The Relationship of NIH(National Institutes of Health) Stroke Scale to Functional Improvement in Stroke Patients (뇌졸중 환자의 임상적 검사NIHSS(National Institutes of Health Stroke Scale)와 기능적 예후와의 상관관계)

  • The purpose of this study was NIHSS(National Institutes of Health stroke scale) to activities of Daily Living stroke patient. We designed a 17 item neurogic Examination NIHSS use in acute stroke therapy trials. In a study of stroke patient. Assessment were measured by the NIHSS and by the MBI (Modified Barthel Index) to evaluate activities of daily living(ADL). Fifty patients were subject in this study. Collected data analysis were completed by using t-test, ANOVA, correlation analysis and Multiple regression analysis. The following were as follow; Among the test of NIHSS(National Institutes of Health stroke scale) was significantly correlated with changes in MBI(Modified Barthel Index) score in 50 stroke patients. Among the subitems of NIHSS, Pupillary Response, a level of consciousness, best motor arm, best motor leg were the best predictors of functional improvement.

Interobserver Reliabilities of Modified Barthel Index, and Motor Assessment Scale in Stroke Patients (Modified Barthel Index 및 Motor Assessment Scale을 이용한 검사자간의 신뢰도 검사)

  • We have conducted a study of the interobserver reliabilities of Modified Barthel Index and Motor Assessment Scale with 30 patients hospitalized with strokes in the department of circulatory internal medicine, Sang Ji University Oriental Hospital. The observations were performed by two staff and residents in the circulatory internal medicine department. Raters were assigned in random pairs to individual patients. Evaluations were performed independently by the two observers. In order to minimize the impact of fluctuations in the patients' clinical status, the second set of observations immediately followed the first. Each patient was used for only one pair of evaluations. The results were as follows. 1. Mean kappa value of 13 items in Modified Barthel Index(MBD was 0.742, which indicated excellent interobserver reliability. The kappa values indicated almost $perfect({\kappa}:\;0.81-1.00)$ for 4, substantial for $9({\kappa}:\;0.61-0.80)$, and moderate for $2({\kappa}:\;0.41-0.60)$ of 13 items. All items. except Grooming item, showed statistically significant interobserver agreement(p<0.01) 2. Mean kappa value of 8 items. except General tonus, in Motor Assessment Scale(MAS) was 0.823, which indicated excellent interobserver reliability. and this value of Motor Assessment Scale was more high than MBI' s value. 0.81. The kappa values indicated almost perfect for 5, substantial for 3 of 8 items. All items showed statistically significant interobserver agreement(P<0.01).

Effects of Added Tong's Acupuncture on NIB Stroke Scale and MBI in Stroke Patients (동씨침(董氏鍼)을 가미(加味)한 치료(治療)가 중풍환자(中風患者)의 NIH Stroke Scale과 MBI상의 기능 회복에 미치는 영향)

  • Objective : The aim of this study was to investigate the effect of tong's acupuncture on recovery of motor disorders in stroke patients. Methods : Twenty two patients with poststroke-hemiplegia were randomized into two groups. Ten patients(test group) treated by 2 methods-tong's acupuncture and body acupuncture. The other twelve patients(control group) treated only by body acupuncture. The activity of daily living was measured with a National Institutes of Health stroke scale(NlHSS) and Modified Barthel Index(MBI). The therapy was performed one a day for 2 weeks. Results: In terms of score of NIHSS. the test group showed statistically meaningful decrease after 2 week treatment. but the control group showed statistically meaningful decrease after I week(p<0.05). And in terms of score of MBI. the test group showed statistically meaningful increase after 2 week treatment. but the control group showed statistically meaningful increase after I week(p<0.05). There was no statistically meaningful difference after 1 and 2 week treatment between the groups. Conclusions: These results support that test group has almost same effectness compared with control group in improvement of the activity of daily living of poststroke-hemiplegic patients.

A Clinical Study about Gait Disturbance of Patient who Diagnosed Spinal Cord Concussion after Traffic Accident (교통사고 후 척수진탕으로 인한 양하지 부전마비 환자 1례에 대한 증례보고)

  • Objectives : The purpose on this study is to report clinical effects of oriental medicine for gait disturbance after traffic accident. Methods : The patient was treated using electro-acupuncture, 8 constitution acupuncture, herbal medicine, moxibustion and physical treatment. The effects for gait disturbance have measured with modified barthel index(MBI) and that for lower back pain have measured with visual analog scale(VAS). Results : 1. The Modified barthel index gradually increased according to treatment time. 2. The VAS of low back pain decreased to less than half. Conclusion : Oriental medical treatment showed positive effect on gait disturbance after traffic accident.

Clinical Usefulness on K-MBI for Decision of Driving Rehabilitation Period in Patients with Stroke: A pilot study (뇌졸중 환자의 운전재활 시기 결정을 위한 K-MBI의 임상적 유용성: 예비 연구)

  • Background & Object: Basic daily activity screening tool such as the Modified Barthel Index (MBI) has been used commonly in rehabilitation clinic and community based rehabilitation setting. Previous studies have shown the significant relations between the level of daily activities and driving ability on stroke or elderly people. However, there is a lack of studies to investigate the usefulness of MBI on prediction of driving ability for stroke patient. This study was to predict driving abilities of stroke survivor using Korean version Modified Barthel Index (K-MBI). Methods: A sample of 48 patients with stroke in rehabilitation hospital was recruited. All participants were tested level of basic daily activities using K-MBI. The driving ability of participants was tested using virtual reality driving simulator. The predictive validity was calculated of the K-MBI among pass or fail group of driving simulator test using receiver operating characteristics curves. Results: The cut-off score of >86.5 on the K-MBI is proper sensitivity to predict on driving performance ability. Conclusion: This pilot result offers clinical reference to therapists and caregivers for reasoning on driving recommendation period during rehabilitation stage of stroke survivors. Further studies need to identify prediction using real on-road test in a large population group.

Correlation Between Walking Ability Assessment Tools for Patients With Spinal Cord Injury Using MBI, FIM, SCIM II, WISCI, Walking Velocity, and Walking Endurance (척수손상 환자의 보행능력 검사를 위한 평가도구의 비교: MBI, FIM, SCIM II, WISCI, 보행속도, 보행지구력)

  • The main purposes of this study were to find the correlation between walking ability assessment tools using the Modified Barthel Index (MBI), Functional Independence Measure (FIM), Spinal Cord Injury Measurement II (SCIM II), Walking Index for Spinal Cord Injury (WISCI), walking velocity, and walking endurance. The study population consisted of 56 patients with spinal cord injury referred to the department of Rehabilitative Medicine in the National Rehabilitation Hospital. All subjects were ambulatory with or without an assistive device. All participants were assessed by MBI, FIM, SCIM II, WISCI, walking velocity, and walking endurance. The data were analyzed using Pearson correlation analysis and X2. There was significant correlation between the MBI, FIM, SCIM II, WISCI, walking velocity, and walking endurance (p<.01). In particular, WISCI has a significant correlation with SCIM II(p<.001). Therefore the WISCI scale is an appropriate assessment tool to predict the gait ability of patients with spinal cord injury. Further study about MBI, FIM, SCIM II, WISCI, walking velocity, and walking endurance is needed using a longitudinal study design.

Correlation Analysis with Modified Barthel Index and Motor Assessment Scale in Stroke Patients (뇌졸중환자의 기능평가척도로서의 MBI와 MAS의 상관성분석)

  • 연구목적 : 뇌졸중환자에 기능회복도의 측정을 위해 가장 신뢰도가 높은 MBI에 대해서 순수하게 뇌졸중환자의 운동기능평가만을 위해 고안된 MAS의 상관성을 각 기간별로 분석하여 MAS의 운동기능평가척도로서의 객관성에 대해 연구한다. 또 한편으로 경과시간에 따른 운동기능 회복도의 차이를 전체그룹과 사상그룹에서 평가한다. 연구방법 : 1998년 1월부터 1998년 6월까지 6개월에 걸쳐 상지대학교 한의과대학 부속한방병원 내과에 입원하여 뇌졸중으로 진단된 환자 중 의식과 언어상태가 명료한 32명을 대상으로 하였다. 입원직후 사상의학과에 의뢰하여 QSCCII 시스템에 의하여 체질진단을 받은 후 본원 운동기능평가실에서 입원직후, 입원 후 2주와 4주 등 3회에 걸쳐 Modified Barthel Index(MBI)와 Motor Assessment Scale(MAS)을 시행하였다. 각 환자에 대한 검사는 2명의 한방순환기내과 전문의와 전공의에 의해 환자에 대한 사전 정보 없이 이루어졌다. 연구결과 : 통계처리는 상관분석을 위해서는 Pearson correlation coefficient, 사상체질에 있어서 각 경과시간에 따른 점수변화는 Repeated ANOVA, 일반적특성 분석에는 Descriptive analysis가 사용되었고, 프로그램은 SPSS/PC+ 7.5 version을 사용하였다. 1. 뇌졸중 환자의 사상의학적인 분포는 태음인이 56.3%로 가장 많았고, 소양인이 28.1%, 소음인이 12.5%, 태양인이 3.1%이었으며, 연령별로는 50대가 32.6%로 가장 많았으며, 평균연령은 60.9세였다. 2. 경과시간에 따른 사상체질간의 MBI, MAS 평균점수변화는 MBI, MAS 모두 반복측정 분산분석 결과 입원당시와 2주 후, 입원당시와 4주 후에서 모두 P<0.001의 범위에서 통계학적으로 유의하게 증가하였고, 경과시간과 사상체질간의 교호작용은 없었다. 3. MBI, MAS 모두 각 항목과 기능별 분류군에서도 경과시간에 따른 평균점수의 변화는 P<0.001 범위에서 통계학적으로 유의하게 증가하여 한방치료의 효과가 있음을 보여주었다. 4. MBI와 MAS의 경과시간별 Pearson 상관성 분석결과 입원당시의 MBI와 입원당시의 MAS에서, 2주 후의 MBI와 입원당시, 2주 후, 4주 후의 MAS에서, 4주 후의 MBI와 입원당시, 2주, 4주 후의 MAS에서 모두 높은 상관성을 보여, 두 평가방법 간에는 상관성이 높다고 본다.

The Relationship between Both Upper Extremity Function and Activities of Daily Living in Stroke Patients (뇌졸중 환자의 양측 상지기능과 일상생활 수행능력의 연관성)

  • The purpose this study was to investigate the relationship between Upper extremity's function and Activities of Daily Living(ADL) in stroke patients. The participants were 112 stroke patients who underwent occupational therapy. Data were analyzed using descriptive statistics, Pearson's correlation coefficient, and multiple linear regression analysis. The results are as foolows. MFT of both unaffected upper limbs and affected upper limbs were significantly correlated with total MBI score. The all area of MFT on the affected upper limbs were significantly correlated with sub-item of MBI. And finger manipulation area of MFT on the unaffected upper limbs were significantly correlated with sub-item of MBI. Significant factors influencing MBI, both unaffected upper limbs and affected upper limbs total score. Significant factors influencing sub-items of MBI, the function of affected upper limbs by MFT were MBI all sub items exculsive bowel, bladder control and the function of unaffected upper limbs by MFT were personal hygiene, bathing, feeding, toilet, bowel & bladder control, chair/bed transfer of MBI sub items. Above results show that ADL is highly correlated with affected upper limbs and unaffected upper limbs function in the stroke patients.

The Correlation between the Balance, Cognition, Motor Recovery and Activity of Daily Living in Stroke Patie (뇌졸중 환자의 균형, 인지, 기능회복, 일상생활 평가도구의 상관성)

  • Background: The purpose of the present study was to determine correlations between the Berg Balance Scale (BBS), Modified Barthel Index(MBI), Mini-Mental State Examination(MMSE) and Fugl-Meyer motor function Assessment(FMA) in persons with strokes. Methods: We recruited 77 stroke patients from the Seoul Bukbu Geriatric Hospital in Seoul, Korea. Balance was measured by BBS. Activity of Daily Living was assessed using MBI. Cognitive function was examined using MMSE. Motor Recovery was measured using FMA. Data was analyzed using Pearson' correlation. Resurts: There were a statistically significant correlation between BBS and MBI, BBS and MMES, BBS and FMA upper extremity, BBS and FMA lower extremity. There were a statistically significant correlation between MBI and MMSE, MBI and FMA upper extremity, MBI and FMA lower extremity. There were a statistically significant correlated between FMA upper extremity and FMA lower extremity. Conclusions: This study will help rehabilitation program for stroke patients and will be the reference data for selection of therapy and evaluation method.

The Effects of Virtual Reality Training on Improving Hand Function and Activities of Daily Living in Patients Living Post-Stroke: A Pilot Study (뇌졸중 후 환자의 일상생활활동 및 손 기능 개선에 대한 가상현실 훈련의 효과 : 예비 연구)

  • The virtual reality training has been suggested as an intervention to improve physical function. But, the effects of virtual reality training focused on improving ADL in patients with acute stroke are unclear. Purpose of this study was to examine the effect of virtual reality training on hand function and activities of daily living (ADL) in patients with acute stroke. Sixteen patients with acute stroke were included in this study. The experimental group (VRA group) received 30 minutes of virtual reality training focused on ADL for each session, while the control group received 30 minutes of conventional virtual reality training. To examine the hand function and ADL of the subjects, the study used Jebsen-Tylor Hand Function Test (JTHFT) and Korean Modified Barthel Index (K-MBI), respectively. Both groups showed significant improvements in hand function of affected and unaffected sides, and in K-MBI total score before and after the intervention. The experimental group showed significantly greater improvements in the self-care domain of K-MBI after intervention than the control group. These findings suggest that the virtual reality training focused on ADL may have a better effect on self-care than conventional virtual reality training in patients with acute stroke.
