A Study on the Ageism and Age-integrated Perception of Healthcare Professional Groups with experience in treating elderly patients (노인의료전문가 집단의 연령주의 및 연령통합 인식에 관한 연구)

  • Using a Consensus Qualitative Research approach, this study aimed to identify the ageism and explore age-integration as a solution of age discrimination that occurs during the delivery of medical services by nine healthcare professionals who have experience in treating elderly patients. There were two-sided confession has shown by health care professionals about the Ageism. They reported that they don't discriminate by age. However, They also appealed an inconvenience due to the elderly. There were real Ageism in the Healthcare Professional site as a way of Unsuitable care, elderly alienation and dependence on caregivers, polarization of medical service and double discrimination against poor elderly. They found it difficult to offer age integrated health care as a means to mitigate or solve. However they have sought to break barriers to communication, provided a comfortable environment not only for senior citizens but also for all others, and have diversified institutional and service standards. To ensure the healthy life and proper medical service of the rapidly increasing elderly patients, we proposed to do critical review of the factors in the Korean medical system that accelerate the Ageism, reorganization of the health care system for the poor elderly, including the curriculum associated with age-integrating within the health care professional education system, raising the Geriatric Medical Service and the relating professionals and improvements in perception of the health care domains for the elderly and older adults.

Improvement of Small Baseline Subset (SBAS) Algorithm for Measuring Time-series Surface Deformations from Differential SAR Interferograms (차분 간섭도로부터 지표변위의 시계열 관측을 위한 개선된 Small Baseline Subset (SBAS) 알고리즘)

  • Small baseline subset (SBAS) algorithm has been recently developed using an appropriate combination of differential interferograms, which are characterized by a small baseline in order to minimize the spatial decorrelation. This algorithm uses the singular value decomposition (SVD) to measure the time-series surface deformation from the differential interferograms which are not temporally connected. And it mitigates the atmospheric effect in the time-series surface deformation by using spatially low-pass and temporally high-pass filter. Nevertheless, it is not easy to correct the phase unwrapping error of each interferogram and to mitigate the time-varying noise component of the surface deformation from this algorithm due to the assumption of the linear surface deformation in the beginning of the observation. In this paper, we present an improved SBAS technique to complement these problems. Our improved SBAS algorithm uses an iterative approach to minimize the phase unwrapping error of each differential interferogram. This algorithm also uses finite difference method to suppress the time-varying noise component of the surface deformation. We tested our improved SBAS algorithm and evaluated its performance using 26 images of ERS-1/2 data and 21 images of RADARSAT-1 fine beam (F5) data at each different locations. Maximum deformation amount of 40cm in the radar line of sight (LOS) was estimated from ERS-l/2 datasets during about 13 years, whereas 3 cm deformation was estimated from RADARSAT-1 ones during about two years.

The Effects of Subjective Health Status on Depression among Older Adults : Focusing on Moderating Effects of Age-friendly public service resources and Age (주관적 건강상태가 고령층의 우울에 미치는 영향 : 고령친화적인 공적서비스 자원과 연령의 조절효과를 중심으로)

  • It has been well known that there is a close relationship between health and depression among older adults. Under the purpose to mitigate and prevent the risk of depression caused by poor health, the changes in subjective health status'effects on depression depending on the level of age-friendly public service resources, and age were respectively examined in this study. Moderating effects analysis was conducted using 492 adults aged 55 years and over from '2017 age integration survey' data which had been collected nation-widely. Major findings are as follows. The inadequate subjective health status raises the level of depression. This tendency is weakened when the better age-friendly public service resources are presented. In contrast, age has no statistically significant effects on the relationship between two variables. As a result of reviewing the specific conditions to bring out the interaction effects, frail subjective health status increased depression of all age groups as the level of age-friendly public service resources is low. Particularly, for the participants aged average(age 66.6) and older, the negative effects of subjective health status on depression are likely to be lowered by the improvement in poor age-friendly public service resources. That is, the buffer effects of age-friendly public service resources are proved. Based on theses findings, several suggestions for health promotion and depression prevention of older adults were discussed.

Trajectories of Drinking problems of the elderly: A Longitudinal Multi-level Growth Curve Model for Change (노인의 음주문제 발달궤적의 예측요인 : 다수준 성장곡선 모형의 적용)

  • A new era of research has focused on examining the growth of change in drinking problems among the elderly. Thus, the purpose of the present study was two fold: (1) to investigate trajectories of drinking problems(CAGE) among the Korean elderly(age$${\geq_-}65$$); and (2) to identify the predicting factors for the intercept and the slope of alcohol problems using multi-level growth curve model. Data come from three waves(1st wave(2006)~3rd wave(2008) of the Korea Welfare Panel(KWP) study. The results indicated that the levels of drinking problems decreased over time and that age, gender, marital status, religion, poverty, self-rated health, and social relationship satisfaction were associated with the baseline CAGE. Further analysis showed that social relationship satisfaction affected the declining slope of drinking problems over time. Specifically, among those who satisfied social relationship, there was a sharp decline of CAGE over time. Overall findings highlight the importance of developing and implementing effective alcohol prevention programs for the elderly in the community settings to mitigate the harmful effects of various psycho-social stressors. Especially, programs to maintain and form healthy social support network are suggested as critical interventions for prevention as well as recovery of alcohol problems in late life.

Bhumipol Dam Operation Improvement via smart system for the Thor Tong Daeng Irrigation Project, Ping River Basin, Thailand

  • The Tor Tong Daeng Irrigation Project with the irrigation area of 61,400 hectares is located in the Ping Basin of the Upper Central Plain of Thailand where farmers depended on both surface water and groundwater. In the drought year, water storage in the Bhumipol Dam is inadequate to allocate water for agriculture, and caused water deficit in many irrigation projects. Farmers need to find extra sources of water such as water from farm pond or groundwater as a supplement. The operation of Bhumipol Dam and irrigation demand estimation are vital for irrigation water allocation to help solve water shortage issue in the irrigation project. The study aims to determine the smart dam operation system to mitigate water shortage in this irrigation project via introduction of machine learning to improve dam operation and irrigation demand estimation via soil moisture estimation from satellite images. Via ANN technique application, the inflows to the dam are generated from the upstream rain gauge stations using past 10 years daily rainfall data. The input vectors for ANN model are identified base on regression and principal component analysis. The structure of ANN (length of training data, the type of activation functions, the number of hidden nodes and training methods) is determined from the statistics performance between measurements and ANN outputs. On the other hands, the irrigation demand will be estimated by using satellite images, LANDSAT. The Enhanced Vegetation Index (EVI) and Temperature Vegetation Dryness Index (TVDI) values are estimated from the plant growth stage and soil moisture. The values are calibrated and verified with the field plant growth stages and soil moisture data in the year 2017-2018. The irrigation demand in the irrigation project is then estimated from the plant growth stage and soil moisture in the area. With the estimated dam inflow and irrigation demand, the dam operation will manage the water release in the better manner compared with the past operational data. The results show how smart system concept was applied and improve dam operation by using inflow estimation from ANN technique combining with irrigation demand estimation from satellite images when compared with the past operation data which is an initial step to develop the smart dam operation system in Thailand.

A study on Communication Robustness Testing for Industrial Control Devices (산업용 제어기기의 통신 견고성 시험 방안 연구)

  • Industrial control systems(ICS) are widely used in various industrial area and critical infrastructure. To mitigate security threats on ICS, the security assurance test for industrial control devices has been introduced and operating. The test includes testing of the security function of the device itself and testing of communication robustness. In this paper, we describe the security requirements of EDSA, Achilles, and Korea's TTA standard(security requirements for ICS). And also, we analyzed the characteristics of communication robustness test(CRT) of each certification. CRT verifies the device's operation of essential function while transmitting fuzzing and stress packets. Existing test methods are mostly focused on the embedded devices and are difficult to apply to various devices. We propose a method to test communication robustness which reflect the characteristics of control H/W, control S/W, field devices and network devices in ICS. In the future, we will apply the proposed communication robustness test to actual products and present solutions for arising issues.

Discrepancies and Validation of Ethanol Level Determination with Osmolar Gap Formula in Patients with Suspected Acute Poisoning (급성 중독환자에서 삼투압 계산식으로 추정된 에탄올 농도의 유효성 검증)

  • Purpose: Osmolar gap (OG) has been used for decades to screen for toxic alcohol levels. However, its reliability may vary due to several reasons. We validated the estimated ethanol concentration formula for patients with suspected poisoning and who visited the emergency department. We examined discrepancies in the ethanol level and patient characteristics by applying this formula when it was used to screen for intoxication due to toxic levels of alcohol. Methods: We retrospectively reviewed 153 emergency department cases to determine the measured levels of toxic ethanol ingestion and we calculated alcohol ingestion using a formula based on serum osmolality. Those patients who were subjected to simultaneous measurements of osmolality, sodium, urea, glucose, and ethanol were included in this study. Patients with exposure to other toxic alcohols (methanol, ethylene glycol, or isopropanol) or poisons that affect osmolality were excluded. OG (the measured-calculated serum osmolality) was used to determine the calculated ethanol concentration. Results: Among the 153 included cases, 114 had normal OGs (OG≤14 mOsm/kg), and 39 cases had elevated OGs (OG>14). The mean difference between the measured and estimated (calculated ethanol using OG) ethanol concentration was -9.8 mg/dL. The 95% limits of agreement were -121.1 and 101.5 mg/dL, and the correlation coefficient R was 0.7037. For the four subgroups stratified by comorbidities and poisoning, the correlation coefficients R were 0.692, 0.588, 0.835, and 0.412, respectively, and the mean differences in measurement between the measured and calculated ethanol levels were -2.4 mg/dL, -48.8 mg/dL, 9.4 mg/dL, and -4.7 mg/dL, respectively. The equation plots had wide limits of agreement. Conclusion: We found that there were some discrepancies between OGs and the calculated ethanol concentrations. Addition of a correction factor for unmeasured osmoles to the equation of the calculated serum osmolality would help mitigate these discrepancies.

Seasonal Distribution of Ticks in Boeun, Chungbuk during 2016-2020 (충북 보은지역의 참진드기류 분포조사 (2016-2020))

  • This study investigated the seasonal distribution of ticks in Boeun, Chungbuk, South Korea, from 2016 to 2020. Over the five-year period, ticks were collected annually from four different sites. A total of 17,704 ticks belonging to three tick species (Haemaphysalis longicornis, Haemaphysalis flava, and Ixodes nipponensis) were collected. H. longicornis was the dominant species across all four sites with the highest density of 68.40% of the total collected specimens, followed by H. flava (3.53%) and I. nipponensis (0.06%). The larvae of unidentified species were also collected: 11.81 T.I. (28.01%). The H. longicornis population peaked during the spring season (May-June), whereas the larval population peaked during August and September. H. longicornis was collected the most from four sites (coniferous forest, broad-leaf forest, mountain path, and copse), with the exception of the larvae, which was collected the most in grassland and grave. H. flava was collected from all sites, but in a small proportion. The SFTS virus was not found in any of the 828 pools of ticks during 2016-2020. Based on the results of this study, the continuous surveillance of the tick population is recommended to mitigate the spread of diseases by these vectors.

Lomens-P0 (mixed extracts of Hordeum vulgare and Chrysanthemum zawadskii) regulate the expression of factors affecting premenstrual syndrome symptoms

  • BACKGROUND/OBJECTIVES: Premenstrual syndrome (PMS) is a disorder characterized by repeated emotional, behavioral, and physical symptoms before menstruation, and the exact cause and mechanism are uncertain. Hyperprolactinemia interferes with the normal production of estrogen and progesterone, leading to PMS symptoms. Thus, we judged that the inhibition of prolactin hypersecretion could mitigate PMS symptoms. MATERIALS/METHODS: Hordeum vulgare L. extract (HVE), Chrysanthemum zawadskii var. latilobum extract (CZE), and Lomens-P0 the mixture of these extracts were tested in subsequent experiments. The effect of extracts on prolactin secretion at the in vitro level was measured in GH3 cells. Nitric oxide and pro-inflammatory mediator expression were measured in RAW 264.7 cells to confirm the anti-inflammatory effect. Also, the hyperprolactinemic Institute for Cancer Research (ICR) mice model was used to measure extract effects on prolactin and hormone secretion and uterine inflammation. RESULTS: Anti-inflammatory effects of and prolactin secretion suppress by HVE and CZE were confirmed through in vitro experiments (P < 0.05). Treatment with Lomens-P0 inhibited prolactin secretion (P < 0.05) and restored normal sex hormone secretion in the hyperprolactinemia mice model. In addition, extracts significantly inhibited the expression of pro-inflammatory biomarkers, including interleukin-1𝛽, and -6, tumor necrosis factor-𝛼, inducible nitric oxide synthase, and cyclooxygenase-2 (P < 0.01). We used high-performance liquid chromatography analyses to identify tricin and chlorogenic acid as the respective components of HVE and CZE that inhibit prolactin secretion. The Lomens-P0, which includes tricin and chlorogenic acid, is expected to be effective in improving PMS symptoms in the human body. CONCLUSIONS: The Lomens-P0 suppressed the prolactin secretion in hyperprolactinemia mice, normalized the sex hormone imbalance, and significantly suppressed the expression of inflammatory markers in uterine tissue. This study suggests that Lomens-P0 may have the potential to prevent or remedy materials to PMS symptoms.

A Performance Comparison of Machine Learning Classification Methods for Soil Creep Susceptibility Assessment (땅밀림 위험지 평가를 위한 기계학습 분류모델 비교)

  • The soil creep, primarily caused by earthquakes and torrential rainfall events, has widely occurred across the country. The Korea Forest Service attempted to quantify the soil creep susceptible areas using a discriminant value table to prevent or mitigate casualties and/or property damages in advance. With the advent of advanced computer technologies, machine learning-based classification models have been employed for managing mountainous disasters, such as landslides and debris flows. This study aims to quantify the soil creep susceptibility using several classifiers, namely the k-Nearest Neighbor (k-NN), Naive Bayes (NB), Random Forest (RF), and Support Vector Machine (SVM) models. To develop the classification models, we downscaled 292 data from 4,618 field survey data. About 70% of the selected data were used for training, with the remaining 30% used for model testing. The developed models have the classification accuracy of 0.727 for k-NN, 0.750 for NB, 0.807 for RF, and 0.750 for SVM against test datasets representing 30% of the total data. Furthermore, we estimated Cohen's Kappa index as 0.534, 0.580, 0.673, and 0.585, with AUC values of 0.872, 0.912, 0.943, and 0.834, respectively. The machine learning-based classifications for soil creep susceptibility were RF, NB, SVM, and k-NN in that order. Our findings indicate that the machine learning classifiers can provide valuable information in establishing and implementing natural disaster management plans in mountainous areas.
