Three Cases Report of Anxiety and Depression Disorder in the Traffic Accident Patients Treated with Prescription of Kami-shoyo-san (우울, 불안 증상을 동반한 교통사고 환자의 가미소요산(加味逍遙散) 치험 3례)

  • Objectives: The purpose of this study was 1. To investigate correlation of subjective stress with patient's prognosis 2. To observe the effect of Kami-shoyo-san on anxiety and depressive disorder. Methods: We investigated 3 cases of in-patients from traffic accidents. Patients were treated with Kami-shoyo-san. The Beck Anxiety Inventory (BAI) and the Beck Depression Inventory (BDI) were used to assess anxiety and depression, respectively. We also used Qi stagnation test and Qi counterflow test according to Terasawa's criteria for diagnosis of anxiety and depression. Pain was evaluated by Numeric Rating Scale (NRS). Results: After treatment, the patient's symptoms improved considerably. Conclusions: 1. There were significant correlations between subjective stress and prognosis. 2. Kami-shoyo-san has good effect on anxiety and depressive disorder.

Effects of the Ergonomic Lap Board for Computer Mouse Padding on Upper Limb Movements, Myoelectric Activities, Task Performance, and Subjective Discomfort (인체공학적 랩보드를 이용한 컴퓨터 마우스 사용이 상지의 움직임 및 근활성도, 작업수행도, 주관적 불편도에 미치는 영향)

  • This study aimed to investigate the effect of a lap board that was developed to prevent musculoskeletal disorders caused by the use of a computer mouse. Study participants were 10 college students in their 20s who did not have any musculoskeletal symptoms. We analyzed the difference in upper limb movements, electromyographic activity, task performance, and subjective discomfort between 2 conditions: use of a mouse with the lap board and use of a mouse placed on a desk. Results of behavior analysis showed that there was a significant difference between the 2 conditions in terms of the average angle of shoulder flexion, shoulder internal rotation, and forearm pronation(p< 0.05). However, electromyographic activity, task performance, and subjective discomfort showed no significant differences between the 2 conditions. When subjects used the mouse with the lap board, their upper extremity was located much closer to the torso than when they used the mouse placed on the desk. Six of 10 participating students preferred the lap board. We expect that advanced research on prevention of musculoskeletal disorders due to the use of the computer mouse will be conducted in the near future.

Hepatic and Renal Safety of Concurrent Use of Conventional and Herbal Medications for Musculoskeletal Disorders: A Retrospective Observational Study (근골격계 질환에 대한 양약 및 한약 병용의 간과 신장에 대한 안전성: 후향적 관찰 연구)

  • Objectives This study aimed to investigate whether the concurrent use of conventional and herbal medications affects liver and kidney function, by examining blood test data. Methods We retrospectively reviewed the electronic medical records of 590 inpatients with musculoskeletal diseases between 2013 and 2017. We investigated cases of drug-induced liver injury (DILI) according to the Roussel Uclaf Causality Assessment Method criteria and cases of drug-induced kidney injury (DIKI) based on the Kidney Disease Improving Global Outcomes definition. Results One case (0.17%) of DILI and one case (0.17%) of DIKI were identified. Significant improvements in serum laboratory data were observed after the concurrent use of both types of medications (p<0.05). The kappa coefficients ranged from 0.26 to 0.72, indicating that the values after the concurrent use of conventional and herbal medications showed a fair similarity to the baseline values of the patients. The linear regression test showed that female sex and high body mass index (BMI) were risk factors for an increase in the serum blood levels of liver function parameters. Conclusions The concurrent use of conventional and herbal medications for musculoskeletal disorders is relatively safe; however, clinicians should exercise caution when prescribing these medications to female patients and patients with a high BMI because of their potential effect on hepatic function.

  • The majority of patients who present with muscle pain does not have a definable myopathic disorder in the usual sense, despite intensive evaluation. Currently, most of these individuals are diagnosed with the syndrome of fibromyalgia, a condition that has been discussed under a number of different names for approximately a century. Fibromyalgia is a form of nonarticular rheumatism characterized by widespread musculoskeletal aching and stiffness, as well as tenderness on palpation at charateristic sites, called tender points. The American College of Rheumatology in 1990 set criteria for the diagnosis of fibromyalgia as that includes at least 3 months of widespread pain and the presence of at least 11 of 18 specific tender points on examination.

Literature Review on One.Handed Manual Material Handling (한손 수동물자취급에 관한 문헌 조사)

  • By referring thirty-seven previous studies on manual material handling (MMH), this paper analyzed guidelines and main factors of one-handed tasks. The previous studies concerned main factors of distance, weight, frequency, posture, gender, age, training, direction of force, height of the force exerted, and object shape and size. Based on these factors, the criteria used to understand one-handed tasks were objective measures of maximum strength, reaction force, etc., psychophysical measures of maximum acceptable frequency and weight, etc., and physiological measures of oxygen uptake, heart rate, electromyography, etc. An allowance threshold model regarding quantitative and objective fatigue and workload would be suggested for future research. This study would be expected that it serve to establish and Korean recommendations of one-handed tasks.

  • Efforts to identify jobs or tasks having known risk factors for work-related musculoskeletal disorder can provide the groundwork for changes aimed at risk reduction. An effective identification method is the ergonomic job hazard analysis which breaks a job into its various elements or actions, describes them, measures and quantifies the ergonomics risk factors. Some analytical methods including OWAS, RULA, REBA, and NLE have been used as tools in quantifying the risk factors. But these traditional methods using worksheet or paper are difficult to explain to participants for performing the job hazard analysis in the field, and take a long time. We develop some software tools to implement the analytical methods using EXCEL programs or computer program. These tools developed in this study are faster and easier to perform the ergonomic job analysis than the traditional methods using worksheet.

  • Sandifer syndrome is a rare manifestation of gastroesophageal reflux in children, occurring in association with abnormal movement of the head and neck and recurrent episodes of seizure after feeding. This syndrome may be misdiagnosed as infantile seizure and musculoskeletal disorder and may be associated with failure to thrive, anemia, aspiration pneumonia, and esophagitis. In this paper, Sandifer syndrome in a four months old male with recurrent aspiration pneumonia, apnea, and seizure after feeding is presented.

  • Recently, modern people are increasing the incidence of various musculoskeletal diseases due to wrong posture. Prevention is possible through proper posture habit, but it is not easy to recognize posture by oneself. Various studies have been conducted to monitor persistent posture in daily life, but most studies using constrained measurement methods and high-cost measurement equipment are not suitable for daily life. In this paper, we implemented a posture discrimination system using a FSR sensor array that can induce posture correction spontaneously through sitting posture monitoring in daily life. The implemented system is designed as a cushion type so it is easy to apply to existing chair. In addition, it can identify five most common postures in everyday life, and can monitor real-time through Android-based smart-phone monitoring application. For the performance evaluation of the implemented system, each posture was measured 50 times repeatedly. As a result, 97.6% accuracy was confirmed.

Domestic and Foreign Research Trend on the Pediatric Chuna Treatment (소아 추나에 대한 국내·외 연구 동향)

  • Objectives The purpose of this review is to investigate the domestic and foreign studies of pediatric Chuna treatment and propose the directions of future studies and clinical applications. Methods We searched for the study at RISS, KISS, DBPIA, Pubmed, CNKI by keywords, '추나', 'Osteopathic', 'Chiropractic', 'Manipulation', '推拿', '導引', '按摩', After 2010. Results 1. Selected 3 domestic studies were categorized as 1 survey study and 2 case reports. Selected 41 foreign studies from Pubmed were categorized as 15 systemic reviews, 8 survey studies, 12 case reports and 6 control studies. Selected 82 foreign studies from CNKI were categorized as 10 systemic reviews, 22 case reports and 50 control studies. 2. 2 clinical domestic studies researched on idiopathic Scoliosis. The foreign clinical studies from Pubmed are 18 cases, and those studies were categorized into Premature baby care (3), Infant colic (2), ADHD (2), Congenital talipes equinovarus (1), Somatic dysfuntion (1), Nonsynostotic occipital plagiocephaly (1), Conversion disorder (1), Lower back pain (1), Chronic bilateral dorsal foot pain and stiffness (1), plantar fasciitis (1), Migraine headaches (1), Cyclic vomiting syndrome (1), Acute otitis media (1) and Cerebral palsy (1). The other 72 foreign clinical studies were from CNKI, and they studied 39 different diseases. Systematically, they studied about digestive diseases (25), respiratory diseases (20), fever (6), musculoskeletal diseases (5), nervous system diseases (5), dermatology diseases (2) and other disease states. The Chuna treatment was used in variety of studies. 3. 2 clinical domestic studies adopted techniques of Osteopathy Chuna. The foreign clinical studies from Pubmed adopted techniques of Osteopathic manipulation (10) and Chiropractic manipulation (8). The other foreign clinical studies from CNKI adopted techniques of Acupressure (69), Abdominal manipulation (23), Spinal manipulation (21), Thoracic manipulation (11), Traction manipulation (2), Muscular manipulation (2), Squeezing Sha manipulation (1), Spine correction (1), Joint manipulation (1) and Fascia manipulation (1). Conclusions In addition to musculoskeletal disorders, variety of pediatric diseases could be treated with Chuna treatment instead of acupuncture.

The Musculoskeletal Pain and Inconvenient Feeling During Hand Instruments with Mannequin and Intra-oral Cavity in Dental Hygiene Students (치위생학과 학생들의 구강 내와 마네킹에서 기구 조작시 근골격계 통증과 불편감)

  • Purpose : The purpose of this study was to analyze some factors that can cause incidence of the muscle and skeletal system trouble, by which Dental Hygiene Students examine the pain and the inconvenient feeling according to kinds in manual implements and the usability in the level of manipulating and maintaining the manual implement, in the actual training of the intra oral cavity along with mannequin given the scaling practice. Methods : Targeting 18 female juniors for the department of dental hygiene, who had directly practiced oral prophylaxis for 2 years, the questionnaire research was carried out right after the mutual practice in the intra oral cavity along with mannequin. Results : It was indicated that there is no big difficulty both in intra oral cavity and mannequin in terms of manipulating implement with a method of maintaining the rightly hand fixing or of grasping the trans formative pencil and of controlling force given manipulating the implement. How to grasp a trans formative pencil was indicated to be more difficult in the intra-oral manipulation(p<.05). Pain and inconvenient-feeling level, which occur in muscle and skeletal system during practicing the scaling in mannequin, were indicated to be in order of shoulder, wrist, neck, waist, elbow, and headache. Conclusion : There was no big difference in the pain and the inconvenient feeling in muscle and skeletal system according to mannequin and intra-oral environment given manipulating tile manual implement. However, there was difference in manipulating the implement with a method of grasping transformative pencil. The pain and inconvenient feeling in muscle and skeletal system were the highest both in shoulder and wrist.
