Case Report; Tetanus, Differential Diagnosis with Trismus (개구장애 환자에서의 감별진단, 파상풍 환자의 증례보고)

  • Tetanus is fatal neurological disease caused by Clostridium tetani on contaminated wound that is characterised by muscle spasm, muscle pain, and autonomic dysfuction. C. tetani exist on contaminated wound frequently that developed clinical tetanus under low oxygen condition. Tetanus have four symptomatic form: generalized, localized, cephalic, and neonatal. The incubation period is about 7 days and mortality is high. The commonest presenting symptom is trismus and other is stiffness of neck and back(opisthotonos), muscle spasm, dysphagia, facial pain, risus sardonicus. Trismus is primary presenting symptom in 50~75% of the cases and this have high possibility of initial visit to dental office. This case report of a patient who visit in our department with trismus as chief complaint.

A Case of Satoyoshi Syndrome Presented with Progressive Muscular Spasm and Alopecia (근 경련과 전신성 탈모증을 보인 Satoyoshi 증후군 1례)

  • Satoyoshi syndrome(generalized Komuragaeri disease) is a rare disorder of unknown cause, characterized by progressive, painful, intermittent muscle spasms and alopecia. Endocrinopathy with amenorrhea, secondary skeletal abnormalities, and diarrhea or unusual malabsorption are frequently seen. It seems that autoimmunity may play a role in its pathogenesis. We report a 13-year-old girl with characteristic manifestations of the syndrome. She was treated with intravenous gammaglobulin and Prednisolone. Painful muscle cramps were gradually improved, but the scalp condition did not change. Satoyoshi syndrome should be considered in children with unexplained muscle spasms and alopecia.

A Study of the character Chi(瘛) in Huangdineijing(黃帝內經) (『황제내경(黃帝內經)』의 '계(瘛)' 자(字)에 대한 고찰)

  • Objective : This study was undertaken to refine the concept of the character Chi(瘛) in Huangdineijing(黃帝內經), find out the difference between Chi(瘛) and Zhi(瘈) and help use them correctly. Methods : This study researched the types and frequency of Chi(瘛) used in Juzhenfangsongbanyinben(聚珍倣宋版印本) edition of Huangdineijing(黃帝內經) and the mixed usage of Chi(瘛) and Zhi(瘈) by comparing some versions of the work. I discerned the meanings of Chi(瘛) and Zhi(瘈) by using opinions of annotators and dictionaries etc. Results & Conclusion : The character Chi(瘛) was used a total of 26 times in Hwangjenaegyeong(黃帝內經). Chi(瘛) appeared by itself just 5 times, whereas it was used as a compound such as in Chizong(瘛瘲), Shunchi(瞤瘛), Xianchi(癎瘛) the rest of the time. Even though Chi(瘛) and Zhi(瘈) were mixed in their use, it appears that the character Chi(瘛) had to be used for the symptomatic myoclonus(筋間代性痙攣症狀) of a chronic convulsion(慢驚風), and the character Zhi(瘈) had to be used for rabies(狂犬病).
