Minimally Invasive Suturectomy and Postoperative Helmet Therapy : Advantages and Limitations

  • Various operative techniques are available for the treatment of craniosynostosis. The patient's age at presentation is one of the most important factors in the determination of the surgical modality. Minimally invasive suturectomy and postoperative helmet therapy may be performed for relatively young infants, whose age is younger than 6 months. It relies upon the potential for rapid brain growth in this age group. Its minimal invasiveness is also advantageous. In this article, we review the advantages and limitations of minimally invasive suturectomy followed by helmet therapy for the treatment of craniosynostosis.

Minimally Invasive Excision of Epidermal Cysts through a Small Hole Made by a $CO_2$ Laser

  • To improve the cosmetic results of removing epidermal cysts, minimally invasive methods have been proposed. We proposed a new minimally invasive method that completely removes a cyst through a small hole made by a $CO_2$ laser. Twenty-five patients with epidermal cysts, which were 0.5 to 1.5 cm in diameter, non-inflamed, and freely movable, were treated. All of the patients were satisfied with the cosmetic results. This method is simple and results in minimal scarring and low recurrence rates without complications.

  • Recently, the incidence of Achilles tendon rupture has been increasing with an increase in the elderly and the sports population. Various kinds of surgical options have been introduced up to now. Among them, the traditional open repair is most commonly used despite the risk of re-rupture or post-operative infections, which in turn can impair the blood flow to the Achilles tendon. Therefore, minimally invasive methods to overcome these complications have been studied with excellent outcomes.

  • Various sorts of dental devices for the dental treatment have been introduced and adopted during past several years. Dental Laser, among all devices, seems to be possible for applying to over the whole treatments and widely employed. Furthermore, this article is to introduce that Dental Laser is intended to care patients secured with the effectual treatment against Conventional procedures based upon its studies. The advantages of Dental Laser treatment-along with LLLT (Low Level Laser Therapy) effect, biostimulation effect, and minimal invasive technique - make the patients reduce their fear of the operation, lighten the inconvenience of post operation, and shorten the treatment period. In particular, it is worth considering that the use of laser-assisted therapies is associated with a marked reduction in the use of analgesics and anti-inflammatory medications compared with conventional procedures. This article is to state advantages and differences of Dental Laser treatment compared with Conventional procedures, and to emphasize to become well-acquainted with the precautions for safety and effective Dental Laser treatment. In case of operating Dental Laser with lack of the instructions, it will cause the unpredicted fatal results; therefore this treatment requires special care in its operation. Henceforward, it is anticipated that infinite treatment protocols will be introduced by applying Dental Laser, and this is to address the utilization of Dental Laser.

"Cohesiveness of Hyaluronic Acid Fillers": Evaluation Using Multiple Cohesion Tests

  • Background Hyaluronic acid fillers can be manufactured using various processes. They have multiple properties, including their concentration, degree of modification, and rheological data. Cohesion is one such property to evaluate gel integrity; however, there is no standardized method for calculating this parameter. This study aimed to evaluate different tests for calculating hyaluronic acid cohesion and discuss the importance of hyaluronic acid cohesion as a consideration when selecting fillers. Methods The cohesion levels of five different hyaluronic acid fillers with different rheological properties were evaluated and compared using the drop weight, compression, tack, and dispersion time tests. Results The cohesion tests yielded different results in the samples. Samples 2 and 4 showed approximately two times the number of droplets when compared with Sample 5 in drop weight test. Samples 1, 2, 3, and 4 were superior to Sample 5 in tack test. Samples 1, 2, and 3 showed cohesive appearances at 95 seconds in most cases in dispersion test. Rheological test results did not reflect the measures of cohesion. Conclusion Although there are no definite standardized tests to evaluate the cohesion of hyaluronic acid fillers, our proposed tests showed similar results for different hyaluronic acid filler products. Further studies are needed to evaluate the cohesion of hyaluronic acid fillers and determine the clinical use of this distinguishing characteristic for clinicians selecting the product of choice. 소형 금속판의 골수강 내 고정을 통한 최소 침습적 무지 외반증 교정 수술: 증례 보고 (Minimally Invasive Surgery for Hallux Valgus Deformity Using Intramedullary Low Profile Plate Fixation: A Case Report)

  • According to a recent systemic review, hallux valgus deformity has a prevalence rate of about 23% among adults aged 18 to 65 years. To date, more than 100 operative methods have been reported for the correction of hallux valgus deformity. For young female with mild to moderate hallux valgus deformity, minimally invasive surgery can be considered for aesthetic demands. Here, we report a case of a young female patient with mild hallux valgus deformity treated by minimally invasive surgery using intramedullary low profile plate fixation. This can be the favorable method for secure fixation of the osteotomy site and prevention of medial skin irritation symptoms derived from a sharp osteotomy margin.

경도 및 중등도 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술의 임상적 유용성 (The Clinical Usefulness of the Minimal Invasive Ulno-humeral Arthroplasty in the Patients with Mild to Moderate Elbow Arthritis)

  • 목적: 경도 및 중등도 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술의 임상적 유용성에 대하여 알아보고자 한다. 대상 및 방법: 2000년 1월에서 2008년 12월까지 척골 신경 증상이 없는 경도 및 중등도의 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술을 시행받은 환자 29예 중 1년 이상 추시가 가능하였던 24예를 대상으로 하였다. 남자 20예, 여자 4예였으며, 평균 연령은 53세 (31~69세)였고, 평균 수술 시간, 관절 운동 범위, 술 후 관절 운동 시작까지의 시간, 술 후 1년 추시에서 Mayo elbow performance score (MEPS)를 조사하였다. 결과: 술 후 평균 1.8일 (1~4일)째 수동적 및 능동적 관절 운동을 시작하였고, 평균 수술 시간은 38분 (25~55분)이었다. 관절 운동 범위는 술 전 평균 25~104도 (신전 0~70도, 굴곡 80~130도)에서 술 후 1년 추시 상 평균 14~133도 (신전 0~45도, 굴곡 90~150도)로 평균 40도 관절운동 범위가 향상되었으며, 술 후 관절 운동 시작까지의 시간은 평균 1.6일 (1~5일)이었다. 술후 1년 추시에서 MEPS는 우수 19예, 양호 5예이었다. 술 후 합병증으로 창상 치유지연 1예, 술후 부종 7예가 있었으나 자연적으로 호전되었다. 결론: 경도 및 중등도 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술은 수술 시간이 짧고 조기 관절 운동이 가능하며 통증이 적으므로 임상적으로 유용한 수술이다.

  • da $Vinci^{TM}$ 수술로봇(da $Vinci^{TM}$ Surgical System)은 삼차원영상시스템과 사람의 손동작을 재현할 수 있는 로봇팔을 갖추고 있어, 최소절개를 통한 제한된 공간에서도 입체시야에서 다양한 수술을 시행할 수 있도록 해줌으로써, 최소침습적수술의 장점을 극대화할 수 있도록 해준다. 저자들은 심방중격결손에 대하여 da $Vinci^{TM}$ 수술로봇을 이용한 개심수술을 성공적으로 시행하였기에 보고하는 바이다.

  • A coronary artery fistula was surgically ligated in a 38-year-old woman via a left anterior mini-thoracotomy without the use of cardiopulmonary bypass. In selected cases, this surgical approach can provide an excellent surgical exposure for coronary artery fistula ligation. It also offers an excellent cosmetic result and shorter hospital stay.

  • Pectus excavatum is rare, but it is the most common type of sternal congenital disorder. There are many surgical methods to correct pectus excavatum such as the Ravitch method, Wada method, Silastic mold method, and the Nuss operation. We report a case of minimal invasive surgery for pectus excavatum using a polyvinyl alcohol sponge.
