  • Objectives:This study was designed to investigate the characteristics of personality changes and emotional distress using the Personality Assessment Inventory (PAI) in patients with traumatic brain injury(TBI), divided into mild (MTBI) and severe (STBI) groups according to the severity of injury. Methods:The subjects were consisted of 25 patients with MTBI, 25 patients with STBI, and 25 normal controls. They were interviewed with the PAI. The data were analyzed by ${\chi}^2$ test, analysis of variance and Tukey test. Results:The results were the followings. First, Negative Impression in validity scales was elevated above cutoff point(T score 70) in both MTBI and STBI groups. Second, the clinical scales of which scores elevated above the cutoff point were Somatic Complaints, Anxiety, Anxiety-Related Disorders, Depression, and Schizophrenia in the MTBI, and Somatic Complaints and Depression in the STBI. Third, the clinical subscales above the cutoff point were Conversion, Somatization, Health Concerns, Affective Anxiety, Physiological Anxiety, Traumatic Stress, Cognitive Depression, Affective Depression, Physiological Depression, Thought Disorder, and Affective Instability in the MTBI, and Health Concerns, Cognitive Depression, Affective Depression, and Physiological Depression in the STBI. Fourth, Suicide Ideation in treatment scales was the only scale above the cutoff point in the MTBI and the others of the treatment and interpersonal scales in the MTBI and all of these scales of the STBI were not elevated above the cutoff point. Fifth, the scales of which scores showed significant difference between the MTBI and the STBI were Somatic Complaints, Anxiety, Depression, and Suicide Ideation, the subscales were Conversion, Somatization, Health Concerns, Affective Anxiety, Physiological Anxiety, Physiological Depression, and Psychotic Experiences. Conclusion:These results suggest that the patients with MTBI had more somatic and anxiety symptoms, depressed mood, and suicidal ideation than the patients with STBI. These characteristics are generally consistent with clinical observation and findings from previous studies of the patients with TBI, and the PAI seems to be a beneficial adjunctive assessment tool for the evaluation of patients with traumatic brain injury.

  • According to ICD-10, The typical Depression are characterized by dejected mood, loss of interest and pleasure, feeling of helplessness. Also, other symptoms such as loss of attentiveness and concentration, guilty conscience, pessimistic attitude for the future, sleeping disorder, lack of appetite, thinking of or doing a self-injury or suicidal act may characterize Depression. These Symptoms of Depression are similar to Wool-Zeng(鬱證), Zeon-Zeng(癲證), Her-ro(虛勞), Bul-Sa-Sik(不思食), Tal-Young-Sil-Zeong(脫營失精), Bul-Myeon(不眠), Ki-Myeon(嗜眠) in Oriental Medicine. In general, Depression is classified into Wool-Zeng type and Zeon-Zeng type. The former is similar to the neurotic type of depression, the latter is similar to the psychopathic type of depression. The clear causes of Depression are not known yet. But psychodynamic and biological factors are assumed to cause Depression. Psychodynamic factors may correspond to Chil-Zeong(七情) and biological factors to Tae-Byeong(胎病) and Dam(痰) in Oriental medicine. In Occidental medicine, Depression is treated by psychotherapy and medication. Electrically-induced spasm therapy and phototherapy also used for the treatment of Depression. In Oriental medicine, Depression is treated by more various therapy such as herb medication, acupuncture and moxibustion, oriental psychotherapy and dirigation(Gi-Gong, 氣功). Depression is not easy to cure. When the treatments of Occidental medicine and Oriental medicine are mutually cooperated, more effective medical care might be expected. Therefore, futher study to treat depression by mutually cooperated therapy is necessary.

  • Reliable and valid diagnostic screening tools in the primary care setting enable accurate estimation of depression in individuals at risk or in need of treatment, and provide patients with an opportunity to receive optimal treatments. Although there have been extensive studies on screening tools for depression used in domestic mental health settings, there is little consideration and lack of a thorough review of the diagnostic validity of screening tools. In the current review, we selected 13 representative screening tools for depression which were evaluated in a total of 19 validation studies conducted in Korea. We summarized DSM-5 target domains, diagnostic indices, sensitivity, specificity, cut-off scores, and diagnostic validity information for each tool. Finally, the depression measurement expert group was constituted to evaluate the current status of screening tools for depression, and their recommendations for a new screening tool were summarized. This study was conducted as part of the Mental Health Technology Development project to develop the Korean screening tool for depression (K-DEP).

A convenience sample or In senile dementia patients and l20 nor-mal elders in a rural area was used. The tools used in the study were the MMSE-K(Mini-Mental State Examination-Korea) for dementia screening test and a questionaire developed by the research team. Data were collected through home visits by Com-munity Health Practitioners. Data were analyzed using descriptive statistics, T-test, and Chi-square test. The findings are as follows : 1. There were significant differences in age, marital status, and religions between the two groups. 2. There was a significant difference in smoling behavior between the two groups. 3. There was a significant difference in past his-tory of cancer between groups. 4. There was a significant difference in past and present elderftmily relationship between the two groups. 5. There were significant differences in intellectual activities, assuming major role in family and seeking other's help in daily life troubles between the two groups. 6. There were significant differences in stress factors such as child problem, family conflict, health problem and illegal behavior between the two groups. 7. The major problems out by families in caring for dementia patient were catastrophic reactions, dirtiness, mood change, devouring and tremor. The most serious problems faced by families was dirtiness. with catastrophic reactions, sleep distrubance, changeableness, and a suspcio-usness following. The care givers expressed chronic fatigue, anxiety, tension, depression, disorder in daily life, shamefulness, blame from neighbours and guiltiness. 8. There is need for geriatric hospitals, nursing homes, burden sharing, and counselling or education for family care givers. • Objectives : The purpose of this study was to assess anger of middle school adolescents in an urban community using State-Trait Anger Expression Inventory (STAXI)-Korean and Korean Child Behavior Checklist (K-CBCL), and to determine whether specific anger expression of adolescents is associated with certain psychopathology including depression. Methods : Data were collected from 395 middle school students and their parents in Incheon city. The students completed the STAXI, Children's Depression Inventory, Korean version of Mood disorder Questionnaire, and Conners-Wells' Adolescent Self-Report Scale (Short Form), and their parents completed the K-CBCL. Results : No significant correlation was observed between aggression subscale K-CBCL and each scale of the STAXI, except anger out (r=0.704). The scores for STAXI total and respective scales were significantly higher for female students than male students. A stepwise logistic regression model was used to explore the possible predictors for depression in adolescents and lower anger control and higher anger suppression were found to be predictors for depression in adolescence. Conclusion : There is a possibility of a large discrepancy between anger that the adolescents actually feel and parents' judgment of their children's anger. It must be considered in assessment of adolescent anger and use of further structured interviews is necessary. In addition, it may be useful to consider the anger expression style in adolescents who report depressive symptoms.

  • Objective : This study aimed to examine impact of anxiety and demographic factors on relationship between severity of depressive symptom and quality of life and disability. Methods : One hundred ninety five patients who met DSM-IV-TR criteria for depressive disorder were enrolled. It includes "Hamilton Rating Scale for Depression (HRSD)", "State-Trait Anxiety Inventory-State (STAI-S)", "General Health Questionnaire/Quality of Life-12 (GHQ/QL-12)", and "Sheehan Disability Scale (SDS)". Correlation analysis was used to see the correlations of each variable. Hierarchical regression analysis was used to see mediating effect of anxiety in the relationship. Sobel test was used to verify mediating effect. Multiple regression analysis was used to see moderating effect of demographic factors in the relationship. Results : There was partial mediating effect of anxiety on the relationship between severity of depressive symptoms and decreased quality of life (z=-11.68, p<.001)/increased disability (z=10.42, p<.001). Only religion was found to be moderating effect on the relationship between depressive symptoms and decreased quality of life. Conclusion : Rapid relief of anxiety along with depressive symptom had important implications for the treatment of patients with depression.

  • Objective : This study examined psychiatric characteristics of wounded persons of Daegu subway fire accident occurring in February 2003 after six years. Methods : 103 wounded persons completed psychometric measures at 2 months and 6 years after the accident. Changes of psychiatric symptoms were compared and factors affecting post-traumatic stress disorder (PTSD) seriousness were examined. Results : In Minnesota Multiphasic Personality Inventory, the scores of Psychopathic Deviate (Pd), Masculinity/ Femininity (Mf), Social Introversion (Si) scales were significantly changed over time. The scores of Symptoms Checklist-90-Revision subscales were mostly decreased over time, however, paranoid ideation (PAR) subsclae scores were not significantly changed over time. In all of the State-Trait Anxiety Inventory (STAI)-State, Beck Depression Inventory, Impact of Event Scale, and PTSD Checklist-Civilian, mean scores at 6 years after the accident were significantly lower than those at 2 months after the accident. However, STAI-Trait score was not significantly changed over time. Total seriousness of PTSD was found to be affected by quality of sleep. Conclusions : Psychiatric symptoms of subjects were largely improved over time, but distrust and doubt of others were continued. In addition, the wounded persons suffered from sleep problems. Therefore, early and continuous interventions of public institutions and public health specialists are needed for the wounded persons with chronic psychiatric disorders.
