A Study of Environmental Hormone Characterisitics on Toxicity from Wooddust (목재분진의 독성에 의한 환경홀몬특성 연구)

  • This study was performed to evaluate tannin exposure by wooddusts for workers in furniture factories and to investigate the relationship between tannin exposure and sino-nasal cancer risk. In order to explore possible cytological changes leading to nasal cancer, we have examined 50 male furniture workers and 50 matched controls using brush cytology. The results we have obtained in this study were as follows: 1. The tannin contents of woods used in woodworking factories have been measured and varied from 0.43 to 8.72 mg tannic acid equivalent per gram wood, for reconstituted softwood and turpentine (Syncarpia glomuliferia) respectively. 2. Airborne tannins in wooddusts were also determined by area and personal exposure. The values of mean exposures for both methods are ranged from 3.1 to $5.0{\mu}g/m^3$ and from 4.6 to $14.5{\mu}g/m^3$ in furniture manufactures. 3. Over nasal cytology scores 2, the scores of study group were slightly more than control group and this kind of metaplasias seemed to be occurred over $2mg/m^3$ wooddust and $6{\mu}g/m^3$ tannin exposure. Keratinising squamous metaplasia was investigated at nasal cytology score 3 and $10{\mu}g/m^3$ tannin exposure. The nasal cytology score 4 was seemed to be atypical squamous metaplasia. 4. To find out contributing factors to nasal cytology change, odds ratio that is one of fundmental biostatistics was applied. Actually the relationship between wooddust, tannin concentration and metaplasia were not meaningful, but the relationship between working experence more than 15 years and metaplasia was calculated as 1.83. This reveals that significant clinical abnormalities could be influenced from the years of woodworking experiences. However further research is required to evaluate the significance of the data, for the purposes of sino-nasal risk assessment, standard setting to prevent nasal cancer occurrences and possibility of changing workplace.

  • Tannins are complex polyphenolic substances present in plants. They may be classified as either hydrolysable or condensed tannins, and extracts of the latter type have displayed carcinogenic properties in animal studies (Hausen, 1981). The International Agency for Research on Cancer has determined that exposure to airborne wood dust, especially hardwood dust during furniture manufacture, is a cause of sino-nasal cancer, notably nasal adenocarcinoma. Stereotactic Radiation Therapy for Nasal Carcinoma with Cribriform Plate Destruction in Three Dogs: A Serial CT Study

  • Three dogs were referred with epistaxis and facial deformity. Computed tomography (CT) scan identified masses in the bilateral nasal cavity with soft tissue attenuation and contrast enhancement. These masses had caused adjacent bones lysis, especially lysis of cribriform plate that extended to the intracranial region. Base on histopathology and CT imaging results, tumors were diagnosed as nasal carcinomas at stage 4. Three dogs were treated with stereotactic radiation therapy (SRT). These dogs received 30-35 Gy from 3-5 daily treatments (7-10 Gy per treatment). The sizes of tumors decreased the most on follow-up CT images at one month after treatment. Recurrence was confirmed between 3 and 5 months after completing SRT. The survival time of dogs treated with SRT were 110, 190, and 210 days, respectively. This study confirmed that SRT could treat canine nasal carcinomas with cribriform plate lysis without causing serious radiation toxicities. Follow-up CT examination is considered at 1 month and 3 to 6 months after SRT to accurately evaluate the prognosis and the timing of recurrence.

High Flow Nasal Cannula Assisted Tracheostomy for Securing Airways in Patients with Acute Respiratory Distress Induced by Glottic Cancer (급성 호흡곤란이 발생한 성문암 환자에서 기도 확보를 위한 고유량 비강 캐뉼라 보조 기관절개술)

  • Tracheostomy is considered as a first treatment, when oral intubation seems to be difficult in patient who present severe upper airway obstruction. Generally, tracheostomy is performed in supine position. But if airway obstruction is severe enough to cause dyspnea, low oxygen saturation and inability to maintain supine position, tracheostomy should be performed in sitting position. The authors report a case of successful tracheostomy using high flow nasal cannula which help to maintain stable oxygen saturation and respiration in patients with laryngeal cancer.

  • Background: Defects of the nasal ala and tip have a complex three-dimensional structure that makes them challenging to reconstruct. Many reconstructive options have been described for nasal ala and tip defects, ranging from primary closure to local flaps and skin grafts. However, it is difficult to determine which method will yield the best cosmetic results in each individual case. Thus, the purpose of this study was to determine which surgical procedures for reconstructing defects of the nasal ala and tip have better cosmetic results. Methods: From 2008 to 2018, 111 patients underwent surgery to reconstruct skin defects after resection of skin cancer in the nasal ala or tip. Their charts were reviewed to obtain data on age, sex, surgical location, size of the defect, surgical method, and cosmetic results using a visual analog scale (VAS). Results: For nasal ala reconstruction, the most commonly used surgical technique was the nasolabial flap (n= 42). This method also had the highest VAS score (7/10). The most commonly selected surgical method for nasal tip reconstruction was the bilobed flap (n= 13), and bilobed flaps and primary closure had the highest VAS score (7/10). Conclusion: Nasolabial flaps showed excellent cosmetic results for the reconstruction of nasal ala defects, while primary closure and bilobed flaps yielded excellent cosmetic results for the reconstruction of nasal tip defects.

Stereotactic Body Radiation Therapy in Canine Nasal Transitional Cell Carcinoma

  • An 11-year-old neutered male Maltese presented for radiation therapy for nasal transitional cell carcinoma. Stereotactic body radiation therapy (SBRT) was provided for the owner's request. Clinical signs improved 6 days after radiation therapy. Adverse effects including alopecia and pigmentation on the facial region were observed at 21 days after radiation therapy. The first follow-up computed tomography (CT) 96 days after SBRT revealed that the nasal tumor had decreased by 60.63% compared with the pre-treatment volume. Clinical signs related with the tumor reassigned 151 days after SBRT, and the second follow-up CT revealed tumor relapse. The patient was died 238 days after SBRT due to tumor relapse. SBRT showed a good tumor control effect with relatively mild radiation toxicity relative to other radiation therapy modalities, in accordance with a previous study. Further studies are needed to establish an effective treatment protocol, such as total dose, fractional dose, and inter-fractional period, in canine malignant nasal tumors.

Accurate Evaluation and Treatment of Dyspnea in Patients with Gastrointestinal Cancer (소화기 암환자 호흡곤란의 정확한 평가와 치료)

  • Dyspnea is a common symptom among patients with gastrointestinal cancer, and a comprehensive evaluation of their respiratory function is essential. Self-reporting aids in the assessment of the degree of dyspnea, while objective examination methods are performed to identify the potential underlying causes when subjective symptoms are present. Standard treatment protocols should be followed for potentially reversible and common causes of dyspnea, such as pleural effusion, pneumonia, airway obstruction, anemia, asthma, exacerbation of chronic obstructive pulmonary disease, pulmonary thromboembolism, or drug-induced interstitial lung disease. Careful and close monitoring is required due to the high frequency of pulmonary thromboembolism and the risk of cardiovascular accidents, drug-induced interstitial lung disease, or other complications from some anticancer drugs. In case of hypoxemia with an oxygen saturation of 90% or less, palliative treatment should comprise standard oxygen therapy such as nasal cannula, mask, or high-flow nasal cannula. If non-pharmacological oxygen therapy is not effective, pain control through systemic narcotic analgesics and anti-anxiety therapy with benzodiazepines may be helpful.

Result of Radiation Therapy of Sino-nasal Cancers Using Partial Attenuation Filter (투과성 필터를 이용하여 방사선 치료를 받은 부비동 및 비암의 치료 결과)

  • [ $\underline{Purpose}$ ]: This study was to evaluate the survival and pattern of failure after radiation therapy of sino-nasal cancer using partial attenuation filer and wedged beams and to help radiotherapy planning of sino-nasal cancer. $\underline{Materials\;and\;Methods}$: Between February 1992 and March 2003, 17 patients with sino-nasal cancers underwent radiation therapy using partial attenuation filter at Dongsan Medical Center, Keimyung university. There were 9 male and 8 female patients. Patients' age ranged from 40 to 75 years (median 59 years). There were 10 patients of maxillary sinus cancer, 7 patiens of nasal cancer. The histologic type was squamous cell carcinoma in 11, adenoid cystic carcinoma in 4 and olfactory neuroblastoma in 2. The distribution of clinical stage by the AJCC system was 3 for stage II, 7 for III and 6 for IV. The five patients were treated with radiation alone and 12 patients were treated with surgery and postoperative radiation therapy. The range of total radiation dose delivered to the primary tumor was from 44 to 76 Gy (median 60 Gy). The follow-up period ranged from 3 to 173 months with median of 78 months. $\underline{Results}$: The overall 2 year survival rate and disease free survival rate was 76.4%. The 5 year and 10 year survival rate were 76.4% and 45.6% and the 5 year and 10 year disease free survival rate was 70.6%. The 5 year disease free survival rate by treatment modality was 91.6% for postoperative radiation group and 20% for radiation alone group, statistical significance was found by treatment modality (p=0.006). There were no differences in survival by pathology and stage. There were local failure in 5 patients (29%) but no distant failure and no severe complication required surgical intervention. $\underline{Conclusion}$: Radiation therapy of sino-nasal cancer using partial attenuation filter was safe and effective. Combined modality with conservative surgery and radiation therapy was more advisable to achieve loco-regional control in sino-nasal cancer. Also we considered high precision radiation therapy with dose escalation and development of multi-modality treatment to improve local control and survival rate in advanced sino-nasal cancer.

Forehead Island Flap For Nasal Reconstruction (이마 섬피판을 이용한 코재건술)

  • The nose is the most prominent area of the face, therefore susceptible to trauma and skin cancer. When small sized defect is in nasal tip, it results in disturbance of the facial harmony even if replantation, composite graft, skin graft or median forehead flap has been used for the reconstruction. So it is needed that the best method reconstruction is performed according to the degree of defect or deformity. And at the same time the physiology and anatomy of nose were clarified and its aesthetic subunits were employed. How can we cover the about 3 cm sized nasal defect in nasal tip with cartilage exposure? At first, we can think forehead island flap is most appropriate. We performed 7 cases of the forehead island flap for reconstruction of the defect in nasal tip(4 cases: cancer, 3 cases: trauma) from March, 2001 to August, 2004. This result was satisfactory in the point of texture, color, donor scar, and there were no complication such as wound disruption, infection, flap atrophy, and hematoma. The advantages of forehead island flap are: 1) No injury of deep vessel and nerve, 2) control of shape and volume, 3) Short operation time, 4) primary closure of donor site, 5) one stage operation. Also, forehead island flap can cover the defect in nose where skin graft and local flap can not cover. But, operator always must take care for flap congestion and donor site scar. We thought forehead island flap is one of the best option of reconstruction of nasal tip defect.

Effect and Tolerability of Nasal Spray of Salmon Calcitonin in the Relief of Pain due to Bone Metastasis (전이성골암에 의한 통증에 대한 Salmon Calcitonin($Miacalcic^{(R)}$) 비강분무의 효능 및 안정성)

  • Metastaic bone tumors are usually accompanied with severe pain. The treatment modalities for this pain are so variable that patients are sometimes afraid of using them. Salmon calcitonin has a function to increase beta-endorphines followed by increasing the blood level of prostaglandin and thromboxan A2, which results in analgesic effect. This drug also has been known to decrease bone resorption. There were a few reports that parenteral use of salmon calcitonin decrease the pain from metastatic bone tumor. We wanted to know the effectiveness and tolerability of nasal spray of salmon calcitonin in relieving bone pain with metastatic tumor. We analyzed the effectiveness in the aspects of pain, sleep, performance status, mobility, supplementary analgesic use. The biologic effect of salmon calcitonin was analysed with CBC, Ca/P, BUN/Cr, uric acid. Simple radiography, alkaline phosphatase, osteocalcin, pyrilink-K were used as parameters for bone change. Eighteen cases of metastatic bone tumors took nasal spray of salmon calcitonin($Miacalcic^{(R)}$, 200IU/day) for 4 weeks, to relieve bone pain. With Wilcoxon Matched-Pairs Signed Ranks Test, we could find pain decreased significantly at 3 week and mobility become improved at 4 week of salmon calcitonin use. Other parameters didn't show any significant changes. We think the analgesic effect is mainly due to effect not on the local bone lesion but on the central nervous system, and that increased dose of salmon calcitonin can induce earlier and stronger analgesic effect.
