Purpose: This study was conducted to assess the factors affecting turnover intention of the nurses working in neonatal intensive care units, including the extent of fatigue, work stresses, and work overload related to infection control. Methods: This was a descriptive correlational study to test factors affecting the turnover intention of the nurses working in neonatal intensive care units. Data was analyzed utilizing descriptive statistics, t-test, One-way ANOVA, Pearsons' Correlation Coefficient, and multiple regressions by using the IBM SPSS Statistics 23.0 Program(IBM Corp., Armonk, NY). Results: The turnover intention of nurses had a positive correlation with the extent of fatigue (r=.54, p<.001), work stress (r=.40, p<.001), and with the work overload (r=.43, p<.001) related to infection control. In addition, factors affecting the turnover intention of nurses included the extent of fatigue (β=.52, p<.001) related to infection control and the number of pediatric patients assigned to each nurse (β=.26, p=.001) and the variances explained by the regression model was 37.0%. Conclusion: It is necessary to establish and implement strategies to lower the extent of fatigue related to infection control by reducing the nurses' workload. Securing additional nurses to ensure an appropriate number of pediatric patients assigned to each nurse, and providing efficient management and available resources to reduce the turnover intention of nurses working in neonatal intensive care units are suggested.
Objective: The occurrence of intramyocardial hemorrhage (IMH) and microvascular obstruction (MVO) in myocardial infarction (MI), known as severe ischemia/reperfusion injury (IRI), has been associated with adverse remodeling. APT102, a soluble human recombinant ecto-nucleoside triphosphate diphosphohydrolase-1, can hydrolyze extracellular nucleotides to attenuate their prothrombotic and proinflammatory effects. The purpose of this study was to temporally evaluate the therapeutic effect of APT102 on IRI in rats and to elucidate the evolution of IRI in the acute stage using cardiovascular magnetic resonance imaging (CMRI). Materials and Methods: Fifty-four rats with MI, induced by ligation of the origin of the left anterior descending coronary artery for 60 minutes, were randomly divided into the APT102 (n = 27) or control (n = 27) group. Intravenous infusion of APT102 (0.3 mg/kg) or placebo was administered 15 minutes before reperfusion, and then 24 hours, 48 hours, 72 hours, and on day 4 after reperfusion. CMRI was performed at 24 hours, 48 hours, 72 hours, and on day 5 post-reperfusion using a 7T system and the hearts were collected for histopathological examination. Cardiac function was quantified using cine imaging and IMH/edema using T2 mapping, and infarct/MVO using late gadolinium enhancement. Results: The extent of infarction (p < 0.001), edema (p < 0.001), IMH (p = 0.013), and MVO (p = 0.049) was less severe in the APT102 group than in the control group. IMH size at 48 hours was significantly greater than that at 24 hours, 72 hours, and 5 days after reperfusion (all p < 0.001). The left ventricular ejection fraction (LVEF) was significantly greater in the APT102 group than in the control group (p = 0.006). There was a negative correlation between LVEF and IMH (r = -0.294, p = 0.010) and a positive correlation between IMH and MVO (r = 0.392, p < 0.001). Conclusion: APT102 can significantly alleviate damage to the ischemic myocardium and microvasculature. IMH size peaked at 48 hours post reperfusion and IMH is a downstream consequence of MVO. IMH may be a potential therapeutic target to prevent adverse remodeling in MI.
Purpose : The purpose of this study was to identify the factors influencing the general hospital nurses' confidence about the advance directives (ADs). Methods : This is a descriptive cross-sectional study. A total of 105 nurses were asked questions about characteristics, knowledge, attitude, learning needs, and confidence the about ADs, using structured questionnaires. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression. Results : The mean scores of knowledge were 7.26±1.47; attitude, 2.80±0.31; learning needs, 4.22±0.62; and confidence, 2.57±0.74. It was observed that knowledge had significant correlations with attitude (r=.43, p<.001), learning needs (r=.29, p=.003), and confidence (r=.40, p<.001), and attitude had significant correlations with learning needs (r=.31, p=.001) and confidence (r=.45, p<.001). Using multiple regression analysis, it was found that confidence was significantly influenced by knowledge (β=.27, p=.007), attitude (β=.21, p=.043), and experience of nursing or counseling a patient who wrote ADs (β=-.19, p=.046). The explanatory power of this model was 37.9% (F=5.16, p<.001, R2=.379). Conclusions : These findings suggest the need to develop a program to enhance nurses' knowledge, positive attitude, and experience of nursing or counseling a patient who wrote ADs.
Anginosus group streptococci (AGS) were classified based on the nucleotide sequences of the 16S rRNA gene (16S rDNA) and comprised Streptococcus anginosus, Streptococcus intermedius, and Streptococcus constellatus. It is known that AGS is a causative factor of oral and systematic diseases. The purpose of this study was to discriminate the 56 clinical strains of AGS isolated from Korean oral cavities using phylogenetic analysis of 16S rDNA and species-specific PCR at the species-level. The 16S rDNA of clinical strains of AGS was sequenced using the dideoxy chain termination method and analyzed using MEGA version 5 software. PCR was performed to identify the clinical strains using species-specific primers described in previous studies and S. intermedius-specific PCR primers developed in our laboratory. The resulting phylogenetic data showed that the 16S rDNA sequences can delineate the S. anginosus, S. intermedius, and S. constellatus strains even though the 16S rDNA sequence similarity between S. intermedius and S. constellatus is above 98%. The PCR data showed that each species-specific PCR primer pair could discriminate between clinical strains at the species-level through phylogenetic analysis of 16S rDNA nucleotide sequences. These results suggest that phylogenetic analysis of 16S rDNA and PCR are useful tools for discriminating between AGS strains at the species-level.
Purpose: This study identified the reasons why tuberculosis (TB) patients withhold treatment in a bid to develop a assessment scale to select patients who needs nursing intervention in the early stage and decrease the risk of discontinuation of treatment. Sample: There were two samples. A sample of 191 patients with TB and having primary treatment and a second sample of N who were under re-treatment Methods: The study design included qualitative and quantitative methods. Qualitative data were collected from in-depth interviews of TB patients under re-treatment. The quantitative data were collected from 191 patients with TB under primary treatment. Results: Exploratory factor analysis revealed 11 factors explaining 69.6% of total variance. These factors were categorized into four subgroups. A depression scale was used to establish concurrent validity. The depression scale had a positive relationship (r=54) with the discontinuing of primary treatment. The internal consistency reliability for the four subgroups was over .84. The confidence coefficient was Cronbach's ${\alpha}$ .95. The final scale was a self-reported four Likert scale including 50 items. Conclusion: Reliability and validity was established for the scale and the scale can be used to examine the risk of treatment discontinuation for TB. The scale is an important resource for nursing interventions in identifying and treating high risk clients.
Oral squamous cell carcinoma (OSCC) is the most prevalent head and neck malignancy, with frequent cervical lymph-node metastasis, leading to a poor prognosis in OSCC patients. The present study aimed to identify potential markers, including microRNAs (miRNAs) and genes, significantly involved in the etiology of early-stage OSCC. Additionally, the main OSCC's dysregulated Gene Ontology annotations and significant signaling pathways were identified. The dataset GSE45238 underwent multivariate statistical analysis in order to distinguish primary OSCC tissues from healthy oral epithelium. Differentially expressed miRNAs (DEMs) with the criteria of p-value < 0.001 and |Log2 fold change| > 1.585 were identified in the two groups, and subsequently, validated targets of DEMs were identified. A protein interaction map was constructed, hub genes were identified, significant modules within the network were illustrated, and significant pathways and biological processes associated with the clusters were demonstrated. Using the GEPI2 database, the hub genes' predictive function was assessed. Compared to the healthy controls, main OSCC had a total of 23 DEMs. In patients with head and neck squamous cell carcinoma (HNSCC), upregulation of CALM1, CYCS, THBS1, MYC, GATA6, and SPRED3 was strongly associated with a poor prognosis. In HNSCC patients, overexpression of PIK3R3, GIGYF1, and BCL2L11 was substantially correlated with a good prognosis. Besides, "proteoglycans in cancer" was the most significant pathway enriched in the primary OSCC. The present study results revealed more possible mechanisms mediating primary OSCC and may be useful in the prognosis of the patients with early-stage OSCC.
The purpose of this study was to identify acculturative stress, social support and self esteem in migrant workers. The data of 181 subjects were collected from June 1st to September 27, 2016, and analyzed using the SPSS WIN 23.0 program. Results: There was a significant negative correlation among acculturative stress, social support(r=-.465, p<.001) and self esteem(r=-353, p<.001). Stepwise multiple regression analysis showed self esteem(${\beta}=-.21$, p<.001) and job(${\beta}=-.02$, p<.05) were significant factors affecting acculturative stress(F=27.672, p<.001). The results of the study indicate that the efforts, to improve self esteem, and to increase social support, are required to decrease acculturatives stress among migrant wokers. The results are expected to be utilized in development of supportive program for migrant workers adaptation. In the following study, the analysis of structural equation modeling about additional factors of acculturative stress among migrant workers will be needed.

This study was conducted to figure out the influential factors on premenstrual syndrome(PMS) of college female students which are to be utilized as the basic data to develop and apply programs for preventing and controlling such symptom. The subjects were 330 college female students. The data were collected from April 2, 2012 to April 6, 2012. From the results, There has been significant correlation between stress and PMS(r=.36, p<.001) and the attitude to menstruation has appeared to have significant positive correlation with PMS as well(r=.34, p<.001). Multiple regression analysis has been employed to identify the influential factors on PMS and the result has shown that menstrual attitude, grade point average for stress, smoking and dysmenorrhea have been the most significant influential factors with 27% of explanatory power. The level of significance has been high in menstrual attitude(${\beta}$=.28, p<.001), grade point average for stress(${\beta}$=.27, p<.001), smoking(${\beta}$=.20, p<.001) and dysmenorrhea(${\beta}$=.15, p<.001) respectively. In conclusion, it needs to find nursing interventions for PMS related to psychosocial factors and suggest a narrative study for improving quality of life of women with PMS.
Background: This study was conducted to evaluate the performance of the Hierarchical Condition Category (HCC) model, identify potentially high-cost patients, and examine the effects of adding prior utilization to the risk model using Korean claims data. Methods: We incorporated 2 years of data from the National Health Insurance Services-National Sample Cohort. Five risk models were used to predict health expenditures: model 1 (age/sex groups), model 2 (the Center for Medicare and Medicaid Services-HCC with age/sex groups), model 3 (selected 54 HCCs with age/sex groups), model 4 (bed-days of care plus model 3), and model 5 (medication-days plus model 3). We evaluated model performance using $R^2$ at individual level, predictive positive value (PPV) of the top 5% of high-cost patients, and predictive ratio (PR) within subgroups. Results: The suitability of the model, including prior use, bed-days, and medication-days, was better than other models. $R^2$ values were 8%, 39%, 37%, 43%, and 57% with model 1, 2, 3, 4, and 5, respectively. After being removed the extreme values, the corresponding $R^2$ values were slightly improved in all models. PPVs were 16.4%, 25.2%, 25.1%, 33.8%, and 53.8%. Total expenditure was underpredicted for the highest expenditure group and overpredicted for the four other groups. PR had a tendency to decrease from younger group to older group in both female and male. Conclusion: The risk adjustment models are important in plan payment, reimbursement, profiling, and research. Combined prior use and diagnostic data are more powerful to predict health costs and to identify high-cost patients.

This study aimed to examine the association between sleep duration, stress levels, and dietary behavior in adolescents. A survey was conducted on 453 students (227 male, 226 female) in high schools in the Chungbuk area. The participants were divided into groups based on daily sleep duration: group I (<5 hrs/d), group II (≥5 hrs/d and <6 hrs/d), group III (≥6 hrs/d and <7 hrs/d), and group IV (≥7 hrs/d). To measure the stress levels of participants, the survey had questions covering a variety of areas, including school life, family life, social relationships, and personal evaluation. To assess the dietary behaviors of participants, the survey included questions encompassing the Nutrition Quotient for Korean adolescents (NQ-A). Among female students, the sleep duration group IV had significantly lower total stress (P=0.022) and significantly lower stress in the personal evaluation area than the sleep duration group I (P=0.004). In the analysis of NQ-A, among female students, the sleep duration group IV showed significantly higher scores in balance (P=0.004), diversity (P=0.016), and total NQ-A score area (P=0.007) compared to other groups. There was a significant negative correlation between sleep duration and stress scores in total students (r=-0.24, P<0.001), while a significant positive correlation was seen between sleep duration and NQ-A scores in female students (r=0.16, P<0.05). To sum up, the longer the sleep duration, the lower the degree of stress, and the quality of meals which can be said to be a significant finding for high school students.
