서울지역 거주 노인들의 만성질환에 따른 식품 및 영양소 섭취 실태 조사 (The intake of food and nutrient by the elderly with chronic disease in the Seoul area)

  • This study was carried out in order to investigate the status of food and nutrient intakes depending on the types of chronic disease of the elderly at welfare centers in Seoul. The total number of subjects was 299 elderly over the age of 60 (82 men and 217 women); they were grouped by the disease status: normal, cardiovascular disease (CVD), diabetes mellitus (DM) and arthritis patients groups. The dietary intakes were obtained by a 24-hour recall and analyzed by CAN-Pro 4.0; they were then compared with the dietary reference intakes for the Korean (KDRIs). In the results of anthropometric data according to chronic disease, the waist circumference of the normal group in males and females was lower than the other groups; in particular, the normal female group showed a lower level of waist-hip ratio, body mass index (BMI) and body fat % than those of the other groups. The arthritis group showed a higher intake of milk products than the other groups (p < 0.05); consequently, calcium intake was significantly higher than that of the other groups (p < 0.001). Vitamin A intake in the arthritis group was significantly higher than the intake in the normal and DM groups (p < 0.01). Normal and arthritis groups showed significantly higher intake of zinc and copper than the CVD group (p < 0.01). In general, about 80% of the subjects in this study revealed insufficient intakes of riboflavin, vitamin C, calcium, dietary fiber, vitamin D, biotin and potassium, compared with the estimated average requirement (EAR) or adequate intake (AI). However, sodium intake of all groups was higher than the AI of KDRIs. In conclusion, nutrient intake, according to the chronic disease, showed significant difference in some of the micronutrients, vitamin A, Ca, Zn and Cu. Therefore, we suggest that dietary guidelines, such as reducing the intake of salty foods and sweet drinks and increasing the intake of foods with sufficient calcium, vitamin D and vitamin C, are necessary to the improvement of eating habits for the elderly.

  • This study was conducted to investigate health status and nutrient intakes among 5th grade elementary students at Korea Centers for Disease Control and Prevention 2010. This study was cross-sectional study on 1,384 children (687 boys, 697 girls) from nine elementary schools located in Seoul and Gyeonggi province. The average height, weight and BMI were 145 cm, 40.6 kg $19.2\;kg/m^2$ for boys and 145.4 cm, 38.2 kg, $18.0\;kg/m^2$ for girls. The prevalence of overweight and obesity were 11.3%, 5.2%, respectively. Serum AST, ALT, glucose, HDL-Cholesterol and RBC levels were significantly higher, while TG levels was significantly lower for boys than for girls. The average energy intake was 1772.4 kcal, which was 98.7% of Estimated Energy Requirement (EER). The boys consumed more energy intake (1800.4 kcal) than the girls (1744.7 kcal). Also, they took insufficient calcium and folate (69.1% and 83.3% of Recommended Intake (RI)) and excess sodium (297.6% of Adequate Intake (AI)) and 85.1% of the subjects had breakfast everyday. In general, the proportion of the children who consumed fruits and vegetables at least once or more a day was low. There was a higher proportion of children in boys who had ramyun and milk with sufficient physical activity than those in girls. As a result of this study, we can find risk factors on obesity and metabolic disorders, and the results can be used for an evidence of nutrition education program and the intervention program.

충남 일부지역 초등학생의 영아기 영양법에 따른 식습관과 영양섭취상태 (Dietary Habits and Nutrient Intakes according to Feeding Method during Infant Period in Elementary School Students)

  • 본 연구에서는 초등학생들을 영아기 영양법에 따라 모유영양군, 인공영양군, 혼합영양군으로 분류하여 영아기 영양법에 따른 식습관과 영양 섭취 상태를 비교 분석함으로써 영아기 영양법이 이후의 식사법에 미치는 영향을 살펴보고자 하였으며, 그 결과는 다음과 같다. 전체 조사대상자의 평균 연령은 9.7세였으며, 신장, 체중, 체질량지수는 각각 139.0 cm, 34.6 kg, $17.8kg/m^2$로 영아기 영양법에 따른 군간 유의한 차이가 없었다. 영아기 영양방법에 대해 조사한 결과 초유를 섭취한 비율은 모유영양군 97.2%, 혼합영양군 92.7%로 인공영양군의 40.9%보다 유의하게 높았으며(P<0.001), 이유 시기 및 첫 이유 식품에서 세 군간 유의한 차이는 보이지 않았다. 또한 식습관 조사 결과 간식을 전혀 먹지 않는다는 비율은 모유영양군 10.0%, 인공영양군 9.4%로 혼합영양군의 3.8%보다 유의하게 높았으며(P<0.05), 식사 및 외식빈도 등은 군간 유의한 차이를 보이지 않았다. 총 식습관 점수와 에너지 및 영양소 섭취량은 영아기 영양법에 따른 군간 유의한 차이를 보이지 않았다. 이상의 연구 결과를 종합하여 볼 때 성장기 아동에서 영아기 영양법에 따라 간식섭취 빈도, 우유 유제품 및 채소 섭취 정도 등과 같은 식습관이 다른 것으로 나타났으며, 추후 영아기 영양법에 따른 성장, 건강, 식습관 및 식사섭취상태 등에 대한 장기적인 추적조사가 필요할 것으로 사료된다.

2020 한국인 영양소 섭취기준 개정: 탄수화물 (The development of the 2020 Dietary Reference Intakes for Koreans: carbohydrate)

  • 2020 한국인 영양소 섭취기준에서 탄수화물은 만성질환 예방을 위한 섭취기준으로 총 에너지 섭취량에 대한 탄수화물로부터의 에너지 섭취비율인 에너지적정비율을 1세 이후 모든 연령에서 55-65%로설정하였다. 그리고 2020년에 처음으로 탄수화물의 평균필요량과 권장섭취량을 설정하였다. 인체의 탄수화물 필요량은 뇌에서 하루에 소비되는 포도당량과 케톤체 생성이 나타나지 않는 양을 기준으로 1세 이후 모든 연령에서 1일 100 g을 설정하고, 권장섭취량은 15% 변이계수를 사용하여 두 배의 변이계수를 더한 130 g/일로 설정하였다. 탄수화물의 평균필요량은 최소필요량의 개념이지 에너지원으로서 적절한 섭취량에 대한 개념은 아니므로 탄수화물의 영양소섭취상태를 평가할 경우에는 평균필요량이나 권장섭취량에 비교하는 것은 적절하지 않고, 에너지적정비율을 활용하여 탄수화물로부터의 에너지 섭취비율의 적절성을 평가하는 것이 적절하다. 탄수화물은 에너지 적정비율범위에서 섭취할 때 만성질환의 위험을 낮출 수 있으며, 이를 탄수화물의 양뿐만 아니라 급원도 고려하는 식생활에 활용해야 할 것이다.

공주지역 여대생의 지방 섭취 양상 조사 (Patterns of Dietary Fat Intake by University Female Students Living in Kongju City: Comparisons among Groups Divided by Living Arrangement)

  • This study was aimed at examining the pattern of dietary fat intake of 120 university female students living in Kongju city in order to provide baseic data for establishing the dietary grideline of desirable fat intake. The subjects were divided into three groups according to living arrangenment ; family home(FH), or dormitory(DM), or self-boarding house(SB). Fat consumption of subjects was surveyed by two-day food records, and profiles of fatty acid and cholesterol taken were calculated based upon the data reported their contents of foods. The result was that mean daily fat intake was 42$\pm$2g for FH, 48$\pm$4g for DM and 41$\pm$3g for SB. The calorie intake by fat was 20.8, 21.5, and 20.4% respectively. The ratio of P/M/S was 1.3/1.7/1 for FH, 1.3/1.6/1 for DM and 1, .5/1.5/1 for SB. And the ratio of n-6/n-3 fatty acid was 5.2/1 for FH, 8.3/1 for DM and 7.2/1 for SB. Daily cholesterol intake was 208$\pm$20mg for FH, 223$\pm$29mg for DM and 251$\pm$27mg for SB. In addition, intake of n-3 fatty acid was 1.2$\pm$0.2g for FH, 0.9$\pm$0.1g for DM and 1.2$\pm$0.2g for DB. Considering the food source of fatty acids, saturated fatty acid and monounsaturated fatty acid were taken primarily from animal foods, especially milk and milk products, and n-6 polyunsaturated fatty acid was taken from vegetable oils and fats. As a major source of n-3 fatty acid, linolenic acid was obtained through vegetable oils and fats, and eicosapentaenoic acid and docosahexaenoic acid were provided by fishes and their products. There patterns of fatty acid intakes did not differ according to living arrangement. The above results showed that intakes of fat and cholesterol, and ratios of P/M/S and n-6/n-3 fatty acid were overall desirable in all groups. However, intake of n-3 fatty acid was low in all groups. Therefore, consumption of perilla oil, legumes and fishes as a source of n-3 fatty acid should be increased by substituting other food source which provide fats and oils because calorie intake by fat was enough in these subject.

부산지역 일부 대학생의 식습관 및 영양소 섭취상태에 관한 연구 (The Comparison in Daily Intake of Nutrients and Dietary Habits of College Students in Busan)

  • The purpose of this study was to investigate the dietary habits and daily intake of nutrients in college students. This survey was conducted using a self-administered questionaire. The average heights and weights were 173.5 cm and 72.3 kg of male students and 161.8 cm and 57.2 kg of female students. The average of BMI was $24.2kg/m^2$ of male and $21.9kg/m^2$ of female, and the value of male students was higher than the value of female students. The response to the daily meals was 54.6% for '$2{\sim}3$ times/week'. The regularity of mealtime was 41.7% for irregular and the frequency eating after nine was 45.7% for '5-6 times/week', respectively. The repast was 72.2% for 'overeating and little eating' and was a significant difference of male and female students (p<0.05). The eating rate was higher '$10{\sim}20min$'. As for breakfast food eaten, skipping breakfast was 23.6% for 'no/week' and female students were higher than male students (p<0.05). The frequency of snacks was 36.0% for 'nothing' of males students and 34.8% for '3-4 times/week' of female students (p<0.05). The type of snack was a significant difference of males and females students (p<0.01), and was the highest 75.0% for carbonated drinks of males and 37.5% for snacks of females. The eating due to stress solution was a significant difference of male and female students (p<0.01), and was the highest 23.0% for 'frequency' of males and 44.7% for 'sometime' of females. As for food intake of male and female students, the meat intake was 66.7% for 'everything of male and female students. The fish intake was 68.1 % for '1-2times/week'. The milk, milk products, eggs and beans were each 40.3%, 58.3%, 56.9%, 47.2% for '1-2 times/week' (p<0.05). The fat intake was 55.6% for '$1{\sim}2$ times/week'. The average consumption of energy was 58% of male and 67% of female of estimated energy requirement (EER). Their mean ratio of carbohydrate: protein: fat was 57 : 15 : 28 of all subjects. The mean intakes of vitamin C and folic acid were 70% and 51% of males and 62% and 52% of females of recommended intake (RI). The mean intakes of Ca, P, Fe and Na were 71%, 140%, 146% of males and 72%, 122%, 76% of female of RI and 273% of males and 233% of females of adequate intake (AI). Therefore, nutritional education is necessary for college students to establish physicall and mentall optimal health conditions though nutritional intervention.

일부 농촌지역 부인의 영양실태 및 혈액성상에 관한 연구 (A Survey of Nutrition and Blood Contents of Rural Women in Korea)

  • The nutritional survey and blood sampling was conducted from May 23 to 27, 1971, in order to investigate the state of the nutritional intake in relation to blood contents of rural women. This survey covered a total of 47 women residing in Youngam-gun, Cholla-namdo, Korea. The daily diet of these subjects was observed by 24-hour recall method and weighing method for 3 days and hemoglobin (Hb), hematocrit(Hct), mean corpuscular hemoglobin concentration (MCHC), serum protein, serum albumin, A/G ratio, serum iron, total iron binding capacity (TIBC), transferrin saturation (TS), serum cholesterol and blood specific gravity (SG) were measured. The results were summarized as follows ; The rate of intake of vegetable foods was high, especially cereals, and that of animal foods was low, especially eggs and milk and milk products. The daily intake of niacin was sufficient, and the intakes of energy and iron were also tend to be adequate. On the contrary, the intakes of vitamin A, $B_2$ and C, calcium and protein were insufficient. The mean red blood cell number was $410.8{\pm}26.4{\times}10^{4}/mm^{3},$ the mean values of Hb, Hct and MCHC were $12.5{\pm}0.9gm%$, $37.5{\pm}2.4%$ and $33.2{\pm}0.5%$ thus the anemic prevalences were 21.3%, 12.8% and 93.6% comparing with the anemic criteria of WHO. The levels of serum protein and serum albumin were $7.1{\pm}0.4gm\;%$ and $4.3{\pm}0.3gm\;%$so the A/G ratio was indicated $1.6{\pm}0.2.$. The levels of serum iron and TIBC were $91.9{\pm}27.5{\mu}g/100ml and $297.5{\pm}31.4{\mu}g/100ml,$ so TS was calculated $31.2{\pm}9.7\;%$. The mean value of SG was $1.044{\pm}0.002,$ and the level of serum cholesterol was $164.3{\pm}25.4mg/dl$. In the relation between each of bloood content, Hb showed positive correlation not only with Hct but also protein at the level of significance of 1 per cent and 5 per cent, and protein also showed a positive correlation with serum cholesterol at the 1 per cent level of significance. In the relation between the nutrient intake and blood contents, vitamin C intake showed a positive correlation with Hb, serum protein and serum iron at the level of significance of 1 per cent, iron intake also showed a positive correlation with serum iron and serum cholesterol at the same significant level, and protein intake showed a positive correlation with serum iron at the at the level of significance of 5 per cent.

한국 당뇨병 노인의 혈액생화학적 특성, 식이 섭취, 당화혈색소 관련 위험 요인 및 위험도: 국민건강영양조사 제4기(2007-2009)와 제7기(2016-2018) 비교 (Blood Biochemical Characteristics, Dietary Intake, and Risk Factors Related to Poor HbA1c Control in Elderly Korean Diabetes Patients: Comparison between the 4th(2007-2009) and the 7th(2016-2018) Korea National Health and Nutrition Examination Surveys)

  • Objectives: The purpose of this study was to investigate the blood biochemical characteristics, comorbidities, dietary intake, and other risk factors leading to poor glycated hemoglobin (HbA1c) control in elderly Korean diabetes patients over 65 years of age. Methods: Data from the 4th (2007-2009) and the 7th (2016-2018) Korea National Health and Nutrition Examination Surveys (KNHANES) were used. Socio-demographic characteristics (age, gender, region, household income, education level, marital status, nutrition education, diabetes duration and diabetes treatment), lifestyle characteristics (drinking, smoking, regular walking, and subjective health perception), anthropometric characteristics (height, weight, and waist circumference), blood biochemical characteristics (HbA1c, high- density lipoprotein cholesterol, triglycerides, total cholesterol, blood urea nitrogen, and blood creatinine), co-morbidities (obesity, hypertension, hypercholesterolemia, hypertriglyceridemia, and anemia), energy and nutrients intake, food group intake, and HbA1c control-related risk factors were compared. Results: Compared to the 4th survey, the 7th survey showed an increase in diabetes prevalence among men, an increase in the prevalence of diabetes in the older patients, and an increase in the duration of diabetes. The energy ratio from carbohydrate consumption in the 7th survey was lower than in the 4th. Compared to the 4th survey, thiamine and riboflavin intake had improved, and the intakes of vitamin A, vitamin C, and niacin had worsened in the 7th. A comparison of food group intakes showed that there was a decrease in the consumption of whole grains, potatoes, and milk and an increase in the intake of beverages and alcoholic beverages. The risk factors for poor control of HbA1c were the duration of diabetes and co-morbid hypertriglyceridemia in the 4th survey, whereas subjective health perception, obesity, and hypercholesterolemia as co-morbidities were found to be risk factors in the 7th in addition to the risk factors highlighted in the 4th survey. Conclusions: For the future management of elderly Korean diabetes patients, greater care is indicated for men over 75 years, and those with low levels of education. It is necessary to increase the intake of milk and vegetables, and reduce the intake of beverages and alcoholic beverages. In addition, it is necessary to reduce the incidence of obesity, hypercholesterolemia, and hypertriglyceridemia for proper control of blood sugar.

성인 남성의 규칙적인 운동이 비만지표와 식이섭취 요인에 미치는 영향 (The Effects of Regular Exercise on Obesity Indices and Dietary Factors in Adult Males)

  • This study was performed to investigate the effects of regular exercise on dietary factors and obesity indices among 407 healthy adult males subjects. Subjects were classified into regular exercise group (REG) and irregular exercise group (IREG). Two hundred and thirteen subjects of REG excercised regularly 3 times (more than 30 minutes/time) per week during more than last 1 month or more. One hundred and ninety-four of IREG (n = 194) didn't regularly exercise during the last 1 month. Obesity indices were BMI (Body Mass Index), WHR (Waist Hip Ratio) and PIBW (Percentage of Ideal Body Weight). And the mean BMI, WHR and PIBW of REG were (22.1, 0.90 and 105.8) significantly lower than those of IREG (25.7, 0.98 and 117.7) respectively. The mean daily starches, seeds, meats, eggs, fish, milk, fats and processed food intakes of REG were significantly lower than those of IREG And the mean daily vegetables, mushrooms and beverages intakes of the IREG were significantly lower than REG. Energy intake of REG and IREG were 1968.2 kcal and 1978.9 kcal respectively. Vitamin C intake of IREG was significantly lower than REG. But niacin and cholesterol intake of REG were significantly lower than the IREG. Exercise regularity was positively related with obesity indices and dietary factors. Therefore, it is necessary to exercise regularly to prevent obesity and cardiovascular disease in Korean adult males.

한국노인을 대상으로 한 영양소 섭취의 주요급원식품과 주요변이식품들을 이용한 간이식품섭취빈도조사지의 개발 (Development of a Simple Food Frequency Questionnaire Using the Contribution of Specific Foods to Absolute Intake and Between-Person Variation of Nutrient Consumption for the Korean Elderly)

  • Contributions of specific foods to absolute intake and between-person variance in 13 nutrients were examined to develop a short food frequency questionnaire (FFQ) for the Korean elderly using dietary data collected by a semiquantitative FFQ with 96 food items. The contributions of specific foods to 90% absolute nutrient intake were measured by assessing their percentage to the total consumption of a nutrient by our samples(n=98). To assess the contribution of the foods to the between-person variance in intake of each specific nutrient, stepwise multiple regression was performed. Cumulative R2 served as the measure of contribution to variation in intake. The most important food source of absolute intake was rice for energy and protein. Important foods of between person variance contribution include bread for energy and pork for protein. Important foods of absolute and variance contribution of nutrient intake were similar for fat(noodles including jajangmyun and kalguksu), calcium(anchovy and milk) and vitamin A (carrot). The number of foods necessary to account for the variation in nutrient intake among older people in Korea differ considerably by nutrient: 18 - 43 and 8-26 foods were accounted for the respective 90 and 80% of absolute intake depending on nutrients. Carbohydrates and vitamin A and C had relatively a few major sources, thus may be assessed well by a small number of foods. Conversely, iron, potassium and B vitamins appear to need longer lists of foods. Fewer numbeer (6 - 12 and 4 - 9) of foods were required for the corresponding percentages of between-person variation for all nutrients. Depending on nutrients, 0.980-0.995 and 0.924-0.987 were respective correlation coefficients of nutrient intakes measured by 96 food items with those by lists of foods for 90% and 80% of absolute intake contribution. These findings suggest that an incomplete food list on the basis of its contribution to absolute and between person variation intakes may still be effective in discriminating among individuals. The results of this study may be useful in the design and use of dietary questionnaire.
