Medication-related osteonecrosis of the jaw: a preliminary retrospective study of 130 patients with multiple myeloma

  • Background: Multiple myeloma (MM) is characterized by a neoplastic proliferation of plasma cells primarily in the bone marrow. Bisphosphonates (BP) are used as supportive therapy in the management of MM. This study aimed to analyze the incidence, risk factors, and clinical outcomes of medication-related necrosis of the jaw (MRONJ) in MM patients. Methods: One hundred thirty MM patients who had previous dental evaluations were retrospectively reviewed. Based on several findings, we applied the staging and treatment strategies on MRONJ. We analyzed gender, age, type of BP, incidence, and local etiological factors and assessed the relationship between these factors and the clinical findings at the first oral examination. Results: MRONJ was found in nine male patients (6.9%). The mean patient age was 62.2 years. The median BP administration time was 19 months. Seven patients were treated with a combination of IV zoledronate and pamidronate, and two patients received single-agent therapy. The lesions were predominantly located in the mandible (n = 8), and the most common predisposing dental factor was a history of prior extraction (n = 6). Half of the MRONJ were related to diseases found on the initial dental screen. Patients with MRONJ were treated with infection control and antibiotic therapy. When comparing between the MRONJ stage and each factor (sign, location, etiologic factor, BP type, treatment, and outcome), there were no significant differences between stages, except for between the stage and sign (with or without purulence). Conclusions: For prevention of MRONJ, we recommend routine dental examinations and treatment prior to starting BP therapy.

  • 사용자 인터페이스 기술은 사용자의 선택에 따른 UI를 적용할 수 있는 기술로 최근 사용자 편리를 위한 인터페이스에 대한 연구가 활발하게 이루어지고 있다. 이를 위해 W3C에서도 다양한 디바이스 환경에서 N-스크린 서비스, 일관된 서비스 제공 및 사용자의 선호도에 따른 UI 적용 서비스를 지원하기 위한 다양한 연구가 진행 중에 있다. 이에 본 논문에서는 사용자 편의를 위한 UI를 개발하는 데 있어 기본적인 모델 기반의 사용자 인터페이스 기술에 대해 연구한다. 이를 위해, 사용자 인터페이스 요소 기술 분석, 사용자 인터페이스 표준 기술 분석, MBUI 표준 규격 개선 방안 도출을 통해 이를 검증하는 과정에 대해 연구하였다. 이는 웹 응용 어플리케이션 적용 방안 기술 확보 및 차세대 웹 어플리케이션을 적용하는데 활용될 것이다.

나노질화 처리를 통한 사출금형 특성 연구

  • 자동차 차체부품에 적용되는 플라스틱 소재는 강도와 내마모성, 내충격성의 충분한 물성확보가 필요하며, 이에 플라스틱 소재의 기계적 특성 향상을 위해 유리 섬유가 다량 함유된 복합소재적용이 증가하고 있다. 반면 플라스틱이 고강도화함에 따라 제품 성형을 위한 사출 금형을 손상시키는 사례가 빈번하게 발생하고, 소재의 유동성 저하에 따른 사출 불량이 증가하고 있어 고강성 플라스틱 복합소재에 대응하는 고경도, 고내마모 특성이 부여된 사출 금형의 개발이 시급한 실정이다. 특히 사출 금형에 사용되는 소재는 기존 소재에 비해 우수한 내마모성과 함께 고광택을 유지하는 것이 더욱 중요해졌으며, 이에 따라 유럽, 일본과 국내 연구진에 의해 다양한 연구가 진행되고 있다. 그 중에서도 일본에서 개발되어 국내에도 소개된 래디칼 질화는 기존 질화법에 표면의 화합물 층만을 제어하는 것으로 다소의 표면 광택 효과는 있으나, 플라스틱 사출에 그대로 적용하기에는 무리가 따르므로 그 용도가 극히 제한적이다. 본 연구에서 적용한 나노 질화기술은 0.1torr 이하의 고진공에서 고밀도의 플라즈마 에너지를 발생시키는 방법으로 화합물층이 없는 나노 크기의 질화층을 소재 표면에 형성시키는 기술로서, 처리 후에도 표면의 색상 및 광택의 변화가 없는 것을 특징으로 한다. 또한 표면 경도 및 피로 특성을 향상시킴으로써 금형의 내구 수명을 향상시킬 것으로 기대된다. 본 연구에서는 KP4 금형 소재를 사용하여 플라즈마 이온 질화 시험 조건에 따른 소재의 경도 및 내마모 특성을 파악하고, 미세 조직 분석 및 XRD 분석 등을 통해 내마모 특성 향상에 대한 기본 특성을 평가하였다. 또한 인장시험을 통해 인장강도, 항복강도 및 연신율을 파악하고, 이를 토대로 고주기 피로시험을 실시함으로써 S-N curve를 얻고, 이를 통해 피로 강도 및 피로 수명에 미치는 나노 질화 처리의 영향을 파악하고자 하였다. 플라즈마 이온 질화 시험은 질소와 수소 비율($N_2:H_2$), 진공도, Screen bias voltage, Bias voltage를 변화시켰으며, 챔버 온도는 $400^{\circ}C$로 고정하였으며, 처리시간도 3시간으로 고정하였다. 질소와 수소의 비율은 3:1일 때 최고의 내마모 특성을 보였으며, 진공도는 내마모 특성에 큰 영향을 미치지 않는 것으로 관찰되었다. KP4의 초기 경도값은 약 302 Hv인 반면 최적의 나노 질화처리를 거친 시편에서는 800Hv 이상의 Vickers 경도값을 보였다. SEM 미세조직 분석과 EPMA를 통한 성분 분석을 시행한 결과 표층으로부터 약 $1.5{\mu}m$의 나노질화층을 확인할 수 있었다.

Power Enhancement of ZnO-Based Piezoelectric Nanogenerators Via Native Defects Control

  • Scavenging electricity from wasteful energy resources is currently an important issue and piezoelectric nanogenerators (NGs) based on zinc oxide (ZnO) are promising energy harvesters that can be adapted to various portable, wearable, self-powered electronic devices. Although ZnO has several advantages for NGs, the piezoelectric semiconductor material ZnO generate an intrinsic piezoelectric potential of a few volts as a result of its mechanical deformation. As grown, ZnO is usually n-type, a property that was historically ascribed to native defects. Oxygen vacancies (Vo) that work as donors exist in ZnO thin film and usually screen some parts of the piezoelectric potential. Consequently, the ZnO NGs' piezoelectric power cannot reach to its theoretical value, and thus decreasing the effect from Vo is essential. In the present study, c-axis oriented insulator-like sputtered ZnO thin films were grown in various temperatures to fabricate an optimized nanogenerator (NGs). The purity and crystalinity of ZnO were investigated with photoluminescence (PL). Moreover, by introducing a p-type polymer usually used in organic solar cell, it was discussed how piezoelectric passivation effect works in ZnO thin films having different types of defects. Prepared ZnO thin films have both Zn vacancies (accepter like) and oxygen vacancies (donor like). It generates output voltage 20 time lager than n-type dominant semiconducting ZnO thin film without p-type polymer conjugating. The enhancement is due to the internal accepter like point defects, zinc vacancies (VZn). When the more VZn concentration increases, the more chances to prevent piezoelectric potential screening effects are occurred, consequently, the output voltage is enhanced. Moreover, by passivating remained effective oxygen vacancies by p-type polymers, we demonstrated further power enhancement.

Phytochemical Screening and Antibacterial Activity Coix lacryma-jobi Oil

  • Coix lacryma-jobi (Hanjeli) is known to posses anti-microbial properties. Therefore, phytochemical compounds of C. lacryma-jobi have been studied to produce novel antimicrobial agents as treatments against antibiotic-resistant bacteria.The objective of this study was to determine the phytochemical composition and antibacterial activity of the C. lacryma-jobi oil against Escherichia coli, Staphylococcus aureus, and Bacillus subtilis. The phytochemical composition of the oil was determined via gas chromatography mass spectrophotometry (GC-MS). Moreover, agar disk and agar well diffusion were employed to screen the antibacterial activity of the oil. An agar well diffusion test was implemented to determinate MIC's (minimum inhibitory concentrations). Dodecanoic acid, tetradecanoic acid, 2,3-dihydroxypropylester, 1,3-dioctanoin, N-methoxy-N-methyl-3,4-dihydro-2H-thiopyran6-carboxamide, propanamide, 5-Amino-1-(quinolin-8-yl)-1,2,3-triazole-4-carboxamide, and pyridine were identified in the C. lacryma-jobi oil. The MIC value of the oil was 0.031 g/L and the MBC of the oil was 0.125 g/L effective in all test bacteria. Dodecanoic acid displayed inhibitory activity against gram-positive and gram-negative bacteria. Therefore, our research demonstrated C. lacryma-jobi (Hanjeli) oil exhibited antibacterial activity against E. coli, S. aureus, and B. subtilis. These research suggest that C. lacryma-jobi root oil could be used for medicinal purposes; however clinical and in vivo tests must be performed to evaluate its potential as an antibacterial agent.

폴리에스터/폴리우레탄 및 나일론/폴리우레탄에 은 문양을 입힌 편직물의 신장-변형 시 전기 전도도 비교 (Comparisons of Electrical Conductivity between Polyester/Polyurethane and Nylon/Polyurethane Woven or Knitted Fabrics with Silver Paste Patterns in Elongation-Strain test)

  • The objective of this study was to investigate electrical conductivity of fabrics from polyester (PET) and Nylon (N) containing polyurethane (PU), with silver paste patterns screen-stenciled in three directions. The PET/PU and N/PU fabrics knitted or woven were uniaxially strain-recovered up to 22.5% in three times when each change in electrical resistance was simultaneously measured. This study established four variables that complexly affected electrical conductivity of these specimens; fabric structures, components, cover factors, and the percolation of silver particles. The woven or knitted fabric structures did not distinctively cause the changes in electrical resistance, however, the woven fabrics with the diagonal patterns showed their relatively high electrical resistance. The PET/PU fabrics with increasing the PET proportion generally presented the opposite propensity to its electrical conductivity. The changes in electric resistance of the PET/PU 85/15 2/1 twill and double plain fabrics instantaneously responded to the rate of elongation. The PET/PU group exhibited a reverse correlation between its cover factor and electrical resistivity. The highest electrical conductivity of the PET/PU 95/5 interlock fabric, with very few fluctuations, was attributed to the deep percolation of the silver particles that bridged the gaps between one loop and another. On the other hand, the occurrence of the silver cracks along with the elongated direction led to the immeasurably high change in electrical resistance as the strain increased.

Autism-Like Behavioral Phenotypes in Mice Treated with Systemic N-Methyl-D-Aspartate

  • Autism spectrum disorder (ASD) having core characteristics of social interaction problems and repetitive behaviors and interests affects individuals at varying degrees and comorbidities, making it difficult to determine the precise etiology underlying the symptoms. Given its heterogeneity, ASD is difficult to treat and the development of therapeutics is slow due to the scarcity of animal models that are easy to produce and screen with. Based on the theory of excitation/inhibition imbalance in the brain with ASD which involves glutamatergic and/or GABAergic neurotransmission, a pharmacologic agent to modulate these receptors might be a good starting point for modeling. N-methyl-D-aspartic acid (NMDA) is an amino acid derivative acting as a specific agonist at the NMDA receptor and therefore imitates the action of the neurotransmitter glutamate on that receptor. In contrast to glutamate, NMDA selectively binds to and regulates the NMDA receptor, but not other glutamate receptors such as AMPA and kainite receptors. Given this role, we aimed to determine whether NMDA administration could result in autistic-like behavior in adolescent mice. Both male and female mice were treated with saline or NMDA (50 and 75 mg/kg) and were tested on various behavior experiments. Interestingly, acute NMDA-treated mice showed social deficits and repetitive behavior similar to ASD phenotypes. These results support the excitation/inhibition imbalance theory of ASD and that NMDA injection can be used as a pharmacologic model of ASD-like behaviors.

대면적 실리콘 태양전지의 PDMS 도포에 의한 반사방지막 특성 (Anti-reflection Coating of PDMS by Screen-printing on Large Area of Silicon Solar Cells)

  • Solar cell is a device that converts photon energy into electrical energy. Therefore, absorption of solar spectrum light is one of the most important characteristics to design the solar cell structures. Various methods have emerged to reduce optical losses, such as textured surfaces, back contact solar cells, anti-reflection layers. Here, the anti-reflection coating (ARC) layer is typically utilized whose refractive index value is between air (~1) and silicon (~4) such as SiNx layer (~1.9). This research is to print a material called polydimethylsiloxane (PDMS) to form a double anti-reflection layer. Light with wavelength in the range of 0.3 to 1.2 micrometers does not share a wavelength with solar cells. It is confirmed that the refractive index of PDMS (~1.4) is an ARC layer which decreases the reflectance of light absorption region on typical p-type solar cells with SiNx layer surface. Optimized PDMS printing with analyzing optical property for cell structure can be the effective way against outer effects by encapsulation.

The Effect of the Active Release Technique on Balance and Functional Movement in Youth Basketball Players

  • Background: This study was conducted to apply active release techniques to male youth basketball players to help improve physical development and damage prevention and improve performance through improved balance and functional movement. Design: Randomized control trial. Methods: The subjects included 33 youth basketball players who were randomly assigned to the experimental group (n=17) and the control group (n=16). For the experimental group, the active release technique was applied to the hip muscles, calf muscles, posterior thigh muscles based on the distribution of injuries surveyed in youth basketball players in the Korean Basksetball League. The Y-balance test and the functional reach test (FRT) were used to assess balance and the Functional Movement Screen (FMS) was used to assess functional movement. Interventions were conducted twice a week for 4 weeks at 40 minutes per session. The experimental group was the active release technique group, and static stretching, a common exercise therapy technique, and self-myofascial release using a foam roller were applied for 20 minutes. The control group received general exercise therapy and placebo active release technique. The placebo active release technique applies pressure only. results:The experimental group showed a greater improvement in balance, as evidenced by the FRT, compared to the control group, which received general exercise treatment. However, there was no statistically significant difference in improvement between the 2 groups. In the case of the experimental group, the difference in the Y balance test before and after the intervention was larger than that of the control group, but there was no statistically significant difference. Significant improvement was found in functional movement, as evidence by the FMS, for the trunk stability test (p < 0.05), in-line lunge test (p < 0.05), rotational stability test (p < 0.05), total score (p < 0.05). Conclusion: In this study, the active release technique improved the balance and functional movement of young basketball players more than general exercise therapy. The application of the active release technique is therefore expected to assist in physical development, prevent damage, and improve the performance of youth basketball players.

Evaluation of Inferior Capsular Laxity in Patients with Atraumatic Multidirectional Shoulder Instability with Magnetic Resonance Arthrography

  • Objective: To compare inferior capsular redundancy by using magnetic resonance arthrography (MRA) images in patients with multidirectional instability (MDI) of the shoulder and control subjects without instability and thereby develop a screening method to identify the presence of shoulder MDI. Materials and Methods: The MRA images of patients with MDI of the shoulder (n = 65, 57 men, 8 women; mean age, 24.5 years; age range, 18-42 years) treated over an eight-year period were retrospectively reviewed; a control group (n = 65, 57 men, 8 women; mean age, 27.4 years; age range, 18-45 years) without instability was also selected. The inferior capsular redundancy was measured using a new method we named the glenocapsular (GC) ratio method. MRA images of both groups were randomly mixed together, and two orthopedic surgeon reviewers measured the cross-sectional areas (CSAs) and sagittal capsule-head ratios on oblique sagittal images, as well as the axial capsule-head ratios on axial images and GC ratios on oblique coronal images. Results: The CSAs and GC ratios were significantly higher in patients than in controls (both, p < 0.001); however, the sagittal capsule-head ratios and axial capsule-head ratios were not significantly different (p = 0.317, p = 0.053, respectively). In addition, GC ratios determined the presence of MDI more sensitively and specifically than did CSAs. A GC ratio of > 1.42 was found to be most suggestive of MDI of the shoulder, owing to its high sensitivity (92.3%) and specificity (89.2%). Conclusion: GC ratio can be easily measured and used to accurately screen for MDI of the shoulder.
