비선형 최적화기법을 이용한 하지근력 예측 인체역학 모형

  • A biomechanical model of lower extremity in seated postures was developed to assess muscular activities of lower extremity involved in a variety of foot pedal operations. It is found that nonlinear optimization method which has been used for modeling the articulated body segments does not predict the forces generated from biarticular muscles reasonably, so the revised nonlinear optimization scheme was employed to consider the synergistic effects of biarticular muscles in the model, assuming that the muscle forces are distributed proportionally based on their physiological cross sectional area and moment arm. The model incorporated four rigid body segments with the nine muscles to represent lower extreimity. For the model valida- tion, three male subjects performed the experiments in which EMG activities of the nine lower extremity muscles were measured. Predicted muscle forces were compared with the corresponding EMG amplitudes and it showed no statistical difference. The developed model can be used to design and to assess the pedals and foot-related equipments design.

윤축을 적용한 좌·우 주관절 신전 동작의 운동역학적 비교 연구 (Comparative Study of Biomechanical Left and Right Elbow Joint Extension Movements After Wheel Axle Application)

  • In this study, we have experimented with 9 players at the national delegate level. Although there were some differences in the average effects of 3 types of one-two straight movements after the application of wheel axle, there were no statistical differences in the case of surface reacting forces, electromyograms, and impact forces. When the right fist was impacted using the one-two straight movements and the wheel axle was applied with 3 segmentations, high impact forces were obtained for the pronation in the following order-72.01 $m/s^2$ (type 2), 70.93 $m/s^2$ (type 3), and 58.19 $m/s^2$ (type 1). Higher values of the surface reacting force were found for type 1 that did not exhibit pronation in the left foot, whereas in the case of the vertical direction of the right foot, type 2 with pronation exhibited higher values and impact forces. In the right electromyogram, high impact forces due to the activation of the muscular electric potential were obtained for lumbar erector (LE) spinae and triceps brachii (TB) with type 1; LE spina, latissimus dosi (LD), and upper trapezius (UT) with type 2; and brachioradialis (BR), UT, and rectus abdominal (RA) with type 3. Due to pronation and complex motions of the 3 pronation segmentations, the efficiency was higher for impacts due to one-two straight movements.

무치악 환자 에서 Neutral Zone 방법을 적용한 임상 증례 (Treatment of Edentulous Patient with Neutral Zone Technique : A Clinical Case)

  • The prosthodontic treatment of severely resorbed edentulous patients has been one of the frustrating areas due to extensive loss of tissues. The integrated neuromuscular balance among tongue, lip, and cheek is compromised. The retention, stability, and support are the three major factors to influence the clinical outcome. Fish described a denture as having three surface, with each surface playing an independent and important role in the over all fit, stability, and comfort of the denture. He recommended that the polished surface should be a series of inclines so that pressure from muscular activity will retain dentures. Within the denture space there is an area that has been termed the neutral zone. The neutral zone is that area in the mouth where, during function, the forces of the tongue pressing outward are neutralized by the forces of the cheeks and lips pressing inward. According to Jacobson and Krol, neuromuscular control interacts to provide retention and the relationship of polished surface of denture base to the surrounding muscular structure of orofacial capsule facilitates the stability and retention. This neutral zone concept has been demonstrated with various modification by a number of authors. The theory used to develop the denture base contours is based on the belief that the muscle should functionally mold not only the border but the entire polished surface. Lott and Walsh reported the clinical success on complete mandibular dentures with application of neutral zone concept. A number of studies demonstrated that denture stability and retention are more dependent on correct position of the teeth and correct contour of external surfaces of the denture in a severely resorbed alveolar ridge. This article presents a prosthodontic approach to treatment of a edentulous patient using neutral zone technique to improve the retention and stability of the prosthesis.

고탄성 런닝화가 생체역학적 요소에 미치는 영향 (Effect of High Elastic Running Shoes on Biomechanical Factors)

  • Objective: Shoes midsole are crucial for reducing impact forces on the lower extremity when someone is running. Previous studies report that the cushioning of running shoes make it possible to use less muscular energies. However, the well cushioned shoes result in energy loss as the shoe midsole is compressed. Cushioning reduces the load on the body, it also results in the use of more muscle energy to create propulsion force. The purpose of this study was to investigate the effect of the difference of shoe hardness & resilience on the running. Method: Shoes midsole are crucial for reducing impact forces on the lower extremity when someone is running. The purpose of this study was to investigate the effect of the difference of shoe hardness & resilience on the running. Results: In vastus lateralis muscle Activation, Type 55 were significantly higher for Type 50 and X (p=0.019, p=0.045). In Gluteus Maximus muscle activation, Type 55 was significantly lower for type 50 (p=0.005). In loading late, Type 55 and X were significantly higher for type 45 (p=0.008, p=0.006). Conclusion: The components of a shoe are very complex, and there can be many differences in manufacturing as well. Although some differences can be found in the biomechanical variables of the high elastic midsole, it is difficult to interpret the performance enhancement and injury prevention.

  • 기능성 반대교합을 가진 성장기 환자의 경우 조기에 원인을 제거하여 골격성으로 이행하는 것을 방지하고 성장조절에 의해 악골관계를 개선함으로써 정상적인 기능과 안모를 유도해야 된다. 반대교합의 치료방법중 기능형 치료장치가 있는데, 그중 $Fr\ddot{a}nkel$장치물은 작용발휘의 기반으로서 vestibular region을 이용하는 것이 특징이며, 비정상적인 근육력을 차단하여 구강내 치열 및 악궁에 대한 힘의 불균형 상태를 개선하고, 결여되어 있는 근육의 긴장도를 증대시킴으로써 근육의 힘을 치아에 직접 미치지 않고, 악골 및 치아, 치조골을 양호하게 발육시킬 수 있다는 점에서 근육의 힘이 장치를 매개로 하여 직접 치아에 전달되어 치아이동을 얻게 되는 다른 장치와 구별된다고 할 수 있다. 이에, 상악중절치 맹출시기의 전치부 반대교합을 FR III로 치료하여 다음과 같은 결론을 얻었다. 1. 상악의 전방성장과 상악전치의 순측경사가 일어났다. 2. 하악의 후하방회전에 의해 하안면고가 증가하였다. 3. 정상적인 하악골과 열성장된 상악골을 가진 경우, 수직피개교합과 역수평피개교합이 더 클수록 더 좋은 예후를 나타냈다.

  • Activity of the trunk muscles is essential for maintaining stability of the lumbar spine because of the unstable structure of that portion of the spine. The central nervous system deals with stabilization of the spine by contraction of the abdominal and multifidus muscles in anticipations of reactive forces produced by limb movement. Recent evidence indicates that the lumbar multifidus muscle and transversus abdominis muscle may be involved in controlling spinal stability. Stabilization training in neutral spine is an integrated approach of education in proper posture and body mechanics along with exercise to improve strength, flexibility, muscular and cardiovascular endurance, and coordination of movement.

C. M. Guzay의 Quadrant Theorem에 대한 고찰 (C. M. Guzay and the Quadrant Theorem)

  • Objectives: The quadrant theorem is a theorem proposed by C. M. Guzay in the field of functional, holistic dentistry. There are not much of scientific literature on the quadrant theorem. This study briefly reviewed basic concepts of quadrant theorem. Methods: A publication by Guzay and research articles were searched and reviewed. The quadrant theorem is depicted as a series of illustrations and accompanied explanations. Results: The primary concept of the quadrant theorem was presented in 1952. Based on geometric biophysics of the occlusion and related anatomical functions, physiological pivotal axis of the mandible is analyzed to occurs at the dens (the sub-atlas area). Composite muscular activity links the mandibular posture with C1-C2, which is then linked with the spinal posture. Twenty illustrations are progressively presented on the physiognomy, occlusion, and analysis of anatomical functions. The balanced distribution of the forces gives the durability of the functions in life. Conclusions: The quadrant theorem provides a functional linkage between the mandibular posture and the upper cervical vertebrae.

  • This is to report functional treatment, as mechano-therapy, of condylar fracture in cases of growing patients. Benoist's appliance was used in 3 cases condylar fractures, provides extraoral elastic forces to external pterygoid muscle. The appliance providing guided exercise of masticatory muscles induces transformative growth of trauma-tic condyles. The results were as follow; 1. Early exercise preventes ankylosis of fractured condyle. 2. Optimal physical exercise therapy induces the normal growth pattern by muscular training and bony remodeling of fractured condyle in children.

드롭랜딩 시 심박수 증가에 따른 하지의 생체역학적 차이 분석 (Analysis on Biomechanical Differences in Lower Limbs Caused by Increasing Heart Rates During Drop-landing)

  • Objective : This study aimed to understand how increased heart rates at the time of drop landing during a step test would affect biomechanical variables of the lower extremity limbs. Background : Ballet performers do more than 200 landings in a daily training. This training raises the heart rate and the fatigability of the lower extremity limbs. Ballet performance high heart rate can trigger lower extremity limb injury. Method : We instructed eight female ballet dancers with no instability in their ankle joints(mean ${\pm}$ SD: age, $20.7{\pm}0.7yr$; body mass index, $19.5{\pm}1.2kg/m^2$, career duration, $8.7{\pm}2.0yr$) to perform the drop landing under the following conditions: rest, 60% heart rate reserve (HRR) and 80% HRR. Results : First, the study confirmed that the increased heart rates of the female ballet dancers did not affect the working ranges of the knee joints during drop landing but only increased angular speeds, which was considered a negative shock-absorption strategy. Second, 80% HRR, which was increased through the step tests, led to severe fatigue among the female ballet dancers, which made them unable to perform a lower extremity limb-neutral position. Hence, their drop landing was unstable, with increased introversion and extroversion moments. Third, we observed that the increasing 80% HRR failed to help the dancers effectively control ground reaction forces but improved the muscular activities of the rectus femoris and vastus medialis oblique muscles. Fourth, the increasing heart rates were positively related to the muscular activities of the vastus medialis oblique and rectus femoris muscles, and the extroversion and introversion moments. Conclusion/Application : Our results prove that increased HRR during a step test negatively affects the biomechanical variables of the lower extremity limbs at the time of drop landing.

한 손 들기 작업과 양 손 들기 작업의 근력 능력 비교 연구 (Comparison of Muscle Strength for One-hand and Two-hands Lifting Activity)

  • Work-related musculoskeletal disorders (WMSDs) are a major problem in industries in which manual materials handling is performed by workers. To prevent these WMSDs, it is necessary to understand the muscular strength capability and use this knowledge to design job and selection and assignment of workers. Even though two-hands lifting activity of manual materials handling tasks are prevalent at the industrial site, many manual materials handling tasks which require the worker to perform one-hand lifting are also very common at the industrial site and forestry and farming. However, a few researches have been done for one-hand lifting activity of manual materials handling tasks. The objective of this study is to compare one-hand and two-hands lifting strength in terms of static and dynamic strength of the lifting activity for the ranging from the height of knuckle to elbow. It is shown in this study that the isometric lifting strength of one-hand is ranging from 54.7 to 63.3% of the one of two-hands. However, it is found that there is no significant difference between a person's isometric lifting strength for left-hand and right-hand. It is also shown that there is no significant difference between the peak force under the dynamic sub-maximal loading with one-hand and two-hands lifting activity. Similar results were obtained for the peak acceleration and peak velocity under the dynamic sub-maximal loading with one-hand and two-hands lifting activity. Isometric lifting strength at the height of knuckle was ranging from 2 to 3 times of the dynamic peak force during sub-maximal lifting. It is concluded that the dynamic peak forces under the sub-maximal loading are not highly correlated with the isometric lifting strength in similar postures.
