric was higher in acidic treating condition and the highest K/S value was showed at pH 5. 5. As the tannin content of silk fabric was increased, the value of ζ -potential of it became negative. 6. As the tannin content of silk fabric was increased, the crystallinity and crystallite size of it were decreased. This meant that the fraction of amorphous region in the fibers increased. And the degree of orientation of it was decreased with the tannin treatment.
