Antibiotic Resistance Among Salmonella spp. Isolated from Feces of Patients with Acute Diarrhea in Gwangju Area, Korea, during 2000-2009 (2000년-2009년 광주 광역시 지역의 급성설사환자의 분변에서 분리한 살모넬라 균주에서의 항생제 내성률 조사)

  • Antibiotic susceptibility was examined for 596 Salmonella isolates from patients with acute gastroenteritis during 2000-2009 in Gwangju area in South Korea. Of 16 antibiotics tested, ampicillin resistance (43%) was the most commonly observed resistance among the 596 Salmonella sp. isolates, followed by tetracycline (35.9%), nalidixic acid (31.5%), and chloramphenicol (26.2%). Antibiotic resistance varied among serotypes: The highest resistance of S. Enteritidis and S. Typhimurium was to ampicillin (51.1%) and tetracycline (77.9%), respectively. A total of 89 resistance patterns were observed, and 26% (155/596) of Salmonella isolates were susceptible to all antibiotics tested in this study. About 21% (127/596) and 15% (87/596) of the isolates were resistant to one and two antibiotics, respectively. The rest of Salmonella isolates (227/596, 38%) were resistant to three or more antibiotic agents. The highest multi-drug resistance (MDR) was observed in serotype S. Paratyphi B (76.5%), followed by S. Typhimurium (58.2%), and S. Enteritidis (40.2%). The most common resistance pattern of MDR isolates was ampicillin-chloramphenicol-nalidixic acid-ticarcillin (36/227, 15.8%), most of which (35/36, 97.2%) were S. Enteritidis.

  • Antimicrobial resistant bacteria has recently emerged and been disseminated in livestock environments because of excessive use of antimicrobial agents for the therapeutic and growth promotion purposes to food animals. In particular, there is potential for multidrug-resistant bacteria that can be transmitted from animals to mankind. In this study, we investigated the genotypes of E. coli strains isolated from humans and chickens using multi-locus sequence typing (MLST) and antimicrobial resistance patterns by disk diffusion method along with integron study involving antimicrobial resistance mechanisms. From July 2013 to July 2014, E. coli strains isolated from clinical specimens (n=44) and poultry chickens (n=34). ST131 (n=20) was most common in human-derived E. coli. ST752 (n=7) was most common in chicken-derived E. coli, with four isolates each for ST117, ST189, and ST69. Of the 44 E. coli strains isolated from humans, 25 of had a class 1 integron, as opposed to only 11 of 34 strains in the E. coli isolated from chickens. There were differences in genotypes and antimicrobial resistance patterns between the chicken-derived and the human-derived E. coli.

  • The incidence of verotoxin-producing Escherichia coli(VTEC) was surveyed in domestic foods including hamburger, raw meats and vegetables from 1997 to 1999. The molecular biological characteristics of the isolates were analyzed. Three VTEC strain were isolated from 1,700 samples. Serotypes of those isolates were 0157 : H7, 026 H4, and 056 : Hl2, respectively. Serotype O26 : H4 produced VT I and VT II, and 055 Hl2 isolate produced VT I, however the 60 MDa plasmid DNA and eae gene were not found from both strains. One 0157 : H7 isolate produced VT II and harbour 60 MDa plasmid DNA, however eae gene was not found in the strain. Although they produced VT, it seemed that the virulence of two strains were relatively weak because of the lack of the eae gene. In addition, the serotype O157 : H7 isolate resistant to ampicillin and streptomycin, while isolates of serotype O26 : H4 and O55 : Hl2 were multi-resistant to antibiotics including ampicillin, carbenicillin , cephalothin, trimethoprim/sulfamethoxazole and tetracycline.   • Antibiotic resistant bacteria were isolated from livestock wastes and the resistance patterns were investigated using various antibiotic agents. Also, a gamma ray was tested regarding the aspects of the effect on resistance pattern and the efficiency of disinfection. Among the isolates, Esherichia coli and Brevibacterium sp. showed the most serious resistance patterns. Esherichia coli had resistance against 9 agents whereas Brevibacterium sp. against 7 agents. It can be suggested from these results that the abuse of antibiotic agents will cause a serious mutation problem even to Esherichia coli which is ubiquitous in the ecosystem. Esherichia coli could be easily controlled but Brevibacterium sp. had a moderate resistance to the gamma ray under low doses. In the case of Brevibacterium sp, more than 2.0 kGy of a radiation dose will be required in order to achieve an enhanced efficiency of disinfection.

Antibiotics Resistance and Molecular Analysis of Enterococcus Isolated from the Han-river in Korea (한강에서 분리한 장구균의 항생제 내성과 분자생물학적 분석)

  • Identification was performed in March 2008 for the 76 Enterococcus strains isolated from the Han-river, which is used as water supply for Seoul citizens. The antibiotic susceptibility, antibiotic resistant structural analysis, trans-conjugation, pulsed-field gel electrophoresis (PFGE) and multi-locus sequence typing (MLST) were also carried out for the isolated strains. Among the isolated strains, 25 strains were E. casseliflavus, 4 strains were E. faecalis and 1 strain was E. hirae. Investigation of antibiotic susceptibility indicated that 15 strains demonstrated tolerance against vancomycin, and that 11 strains of E. faecium and 4 strains of E. casseliflavus were VRE. The vanA gene detection of the VRE strains revealed that 6 E. faecium strains were vancomycin-resistant Enterococcus faecium (VREF) possessing vanA. Analyses of transposon Tn1546 structure containing vanA demonstrated that Km36 and Km37 belonged to Tn1 type, Km20 and Km38 was Tn2 type, and Km39 and Km40 was Tn3 type. PFGE disclosed that among the 6 VREF strains, Km36 and Km37 exhibited equivalent subtype, while the rest 4 strains showed subtypes different to each other. MLST for the 6 VREF strains disclosed that 3 strains were ST78, while the rest 3 strains were ST18, ST192 and ST230, respectively. All these clonal complexes were derived from CC17 which has been isolated from clinical sources. 4 strains belonged to CC78, while the rest 2 strains were CC18 and CC192, respectively.

Genetic characterization of Shigella flexneri isolated from the diarrheic patients in Seoul region (서울지역 설사환자로 부터 분리된 Shigella flexneri의 성상과 유전적 특성)

  • The shigellae are common etiological agents of bacillary dysentery in humans and primates. During four years from 2002 to 2005, 22 strains of Shigella spp. were isolated from the diarrheic patients in Seoul region. All of them were identified as S. flexneri by biochemical tests and serotyping. The prevalence of serotypes were variable by year, but the major serotypes were 2a and 3a. In an antimicrobial susceptibility test, all of the isolates were resistant to streptomycin and tetracycline, and susceptible to amikacin, kanamycin, cefoxitin, and gentamicin. All of the isolates showed the multi-resistant patterns over 3 drugs. By analysis of the plasmid profile the isolates were classified into 7 groups (P1~P7). Serotypes 2a and 2b were distributed to P1, P2, P3, and P4. Serotype 3a was differentiated to P5 and serotype 3b, to P6 and serotype 4a, to P7. PCR results showed that all isolates were positive for two virulence genes, ipaH and ial, but none of the strains had stx gene. The set1A and set1B genes were detected from 12 isolates (54.5%) that belonged to serotype 2a and 2b. The sen gene was detected from 19 isolates (86.4%). The 22 isolates showed 12 to 17 DNA fragments in the sizes ranging from 20.5 kb to 1135 kb, resulting in 13 patterns by the PFGE with Not I digestion. The PFGE patterns of the isolates showed the close relation with the serotypes, but no relations with year of isolation and antimicrobial resistance.

Survey of drug resistance in Edwardsiella tarda isolated from diseased eels(Anguilla japonica) (뱀장어 병어로부터 분리한 Edwardsiella tarda의 약제내성)

  • Ninety-six isolates of Edwardsiella tarda recovered from outbreaks of Edwardsiellosis in cultured eels(Anguilla japonica) in Kunsan, were examined for drug susceptibility, distribution and transferabilities of R plasmid. All of the E. tarda isolates examined were sensitive to gentamicin(GM), streptomycin(SM), norfloxacin(NF), and amikacin(AK). But most isolates were resistant to sulfadimethoxine(SD, 86 strains), ampicillin(AM, 84 strains), penicillin G(PM, 80 strains), nalidixic acid (NA, 67 strains), oxytetracycline(OT, 44 strains), and oxolinic acid(OA, 37 strains). Twenty different combinations of drug resistance patterns were observed : the frequently encountered pattern was SD-AM-PM-NA-OA(16 strains), SD-AM-PM-NA(14 strains), SD-AM-PM-NA-OT-OA(12 strains), SD-AM-PM-OT(10 strains), and SD-AM-PM-NA-OT(8 strains). Transferable R plasmids were found out to be carried in 78 out of 94 resistant strains, indicating that these isolates carry conjugally transferable R plasmids associated with single or multiple drugs. The frequently observed transferarble R plasmids were AM(8 strains), AM-PM-NA(8 strains), Am-SD(6 strains), PM(6 strains), and SD(6 strains) These results suggest that high dose of various antibacterials might have already been introduced to eel culture system leading to the acquirement of multi-drug resistance to wide range of antibacterials.

Apoptotic Effect of Ethanol Extracts of Bojungbangamtang and Acidic Polysaccharide of Korea Red Ginseng in a MCF7/adR Multidrug-resistance Breast Cancer Cells (항암제 내성 유방암 MCF7/adR 세포주에 대한 보정방암탕과 홍삼산성다당체의 세포고사 유도효과)

  • This study was undertaken to determine whether the 9 herbal complex induces apoptosis in human breast cancer MCF-7 cells and adriamycin-resistant MCF7/adR cells. Ethanol extracts of Bojungbangamtang (BBTE) and acidic polysaccharide of Red Ginseng (GIN) induced cell death in both MCF-7 and MCF7/adR cells. Ethanol extracts of Bojungbangamtang and acidic polysaccharide of Red Ginseng also induced $G_2/M$ cell cycle arrest and increased TUNEL positive cells in MCF7/adR cells. In addition, flow cytometric analysis revealed the decreased expression of P-glycoprotein (P-gp) in ethanol extracts of Bojungbangamtang and acidic polysaccharide of Red Ginseng treated MCF7/adR cells. Similarly, decreased protein levels of P-glycoprotein and multidrug resistance associated proteins-1 were also determined by immunocytometry in ethanol extracts of Bojungbangamtang treated MCF7/adR cells. Taken together these data indicate that ethanol extracts of Bojungbangamtang and acidic polysaccharide of Red Ginseng inhibit the function of ABC transporters such as multi drug resistance associated proteins (MRPs) and P-glycoprotein as well as induce apoptosis in MCF7/adR cells. Thus, these data suggest that ethanol extracts of Bojungbangamtang and polysaccharide of Red Ginseng can be candidates for the treatment of multidrug-resistant MCF7/adR cells.

Prevalence of Drug-resistances in Patients with Pulmonary Tuberculosis and Its Association with Clinical Characteristics at One Tertiary Referral Hospital in Pusan, Korea (부산지역 한 3차 병원으로 내원한 폐결핵 환자에서 약제 내성률과 예측인자간의 연관성)

  • Background : The incidence of drug-resistant tuberculosis has recently decreased in Korea. However, it is still one of the major obstacles in treating pulmonary tuberculosis. This study was performed to determine the prevalence and clinical characteristics associated with drug-resistance in pulmonary tuberculosis at the tertiary referral hospital in Pusan, Korea. Methods : The medical records of 138 patients, who had been diagnosed as active pulmonary tuberculosis were retrospectively reviewed, and results of drug susceptibility from May 1997 to June 2000. The relationships among those with a history of previous tuberculosis treatment, the extent of lung involvement, the presence of cavities on the initial chest X-ray films and patterns of drug resistance were analyzed. Results : The total number of patients who had drug resistance to at least one drug was 55(39.9%). Among them 34(24.6%) had resistance to isoniazid(INH) and rifampin(RFP). There was drug resistance in 20(22%) of 91 patients without previous tuberculosis therapy, and among them 9(9.9%) were multi-drug resistant. Thirty-two(74.5%) out of 47 patients with previous therapy were drug-resistant and 25(53.2%) were multidrug resistant. For all 138 patients, resistance to INH was the most common(34.1%), followed by RFP(26.1%) and ethambutol(EMB)(14.5%). Drug resistance to INH, RFP, PZA and streptomycin(SM) were independently associated with a history of previous treatment(odds ratio; 9.43, 9.09, 8.93 and 21.6 respectively, p<0.01). The extent of lung involvement on the chest films was significantly associated with the drug resistance to INH and RFP(odds ratio; 2.12 and 2.40 respectively, p<0.01). The prevalence of drug resistance to RFP, INH and RFP was significantly more common in patients with a cavitary lesion on the chest films by multivariate analysis(odds ratio; 4.17 and 4.81 respectively, p<0.05). Conclusion : This study revealed that patients with a prior treatment history for pulmonary tuberculosis, and the presence of a cavitary lesion on chest films had a higher prevalence of anti-tuberculosis drug resistance. A very careful clinical and microbiological examination is needed for patients with such characteristics.

Fabrication of tunable all-fiber feedback laser and its application (파장가변이 가능한 완전 광섬유형 궤환 레이저 구현 및 응용)

  • In this paper, a tunable fiber feedback laser based on the use of Sagnac loop filter and fiber Bragg grating (FBG) is proposed. The Sagnac loop filter using a high birefringence fiber provides precise 0.33-nm channel spacing as a multi-wavelength grid filter. Ni-Cr wire wound on the FBG is useful to induce the thermo-optic effect of the fiber. Two types of FBG structures, which have a different length of wire, are demonstrated to show the wavelength shift and separation. To tune FBG by resistant heat, some current is supplied into the wire. When the wavelength matched with one of the cavity modes of Sagnac loop filter, the mode-locked lasing is occurred. The electrical power sensitivity of the resonant wavelength is measured to 1.75pm/mW. This laser configuration can be applied the electrical power system for monitoring the power fluctuation.
