쑥뜸치료(Moxibustion)로 호전된 비소세포성 폐암 환자의 암성 통증 1례 (A Clinical Case of Patient with Cancer Pain Treated by Moxibustion)

  • Background & Objective: Regardless of types of cancer, cancer pain means the pain occurred by tumor itself, caused by complication or side effects during treatment and which is neither directly related to tumor nor treatment. This case was performed to relieve the cancer pain and study the effect of moxibustion to the cancer pain. Method: For 27 days, the patient was treated by moxibustion($MANINA^{(R)}$), acupuncture, herbal medicine and narcotic analgesics. We planned to maintain moxibustion on 8 points of Bladder meridian line(BL13, BL17, BL42, BL43) for 20 minutes. The improvement of the symptom was evaluated by BPI/VAS, and the amount of narcotic analgesics applied. Results: The BPI/VAS was declined and amount of applying narcotic analgesics was decreased. Conclusion: It is suggested that moxibustion on BL13, BL17, BL42, BL43 is useful to relief of cancer pain in left flank area.

한방의료에 대한 인식 및 만족도 조사 (The Research about Recognition and Satisfaction on Korean Medicine)

  • Object: Recently, demend for Korean Medicine have been rapidly increasing in Korea. However, there are still a few studies for recognition, satisfaction and satisfaction factors of Korean Medicine to increase competitiveness of Korean-Medical service. Thus, the purpose of this study is to grope ways to strengthen the competitiveness of Korean-Medical service by investigating recognition and satisfaction factors on Korean medicine. Method: People who have been experienced Korean Medicine were targets of investigation. Questionnaire was consisted of questions about awareness for the Korean Medicine, awareness of Korean-Medical treatment, satisfaction level for Korean Medicine, satisfaction and dissatisfaction factors. This questionnaires were conducted through personal interviews. Result: Total 315 members responded to the survey and the results of the analysis were as follows. 1. Korean Medicine treatment was recognized as the scientific medical act to protect human body wellness. 2. Korean-Medical service can be represented by acupuncture and moxibustion therapy, physical therapy, Korean medicine. And care for diseases such as pain disorders, chronic fatigue, anti-aging, and geriatric disease were wanted. 3. The factors that influence on satisfaction of the Medical Services was the perception that Korean Medicine is scientific and safe. Conclusion: To strengthen the competitiveness of Korean-Medical service, first, efforts to enhance the effectiveness of the basic treatment such as acupuncture and moxibustion, herbal medicine are required. Second, scientific development of Korean Medicine is necessary. Finally, the effort to offer various services of Korean Medicine are needed.

영상 의학적 소견과 상반된 증상으로 내원한 요추 추간판탈출증 환자 2례 (Two Cases of Patients Visiting with Herniated Intervertebral Disc of Lumbar Spine Showing Symptoms Opposed to Radiological Findings)

  • Objective: The purpose of this study is to investigate the effects of oriental medicine conservative treatment on patients who show symptoms at the legion opposed to that shown in Magnetic Resonance Image(MRI) findings. Subject & Method: Oriental medicine treatment such as acupuncture and herbs was applied to two patients who were diagnosed as lateral herniated intervertebral disc of lumbar spine via MRI but whose pain and muscular force attenuation were shown at the opposed lower limb, for measuring its effects. Results: As for the two cases, the patients were favored in the Visual Analog Score (VAS) and the Straight leg Raising(SLR) test after the oriental medicine conservative treatment. In one case, the muscular force in the lower limb was favored. Conclusion: Based on the results in this study, oriental medicine conservative treatment might be effective on patients who show symptoms at the legion opposed to that shown in MRI findings.

비특이적 목 통증에 사용된 침구치료에 대한 체계적 문헌고찰 (A Systematic Review of Acupuncture-Moxibustion Treatments for Nonspecific Chronic Neck Pain)

  • Objectives : The aim of this study was to describe the details and to assess the clinical evidence of acupuncture and moxibustion for non-specific chronic neck pain. Methods : We searched seven databases including Korean, English, and Chinese databases through July 2016. Studies using acupuncture, moxibustion, pharmacopuncture, electroacupuncture, auricular-acupuncture, acupressure for non-specific chronic neck pain were included. Only controlled clinical trials or randomized clinical trials were assessed. Study design, number of subject, inclusion criteria, intervention, and results were extracted. In addition, details of intervention including needle type, retention time, acupoints were analyzed. Results : Total 64 studies(39 acupuncture, 9 laser, 6 pharmaco-acupuncture, 3 electro-acupuncture, 3 auricular-acupuncture, 3 moxibustion, 1 acupressure) were included. Among 39 acupuncture studies, 35 used acupuncture as sole intervention. Sham treatment was the most common intervention for control group, followed by no intervention. Various outcome including pain, disability, quality of life, range of motion was used as outcome measurement. The effect of acupuncture and moxibustion was different depending on the type of control and outcome measurement. The most commonly used method in acupuncture for neck pain was GB21, SI3, GB20, LI4, BL11 acupoints, 10~30 mm insertion depth, 20~30 retention time, and 1~2 times per week. Conclusions : Analyzing the details of acupuncture and moxibustion treatment could be helpful for researches and clinics. Further studies should consider the characteristics of study design, intervention, and outcome to assess the effect of TKM.

  • By process of treatment for a case which diagnosed as HNP of left posterolateral aspect of L4/5 disc and treated from the 10th, May 2001 to the 23rd, Jun 2001, the results are as follows. Method & Results : This patient was medicated Hwallaktang-gami, taken acupuncture, phototherapy, TENS, electric acupuncture, exercises, Mori Cortex-bee venom acupuncture. As a result, the patient's clinical symptom were improved but a computed tomagraphy confirmed L4/5 and L5/S1 HNP was not changed as seen on repeated CT scanning. Conclusion : These results suggest that among conservative therapies the oriental medical treatments including Mori Cortex-bee venom acupuncture improve clinical symptom effectively. But in this case, the structure of herniated disc was not changed.

Analysis of Medical Disputes in Korean Medicine : With a focusing on Korean medicine treatments in Korean Acupuncture & Moxibustion Medicine Society official documents

  • Background: This paper was to investigate Korean medical disputes through the cases of asking Korean Acupuncture and Moxibustion Medicine Society(KAMMS) for medical consultation Methods: In this study, it was investigated 66 medical disputes requested to KAMMS for medical consultation from April, 2013 to December, 2017. The cases of disputes were classified according to the year, month, sex, age, area, original disease, treatment method and type of occurrence. Results : There were 66 cases from April, 2013 to December, 2017 that able to investigate. There were no annual increases and decreases or monthly trends in medical disputes. In characteristics of patients, female (53.03%) were more likely than male, and the age distribution was in in 50s (24.24%). It occurred in area, followed by Gyeongsang (33.33%), Gyeonggi-Incheon (30.30%), and Seoul (13.64%). The majority of original disease was musculoskeletal disease (81.82%), and treatment methods that have been assumed to cause medical disputes were 38 cases (57.58%) of acupuncture, followed by 12 cases (18.18%) of combined treatment. Analysis of occurred disease showed that 23 cases (34.85%) of musculoskeletal diseases were the most common, followed by 17 cases (25.76%) of infection. When original disease was musculoskeletal disease, the greatest type of occurrence was musculoskeletal disease (30.30%), but there was no statistical significance. Musculoskeletal disease was common after acupuncture (28.80%), and infection was common after combined treatment (12.10%). It was statistically significant. Although no statistically significant, pharmacopuncture tended to cause the immune response, while moxibustion tended to cause burns.

내장감각과민의 침구 치료에 대한 실험연구 현황: PubMed를 중심으로 (Experimental Study Trends on the Acupuncture Moxibustion Treatment for Visceral Hypersensitivity: Based on the Data of PubMed)

  • Objectives : The aim of this study is to review the current trends in experimental studies on the acupuncture moxibustion treatment for visceral hypersensitivity. Methods : PubMed was searched for experimental studies about visceral hypersensitivity and acupuncture/moxibustion. Data were extracted and tabulated from the selected articles about experimental method, intervention, result and mechanism. Results : Total 23 articles were reviewed. Chronic visceral hypersensitivity animal model was applied in 17 studies (74%). Visceral hypersensitivity was measured by abdominal withdrawal reflex scoring or/and abdominal electromyogram. Acupoints like ST25, ST36, ST37, BL25, LI11, BL32 and PC6 were treated by electroacupuncture or moxibustion. All articles reported that electroacupuncture or moxibustion treatment is significantly effective in reducing visceral hypersensitivity. Treatment mechanisms were studied, related to mast cell, serotonin (5-HT) and receptor (5-HT3R and 5-HT4R), substance P (SP), vasoactive intestinal polypeptide (VIP), c-fos positive cell, corticotropin-releasing hormone (CRH), purinergic 2X (P2X)2, P2X3, P2X4, P2X7, N-methyl-D-aspartate (NMDA) receptor (NR1 and NR2B), prokinectin (PK) 1 and PK2. Conclusions : Evidences on acupuncture/moxibustion treatment for visceral hypersensitivity in animal studies warrant more research on effective acupoins, electro-acupuncture methods and treatment durations.

Clinical Research Using Salviae Miltiorrhizae Radix-Pharmacopuncture for Lumbar Herniated Intervertebral Disc: Analysis of Trends

  • This review of national and international randomized controlled trials of Salviae miltiorrhizae radix pharmacopuncture for lumbar herniated intervertebral discs was performed to assess its clinical efficacy. There were 5 online databases (PubMed, EMBASE, CNKI, NDSL, OASIS, and RISS) searched on June 1st, 2020. Studies were selected according to the inclusion and exclusion criteria and were reviewed by risk of bias assessment. This review included 14 Chinese studies. The sample sizes ranged from 50 to 100. The numbers of treatments ranged from 20 to 30, with most patients receiving 20 treatments. The longest treatment periods were 10-15 and 15-20 days, of which each accounted for 29% of the studies. The most frequently used evaluation indices were the Japanese Orthopedic Association and the Visual Analog Scale scoring method. The most frequently used acupoints were EX18 and BL25, which accounted for 31% of the total number of acupoints. In 50% of the studies, the pharmacopuncture injection volume was 2 mL. Acupuncture treatment was the most common control group. Eleven studies reported that the intervention group had significantly improved symptoms. However, most of the included studies were of low quality.

A Literature Review of Clinical Studies Using Sa-am Acupuncture

  • Sa-am acupuncture originated in the Chosun Dynasty and is a distinct feature of Korean medicine. It has been used to treat various diseases and conditions in clinical practice however, there is insufficient scientific evidence to support the use of Sa-am acupuncture. We aimed to comprehensively review the clinical studies of Sa-am acupuncture retrieved from national and international databases (MEDLINE, EMBASE, the China National Knowledge Infrastructure, and 3 Korean databases). There were 52 articles reviewed including 29 case studies, 19 randomized controlled trials (RCTs), and 4 uncontrolled trials. Neurological disorders were the most frequently studied, and kidney tonification, and directional supplementation and draining were the most frequently used methods. Overall, the outcomes were generally positive however, there were many additional treatments together with Sa-am acupuncture reported in the case reports, and the quality of evidence was low in the RCTs. Future studies should report the detailed method of practicing Sa-am acupuncture treatment and focus on the specific effect of Sa-am acupuncture with rigorous design to scientifically support the clinical use of Sa-am acupuncture.

화침의 안전성 평가에 관한 고찰 (The Safety Assessment of Fire needling)

  • Objectives : Fire needling has been applied as the treatment for various diseases and been getting much attention from Oriental medicine due to its excellent effectiveness as the results of clinical studies have reported. However, the research findings on the safety of treatment method, materials for the Fire needling needle materials and the possibility of burn injury during the procedure are still insufficient. Methods : A thermo imaging camera was used to confirm the temperature distribution on acupuncture needle and the treatment area during the fire needling therapy. Then the degree of thermal injury was observed by H&E stain and TUNEL assay. In addition, in order to assess the safety of acupuncture materials, we conducted MTT assay using a L6 cell line. Results : The average temperature of the skin surface was observed at $47{\sim}51^{\circ}C$ after classic fire needling and $30^{\circ}C$ after warming fire needling. Warming fire needling therapy does not induce a burn on the tissue and a third degree burn was observed locally in the muscle and skin layers after classic fire needling treatment. This confirms that hwa-acupuncture therapies do not cause major burns. According to the safety assessment test result, no cytotoxicity was detected in the warming fire needling materials. This confirms the safety of the acupuncture materials Conclusions : Various research results on the biological safety of fire needling. Since fire needling therapy induces a burn locally without leaving any scar, and as other results indicate, it is considered a safe treatment method.
