Treatment of Multidisciplinary Approach of Critical Ischemic Limb with Diabetic Foot (당뇨발과 동반된 중증 허혈성 하지에서의 다각적 접근 방법의 치료)

  • Purpose: The purpose of this study is to evaluate treatment results of multidisciplinary approach of critical ischemic limb with diabetic foot. Materials and Methods: From March 2005 to March 2012, 674 diabetic foot patients were analyzed. Among them, 85 patients were neuroarthropathic type, 383 patients were infectious type, and 206 patients were ischemic type. The subjects were 206 patients who had critical ischemic limbs and major or minor amputations were done. Various single or combined treatment method before amputation was performed. We investigated their ABI, HbA1c, main occlusion lesion, limb salvage and hospitalization period by various treatment method. Results: Major amputation was 27 cases, minor amputation was 179 cases. Mean HbA1c was 8.2%, and mean ABI was 0.66. Main occlusion lesion was 6 cases at common iliac artery, 13 cases at external iliac artery, 9 cases at internal iliac artery, 11 cases at common femoral artery, 23 cases at deep femoral artery, 52 cases at superficial femoral artery, 35 cases at popliteal artery, 40 cases at posterior tibia artery, 35 cases at anterior tibial artery, 28 cases at peroneal artery, and 13 cases at dorsalis pedis artery. Major amputations were decreased, minor amputations were increased, and hospitalization period was reduced by treatment of multidisciplinary approach. Conclusion: Treatment of multidisciplinary approach, which include preoperation percutaneus transluminal angioplasty, vascular surgery, and amputation, of critical ischemic limb with diabetic foot had advantages of limb salvage and hospitalization period reduction.

  • This work identifies the key factors influencing the success of multidisciplinary, interdisciplinary, and trans-disciplinary R&D projects in transition economies by integrating knowledge management, organizational, inter/intra-collaboration (open-innovation), and leadership perspectives, while also addressing the perspective of the science of team science, which is an integrative approach to R&D. This is followed by providing the major sub-constructs of team science and policy implications to better facilitate multidisciplinary, interdisciplinary, and transdisciplinary R&D projects in knowledge-based transition economies.

  • Kampo medicine, a Japanese traditional herbal medicine, has been used in clinical practice in Japan. The most appropriate Kampo formula should be chosen for each individual by the four diagnostic procedures. Fuku shin, the abdominal exam, is one of the most important approaches in the procedures. There are several abdominal conformations (signs) when administering Fuku shin. In Kampo medicine, psychiatric illness-marked by depression and anxiety-has been shown to be related with an abdominal conformation, Shin ka hi koh (Epigastric Obstructive Hardness). The aim was to see the occurrence of abdominal conformations in each level of depression and anxiety symptoms. Two hundred fifteen patients were assigned to high-, moderate-, or low-level psychiatric comorbidity based on the Hospital Anxiety and Depression Scale and were studied regarding the occurrence of major abdominal conformations. Moderate and high psychopathological groups showed the higher occurrence of Shin ka hi koh [Low, 21%; Moderate, 67%; High, 74%] ($p$ < 0.0001). In conclusion, moderate and high psychopathological patients showed the higher occurrence of a specific abdominal sign.

  • This study particularly looked at educational philosophy and content of Design Academy Eindhoven in Holland which provides multidisciplinary design education. For it might be important data for examining the educational substance and directivity in design that help construct education system in design for a new era among the varying design paradigm. The biggest feature of DAE education system designed for students to optimize their ability with systematic education of 8 contexts with 4 approaches through innovative specialty system. Besides, their interaction helps students major in what is individual specialty. Most of all, the study on the contents of DAE's multidisciplinary education is expected to be significant data for our design education that lacks integrative perspective in the respect that students might be able to develop their identity as an individual designer in changing society beyond the mere technical and formative level of design.

  • A multistage plan and multidisciplinary approach are the keys to successful repair in patients with pulmonary atresia (PA) with ventricular septal defect (VSD) and major aortopulmonary collateral arteries (MAPCAs). In this article, we present a multidisciplinary approach adopted to treat a patient with PA with VSD and MAPCAs associated with left pulmonary artery interruption.

  • As the importance of information security grows, the demand of professionals in information security field is continuing to increase. In developing as information security professionals, however, there are practical problems to be solved in advance. This study defines the body of essential knowledge(EBK) for information security professional development; on the other hand, this study suggests a education program as a multidisciplinary major based on the EBK.

Multidisciplinary Approach to Low Fertility Issue in Korea (저출산 대책에 대한 다학제적 접근)

  • A rapid decrease of total fertility rate to 1.08 in 2005 prompted the Korean government to plan and implement a '5-year plan for ageing society and population policy' starting from 2006. The 1st and 2nd 5-year plans had not shown any discernible impact on the fertility and the 3rd 5-year plan was launched in 2016. However, the fertility rate is going down further. The author reviewed the contents and assessment reports of the fertility promotion plan to suggest ideas for complementing the shortcomings of it. Author defined the major determinants of marriage and child birth as philosophy, politics, sense of value, social norm, culture, healthcare, and education. The plan was examined in view of these determinants. Transformation of Korea from an agricultural society to an industrialized society in a short period of time had brought about changes in most of the determinants of marriage and child birth; in particular philosophy and sense of value. These aspects were not put into consideration in the plan. Author suggested to launch a social education program for the general public to establish a sound philosophy of life, reform the sense of value on family, child birth and education, and cultivate the skill to draw a consensus through discussions on the social issues. A special program to promote marriage of women at the optimum age for child birth was proposed. The government should implement well balanced policy for economic development and labor. Multidisciplinary approach was recommended for these tasks.

Enhancing value of quality assurance rounds in improving radiotherapy management: a retrospective analysis from King Hussein Cancer Center in Jordan

  • Purpose: The quality assurance (QA) chart rounds are multidisciplinary meetings to review radiation therapy (RT) treatment plans. This study focus on describing the changes in RT management based on QA round reviews in a single institution. Materials and Methods: After 9 full years of implementation, a retrospective review of all patients whose charts passed through departmental QA chart rounds from 2007 to 2015. The reviewed cases were presented for RT plan review; subcategorized based on decision in QA rounds into: approved, minor modifications or major modifications. Major modification defined as any substantial change which required patient re-simulation or re-planning prior to commencement of RT. Minor modification included treatment plan changes which didn't necessarily require RT re-planning. Results: Overall 7,149 RT treatment plans for different anatomical sites were reviewed at QA rounds. From these treatment plans, 6,654 (93%) were approved, 144 (2%) required minor modifications, while 351 (5%) required major modifications. Major modification included changes in: selected RT dose (96/351, 27%), target volume definition (127/351, 36%), organs-at-risk contouring (10/351, 3%), dose volume objectives/constraints criteria (90/351, 26%), and intent of treatment (28/351, 8%). The RT plans which required major modification according to the tumor subtype were as follows: head and neck (104/904, 12%), thoracic (12/199, 6%), gastrointestinal (33/687,5%), skin (5/106, 5%), genitourinary (16/359, 4%), breast (104/2387, 4%), central nervous system (36/846, 4%), sarcoma (11/277, 4%), pediatric (7/251, 3%), lymphoma (10/423, 2%), gynecological tumors (2/359, 1%), and others (11/351, 3%). Conclusion: Multi-disciplinary standardized QA chart rounds provide a comprehensive and an influential method on RT plans and/or treatment decisions.

Multidisciplinary Design Optimization of Earth Observation Satellite Conceptual Design using Collaborative Optimization (Collaborative Optimization을 이용한 지구관측위성의 다분야 통합 최적 개념설계)

  • In this paper, the conceptual design procedure and results of Earth observation satellite through Multidisciplinary Design Optimization (MDO) are described. The conceptual design equations for major parameters are developed based on the established database of Earth observation satellite so far. The MDO conceptual design tool for Earth observation satellite was developed by applying the Collaborative Optimization (CO) architecture amongst several MDO architecture techniques available today. The objective for this research was set to minimize the total mass of satellite as well as satisfy all design constraints by utilizing the Sequential Quadratic Programming (SQP) algorithm. Eventually the effectiveness of MDO conceptual design tool was verified through proposing a comparison between the conceptual design results with MDO applied and the design specification of ASNARO-1 & IKONOS-2 Earth observation satellite.

  • This paper presents a framework, MADAM(Multidisciplinary Analysis and Design for Advanced Mobility). For the actual UAM operation, not only aircraft performances but also demand, cost and flight scenarios are in connection; the overall framework is essential for the multidisciplinary design. In this study, the framework is developed and introduced. Demand and cost analysis of Gimpo-Samseong line in the Seoul area using the framework is conducted as an example result. Also, future ticket prices are estimated by applying changes in the aspects of major cost components and the price, ₩76,000, is calculated with the target for maximizing the total profit in the year 2035.
