진폐환자 가족의 특성 (The Characteristics of the Families with Pneumoconiosis Patients)

  • Pneumoconiosis is a pathological change which is caused by coal dusts. It is a chronic disease that is not cured thoroughly but need treatment and nursing care through all lifetime of the patient. The family of the patient will be suffered from the physical and mental difficulties in the consequence of pneumoconiosis. The study is to find out the characteristics of the families with pneumoconiosis patients. The subjects of the study were 300 families with pneumoconiosis patients who were under medical treatment in Taeback, Donghae, Jeongsun Occupational Medical Center. The period of the study was from the 8th to the 31st of August, 1998. The data were collected by the structured questionnaires included the family intensity measurement which was translated by "Oh". The general properties of the subjects were calculated by frequency and percentage with SAS program. The followings are the summaries of the study. 1) The mean age of the pneumoconiosis patients was 62.3 years. The mean duration of diseases was 11 years and 7 months and the mean duration of hospital stay was 6 years and 4 months. 2) The mean age of the primary care giver was 55.7 years. The proportion of highschool education was 9.6% and it was quite low level compared to other primary care giver groups. 3) The average number of family members were 1.76 person(2.76 persons included patient). The economic status was somewhat high compared to other families with chronic patients. 4) The mean score of family intensity was 41.2(item mean=3.4). With the result, it is recommended to develop a program to improve the quality of family life. For example, there will be social support program for pneumoconiosis patients family sponsored by Social Insurance for Occupational Diseases.

보육교사의 성인애착, 효능감, 훈육방식과 유아의 적응간의 구조모형 분석 (Analysis on Structural Models between Educare Teachers' Adult Attachment, Self-Efficacy, Discipline Styles and the Young Children's Adjustments)

  • This research attempted to examine the effects of adult attachment which belongs to the psychological characteristics of the educare teachers on the behavior young children's adjustments through the educare teacher's self-efficacy and styles of discipline. To achieve this, a survey was conducted on 866 children aged 2-5 years attending 23 educare centers located in Seoul, the region of Gyeonggi and Gyeongsangbuk-do, and their 135 teachers in charge. The results of this research demonstrated that first, educare teachers' adult attachment had indirect effects on the young children's adjustments by the media of self-efficacy. Second, the educare teachers'adult attachment was found to have indirect effects on the young children's adjustments by the media of discipline styles. Third, the educare teachers' adult attachment was found to have effects on the young children's adjustments through self-efficacy and discipline styles. In conclusion, with regard to the effects of the educare teachers' adult attachment on the young children's adjustments the mediated effects of self-efficacy and discipline styles were confirmed. This research enables one to have an in-depth understanding of the process in which the educare teacher's psychological characteristics have important effects on the social and emotional development of young children and it is expected that it will become evidence data for realizing the need for developing practical educational programs regarding the character development and mental health of the educare teachers.

2011 동아시아 간호포럼(EAFONS) 초록분석을 통한 아시아 간호연구의 동향 분석 (Nursing Research Trends Analysis Using 2011 East Asian Forum of Nursing Scholars (EAFONS) Abstract)

  • Purpose: The purpose of this study was to identify the current status of Asian research and suggest a direction for the future development of nursing research in Asian countries Methods: To examine the current status of Asian nursing research, 539 abstracts presented at the 2011 East Asian Forum of Nursing Scholars in Seoul were analyzed according to the structured analysis format. Results: The results showed that most of the studies (77.6%) were quantitative design, but qualitative design was also conducted. Most of quantitative studies were quasi experimental designs and questionnaires are most frequently used for data collection. Only 8.5% of the studies used physiological measures. Key words were categorized into four nursing metaparadigms: clients, environment, health and nursing. The most frequently mentioned domain was health. Main themes of research were elderly, chronic disease, health promotion, and nurse/nursing management. Most frequently used key words were elderly, social support, depression, and stress. Conclusion: Major trends were similar in Asian countries, and mostly conducted with quantitative designs. Research topics were varied and major interests in nursing research topics were elderly, health promotion, and mental health in all countries. We need to develop nursing science based on closer communication and cooperation among Asian countries.

한국 근로자의 업무관련 불안감 및 우울감 경험에 대한 직장폭력의 영향 (The Influence of Workplace Violence on Work-related Anxiety and Depression Experience among Korean Employees)

  • Purpose: Work-related anxiety and depression are frequent work-related mental health problems. In this study the relationship between workplace violence and work-related anxiety and/or depression among Korean employees was evaluated. Methods: Data were obtained from the Korean Working Condition Survey of 2006. Participants were 9,094 Korean workers aged 15-64 yr. Multiple logistic regression using SAS version 9.1 was used. Results: The incidence of work-related anxiety, work-related depression and workplace violence were 4.5%, 3.5%, and 1.8% respectively. When personal and occupational risk factors were adjusted, workplace violence was significantly associated with work-related anxiety and depression (OR for anxiety: 4.07, CI: 2.62-6.34; OR for depression: 4.60, CI: 2.92-7.25). Work-related anxiety was significantly related to type of employment, working period at present workplace, work time, shift work, job demand, and social support from superiors. Factors influencing work-related depression were gender, education, alcohol consumption, company size, type of employment, working period at present workplace, work time, shift work, and job demand. Conclusion: To promote psychological health in workers there is a need to develop work-related anxiety and depression prevention programs and to decrease in workplace violence. In developing these programs, consideration should be given to personal factors, working conditions, and psychosocial working environments.

산업장의 소음폭로수준과 근로자의 스트레스 증상간의 관련성 (The Relationship between Noise Exposure Level in Worksite and Workers' Stress Symptoms)

  • Health services on industrial noisy environment have been provided only for noise-induced hearing loss management until now. But gradually, modem diseases and death have come to be related to stress and mental health deeply, therefore noise-induced mental disorder, like a stress became very important. In this point, the purpose of this study was to analyze the relationship between noise exposure level in worksite and workers' stress symptoms. This study included a survey of 786 manual workers selected from 89 worksites in 21 factories in Puchon. The results were as follows: 1. For demographic characteristics, most of the workers were males(80.8%), the $20\sim29years$ old were 34.5% and those who graduated from high school were 65.3%. The workers whose monthly income ranged from 100,000 to 800,000 won were 37.5% and who have a religion were 47.9%. 2. For occupational characteristics, workers who had worked $1\sim5$ years in the factories were 33.0%. Those who were much for them workload were 43.9% and who worked more than 8 hours a day were 73.9%. Those who were disatisfied with their job and pay were 31.9% and 50.6%. The workers who responsed ventilation condition of their worksites were bad were 51.9% and the dissatisfed with working environment of their worksites were 45.9%. 3. Workers who were suffering from tinnitus were 53.3% and those who perceived hearing loss were 50.1%. Persons who reported they always wore earplugs at work were 35.4%. Those who felt earplugs bigger than their ears were 30.6% and those who experienced eardiseases caused by earplugs were 25.6%. 4. For the noise exposure level in worksite, workers who were exposed to $80\sim90dB$ were 30.3%, $90\sim100dB$ were 26.4% and $50\sim70dB$ were 19.2%. 5. Workers' stress symptoms were significantly related to marital status and their monthly income(p<0.05). Workers who were single and had lower monthly income showed higher PSI (Psychiatric Symptom Index) scores than those who were married and had higher monthly income. Higher PSI scores were also significantly related to $1\sim2$ days hight-work per week, much for them workload, dissatisfaction with their job, and bad relationship with their bosses and coworkers. 6. Higher PSI scores were significantly related to severe tinnitus and perceived hearing losso(p<0.001). Workers who felt the earplugs they use did not fit their ears showed significantly higher PSI scores(p<0.01). Workers who reported that they did not feel they need earplugs showed. significantly higher PSI scores (p<0.05). Increased experience of eardisease caused by earplugs that did not fit were also significantly related to higher PSI scores(p<0.01). 7. The higher noise exposure level in worksite from 80dB was, the more severe stress symptoms including PSI subparts were reported; Anxiety, Anger, Depression, and Cognitive disorder(p<0.001). 5. According to the results of stepwise multiple regression analysis, factors affecting workers' PSI scores were perceived hearing loss$(R^2=0.160)$, noise exposure level in worksite$(R^2=0.110)$, realtionship with coworkers, amount of workload, monthly income and relationship with bosses orderly and the total $R^2$ of this 6 factors was 0.371. 9. The most significant factors that have impact on manual workers' stress symptoms were perceived hearing loss and noise exposure level in worksite, especially noise exposure level in the worksite was the most affective factor on the depression symptom.

대학생의 성별에 따른 스트레스 및 성격특성 비교 - Lim's Character Style Inventory를 중심으로 (Gender Differences of Perceived Stress Type, Stress Response and Personality Characteristics of Lim's Character Style Inventory and Vulnerability of Personality in College Students)

  • Purpose: The purpose of this study was to investigate perceived stressor, the characteristics of personality and stress responses of college students and to compare their gender differences. Method: This study was cross-sectional survey and participants were 146 college students. Data was collected by Perceived Daily Life Stressor Scale of college students, Lim's Character Style Inventory, Vulnerability of Personality, and Stress Response Index, demographic questionnaire from May to September, 2012 in two cities. Results: Friend relationship stressor(p=.024) and professor relationship stressor(p=.032) among daily life stressors of female students were significantly higher than male students. Fatigue(p=.009), anger(p=.046), somatization(p<.001) and total stress response(p=.049) of female students were significantly higher than male student. Challenge(p=.034) of LCSI of female students was significantly higher than male student and stability(p=.018) of male students was significantly higher than female students. Regression model of stress response in male college students was significant(F=35.18, p<.001) and influencing factors were vulnerability of personality and challenge($R^2$=.544). Regression model of stress response in female college students was significant(F=39.38, p<.001) and influencing factors were vulnerability of personality, perceived stress type, and sociability($R^2$=.612). Conclusion: These findings need to be reflected to develop stress management programs and mental health promotion programs for college students.

간호학 교과과정 개선을 위한 조사 연구 (A Study on improvement of curriculum in Nursing)

  • This Study involved the development of a survey form and the collection of data in an effort-to provide information which can be used in the improvement of nursing curricula. The data examined were the kinds courses currently being taught in the curricula of nursing education institutions throughout Korea, credits required for course completion, and year in-which courses are taken. For the purposes of this study, curricula were classified into college, nursing school and vocational school categories. Courses were directed into the 3 major categories of general education courses, supporting science courses and professional education course, and further subdirector as. follows: 1) General education (following the classification of Philip H. phoenix): a) Symbolics, b) Empirics, c) Aesthetics. 4) Synthetics, e) Ethics, f) Synoptic. 2) Supporting science: a) physical science, b) biological science, c) social science, d) behavioral science, e) Health science, f) Educations 3) Professional Education; a) basic courses, b) courses in each of the respective fields of nursing. Ⅰ. General Education aimed at developing the individual as a person and as a member of society is relatively strong in college curricula compared with the other two. a) Courses included in the category of symbolics included Korean language, English, German. Chines. Mathematics. Statics: Economics and Computer most college curricula included 20 credits. of courses in this sub-category, while nursing schools required 12 credits and vocational school 10 units. English ordinarily receives particularly heavy emphasis. b) Research methodology, Domestic affair and women & courtney was included under the category of empirics in the college curricula, nursing and vocational school do not offer this at all. c) Courses classified under aesthetics were physical education, drill, music, recreation and fine arts. Most college curricula had 4 credits in these areas, nursing school provided for 2 credits, and most vocational schools offered 10 units. d) Synoptic included leadership, interpersonal relationship, and communications, Most schools did not offer courses of this nature. e) The category of ethics included citizenship. 2 credits are provided in college curricula, while vocational schools require 4 units. Nursing schools do not offer these courses. f) Courses included under synoptic were Korean history, cultural history, philosophy, Logics, and religion. Most college curricular 5 credits in these areas, nursing schools 4 credits. and vocational schools 2 units. g) Only physical education was given every Year in college curricula and only English was given in nursing schools and vocational schools in every of the curriculum. Most of the other courses were given during the first year of the curriculum. Ⅱ. Supporting science courses are fundamental to the practice and application of nursing theory. a) Physical science course include physics, chemistry and natural science. most colleges and nursing schools provided for 2 credits of physical science courses in their curricula, while most vocational schools did not offer t me. b) Courses included under biological science were anatomy, physiologic, biology and biochemistry. Most college curricula provided for 15 credits of biological science, nursing schools for the most part provided for 11 credits, and most vocational schools provided for 8 units. c) Courses included under social science were sociology and anthropology. Most colleges provided for 1 credit in courses of this category, which most nursing schools provided for 2 creates Most vocational school did not provide courses of this type. d) Courses included under behavioral science were general and clinical psychology, developmental psychology. mental hygiene and guidance. Most schools did not provide for these courses. e) Courses included under health science included pharmacy and pharmacology, microbiology, pathology, nutrition and dietetics, parasitology, and Chinese medicine. Most college curricula provided for 11 credits, while most nursing schools provide for 12 credits, most part provided 20 units of medical courses. f) Courses included under education included educational psychology, principles of education, philosophy of education, history of education, social education, educational evaluation, educational curricula, class management, guidance techniques and school & community. Host college softer 3 credits in courses in this category, while nursing schools provide 8 credits and vocational schools provide for 6 units, 50% of the colleges prepare these students to qualify as regular teachers of the second level, while 91% of the nursing schools and 60% of the vocational schools prepare their of the vocational schools prepare their students to qualify as school nurse. g) The majority of colleges start supporting science courses in the first year and complete them by the second year. Nursing schools and vocational schools usually complete them in the first year. Ⅲ. Professional Education courses are designed to develop professional nursing knowledge, attitudes and skills in the students. a) Basic courses include social nursing, nursing ethics, history of nursing professional control, nursing administration, social medicine, social welfare, introductory nursing, advanced nursing, medical regulations, efficient nursing, nursing english and basic nursing, College curricula devoted 13 credits to these subjects, nursing schools 14 credits, and vocational schools 26 units indicating a severe difference in the scope of education provided. b) There was noticeable tendency for the colleges to take a unified approach to the branches of nursing. 60% of the schools had courses in public health nursing, 80% in pediatric nursing, 60% in obstetric nursing, 90% in psychiatric nursing and 80% in medical-surgical nursing. The greatest number of schools provided 48 crudites in all of these fields combined. in most of the nursing schools, 52 credits were provided for courses divided according to disease. in the vocational schools, unified courses are provided in public health nursing, child nursing, maternal nursing, psychiatric nursing and adult nursing. In addition, one unit is provided for one hour a week of practice. The total number of units provided in the greatest number of vocational schools is thus Ⅲ units double the number provided in nursing schools and colleges. c) In th leges, the second year is devoted mainly to basic nursing courses, while the third and fourth years are used for advanced nursing courses. In nursing schools and vocational schools, the first year deals primarily with basic nursing and the second and third years are used to cover advanced nursing courses. The study yielded the following conclusions. 1. Instructional goals should be established for each courses in line with the idea of nursing, and curriculum improvements should be made accordingly. 2. Course that fall under the synthetics category should be strengthened and ways should be sought to develop the ability to cooperate with those who work for human welfare and health. 3. The ability to solve problems on the basis of scientific principles and knowledge and understanding of man society should be fostered through a strengthening of courses dealing with physical sciences, social sciences and behavioral sciences and redistribution of courses emphasizing biological and health sciences. 4. There should be more balanced curricula with less emphasis on courses in the major There is a need to establish courses necessary for the individual nurse by doing away with courses centered around specific diseases and combining them in unified courses. In addition it is possible to develop skill in dealing with people by using the social setting in comprehensive training. The most efficient ratio of the study experience should be studied to provide more effective, interesting education Elective course should be initiated to insure a man flexible, responsive educational program. 5. The curriculum stipulated in the education law should be examined.

노인의 정보화 수준에 영향을 미치는 건강특성 연구 (The effect of health related characteristics on the use of information and communication technology of older adults)

  • 노년기 건강과 삶의 질 증진을 목적으로 정보통신기술(Information and Communication Technology: ICT)을 개발하고, 효과적으로 활용하기 위해서는 사용자인 노인의 개인적 특성을 다각적으로 파악하는 것이 중요하다. 본 연구는 건강 관련 요인을 세부적으로 구분하여 노인의 건강특성과 연령집단별 정보화 수준을 비교하고, 정보화 수준에 영향을 미치는 건강특성을 연령집단별로 탐색하는 것을 목적으로 한다. 정보화 수준은 정보통신기술의 9가지 기능을 사용할 수 있는 능력으로 측정하였다. 본 연구는 2017년 노인실태조사 자료를 사용하였으며, 연구대상은 65세 이상 노인 총 10,058명이다. 분석결과, 첫째, 연령집단별로 정보화 수준의 차이를 살펴보면 전기 노인(65~74세)의 정보화 수준이 가장 높고, 그다음 중기 노인(75~84세), 후기 노인(85세 이상) 순이다. 둘째, 건강특성별로 정보화수준을 비교한 결과 진단받은 만성질환 수가 적을수록, 주관적 건강상태가 높을수록, 그리고 인지기능, 우울, 시력, 청력, 보행기능, IADL이 정상인 경우는 그렇지 않은 집단에 비해 정보화 수준이 높게 나타났다. 셋째, 노인의 정보화 수준에 영향을 미치는 유의미한 요인은 연령, 학력, 가구소득, 주관적 건강, 우울, 인지, 시력제한, 보행기능제한이었다. 넷째, 정보화 수준에 영향을 미치는 건강특성은 연령집단별로 차이가 존재하였다. 전기노인(65~74세)의 경우 주관적 건강, 우울, 인지기능, 보행기능제한, IADL장애가 유의미한 영향요인이며, 중기 노인(75~84세)의 경우는 주관적 건강, 우울, 인지기능, 시력제한, 청력제한, 보행제한이 유의미한 영향요인으로 나타났다. 후기 노인(85세 이상)은 정보화 수준이 다른 연령집단에 비해 매우 낮으며, 이에 따라 건강특성의 영향도 우울과 인지기능만 유의미하였다. 본 연구결과를 통해 노인의 건강관리와 삶의 질 증진을 위한 정보통신기술 개발 및 이를 활용한 개입프로그램을 마련할 때 다양한 건강특성에 대한 고려가 필요함을 제시하였다.

소방공무원의 스트레스 및 우울의 위험 요인 : 개인 및 직무요인을 중심으로 (Risk Factors for Stress and Depression in Firefighters : Comparison of Individual and Job Related Factors)

  • Objective : The aim of this study was to investigate the individual and job related factors as risk factors for mental health of firefighters. Methods : The data of 202 fire-fighters was analyzed by using a multinomial logistic regression analysis. All participants completed self-reported questionnaires including demographics (sex, age, work duration), the Childhood Trauma Questionnaire, the Ways of Coping Checklist, the Colleague related traumatic events, the Korean occupational stress scale, the Korean Perceived Stress Scale-10, and the Beck Depression Inventory. Participants were divided into three groups according to the level of stress and depressive symptom scores: Low Stress-Low Depression (LS-LD), High Stress-Low Depression (HS-LD), and High Stress-High Depression (HS-HD). Results : A job related factor-organizational injustice-was a significant factor related to HS-LD, while individual factors such as active coping level and childhood trauma experience and a job related factor-difficult physical environment-were significantly associated with HS-HD. Conclusion : These results imply the need to take both individual and environmental approaches into account when managing the stress and depression of firefighters. More specifically, psycho-education to facilitate active coping strategy and adaptive emotional regulation at the individual level and the improvement of physical work environment of firefighters should be supported.

Comparison of Attention-Deficit/Hyperactivity Disorder Practice in Adults According to a Training Background in Child Psychiatry

  • Objectives: Awareness of attention-deficit/hyperactivity disorder (ADHD) in adults has significantly increased; however, clinical data specific to the Korean population are insufficient. Clinical experience of ADHD may differ based on whether psychiatrists have received pediatric psychiatry-specific training. In order to prepare a practice parameter for adult ADHD patients in Korea, we examined questionnaire data to observe how pediatric psychiatry training could affect clinical practice for adults with ADHD. Methods: A questionnaire about the diagnosis and treatment process was distributed to both general psychiatrists (GPsy) and child and adolescent psychiatrists (CAP) at the summer and winter workshop meetings of Korean Academy of Child and Adolescent Psychiatry. Results: In total, 142 psychiatrists participated in the survey (86 GPsy, 56 CAP). GPsy and CAP preferred pharmacotherapy (GPsy 82.40%, CAP 64.30%) as the primary treatment option and answered that the clinical psychiatric interview is the most necessary step in diagnostic assessment (GPsy 22.16%, CAP 19.00%). The GPsy responded with an optimal and average treatment duration that was shorter than that reported by CAP. Conclusion: Identification of the initial presenting symptom as the correct diagnosis and the optimal duration of pharmacotherapy differed between GPsy and CAP in practice, whereas concepts in diagnosis and treatment of ADHD in adults were similar for both groups. These results suggest the urgent need for the Continuing Medical Education program for psychiatrists treating adults with ADHD.
