뚜렛 증후군에서 보툴리눔 톡신의 임상 효과 : 증례보고 및 고찰 (The Clinical Effect of Botulinum Toxin in a Patient with Tourette's Syndrome: A Case Report and Review)

  • Botulinum toxin, a neurotoxin, is known to be an inhibitor of cholinergic neuromuscular transmission. Recently, it was reported that the administration of botulinum toxin is effective for the treatment of focal neurological motor disorders such as cervical dystonia, blepharospasm, hemifacial spasm, spasmodic dysphonia, and writer's cramp. Several case studies reported that the botulinum toxin was administered for the treatment of motor tic or vocal tic. It was found that this toxin reduces the frequency and severity of the tic as well as the premonitory urge and symptoms. In our case study, a noticeable decrease of motor tic symptom was observed after an intramuscular injection of 300mg of botulinum toxin in an 18-year-old patient with Tourette's disorder who showed only a little improvement of motor tic and vocal tic symptoms after treatment with antipsychotic drugs for several years. This case is reported in our study and literature survey was undertaken for reviewing similar cases. In our study, an 18-year-old boy diagnosed with Tourette's disorder based on Diagnostic and Statistical Manual of Mental Disorders, fourth edition presented with the following scores : the Clinical Global Impression scale, Yale Global Tic Severity Scale (motor/vocal/severity), Premonitory Urge Score, Korean Attention-Deficit Hyperactivity Disorder Rating scale, and Kovac Depression scale which were performed prior to the treatment were 5, 21/5/50, 100, 17, and 18 points, respectively. Two weeks after the injection of botulinum toxin, the scores were 4, 17/5/40, 50, 16, and 19 points, respectively. Eight weeks after the injection of botulinum toxin, they had become 3, 15/5/30, 25, 16, and 20 points, respectively, which clearly indicates a noticeable decrease of motor tic symptom.

유동가압성형(TPM)을 이용한 휴대용 유도무기용 연소관 조립체 제작공정연구 (A study on the manufacturing of motor case assembly for K-PSAM propulsion system by Trans. power molding(TPM) process)

  • 복합재료와 금속 튜브 사이의 좁고 긴 원통형 공간을 EPDM(Ethylene propylene dien ter polymer)으로 충전하는 연구를 수행하였다. 휴대용 유도무기의 추진기관을 개발하기 위하여 소형 연소관 조립체를 설계하고 제작하였다. 연소관 조립체는 연소관 금속튜브, 연소관 내열재, 그리고 연소관 단열재로 이루어져 있는데 연소관 내열재는 탄소/페놀릭을 사용하여 롤링 공법으로 제작하였으며 연소관 단열재는 EPDM을 사용하여 TPM 공법으로 제작하였다. EPDM의 조성 선정을 위하여 내열 삭마특성, 열분해특성 실험을 수행하였으며 흐름성, 접착성, 경도측정 등을 통하여 최종 조성을 결정하였다. 연소관 조립체를 제작하기 위한 EPDM 충전을 위하여 TPM 공법을 설계하고 금형과 치구를 제작하였다. 충전된 연소관 조립체는 비파괴검사를 통하여 접착, 기공 등을 검사하여 최종 선정되었다.

대칭 적층 복합재 연소관의 탄성파 전파에 관한 실험적 연구 (An Experimental Study on Elastic Wave Propagation in a Symmetrically Filament-Wound Composite Motor Case)

  • 복합재 연소관의 수압/AE 시험의 성공적 수행을 위한 핵심 과제중의 하나는 수압으로 인해 방출되어 연소관을 통해 전파하는 탄성파의 특성을 분석함으로써 수압/AE 시험시 감지해야할 탄성파의 최적 성분을 결정하는 것이다. 이러한 문제를 해결하기 위해 본 연구에서는 대칭 적층 구조를 갖는 복합재 연소관에 광대역 초음파를 발진시키고 전파방항과 거리를 변화시킨 105지점에서 광대역 음향방출 센서를 이용하여 복합재 내를 전파한 탄성파를 수신하고, 그 특성(주파수, 전파거리, 전파속도)을 분석하였다. 이와 같은 실험을 내부가 비어 있는 연소관과 내부를 물로 채우고 수압을 가한 연소관에 대해 반복 수행함으로써, 수압이 탄성파의 전파특성에 미치는 영향을 분석하고, 이를 바탕으로 수압/AE 시험시 검출할 탄성파의 최적 성분을 결정하였다.

T800/AD6005계 복합재 연소관의 습기 투과에 관한 연구 (Water Vapor Transmission for T800/AD6005 Based Composite Motor Case)

  • 수분이 복합재를 통하여 투과되는 정도를 고찰하기 위하여 $20^{\cire}C$, 90%RH의 온/습도를 유지하는 수조를 제작하였고, 이 수조내에 보관한 복합재 연소관의 실린더 벽면으로 투과되는 습기를 측정하기 위하여 연소관 내에 습도 센서를 투입하여 상대습도를 직접 측정하였다. 습기의 투과는 내부에 라이너/인슬레이션이 피복 되거나 또는 추진제가 충전된 경우 많은 감소효과를 보였다. Epoxy filament winding으로 제조된 순수 복합재 연소관의 경우 수조에 넣고 평형에 도달한 후로부터 약 8개월 간의 습기 투과 상태를 볼 때 벽면을 통하여 들어가는 water vapor flux는 $20^{\cire}C$, 90%RH에서 평균적으로 2.88${\times}$$10^{-9}$g/$\textrm{m}^2$sec로 나타났다. 이때 습기가 투과되는 연소관의 국지점을 평판으로 가정하고 Fick's law를 이용하여 구한 습기에 대한 복합재의 확산계수는 D=7.98${\times}$$10^{-7}$$\textrm{mm}^2$/sec 였다. 밀폐된 장치 내의 습도 변화를 직접 측정하는 본 시험 방법은 실린더 형태 및 밀폐된 용기를 이루는 곡면형 재료의 환경에 따른 습기 투과량 및 재료의 확산 계수 측정에 유용할 것으로 생각된다.

Targeting motor and cognitive networks with multichannel transcranial direct current stimulation along with peripheral stimulation in a subacute stroke survivor: single case study

  • Objective: Reacquisition of motor functions following stroke depends on interhemispheric neural connections. The intervention highlighted in the present case is an insight for augmenting motor recovery by stimulating the lesioned area and adjacent areas governing the motor behaviour of an individual. The purpose of this study was to determine the changes in the motor and cognitive outcomes through multi target stimulation of cortical areas by application of multichannel transcranial direct current stimulation (M-tDCS) in a stroke survivor. Design: A case report. Methods: The patient was a participant of a trial registered with the clinical trial registry of India (CTRI/2020/01/022998). The patient was intervened with M-tDCS over the left primary motor cortex i.e. C3 point and left dorsolateral prefrontal cortex i.e. F3 point with 0.5-2 mA intensity for the period of 20 minutes. SaeboFlex-assisted task-oriented training, functional electrical stimulation over the lower extremity (LE) to elicit dorsiflexion at the ankle and eversion of the foot, and conventional physiotherapy rehabilitation including a tailored exercise program were performed. Outcome assessment was done using the Fugl-Meyer assessment scale (FMA) for the upper and lower extremity (UE and LE), Montreal Cognitive Assessment (MOCA), Wisconsin Gait Scale (WGS) and the Stroke Specific Quality of Life (SSQOL) measures. Assessment was taken at Day 0, 15 and 30 post intervention. Results: Improvement was observed in all the outcome measures i.e FMA (UE and LE), MOCA, SSQOL and WGS across the span of 4 weeks. Conclusions: M-tDCS induced improvement in motor functions of the UE and LE, gait parameters and cognitive functions of the patient.

  • The characteristics of an induction motor vary with the number of parameters and the performance relationship between the parameters also is implicit. In case of the induction motor design, we generally should estimate many objective physical quantities in the optimization procedure. In this article, the multi objective design optimization based on genetic algorithm is applied for the three phase induction motor. The efficiency, starting torque, and material cost are selected for the objectives. The validity of the design results is also clarified by comparison between calculated results and measured ones.

  • Theoretical IPMSM control technique is complicated, and reliability is low because of the changing parameters. Further, in case of general look-up table designing method which obtains torque characteristics (according to current and speed) or torque characteristics (according to magnetic flux through the entire control region), obtaining a precise result can be difficult and has the disadvantage taking too much time to establish a current look-up table. In this paper, the new auto maximum torque point tracking (MTPT) algorithm that automatically finds the optimum stator d - q axis electric current reference through the entire speed region is devised; consequently, it could establish a 3D look-up table with torque characteristics according to current and speed. In case of using the devised auto MTPT algorithm, the result value detailed was obtained in comparison with the generalized look-up design technique, and checked to reduce the current look-up table establishment time.

  • Acute motor axonal neuropathy (AMAN) is a subtype of Guillain-Barre syndrome and characterized by selective involvement of motor fibers. Acute disseminated encephalomyelitis (ADEM) is a demyelinating disease of central nervous system. The coincidence of central and peripheral nervous system involvement has been reported rarely. We described a 37-year-old male patient presented with fever and altered consciousness. The examination of cerebrospinal fluid and brain magnetic resonance imaging was compatible with acute disseminated encephalomyelitis. Several days after admissionb his mentality was improved but quadriparesis, multiple cranial neuropathies, and areflexia were detected. Electrophysiologic studies suggested axonal form of motor dominant polyneuropathy. We report a case of acute motor axonal neuropathy combined with ADEM. We consider that this case is an example of simultaneous immunologic process to the common pathogenic epitope of central nervous system and peripheral nervous system.

  • The paper presents the effect of the backlash magnitude and the supplied voltage of motor on the gear reduction servo system. The bandwidth of the system depends heavily on the supplied voltage of motor as well as the backlash magnitude. Specially, the bandwidth of the system increases as the supplied voltage of motor increases. 1t is shown that the system has the bandwidth of zero backlash in case of motor supplied voltage is infinite. It is also found that the magnitude of the supplied voltage of motor is very important in case of the analysis of the system bandwidth.

Effects of Goal-Oriented Functional Tasks on Gross Motor Function and Activities of Daily Living in Children with Cerebral Palsy -A Single Case Study-

  • Purpose: The study sought to determine whether goal achievement, performance, satisfaction, activities of daily living, and gross motor function in children with spastic cerebral palsy are positive affected by goal-oriented functional task training. Methods: In a single case study, two 7-year-old girls diagnosed with spastic cerebral palsy were given functional task training based on individualized goals for 24 60-minute sessions over a period of 6 months. The individualized goals reflected the needs of both children and parents. The Canadian occupational performance measure (COPM) and the goal attainment scale (GAS) were used to measure goal performance and satisfaction before and after intervention. Independence of activities of daily living before and after intervention were assessed using the functional independence measure for children (Wee-FIM). And change of gross motor function were assessed using gross motor function measure (GMFM). Results: Clinically significant changes were observed in COPM and GAS, as well as positive changes in independence of gross motor function and activities of daily living. Conclusion: The findings indicate that goal-oriented functional task training addresses the needs of children with spastic cerebral palsy, which increases treatment satisfaction and has a positive effect on independent activities and participation in daily life.
