소사체 치아에서의 유전자지문 분석을 위한 실험적 연구 (Experimental Study for DNA Fingerprint from Teeth of Charred Body)

  • In the field Of individual identification in forensic Science, if the body is charred, it is sometimes impossible to identify the morphologic changes and charred tissue such as blood, muscle and bone can not be identified by forensic microbiologic method such as DNA typing. So the author used the characteristics of teeth which is relatively firm compare to other organs and stable to external environment such as heat and also possess cells needed for the DNA typing. The author conducted the experiment on teeth to detect DNA related to individual identification regarding to temperature in which other charredorgans can not be detected. The experiment was done on 64 extracted third molars consisted of unheated ones, and heated teeth to $100^{\circ}C$, $150^{\circ}C$, $200^{\circ}C$ for 45 min, 90 min, and 120 min respectively and to $250^{\circ}C$ for 45 min. DNA was extracted from each tooth and amplified fragment length polymorphism procedure(AMP-FLPs) using polymerase chain reaction(PCR) was applied and observed for the matching DNA in HumTH01 and HumCD4 locus and the followings Are the results : 1. It was able to detect matching DNA in HumTH01 and HumCD4 locus in every teeth which no heating has been done. 2. It was able to detect matching DNA in HumTH01 and HumCD4 locus in every teeth heated to $100^{\circ}C$ for 45, 90 and 120 min. 3. It was able to detect matching DNA in HumTH01 and HumCD4 locus in teeth heated to $l00^{\circ}C$, $200^{\circ}C$ for 45, 90, 120 min. 4. It was impossible to detect matching DNA in HumTH01 and HumCD4 locus in teeth heated to $250^{\circ}C$. So, it is possible to extract DNA from teeth that otherwise can not be extracted from other organs in the charred body and it can be concluded that teeth are highly reliable and applicatable as forensic odontology for individual identification.

멀리건의 견인을 동반한 하지직거상 기법이 요통 환자의 제한된 하지직거상에 미치는 즉각적인 영향에 관한 연구 (A Study on the Immediate Effects of Mulligan's Straight Leg Raise with Traction Technique on Limited Straight Leg Raise of Low Back Pain Patients)

  • Background: The purpose of this study was to investigate the immediate effects of mulligan's straight leg raise with traction technique on angle of passive straight leg raise and length of hamstring muscle in patients with low back and radiate pain. Methods: Thirty one subjects participated in this study. The subjects were assigned to either the low back pain group (n=17) or the radiate pain group (n=14). Subjects in both groups received 3 times mulligan's straight leg raise with traction. All subjects were examined for the range of motion of lower extremity. The range of motion of lower extremity was composed of angle on straight leg raise and 90-90 straight leg raise. The range of motion of lower extremity was measured using a goniometer. Results: After 3 times of mulligan's straight leg raise with traction, significant improvements on the angle of straight leg raise and 90-90 straight leg raise were observed in the both groups (p<.05). However, there are no significant difference was observed between groups. Conclusion: These results suggest that mulligan's straight leg raise with traction provides an immediately effective in range of motion of lower extremity in patients with low back pain as well as radiate pain. Although more research is required on the effects of long-term mulligan's straight leg raise with traction on range of motion of lower extremity, our results can be useful to establish the standard parameters for range of motion of lower extremity in the clinical setting.

Trichinella spiralis Infection Suppressed Gut Inflammation with $CD4^+CD25^+Foxp3^+$ T Cell Recruitment

  • In order to know the effect of pre-existing Trichinella spiralis infection on experimentally induced intestinal inflammation and immune responses, we induced colitis in T. spiralis-infected mice and observed the severity of colitis and the levels of Th1, Th2, and regulatory cytokines and recruitment of $CD4^+CD25^+Foxp3^+$ T (regulatory T; $T_{reg}$) cells. Female C57BL/6 mice were infected with 250 muscle larvae; after 4 weeks, induction of experimental colitis was performed using 3% dextran sulfate sodium (DSS). During the induction period, we observed severity of colitis, including weight loss and status of stool, and evaluated the disease activity index (DAI). A significantly low DAI and degree of weight loss were observed in infected mice, compared with uninfected mice. In addition, colon length in infected mice was not contracted, compared with uninfected mice. We also observed a significant increase in production of pro-inflammatory cytokines, IL-6 and IFN-${\gamma}$, in spleen lymphocytes treated with DSS; however, such an increase was not observed in infected mice treated with DSS. Of particular interest, production of regulatory cytokines, IL-10 and transforming growth factor (TGF)-${\beta}$, in spleen lymphocytes showed a significant increase in mice infected with T. spiralis. A similar result was observed in mesenteric lymph nodes (MLN). Subsets of the population of $T_{reg}$ cells in MLN and spleen showed significant increases in mice infected with T. spiralis. In conclusion, T. spiralis infection can inhibit the DSS-induced colitis in mice by enhancing the regulatory cytokine and $T_{reg}$ cells recruitment.

  • Purpose : The purpose of this study is to investigate the effect of squat exercise using a ball on the gap between knees and Q angle of a subject with a genu-varum, and to prove the effect, to provide a clinical basis for developing into a knee correction exercise program. Methods : As a result of posture measurement through GPS, 26 female with genu-varum with a knee length of 5 cm or more were studied. The group was randomly assigned to 13 squat exercise group using ball (experimental group) and 13 general squat exercise groups (control group). The experimental group placed the ball between both knees in a position where the distance between both feet was slightly wider than the shoulder width on a flat support surface, and fixed the ball, and then squat with the start signal. The control group squats without a ball. Each group performed three sets of exercise three times a week for six weeks. Before their exercise, after three and six weeks, EMG, GPS, digital goniometer measurement, the vastus medialis (VM), the vastus lateralis (VL), and the Q-angle were measured in the squat exercise posture. EMG was measured in squat exercise posture. Results : The distance between the knees was reduced. EMG is activated in group A, the group B experimental results showed the high activity of the VL. Q-angle had increased. But the experimental group increased more than the control group. Conclusion : We have confirmed through our experiments that the distance interval between the knees during squat exercises using a ball can be reduced. Furthermore, it would also be helpful to ensure the treatment of genu-varum.

한국재래산양의 태아 및 신생아의 결장 발달에 관하여 (Development on the colon of fetuses and neonates in Korean native goats)

  • The development of colon in fetuses between 60, 90, 120 days of gestation and neonates of Korean native goats was investigated by light scanning electron microscopy. The results were summarized as follows : 1. The colonic wall appeared to be differentiated into the epithelium, lamina propria, submucosa, tunica muscularis and serosa in 60-day-old fetus. 2. The epithelium of the colonic villi was stratified in some areas and simple columnar in others at 60- and 90day-old fetus but was only observed simple columnar epithelium at 120-day-old fetus. 3. The goblet cells and the intestinal glands appeared in the colonic mucosa at 90 days of gestation and continued gradually to increase in number during gestation. 4. The well-developed villi of the colon appeared in columnar shape at 60 days of gestation and increased in length and width until 90 days of gestation but the villi appeared to be shorter and degenerated after 120 days of gestation. At birth only the rudiment trace of the villi remained. 5. The tunical muscularis of the colon was continuously developed during gestation period. The inner circular and outer longitudinal muscle layers were distinguishly observed in the colon of 90-day-old fetuses. 6. Scanning electron microscopically, the colonic villi developed in columnar shape or finger-like of fetuses at 60 days of gestation. The of the colonic villi became degenerated after 120 days of gestation. At birth only the rudiment trace of villi remained.

Comparison of Three Normalization Methods for 3D Joint Moment in the Asymmetric Rotational Human Movements in Golf Swing Analysis

  • Purpose: From the perspective of biomechanics, joint moments quantitatively show a subject's ability to perform actions. In this study, the effect of normalization in the fast and asymmetric motions of a golf swing was investigated by applying three different normalization methods to the raw joint moment. Methods: The study included 13 subjects with no previous history of musculoskeletal diseases. Golf swing analyses were performed with six infrared cameras and two force plates. The majority of the raw peak joint moments showed a significant correlation at p < 0.05. Additionally, the resulting effects after applying body weight (BW), body weight multiplied by height (BWH), and body weight multiplied by leg length (BWL) normalization methods were analyzed through correlation and regression analysis. Results: The BW, BWH, and BWL normalization methods normalized 8, 10, and 11 peak joint moments out of 18, respectively. The best method for normalizing the golf swing was found to be the BWL method, which showed significant statistical differences. Several raw peak joint moments showed no significant correlation with measured anthropometrics, which was considered to be related to the muscle coordination that occurs in the swing of skilled professional golfers. Conclusions: The results of this study show that the BWL normalization method can effectively remove differences due to physical characteristics in the golf swing analysis.

한국형 재활환자분류체계 버전 1.0 개발 (The Development of Korean Rehabilitation Patient Group Version 1.0)

  • Background: Rehabilitations in subacute phase are different from acute treatments regarding the characteristics and required resource consumption of the treatments. Lack of accuracy and validity of the Korean Diagnosis Related Group and Korean Out-Patient Group for the acute patients as the case-mix and payment tool for rehabilitation inpatients have been problematic issues. The objective of the study was to develop the Korean Rehabilitation Patient Group (KRPG) reflecting the characteristics of rehabilitation inpatients. Methods: As a retrospective medical record survey regarding rehabilitation inpatients, 4,207 episodes were collected through 42 hospitals. Considering the opinions of clinical experts and the decision-tree analysis, the variables for the KRPG system demonstrating the characteristics of rehabilitation inpatients were derived, and the splitting standards of the relevant variables were also set. Using the derived variables, we have drawn the rehabilitation inpatient classification model reflecting the clinical situation of Korea. The performance evaluation was conducted on the KRPG system. Results: The KRPG was targeted at the inpatients with brain or spinal cord injury. The etiologic disease, functional status (cognitive function, activity of daily living, muscle strength, spasticity, level and grade of spinal cord injury), and the patient's age were the variables in the rehabilitation patients. The algorithm of KRPG system after applying the derived variables and total 204 rehabilitation patient groups were developed. The KRPG explained 11.8% of variance in charge for rehabilitation inpatients. It also explained 13.8% of variance in length of stay for them. Conclusion: The KRPG version 1.0 reflecting the clinical characteristics of rehabilitation inpatients was classified as 204 groups.

Isolation, Molecular Phylogeny, and Tissue Distribution of Four cDNAs Encoding the Apolipoprotein Multigene Family in Barred Knifejaw, Oplegnathus fasciatus (Teleostei, Perciformes)

  • Lipoproteins are complexes of lipids and specific apolipoproteins that are involved in lipid transport and redistribution among various tissues. In this study, we isolated full-length apolipoprotein cDNA sequences encoding apolipoprotein A-I (apoA-I), apoE, apoC-II, and apo-14 kDa in barred knifejaw, Oplegnathus fasciatus. In addition, we reconstructed phylogenetic trees and investigated mRNA tissue distributions. Alignment analyses of amino acid sequences revealed that secondary structures of the polypeptides apoA-I, apoE, and apoC-II in barred knifejaw are well conserved with their teleostean and mammalian counterparts in terms of characteristic tandem repetitive units forming amphipathic ${\alpha}$-helices. Both the sequence alignment data and cleavage sites of apo-14 kDa indicated a clear differentiation between Percomorpha and Cypriniformes. Meanwhile, the phylogenetic trees of apolipoprotein sub-families suggested that the common ancestor prior to the split of the Actinopterygii (ray-finned fishes) and Sarcopterygii (tetrapods) would have possessed the primordial protein-encoding genes. Tissue distribution of each apolipoprotein transcript determined by semi-quantitative RTPCR showed that barred knifejaw apoA-I transcripts were more or less ubiquitously expressed in the liver, intestines, brain, muscle, spleen, and kidney. The most striking difference from previous observations on barred knifejaw was the ubiquitous expression of apoE across all somatic tissues. Barred knifejaw apoC-II showed tissue-specific expression in the liver and intestines, while the liver and brain were the major sites of apo-14kDa mRNA synthesis.

구개에 발생한 혈관성 평활근종의 보고 (Vascular leiomyoma of the palate)

  • 평활근종(leiomyoma)은 평활근에서 발생하는 양성종양으로, 두경부 영역에서는 흔치 않은 질환이다. 구강내 평활근종의 경우 혈관성 평활근종이 가장 흔하다. 구강내 혈관성 평활근종은 비교적 단단하고, 매끈한 표면을 가지며, 경계가 명확하고, 둥글고 돌출되어 있다. 대부분 무통성으로 느리게 성장하기 때문에 환자가 병원을 찾게 될 때까지 수개월에서 수년이 걸리며, 병력 및 임상검사, 방사선학적으로 특징적인 소견을 나타내지 않아 수술 후 병리조직학적 소견으로만 진단이 가능하다. 본 증례는 구개부에 연성종양을 주소로 내원한 53세의 여자 환자에서 종양 적출술을 시행, 병리조직학적으로 혈관성 평활근종으로 확진된 1예를 문헌고찰과 함께 보고한다.

일개 종합병원 입원환자의 낙상관련 특성 및 낙상발생 예측요인 (Characteristics and Risk Factors for Falls in Tertiary Hospital Inpatients)

  • Purpose: The aim of this study was to explore characteristics of and risk factors for accidental inpatient falls. Methods: Participants were classified as fallers or non-fallers based on the fall history of inpatients in a tertiary hospital in Seoul between June 2014 and May 2015. Data on falls were obtained from the fall report forms and data on risk factors were obtained from the electronic nursing records. Characteristics of fallers and non-fallers were analyzed using descriptive statistics. Risk factors for falls were identified using univariate analyses and logistic regression analysis. Results: Average length of stay prior to the fall was 21.52 days and average age of fallers was 61.37 years. Most falls occurred during the night shifts and in the bedroom and were due to sudden leg weakness during ambulation. It was found that gender, BMI, physical problems such elimination, gait, vision and hearing and medications such as sleeping pills, antiarrhythmics, vasodilators, and muscle relaxant were statistically significant factors affecting falls. Conclusion: The findings show that there are significant risk factors such as BMI and history of surgery which are not part of fall assessment tools. There are also items on fall assessment tools which are not found to be significant such as mental status, emotional unstability, dizziness, and impairment of urination. Therefore, these various risk factors should be examined in the fall risk assessments and these risk factors should be considered in the development of fall assessment tools.
