  • A successful design approach for noise barriers should be multidisciplinary because noise reduction goals influence both acoustical and non-acoustical considerations, such as maintenance, safety, physical construction, cost, and visual impact. These various barrier design options are closely related with barrier dimensions. In this study, we have proposed an optimal design method of straight noise barriers using genetic algorithm, providing a barrier having the smallest dimension and achieving the specified noise reduction at a receiver region exposed to the industry and traffic noise, to help a successful barrier design.

High stakes anesthesia: Anesthetic considerations and implications for complete dental extraction in a patient with complex comorbidities

  • This case report describes a frail, middle-aged woman with multiple comorbidities who was scheduled to undergo extraction of all remaining teeth in anticipation of cardiac quadruple valve intervention. Dental and anesthetic management of the patient are discussed. Medical care of the patient with a high burden of comorbidities requires a multidisciplinary approach even for a routine dental procedure.

  • Chronic cough is a common problem that can be refractory to medical treatment. Nonpharmaceutical management of chronic cough has an important role in well selected patients. This review article outlines the history of chronic cough management, current approaches to speech pathology management of the condition and new modalities of nonpharmaceutical treatment. There is a need for further research into nonpharmaceutical options with well described randomised control trials.

  • Locally advanced (Stage III) non-small cell lung cancer (NSCLC) accounts for approximately one third of all cases of NSCLC. Few patients with locally advanced NSCLC present with disease amenable to curative surgical resection. Historically, these patients were treated with primary thoracic radiation therapy (RT) and had poor long term survival rates, due to both progression of local disease and development on distant metastases. Over the last two decades, the use of multidisciplinary approach has improved the outcome for patients with locally advanced NSCLC. Combined chemoradiotherapy is the most favored approach for treatment of locally advanced unresectable NSCLC. There are two basic treatment protocols for administering combined chemotherapy and radiation, sequential versus concurrent. The rationale for using chemotherapy is to eliminate subclinical metastatic disease while improving local control. Sequential use of chemotherapy followed by radiotherapy has improved median and long term survival compared to radiation therapy alone. This approach appears to decrease the risk of distant metastases,, but local failure rates remain the same as radiation alone. Concurrent chemoradiotherapy has been studied extensively. The potential advantages of this approach may include sensitization of tumor cells to radiation by the administration of chemotherapy, and reduced overall treatment time compared to sequential therapy; which is known to be important for improving local control in radiation biology. This approach Improves survival primarily as a result of improved local control. However, it doesn't seem to decrease the risk of distant metastases probably because concurrent chemoradiation requires dose reductions in chemotherapy due to increased risks of acute morbidity such as acute esophageal toxicity. Although multidisciplinary therapy has led to improved survival rates compared to radiation therapy alone and has become the new standard of care, the optimal therapy of locally advanced NSCLC continues to evolve. The current issues in the multidisciplinary management of locally advanced NSCLC will be reviewed in this report.

  • The objectives of this study are to investigate core learning ability necessary for the 21st century and to develop core human resources required for social and national development in the future. The '21st Century Learning Ability Project' is to approach the existing knowledge and learning multidisciplinary and interactively based on the learning ability of each learner and individually and socially valuable themes. Even in Korea, a variety of intersubject integrative educational curriculum was attempted to cultivate learning ability. However, there are not enough teachers who can teach the differences between the content and characteristics for each subject by fully understanding them. Thus, it is difficult to apply them easily to the field of education. Thus, this study develops the multidisciplinary customized educational curriculum in order to develop the learning ability necessary for the 21st century, analyzes the reality of the educational curriculum integrating the academic knowledge in order to support this effectively. As a result, this study offers the interdisciplinary customized integrated model applicable to elementary and middle schools.

Long-term stability after multidisciplinary treatment involving maxillary distraction osteogenesis, and sagittal split ramus osteotomy for unilateral cleft lip and palate with severe occlusal collapse and gingival recession: A case report

  • In this report, we describe a case involving a 34-year-old woman who showed good treatment outcomes with long-term stability after multidisciplinary treatment for unilateral cleft lip and palate (CLP), maxillary hypoplasia, severe maxillary arch constriction, severe occlusal collapse, and gingival recession. A comprehensive treatment approach was developed with maximum consideration of strong scar constriction and gingival recession; it included minimum maxillary arch expansion, maxillary advancement by distraction osteogenesis using an internal distraction device, and mandibular setback using sagittal split ramus osteotomy. Her post-treatment records demonstrated a balanced facial profile and occlusion with improved facial symmetry. The patient's profile was dramatically improved, with reduced upper lip retrusion and lower lip protrusion as a result of the maxillary advancement and mandibular setback, respectively. Although gingival recession showed a slight increase, tooth mobility was within the normal physiological range. No tooth hyperesthesia was observed after treatment. There was negligible osseous relapse, and the occlusion remained stable after 5 years of post-treatment retention. Our findings suggest that such multidisciplinary approaches for the treatment of CLP with gingival recession and occlusal collapse help in improving occlusion and facial esthetics without the need for prostheses such as dental implants or bridges; in addition, the results show long-term post-treatment stability.

  • The purposes of this exploratory study were to interrelate the scholastic discipline of the ‘development communication’ into ‘community development’ and ‘adult education’ in terms of interests, views and theoretical backgrounds of these fields of social sciences; and to draw some implications for developing scholastic interactions among these fields to pursue common social changes of human society. Development communications provide opportunities to set goals, to decide contents, and to utilize communication media in developmental efforts. Contemporary trends of development communication, community development and adult education are concerned with indigenous, two-way, bottom-up and people-centered communication from exogenous, one-way, top-down and institution-centered communication of the past. Multidisciplinary approaches to communication concepts and methodology may increase the potentials of community development and adult education in terms of efficiency and effectiveness. Some of the development communication approaches such as traditional and folk media approach. new media or ICT(information & communication technology) approach, participatory communication for development approach, communication support development approach and mass media approach may be applicable for community development and adult education. Better understanding on development communication approaches will be needed for the adult educators as well as community development practitioners.

Therapeutic Difficulty in a Case of Supratentorial Primitive Neuroectodermal Tumor Diagnosed during Pregnancy

  • We report a supratentorial primitive neuroectodermal tumor (sPNET) in 17-year-old primipara in the second trimester her pregnancy. Magnetic resonance imaging revealed a left frontoparietal mass with solid and cystic component. Gross-total resection was achieved via a left frontoparietal craniotomy. It was decided to suspend the radiotherapy and chemotherapy until the 30 weeks of gestation. But, a sudden uncal herniation was developed due to the reccurrence of the tumor and bleeding into the tumor at the 25 weeks of gestation and the patient died after urgent decompressive surgery. sPNETs is an extremely rare brain tumor in pregnancy and only two cases were reported in the literature to date. There is no universally agreed treatment protocol for sPNETs during pregnancy and a multidisciplinary approach is required in treatment. In the present study, the clinical, histopathological features and therapeutical difficulties of sPNETs diagnosed during pregnancy was discussed with the literature review.

  • This paper proposes a modeling and controller design approach for a wind-diesel hybrid system including dump load. Wind turbine depends on nature such as wind speed. It causes power fluctuations of wind turbine. Excessive power fluctuation at stand-alone power grid is even worse than large-scale power grid. The proposed control scheme for power quality is fuzzy PI controller. This controller has advantages of PI and fuzzy controller. The proposed model is carried out by using Matlab/Simulink simulation program. In the simulation study, the proposed controller is compared with a conventional PI controller. Simulation results show that the proposed controller is more effective against disturbances caused by wind speed and load variation than the PI controller, and thus it contributes to a better quality wind-diesel hybrid power system.

  • The paper describes the improvement on concurrent subspace optimization(CSSO) via automatic differentiation. CSSO is an efficient strategy to coupled multidisciplinary design optimization(MDO), wherein the original design problem is non-hierarchically decomposed into a set of smaller, more tractable subspaces. Key elements in CSSO are consisted of global sensitivity equation, subspace optimization, optimum sensitivity analysis, and coordination optimization problem that require frequent use of 1st order derivatives to obtain design sensitivity information. The current version of CSSO adopts automatic differentiation scheme to provide a robust sensitivity solution. Automatic differentiation has numerical effectiveness over finite difference schemes tat require the perturbed finite step size in design variable. ADIFOR(Automatic Differentiation In FORtran) is employed to evaluate sensitivities in the present work. The use of exact function derivatives facilitates to enhance the numerical accuracy during the iterative design process. The paper discusses how much the automatic differentiation based approach contributes design performance, compared with traditional all-in-one(non-decomposed) and finite difference based approaches.
