집속 초음파 뇌 질환 치료를 위한 두부 유사 팬텀의 개발 (Development of Human-Head-Mimicking Phantom for Brain Treatment Using Focused Ultrasound)

  • 본 연구에서는 집속형 초음파를 이용하는 뇌 질환 치료의 연구를 위한 인체 두부 유사 팬텀을 개발하였다. 문헌 연구를 통하여 피부 조직, 두개골, 뇌 조직의 음향학적 및 물리적 특성을 조사하였으며 근사한 값을 가지는 적합한 각 조직 대체 물질을 제시하였다. 피부 조직의 경우 글리세롤 기반 연부 조직 유사 팬텀의 성분비를 조정하여 실제 조직과 유사한 음향학적 특성을 가지도록 하였으며 고분자 합성수지의 음향학적 특성을 측정하여 두개골 유사 물질로써의 적합성을 평가하였다. 뇌 조직은 투명한 egg white 팬텀을 이용하여 집속형 초음파의 가열 특성을 확인할 수 있도록 하였다. 또한 뇌 질환 치료 프로토콜 개발을 위한 시험 조사가 가능하도록 대체 물질들을 결합한 두부 유사 팬텀을 제작하였고 제작된 팬텀의 유효성 및 활용성 평가를 위해 초음파 조사 조건에 따른 팬텀의 변성을 관찰하였다.

  • Porous scaffolds composed of gelatin and polysaccharides such as hyaluronic acid and $\beta$-glucan were prepared by using the freeze-drying method after cross-linking with l-ethyl-(3-3-dimethylaminopropyl) carbodiimide hydrochloride (EDC). The scaffold had an inter-connected pore structure with the sufficient pore size for use as a support for the growth of fibroblasts. Results for the contact angle and cell attachment confirmed that high gelatin content in a mixture was suitable for cellular attachment and distribution in two- or three-dimensional fibroblast cultures. However, the addition of polysaccharides aroused the synergistic effects of morphologic and mechanical property of gelatin-based scaffolds. To prepare the artificial dermis for the wound dressing to mimic the normal human dermal skin, fibroblasts were isolated from a child's foreskin, and cultured in gelatin-based scaffolds. An in vivo study showed that the artificial dermis containing the fibroblasts enhanced the wound healing rate and re-epithelialization of a full-thickness skin defect rather than the acellular scaffold after one week.

Medial and Lateral Canthal Reconstruction with an Orbicularis Oculi Myocutaneous Island Flap

  • Background The eyelid and canthal areas are common locations for cutaneous tumors. The medial canthus includes, among many other apparatuses, the canthal tendon and lacrimal canaliculi, and its characteristic thin and supple skin is hard to mimic and restore using tissue from other regions. Accordingly, reconstruction of the canthal area can prove challenging for surgeons. Although various methods, such as skin grafts and local flaps from adjacent regions, have been utilized for reconstructive purposes, they present known disadvantages. However, we were able to successfully reconstruct both lateral and medial canthal area defects by using orbicularis oculi myocutaneous island flaps. Methods Our study included seven patients who underwent medial or lateral canthal region reconstruction, using orbicularis oculi myocutaneous island flaps, between 2011 and 2014, following either cutaneous tumor excision or traumatic avulsion injury. Results Five patients had basal cell carcinoma, one had squamous cell carcinoma of the eyelid, and one had sustained a traumatic avulsion injury of the eyelid and canthal area. Entire flap loss was not observed in any patient, but one-a heavy smoker-showed partial flap loss, which healed with secondary intention and yielded acceptable results. Donor site morbidity was not observed, and all patients were satisfied with their surgical outcomes. Conclusions The canthal regions can be successfully reconstructed with orbicularis oculi myocutaneous island flaps. These flaps offer several key advantages, including similarity in texture, color, and thickness to the recipient site and a negligible incidence of donor site morbidity.

눈대상포진이 병발된 경미한 얼굴의 화상 (Herpes Zoster Ophthalmicus in Minor Facial Burn)

  • Purpose: Many conditions can mimic the presentation of burns. Herpes zoster is one of them. The characteristic features of herpes zoster such as vesicles, pustular lesions and crusts can also be found in burns. Herpes zoster ophthalmicus is a disease caused by recurrent infection of varicella - zoster virus in the ophthalmic division of the trigeminal nerve. This virus frequently affects nasociliary branch and serious ocular complications can occur. Thus, early diagnosis and proper treatment of this disease is important to prevent further ocular manifestations. We report a man who sustained minor facial burn injury that was complicated with herpes zoster ophthalmicus. Methods: A 66 - year - old man visited emergency room with multiple whitish vesicles with serous discharge on right forehead, right medial canthal area and nose. At first he was thought to have a secondary infection of facial burn injury. The vesicles on his face began to form crusts on the next day. Since his skin lesion was located on the ophthalmic division of trigeminal nerve, we also suspected herpes zoster ophthalmicus. He was referred to dermatologist and ophthalmologist. Results: We used antiviral agent (Acyclovir) and NSAIDs for treatment. The patient had no ocular complications. His skin lesion was almost healed after 1 month and remained scars. We treated a patient with minor facial burn complicated with herpes zoster ophthalmicus with antiviral agent. Conclusion: In this work, we describe a case of old patient with herpetic infection and emphasize the need for careful examination to diagnose accurately.

식물성 재료원 비누의 피부 위 미세먼지 세정, 인체 적용 및 안전성 효과에 관한 연구 (Cleansing of Fine Dust on the Skin, Application to the Human Body and Safety Effect of Botanical-sourced Soap)

  • 배경: 미용비누는 일반적으로 세정효과가 있고 인체에 안전하다고 보고되어 있으나, 새 제품은 항상 평가의 대상이 된다. 목적: 다지워 비누(Daziwar soap bar)의 미세먼지 모사체의 세정효과에 대한 인체적용 시험 및 안전성 평가를 여성 52명에게 수행하는 것이 목적이었다. 방법은 미세먼지 세정효과는 건강한 성인 여성 22명의 아래팔 안쪽 피부 부위에 모사체 미세먼지를 분사한 직후에 세척 전과 후의 양적 변화를 측정하였다. 안전성 평가는 30명의 여성의 등에 첩포를 한 후에 24시간이 지나서 30분, 24시간 48시간 때에 반응을 평가하였다. 결과: 팔 안쪽 부위에 모사체 미세먼지를 분사한 직후에, 다지워비누로 1회세척 후에 잔존 모사체량은 시험군이나 대조군 모두 통계적으로 유의하게 감소하였다(p < .001). 그러나 세척률에서는 실험군에서는 99.96%, 대조군에서는 75.58%로 나타났다. 시험대상자의 주관적 설문 평가에서는, 비누를 사용한 직후에 유효성이 있는 항목에서 '좋은(Good)' 이상으로 평가되었다. 피부과 전문의사의 미세먼지 세척 후 이상 반응 평가에서는, 시제품을 사용하는 동안 시험 대상자에게서 특별한 피부 이상 반응에 대한 보고나 관찰이 없었고, 피부의 이학적 검사에서도 이상 소견은 관찰되지 않았다. 안전성평가에서, 등 부위에 24시간 첨포 후에 30분, 24시간, 48시간 때에 피부 반응 평가에서 모두 무자극으로 판정되었다. 결론: 시험제품인 다지워 비누는 인체 상의 미세먼지의 세정에 크게 도움을 주며 인체에 안전하다는 것을 알 수 있었다.

기저세포모반 증후군과 관련된 다발성 낭종의 장기 치료결과: 증례보고 (LONG TERM FOLLOW-UP OF MULTIPLE ODONTOGENIC KERATOCYSTS ASSOCIATED WITH BASAL CELL NEVUS SYNDROME: A CASE REPORT)

  • Multiple jaw cysts are one of the most constant features of the basal cell nevus syndrome. Basal cell nevus syndrome is inherited as an autosomal dominant trait with variable expressiveness. This syndrome comprises a number of abnormalities such as multiple nevoid basal cell carcinomas of the skin, skeletal abnormalities as bifid rib and fusion of vertebrae, central nervous system abnormalities as mental retardation, eye abnormalities with multiple jaw cysts. The odontogenic keratocysts in patients with this syndrome are often associated with the crowns of unerupted teeth and huge size; on radiographs they may mimic dentigerous cysts. The most important feature of the cyst is its extraordinary recurrence rate. Since recurrence may be long delayed in this lesion, follow-up of any case of odontogenic keratocyst with roentgenograms and clinical examination of basal cell carcinoma are essential for at least five years after surgery. We report the result of 7-year follow up after cyst enucleation associated with basal cell nevus syndrome with the literature of review.

헤르페스 바이러스 감염으로 발생한 대식 세포 활성 증후군을 첫 증상으로 한 소아 전신 홍반 루푸스 (Macrophage Activation Syndrome Triggered by Herpes Viral Infection as the Presenting Manifestation of Juvenile Systemic Lupus Erythematosus)

  • 대식세포 활성 증후군(MAS, Macrophage activation syndrome)은 전신 홍반성 루푸스(SLE, systemic lupus erythematous) 환자에서 감염에 의해 나타날 수 있는 드문 합병증이다. MAS는 기저의 자가면역질환의 임상양상과 유사하게 나타나거나 혹은 감염성 합병증과 혼돈될 수 있어 감별에 주의 하여야 한다. 14세 여환이 2주간 지속되는 발열과 통증을 동반하는 얼굴의 피부 발진을 주소로 내원하였다. 피부 발진과 간비대, 범혈구 감소증, aspartate aminotransferase, lactate dehydrogenase, 혈청 ferritin이 상승하여, MAS를 의심하였다. 피부 병변과 항핵체 양성, C3와 C4의 감소, 간접 쿰스검사 양성으로 SLE를 진단하였다. 따라서 본 증례는 MAS가 HSV에 의하여 촉발된 것을, SLE의 첫 증상으로서 나타낸 증례로서, 촉발 요인 및 기저질환을 치료함으로써 중증의 합병증 없이 호전되었다.

하지정맥류 모사 생체조직 팬텀과 조직등가 팬텀을 이용한 초음파 치료효과 평가에 관한 연구 (A Study on the Efficiency Evaluation of Ultrasound Therapy Using Varicose Vein Simulated Tissue Phantom and Tissue Equivalent Phantom)

  • 비침습적 치료 효과에 대한 기대로 집속 초음파를 이용한 하지정맥류 치료에 대한 많은 연구가 진행되고 있다. 본 연구에서는 초음파 정맥류 정맥 치료 효과 평가에 적용할 수 있는 생체 조직 팬텀과 조직 등가 팬텀을 제시 하였다. 제작된 팬텀은 음향학적 특징과 초음파 조사에 따른 수축률을 평가함으로써 유용성을 평가하였다. 하지정맥류가 발생하는 조직의 구조를 고려하여 피부, 지방, 근육의 세 층으로 된 다층 구조 팬텀을 제작하였으며, 각 층을 구성하는 물질은 인체와 유사한 특성을 갖도록 제작하였다. 또한 혈관 유사물질을 매식한 다층 구조 팬텀과 동물 혈관을 이용한 다층 구조 팬텀, 동물 조직을 이용한 다층 구조 팬텀을 제작하였으며, 초음파 조사에 따른 혈관 유사 물질 및 혈관 조직의 수축 정도를 B-mode 영상을 이용하여 평가하였다. 본 연구의 결과를 통하여 제시된 팬텀이 초음파 하지정맥류 치료 평가에 유효하게 활용될 수 있을 것으로 판단되었다. 또한, 집속 초음파를 이용한 하지정맥류 치료 장비 개발 및 치료 효과 검증에 중요한 역할을 할 것으로 판단된다.

Structure and Bacterial Cell Selectivity of a Fish-Derived Antimicrobial Peptide, Pleurocidin

  • Pleurocidin, an $\alpha$-helical cationic antimicrobial peptide, was isolated from skin mucosa of winter flounder (Pleuronectes americamus). It had strong antimicrobial activities against Gram-positive and Gram-negative bacteria, but had very weak hemolytic activity. The Gly$^{13,17}\rightarrow$Ala analog (pleurocidin-AA) showed similar antibacterial activities, but had dramatically increased hemolytic activity. The bacterial cell selectivity of pleurocidin was confirmed through the membrane-disrupting and membrane-binding affinities using dye leakage, tryptophan fluorescence blue shift, and tryptophan quenching experiments. However, the non-cell-selective antimicrobial peptide, pleurocidin-AA, interacts strongly with both negatively charged and zwitterionic phospholipid membranes, the latter of which are the major constituents of the outer leaflet of erythrocytes. Circular dihroism spectra showed that pleurocidin-AA has much higher contents of $\alpha$-helical conformation than pleurocidin. The tertiary structure determined by NMR spectroscopy showed that pleurocidin has a flexible. structure between the long helix from $Gly^3$ to $Gly^{17}$ and the short helix from $Gly^{17}$ to $Leu^{25}$. Cell-selective antimicrobial peptide pleurocidin interacts strongly with negatively charged phospholipid membranes, which mimic bacterial membranes. Structural flexibility between the two helices may play a key role in bacterial cell selectivity of pleurocidin.

  • Rheumatoid arthritis (RA) is a chronic autoimmune disorder characterized by progressive joint deterioration; Furthermore, RA can also affect body tissues, including the skin, eyes, lungs, heart and blood vessels. The early stages of RA can be difficult to diagnose because the signs and symptoms mimic those of many other diseases. It is not known exactly what triggers the onset of RA and how to cure the disease. But recent discoveries indicate that remission of symptoms is more likely when treatment begins early with strong medications known as disease-modifying anti-rheumatic drugs (DMARDs). Tumor necrosis factor (TNF) inhibitors are typical examples of biotherapies that have been developed for RA. The substances may occur naturally in the body or may be made in the laboratory. Other biological therapies care biological response modifiers (BRMs)such as monoclonal antibodies, interferon, interleukin-2 (IL-2) and a protein binder using repeat units. These substances play significant anti-inflammatory roles. Proteins with recurrent, conserved amino acid stretches mediate interactions among proteins for essential biological functions; for example, ankyrin (ANK), Heat repeat protein (HEAT), armadillo repeat protein (ARM) and tetratricopeptide repeats (TPR). Here, we describe Leucine rich repeats (LRR) that ideally fold together to form a solenoid protein domain and is more applicable to our current study than the previously mentioned examples. Although BRMs have limitations in terms of immunogenicity and effector functions, among other factors, in the context therapeutic use and for proteomics research, We has become clear that repeat-unit-derived binding proteins will increasingly be used in biotechnology and medicine.
