Database Design in Ward Nursing Information System (병동간호업무 전산화를 위한 데이터베이스구축;간호업무기록지를 중심으로)

  • In the complexity and diversity of modern society, there is an urgent need for an information system which can systematically collect, manage and analyze data. Especially in the discipline of nursing, a nursing informarion system is necessary to maximize nursing resources and improve nursing care in the present system which is faced with increases in client needs and multiple changes in hospital environments. This research was done to provide a basis for the development of an integrative nursing information system for the future, by designing dababases items which were extracted from an analysis of the ward nursing information system on general wards excluding the OPD, ICU, OR and CSR with functions using a different system from the wards, and the design of output screen used the database items. The ward nursing information system was analysed through analysis of nursing practice related to recordings, such as the worksheet, kardex, and other nursing practice recordings, on 25 wards. The development of the database was the part of the construction of hospital information system and used the database development life cycle which is related to the system development life cycle. The database development steps included selection of database management system and design of a physical database following the principles of the order communication system which is been developing at Y University Hospital. Conceptual database and Logical database were designed using the base of 25 data items and fields derived from analysing the worksheet, the data items and fields derived from the kardex and other nursing practice recording, from these 19 data base tables were framed through transforming the relational database. Through this process, four types of output material for nursing practice recording which nurses can carry and use during their nursing practice were produced.

Involvement of Phosphatidylinositol 3-Kinase in the Insulin Signaling in Preimplantation Mouse Embryos (생쥐 착상전 배아의 인슐린 신호전달 과정에 Phosphatidylinositol 3-Kinase의 관련성)

  • A phosphatidylinositol 3-kinase (PI3K) is a upstream component of insulin signaling by which protein synthesis can be stimulated in many systems. To elucidate involvement of PI3K and its downstream mammalian target of rapamycin (mTOR) in the insulin signaling in pleimplantation mouse embryos, 8-cell embryos were cultured to blastocysts in the presence or absence of insulin and/or inhibitor drugs. The number of blastomeres per blastocyst, protein synthesis, and protein phosphorylation were examined. There was significant difference in embryonic development to blastocyst stage and hatching was potentiated by the insulin supplementation. The increase in the mean celt numbers per blastocyst was apparent in the insulin culture. Wortmannin, a PI3K inhibitor and rapamycin, an inhibitor of mTOR abolished the stimulatory effect of insulin on morphological development mitosis and protein synthesis. In autoradiography, phosphoproteins pp22 and pp30 which undergo phosphorylation in response to insulin were identified. Taken together, it can be suggested that PI3K and mTOR engaged in insulin signaling in the mouse embryo 8-cell onward and mediate embryotropic offset of insulin.

Predicting Factors on Fatigue in Patients with Parkinson's Disease (파킨슨병 환자의 피로와 영향요인)

  • Purpose: Fatigue is a common problem in Parkinson's disease (PD), affecting 30~65% of patients with that diagnosis. Only recently has fatigue been recognized as an important clinical feature of PD. The aim of this study was to investigate the level of fatigue and related factors in patients with PD. Methods: Between March 1, and September, 30, 2010, a sample of 181 PD patients agreed to be interviewed. Results: The female patients' PFS (Parkinson Fatigue Scale) score was higher than those of the male patients. Household income and having a Job were significantly correlated with the PFS scores. Among the disease characteristics, motor fluctuations, dyskinesia and modified Hoehn and Yahr stage were significantly correlated with the PFS scores. On stepwise regression analysis, the most important factors related to the PFS scores were depression and sleep disturbance. Conclusion: Fatigue in patients with PD was associated with many factors and strongly associated with depression and sleep disturbance. Fatigue is a multidimensional construct; therefore, multidimensional strategies for relieving specific aspects of fatigue are needed.

Voters' Third-Person Perceptions -based on the Media Effect on the Presidential Candidates Images and Choice- (유권자의 제3자 효과 지각 연구 -후보자 이미지와 후보 선택에 미치는 미디어 효과를 중심으로-)

  • Based on the third-person effect hypothesis, this study conducted a nation-wide online survey to assess how Korean voters perceived the mass media's effect on the candidates' image and voting behavior during the 17th presidential election. The research results showed that the voters tended to perceive that the mass media such as newspaper, television and the Internet had a greater effect on others than on themselves with regards to the formation of the three candidates' images. The third-person effect on the voting behavior was also revealed differently in terms of the medium according to age and political tendency of the voters. For instance, the younger and liberal voters were likely to see newspaper as having a greater influence on other voters' choice of candidate, while the older voters saw TV as having a greater effect on other voters. The conservative tendency did not affect the perception of the voters at all. Another noteworthy result was that personal characteristics of the candidates' images such as appearances and communication skills did not affect the voters' behaviors in the election process.

  • It is well known that the Capon algorithm offers better resolution compared to that of the FM (Fourier method) algorithm by minimizing the total output power while maintaining a constant gain in the look direction. Unfortunately, the DoA (Direction of Arrival) estimation performance of the Capon algorithm is drastically degraded when the SNR of received signal is low and thus, it cannot distinguish among signal sources which have similar incidence angles. In this paper, we propose a novel scheme enhancing the resolution of the Capon algorithm by ing all rows except the first row of an inverse covariance matrix.

A Pilot Study of APN-led Self-management Program to Improve Cardiovascular Health Status among Korean Women with Risk Factors

  • Purpose: The aim of this study was to examine the effects of an Advanced Practice Nurse (APN)-led self-management program on cardiovascular health status among Korean women at risk of developing or progressing cardiovascular disease. Methods: This pilot study used one-group pre- and post- test experimental design. At health fairs in a community, 30 women who had one or more risk factors for metabolic syndrome were recruited and agreed to participate in the study. A total of 25 women completed the study. The intervention consisted of weekly follow-up calls and self-monitoring diary after an hour of individual counseling regarding risk factors, fast walking, and healthy diet tailored to the participants' needs. Physical activity was assessed with the World Health Organization International Physical Activity Questionnaire and a pedometer. Results: Participants showed statistically significant improvements in blood pressure, body mass index, levels of triglyceride, total cholesterol and low density lipoprotein, numbers of metabolic syndrome factors, and the 10-year CV risk estimate after one month of concentrated intervention. In addition, their physical activity behavior significantly improved after the intervention. Conclusion: This APN-led self-management program targeting modifiable risk factors by offering tailored counseling and concentrated support during the transition might be effective in preventing progression to the cardiovascular disease.

Efficient Gene Delivery through the Human Transferrin Receptor of Fibroblast-like Synoviocytes Stimulated with bFGF: a Potential Target Receptor for Gene Transduction in Rheumatoid Arthritis

  • Efficient gene delivery to specific tissues, such as inflammatory and cancerous tissues, is currently a major concern in disease treatment. The human transferrin receptor (hTR) has been detected in the synovium and fibroblast-like synoviocytes (FLS), which raises the possibility that expression of hTR is associated with the pathogenesis of rheumatoid arthritis (RA). To investigate whether the hTR is a useful target for gene transduction into the FLS of RA patients, recombinant adenoviruses with wildtype fiber (AdLac) and transferrin peptide-tagged fiber (Tf-AdLac) were used. The hTR expression level in FLS was notably increased by basic fibroblast growth factor (bFGF). Gene transduction to FLS was significantly higher by the hTR-targeted adenovirus than by the AdLac adenovirus, and treatment of the FLS with bFGF resulted in increased gene transduction by Tf-AdLac. Taken together, these data support Tf-AdLac as a new strategy for gene transduction in the treatment of RA patients.

  • Development of various medical systems was accomplished through the progress of biotechnological method for therapy of human diseases. Furthermore, drug delivery systems have been investigated to carry the bioactive materials such as drug or gene in the body effectively. The most important thing in this system is to develop biomedical polymers having biocompatibility, biodegradability, and non-toxicity. Chitosan, a natural polymer, has been importantly considered as biomedical materials due to its good biocompatibility and various bio-active characteristics. Since the property of chitosan is differently explained according to the crystalline structures of chitin, the study for structural analysis of chitin has to proceed to apply as a biomaterial. From this point of view, this article introduced the analysis of crystalline structural of chitin, general property of chitosan and potential characteristics of low molecular weight water-soluble chitosan (LMWSC) as a biomaterials. Furthermore, chemical modification of LMWSC using various functional groups was also performed to enhance its bioavailability and emphasize their potential as drug delivery carriers (DDS).

Preparation and Characterization of the Histidine-graft-Low Molecular Weight Water-Soluble Chitosan as a Gene Carrier (유전자 전달체로서 히스티딘이 결합된 저분자량 수용성 키토산의 제조와 특성)

  • To improve transfection efficiency, we prepared histidine-low molecular weight water-soluble chitosan (LMWSC) having the potential to form complex with DNA as a cationic polymer. Histidine-LMWSC was synthesized by the esterification reaction and removing phthaloyl group. The histidine-LMWSC was characterized using FT-IR, $^1H$ NMR spectra. Histidine-LMWSC was complexed with plasmid DNA (pDNA) in various polymer/DNA (N/P) weight ratios, and the complex was identified using gel retardation assay. The particle sizes of the hisitidine-LMWSC/DNA complexes were measured on a DLS instrument by fixing the histidine-LMWSC/DNA weight ratio of 10/1. Owing to the utilization of a large excess amount of cationic LMWSC against anionic DNA, the particle size of histidine-LMWSC/DNA complexes was in the range of 100~200 nm. Therefore, histidine-LMWSC will be useful in the development of gene carriers.

Antibacterial Activity of Low Molecular Weight Water-Soluble Chitosan (저분자량 수용성 키토산의 항균 활성에 관한 연구)

  • Chitosan is a natural polymer derived from chitin that has been widely used as a dietary supplement and in a variety of pharmacological and biomedical applications. In addition, water-soluble chitosan has been used to enhance the stability of chitosan in water and reduce cytotoxic activity induced by acetic acid. In this study, the antibiotic activity and mechanism of low molecular weight water-soluble chitosan (LMWSC; MW1, MW3, MW5, and MW10) were examined in pathogenic bacteria cells and vesicles containing bacterial membrane lipids. MW10 displayed potent antibacterial activity against pathogenic bacteria strains and no cytotoxicity against mammalian cells. In addition, the degree of calcein leakage was examined as a function of lipid composition (PE/PG=7/3 w/w). The results of these experiments demonstrated that MW10 promoted leakage in negatively-charged membranes. Furthermore, confocal microscopy revealed that MW10 was located in the plasma membrane.
