대만 의료보장개혁과 교훈 (Tawian's Health Care Reform and Its Lessons)

  • Taiwan has experienced rapid economic growth during the past two decades. As a result, the demand for health care in Taiwan has increased rapidly. To meet the rising demand, Taiwan implemented a National Health Insurance (NHI) program on March 1, 1995. This program now covers more than 96 percent of Taiwan's citizens. Implementation of the NHI in 1995 represents fulfillment of a primary social and health policy goals of Taiwan. The goals of the NHI program is to eliminate financial barriers of health care for the citizens, to improve the quality of care. To achieve these goals, the NHI was designed on the following principles: 1. All Taiwan citizens are compul내교 joined the NHI program by law; 2. The NHI program provides comprehensive services; 3. The NHI is run by one single govt' subsidy; 5. The NHI adopt fee-for-services scheme to pay medical expenses and copayment to avoid abouse of medical services.   • Today, the basic problem of functional safe management of Korean Disaster Control system was separately administrated 33 acts that are relating to safe management in 13 ministries. Because of the facts that the fire service is not provided practically, the control system and risk management for safe administration are not operated, the information can not be shared with each other, and the various laws have the lacks of linkage, the National Safe System was appeared unsteady. The roles and functions of fire service have started with restriction to operate structurally and institutionally, which operational structure of fire service is becoming weak. As a result, the federal and local fire organizations have not reached yet to the institutionalization and the local fire service agencies have bias with the task regarding the fire service because of the relation between organizational structure and the local fire agency. With the enforcement of the federal and local fire system, professionality and autonomy for making policy, and dealing with changes of fire service positively, the national fire service on the policy performance can be established. Promotion of research and development and education training to strengthen innovation in technology and competition in fire industry will contribute to the firmly establishment of control system to prevent from fire, flood, terror and national disaster. This article proposed that (a) the established law and administration, agency are required efforts to effectively operate fire service system; (b) the national fire service agency, national college of fire, national institute of science fire, and national fire service hospital should be early established to make firmly policy to operate effectively and practically. These kinds of innovational acts are known the best ways of operating solid policy of national fire service system.

  • Background : In order to recognize the problems of current Korean physical therapy which does not conform to the current trend of modern society and to suggest improvement directions, we will establish a law for exclusive use of physical therapy that can guarantee the health rights of the people and contribute to the development of Korean physical. Methods : Korea's current physiotherapy system is compared with OECD member countries and WCPT member countries, and considering the expected effects and necessities that arise when the sole law is enacted, the sole law for the global trend is presented. Result : If the sole law is enacted and the physical therapist is treated solely, the effect is as follows. 1. Provision of high-quality physiotherapy services through establishment of physical therapy expertise 2. Convenient service provision 3. Reduced treatment costs due to reduced National Health Insurance fiscal expenditure 4. contributing to the improvement of medical welfare for the elderly and the disabled 5. Decreased unemployment rate due to job creation.

The Scope of Practice for Registered Nurses in 64 South Korean Laws

  • Purpose: The role of registered nurses is expanding in scope as the healthcare paradigm shifts from acute, hospital-based care to community and population-based care. Given this paradigm shift, this study explores the legal aspects of the role of a registered nurse. Methods: We used document analysis for extracting laws and legal orders related to nursing from the entirety of Korean law. Using textualism approach, we examined the contents utilizing a framework that was developed based on the role classification of community nurses by Clark in this study. Results: A total of 119 items related to nursing were derived from 64 laws. Of these, 71.4 % can be performed by people in multiple types of occupations including nurses. As a result of analyzing required qualifications, 45.4% of 119 items required additional qualifications besides registered nurse license. Analysis of workplace and activity type demonstrated that 26.1% of the 119 items were related to medical institutions, with nurses performing mostly "Client-oriented role." More than half (68.9%) were non-medical institutions, with nurses performing mostly "Delivery-oriented role." Some, however, did not stipulate the nurse's roles clearly. Conclusion: Therefore, to match the enhanced scope and responsibilities of registered nurses and to appropriately recognize, guide, and hold these nurses accountable, laws and policy must reflect these changes. In doing so, these updated laws and policies will ultimately serve as a basis for improving the quality and safety of nursing services.

  • 국내 의료기사 등에 관한 법률에서 의료기사는 의사 또는 치과의사의 지도 아래 업무를 행하는 것으로 되어있다. 하지만 실제 업무행태 및 국내외적 경향, 그리고 의료기사의 교육 수준을 고려한다면 전문성을 인정해주어야 한다. 이러한 전문적이고 독립적인 운영시스템은 의료기사의 자율성을 보장해주어야 하며, 법률적으로도 변화가 필요하다고 할 수 있다. 즉, 우수한 교육과정과 많은 나라에서 "요청", "협조" 등의 개념을 적용하는 시대적 요구에 따라 "지도" 보다는 "의뢰 및 처방"으로 변경이 필요하다. 이러한 변화는 시대적 요구라 할 수 있고 의료서비스의 향상, 그리고 사회적 기여를 위한 변화라고 할 수 있다.

재해방지정책의 개선방안에 관한 고찰 - 자연재해를 중심으로 - (A Study on the Natural Disaster Response Policy in Korea)

  • Recently, natural disasters happened to the Korea frequently. Disaster in the present society has been changed and increased it impacts and extent of danger as the society develops. So, Korean people feel uneasy about the unpredictable disaster. Therefore the capability of administration system, and cooperation and support between related organization become more necessary. This study aims to find out better ways of disaster management system which can protect people's lives and fortune from all sort of national disasters. To research policy reform, it is necessary to analyze disaster response organization(national disaster prevention countermeasures headquarters and so on), interorganizational relationship, law system etc. Especially, in this study, role appreciation of government was analyzed to get the rational policy reform measure. Suggestion : First, it is necessary Establishing a consolidated organization for disaster managing and united response organization is necessary such as Federal Emergency Management Agency(FEMA) which controls an over all aspects of disaster management. And it is necessary to develop Integrated Administrative Control System based on comprehensive plan. When the accidents happen, it needs to be many equipments and various specialized personnel. So, to manage a large scale of disaster, it is necessary to have interorganizational network system which many specialized organization including volunteer group are integrated. Also, as soon as the natural disaster happen, many professionals and volunteers are distributed to the spots of disaster in the right man in the right place. Finally, All the citizens have to keep safety rules, and also to take a safe action in ordinary daily life.

근대부터 건국 초기까지의 의약체계 법령 고찰 - 이원적 의약체계 정립을 중심으로 - (A study on the Laws and Regulations of the Medical and Pharmaceutical System in Korea from the Modern Period to the Early Days of the Republic - Focusing on the Establishment of the Dualistic Medical and Pharmaceutical System -)

  • Purpose : The purpose of this study was to analyze the history and characteristics of laws and regulations of the medical and pharmaceutical system in Korea-focusing on the Korean (Oriental) medical and pharmaceutical system-from the modern period to the early days of the Republic. We reviewed how traditional notions and categories of Oriental medicine, which were regarded as experiential and conventional, became part of the current dualistic medical and pharmaceutical system, and examined problems and effects during the course of positioning. Methods : We classified the development of the medical and pharmaceutical laws and regulations chronologically, from the Korean Empire to the beginning of the Republic. The abolishment of the traditional medical system that was based on laws and regulations of the Joseon Dynasty, the implementation of dualistic medical system in the Korean Empire, the attempt to demolish Korean (Oriental) medicine under the Japanese colonial rule, and the process of developing a statute-based continental law system were thoroughly reviewed. Results : Although the dualistic medical system was specified in legislation via the enactment of the National Medical Services Law in 1951, we found that it was actually enacted in 1963, when the laws and systems regarding the educational institution of Korean (Oriental) medicine were stably established. Moreover, the dualistic pharmaceutical system was specified in legislation through the partial amendment of the Pharmaceutical Affairs Act in 1994, but we concluded that the actual enactment was rather in 2000, when the first Korean (Oriental) pharmacist was produced. Discussions and conclusions : An effort to establish a dualistic medical system of Korean (Oriental) medicine and Western medicine during the Korean Empire bore fruit a few decades later, after the Republic of Korea was founded. It means the basis for the legal system finally took shape in spite of the numerous attempts during the Japanese colonial era and the beginning of the Republic to abolish Korean (Oriental) medical and pharmaceutical system.

  • As the rate of the elderly is proportionally increasing in society, the demands of this group and the services pertaining to them is increasing. The demand for improved services is in relation to national income, the development of medical techniques and the interests of elderly. Therefore, the demand for aged social welfare facilities and management of them is increasing. This paper will focus on the types and states of aged social welfare facilities under the revised aged social welfare law in 2008. Following this research, The Social Welfare Corporation is the highest management level. To date the establishment of more infrastructure has increased rapidly, since 2000. There are many small facilities and a few large facilities.

3년제 대학 응급구조과의 교육과정 비교 분석 - 2006학년도 시행 교육과정을 중심으로 - (Comparative Analysis on Three-Year Period Curriculum of Emergency Medical Technology of College)

  • Purpose: This study was carried out in order to provide the basic data for the curriculum standardization of emergency medical technology by analyzing the three-year period curriculum of 9 colleges. Method: This is the descriptive analysis of the curricular of 9 colleges. The analyzed variables were the distribution, credit, mean, frequency of the liberal arts, majors, clinical and on-the-job(OJT) training courses, and teaching profession subject. Results: 1. The number of whole subjects was 61.0, the number of liberal arts was 10.3, and the number of majors was 50.7. The completion credit was 128.3, credits of liberal arts were 15.5(12.2%), and credits of majors were 112.8(87.8%). 2. The range of credits of liberal arts was 8 to 21, and most of the liberal arts were done in the first year of college. 3. The distribution of the credits of the national examination for the license was as follows; the itemized emergency care subjects were 27.9 credits, the general emergency care was 18.5 credits, basic sciences were 17.7 credits, emergency patient care was 9.5 credits, and emergency medicine law was 3.2 credits. 4. The number of other major subjects were 10.0 and showed even distribution in each semester. 5. The clinical and on-the-job(OJT) training were 4.5 subjects, the credits of completion were 14.9 and these subjects were not in the first year of college. Conclusion: This results will be helpful data for the advanced development and standardization of the new curriculum by keeping pace with the environmental change, competency improvement and the need of the learners of emergency medical technology.

지역사회에서의 가정간호 접근성 제고 방안 - '서울시간호사회' 가정간호사업 분석을 토대로 - (A Study for the Enhancement of Accessibility to Community Home Nursing Care Services - The Home Nursing Care Program of Seoul Nurse Association -)

  • Recently, there has been an increasing need for long-term care and comprehensive health care services in community settings. The Ministry of Health and Welfare introduced the Hospital-Based Home Nursing Care Program in 2000. Before this initiative, there was a Home Nursing Demonstration Center, affiliated with the Seoul Nurse Association, had offered home nursing services with the financial support from the local government. since 1993, the Center's nursing staff has been engaged in a general hospital in an effort to provide home nursing care services within Korea's health care system. The purpose of this study was to analyze and identify characteristics of community-based home nursing care supplied by a community-based home nursing team engaged in a general hospital. Also. visit nursing care services provided by public health centers were evaluated in terms of accessibility and supply versus demand, to enhance the accessibility of low-income patients living in Seoul to home nursing care services. Data were collected from home nursing insurance reimbursement claims submitted by the community-based home nursing care team from March 1 to October 30 in 2001 and a questionnaire survey on home-visit nursing services of 25 public health centers in Seoul. The subjects consisted of 197 patients and 12 public health centers. The result were as follows. First, medical institution's community-based home nursing care program was better in technical quality than health-center-based home-visit nursing care. In addition. the pattern of the subject patients was similar to that of hospital-based home nursing care program. Second, there was a high demand for community-based home nursing care while only a small number of home-visiting nurses served at public health centers in Seoul. As a result, many patients could not receive adequate care. Finally, we suggest that community-based home nursing care program should be introduced in the national health system to meet the at-home care needs of severely ill low-income patients. Furthermore, to better utilize home nursing and visit-nursing care resources and offer continued care for patients in community settings, an efficient referral network should be built among related institutions. This would require improvement of reimbursement system and amendment of the law related to health insurance system and community-based home nursing care services.
