Structural and Functional Relationship of the Catalytical Subunit of Recombinant Pyruvate Dehydrogenase Phosphatase (rPDPc): Limited Proteolysis (Pyruvate dehydrogenase phosphatase의 catalytical subunit의 구조와 활성에 대한 연구)

  • Pyruvate dehydrogenase phosphatase (PDP)와 kinase는 당대사시 해당과정에서의 대사 산물인 pyruvate를 acetyl CoA로 만들어 구연산 회로로 진입시켜 주는 효소인 pyruvate dehydrogenase complex (PDC)의 활성을 조절하는 중요한 효소이다. PDP의 catalytic subunit는 PDC의 dihydrolipoamide acetyltransferase (E2), PDP regulatory subunit (PDPr), 그리고 칼슘 결합 도메인 등으로 구성되어 있는 것으로 추측되어지고 있다. 본 연구에서는 그 구조와 기능과의 상관관계를 알아보기 위해 PDPc를 E. coli JM101에서 발현시켜 순수 정제 후 단백분해 효소를 이용한 제한적 가수분해 방법을 이용해 그 구조와 기능과의 상관관계에 대해 연구하고자 하였다 정제된 PDPc는 trypsin, chymotrypsin, Arg-C 그리고 elastase를 이용하여 3$0^{\circ}C$ 그리고 pH 7.0에서 제한적으로 분해시켰으며 각 분해산물의 아미노 말단의 아미노산 배열을 분석하였다. 그 결과 PDPc는 trypsin, chymotrypsin, elastase에 의해 N-terminal의 50 kD과 C-terminal의 10 kD의 두개의 분해산물을 만들었으며, Arg-C에 의해 50kD의 분해산물은 약 35kD와 15kD으로 더 가수분해가 되었다. 이러한 결과로 볼 때 PDPc는 앞에서 추측한데로 세개의 주요한 기능적 도메인으로 이루어져 있음을 알 수 있었다 또한 C-terminal의 10kD은 PDPc의 활성에는 영향을 주지 않는 것으로 밝혀졌으나 다른 도메인의 기능은 더 연구가 되어져야 할 것으로 생각된다.

Medication Status and Adherence of the Elderly under Home Care Nursing (가정간호 노인대상자의 처방약물복용 실태 및 복용 이행도 영향요인)

  • Purpose: This study is a descriptive research intended to clarify the medication status of community-dwelling elders and to identify factors affecting their medication adherence. Methods: Data were collected using questionnaires and interviews from 101 subjects who had taken prescribed drugs for at least 7 days sampled among elderly people using home care nursing at a general hospital in Seoul. Results: According to the results of this study, medication adherence measured by pill counting was 88.3% and that measured by self-reporting was 94.6%. There were statistically significant differences in medication adherence according to major disease (p=.006), the number of admissions (p=.032), the number of drugs (p=.051), the frequency of medication (p=.026), and depression (r=-.205). In addition, depression was found to be a significant variable explaining the medication adherence with explanatory power 3.8% (p=.035). Conclusion: The presence of depression affected the elderly subjects' the medication adherence. Therefore, more concern and educational approaches are required to encourage elderly people to comply correctly with medication regimens particularly for elderly patients who have a malignant or long-lasting disease or who have to take multiple drugs or maintain a daily dosing frequency.

Evaluation of Nanjido Landfill Site Stabilization by Leachate Component (침출수 성분에 따른 난지도 매립지의 안정화 평가)

  • This study was focused on the evaluation of Nanjido landfill site by leachate analysis. Several parameters, for example pH, BOD, $COD_{Mn},\;COD_{Cr}$, SS, TP, $NH_3-N$, Color, were selected as major leachate quality parameters. $BOD/COD_{Cr}$. was used to estimate the biodegradable portion in organic matter. Samples were collected at the leachate storage wells and leachate treatment facility inflow in each quarter of 2003. The results were as follows : 1. Inflow quality of treatment plant in 2003 was analysed as $pH\;7.3\~8.0,\;BOD\;12.4\~30.0mg/L,\;COD_{Mn}\;101.2\~130.3mg/L,\;COD_{Cr}\;122.5\~238.0mg/L,\;SS\;16.1\~115.3mg/L$, $T-P\;0.27\~0.80mg/L,\;NH_3-N\;208.0\~~354.0mg/L$, Color $110.3\~129.0$ unit. 2. $BOD/COD_{Cr}$ of inflow ranged between $0.07\~0.16$ indicating that the landfill had the properties of old-fill and organic portion in leachate was mostly persistent organic matter. 3. Though concentrations of BOD, COD, $NH_3-N$, Color in leachate storage wells were mostly higher in first landfill than in second landfill, $BOD/COD_{Cr}$ ranging from 0.03 to 0.20, showed reversely. 4. Correlation coefficient$(R^2)$ between color and other parameters related to organic matter showed mostly above 0.7 and especially highest value with $COD_{Mn}$ of 0.7985.

Comparative Study of White and Steamed Black Panax ginseng, P. quinquefolium, and P. notoginseng on Cholinesterase Inhibitory and Antioxidative Activity

  • This study evaluated the anti-cholinesterases (ChEs) and antioxidant activities of white ginseng (WG) and black ginseng (BG) roots of Panax ginseng (PG), P. quinquefolium (PQ), and P. notoginseng (PN). Ginsenosides $Rg_1$, Re, Rf, $Rb_1$, Rc, $Rb_2$, and Rd were found in white PG, whereas Rf was not found in white PQ and Rf, Rc, and $Rb_2$ were not detected in white PN. The major ginsenoside content in steamed BG including $RK_3$, $Rh_4$, and 20(S)/(R)-$Rg_3$ was equivalent to approximately 70% of the total ginsenoside content. The WG and BG inhibited acetylcholinesteras (AChE) and butyrylcholinesterase (BChE) in a dose dependent manner. The efficacy of BG roots of PG, PQ, and PN on AChE and BChE inhibition was greater than that of the respective WG roots. The total phenolic contents and 2, 2-diphenyl-1-picryl-hydrazyl (DPPH) scavenging activity were increased by heat treatment. Among the three WG and BG, white PG and steamed black PQ have significantly higher contents of phenolic compounds. The best results for the DPPH scavenging activity were obtained with the WG and BG from PG. These results demonstrate that the steamed BG roots of the three studied ginseng species have both high ChEs inhibition capacity and antioxidant activity.

EFFECT OF CANAL TAPERING IN TEETH OF VARIOUS APICAL SIZE & CROSS-SECTIONAL CONFIGURATION ON MICROLEAKAGE (다양한 치근단공 크기와 근관단면의 형태를 가지는 치아에서 taper의 정도가 미세누출에 미치는 영향)

  • The aim of this study was to evaluate the microleakage of teeth according to root canal preparation with & without apical enlargement in various size of apical foramen. 60 extracted one canal roots were cross-cutted at 5 mm from root apex and divided into two groups according to their apical foramen size of large (L) and small (S). Each group was subdivided into two groups accordance with their cross-sectional configuration at 5 mm from apex, round (R) and ovoid (O); SR Group, SO Group LR Group, LO Group. Each group was shaped in .02 taper by Quantec series Nickel-Titanium (NiTi) rotary file, obturated by lateral condensation method. Leakage was measured using a fluid transport model under 40 $cmH_2O$ pressure. After the leakage test, blocks which had showed the leakage retreated with .04 taper and ,06 taper and evaluated the degree of fluid filtration in each group. The data was analysed statistically using chi-square test and fisher's exact test. The results obtained were as follows : 1. Significant difference in leakage was found in groups which had different apical foramen size in .02 taper instrumentation (p < 0.05), but not in .04 taper instrumentation (p > 0.05) 2. The difference in microleakage according to the shape of canal was not evident at 5 mm from apex (p > 0.05). 3. There was correlation between .02 taper instrumentation and .04 taper instrumentation in LR group, LO group (p < 0.05).

Donor Cell Source (Miniature Pig and Landrace Pig) Affects Apoptosis and Imprinting Gene Expression in Porcine Nuclear Transfer Embryos

  • This study investigated the developmental ability and gene expression of somatic cell nuclear transfer embryos using ear skin fibroblast cells derived from miniature pig. When miniature pig (m) and landrace pig (p) were used as donor cells, there were no differences in cleavage (79.2 vs. 78.2%) and blastocyst rates (27.4 vs. 29.7%). However, mNT blastocysts showed significantly higher apoptosis rate than that of pNT blastocysts (6.1 vs. 1.7%) (p<0.05). The number of nuclei in pNT blastosysts was significantly higher than that of mNT (35.8 vs. 29.3) (p<0.05). Blastocysts were analyzed using Realtime RT-PCR to determine the expression of Bax-${\alpha}$, Bcl-xl, H19, IGF2, IGF2r and Xist. Bax-${\alpha}$ was higher in mNT blastocyst than pNT blastocyst (p<0.05). There was no difference in Bcl-xl between two NT groups. Bax-${\alpha}$/Bcl-xl was, however, significantly higher in mNT blastocyst compared to pNT. The expression of imprinting genes were aberrant in blastocysts derived from NT compared to in vivo blastocysts. H19 and IGF2r were significantly lower in mNT blastocysts (p<0.05). The expression of IGF2 and Xist was similar in two NT groups. However, imprinting genes were expressed aberrantly in mNT compared to pNT blastocysts. The present results suggest that the NT between donor cells derived from miniature pig and recipient oocytes derived from crossbred pig might affect reprogramming of donor cell, resulting in high apoptosis and aberrant expression patterns of imprinting genes.

Influencing Factors on the Satisfaction of the Paramedic Students in Clinical Training (응급구조학전공 학생의 병원 임상실습 만족도에 영향을 미치는 요인)

  • Purpose: This study aims to assess the influencing factors on the level of satisfaction with clinical training and to provide basic data for more efficient clinical training. Method: The study was conducted on 402 paramedic students who have experienced clinical training from September 6 until October 12 in 2011. The questionnaires consisted of 40 questions. We used SPSS 18.0 frequency analysis, technical statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Result: 1. The level of satisfaction with clinical training showed significant difference between the frequency (F=8.837, p=.000) and clinical training managers (F=5.418, p=.001). 2. The level of satisfaction with clinical training showed the strongest positive correlation with the satisfaction of clinical training hospitals (r=.694, p=.000). 3. Multiple regression analysis revealed the most powerful predictor for satisfaction with clinical training was the satisfaction level of clinical training hospitals(48.2%) and the frequency of clinical training experiences(.8%), the preparation before the clinical training(5.4%), the total duration of clinical training(.7%), and the satisfaction of emergency department education(1.0%). These five variables accounted for 56.1% of the satisfaction of clinical training among paramedic students. Conclusion: As student's satisfaction of the clinical training increases, the quality of paramedics is expected to improve in the future. As a result, the future paramedics can be nurtured to be highly skilled in on-the-scene emergency situations after graduation.

A Study on Glomerular Filtration Rate Comparisons between Gates Method and Modified Gates Method used in Dynamic Renal Scintigraphy (동적 신장 신티그래피 검사 시 사용되는 Gates 법과 Modified Gates 법의 사구체 여과율 비교에 관한 고찰)

  • Purpose: Glomerular filtration rate is an important index for assessment of renal function, early discovery of renal disease, and progress observation of chronic renal disease patients. In the present study, the objective is to conduct a comparative analysis of differences between Gates and Modified Gates method in dynamic renal scintigraphy based on MDRD (Modification of Diet Renal Disease) formula using blood collection. Materials and Methods: Renal scintigraphy was performed for 45 patients who visited our hospital between November 2010 and August 2011. For 20 patients of those tested, glomerular filtration rates from Gates method and MDRD formula using AGUS equipment, were compared. For the other 20 patients, glomerular filtration rates from Modified Gates method and MDRD formula using INFINIA equipment. Finally, Gates and Modified Gates method were compared for 5 patients who indicated no change in glomerular filtration rates from MDRD formula during progress observation. Results: Glomerular filtration rates from both Gates and Modified Gates method showed a high correlation with those from MDRD formula ($p$<0.01, r=0.903, r=0.867), with a paired difference mean for Gates method of $2.05{\pm}2.54mL/min/1.73m$, and that for Modified Gates method of $25.2{\pm}3.71mL/min/1.73m$. Finally, the values for Gates method and those for Modified Gates method showed a high correlation for the five patients ($p$<0.05, r=0.949), with a paired difference mean of $20.4{\pm}8.84mL/min/1.73m$. Conclusion: Glomerular filtration rates from Gates method, Modified Gates method and MDRD formula showed mutually high correlations. If the tests are performed with recognition for the correlations between Gates and Modified Gates method used in a dynamic renal scintigraphy, then an accurate assessment of renal function is considered possible with an improved diagnostic ability.

Remifentanil Protects Human Keratinocyte Through Autophagic Expression

  • Background: Remifentanil, an ultra-short-acting mu-opioid receptor agonist, is unique from other opioids because of its esterase-based metabolism, minimal accumulation, and very rapid onset and offset of clinical action. Remifentanil can prevent the inflammatory response and can suppress inducible nitric oxide synthase expression in a septic mouse model. However, the effects of remifentanil on human keratinocyte and autophagy have yet to be fully elucidated during hypoxia-reoxygenation. Here we investigated whether remifentanil confers protective effect against hypoxia-reoxygenation in human keratinocyte and, if so, whether autophagy mediates this effect. Methods: The human keratinocytes were cultured under 1% oxygen tension. The cells were gassed with 94% $N_2$, and 5% $CO_2$ and incubated for 24 h at $37^{\circ}C$. To determine whether the administration of affects human keratinocytes hypoxia-reoxygenation injury, cells were then exposed to various concentrations of remifentanil (0.01, 0.1, 0.5 and 1 ng/ml) for 2 h. After remifentanil treatment, to simulate reoxygenation and recovery, the cells were reoxygenated for 12 h at $37^{\circ}C$. Control group did not receive remifentanil treatment. Normoxia group did not receive hypoxia and remifentanil treatment for 36 h. 3-MA group was treated 3-methyladenine (3-MA) for 1h before remifentanil treatment. Cell viability was measured using a quantitative colorimetric assay with MTT, showing the mitochondrial activity of living cells. Cells were stained with fluorescence and analyzed with Western blot analysis to find out any relations with activation of autophagy. Results: Prominent accumulation of autophagic specific staining MDC was observed around the nuclei in RPT group HaCaT cells. Similarly, AO staining, red fluorescent spots appeared in RPT group HaCaT cells, while the Normoxia, control and 3-MA groups showed mainly green cytoplasmic fluorescence. We here examined activation of autophagy related protein under H/R-induced cells by Western blotting analysis. Atg5, Beclin-1, LC3-II (microtubule-associated protein 1 light chain 3 form II) and p62 was elevated in RPT group cells. But they were decreased when autophagy was suppressed by 3-MA (Fig. 5). Conclusions: Although the findings of this study are limited to an in vitro interpretation, we suggest that remifentanil may have a beneficial effect in the recovery of wound from hypoxia-reoxygenation injury.

A Study on the Concentration of Hippuric Acid in Urine of Workers Exposed to Toluene (톨루엔에 폭로된 근로자의 뇨중 마뇨산량에 관한 연구)

  • The objective of this study is to examine the correlation between the toluene concentration in the work environment of organic solvents and the concentration of hippuric acid in urine of workers exposed toluene, and to acquire the fundamental materials of workers' health care and to assist the assessment of the biological permisible exposure limit. The control groups are 72 workers who had never been exposed to toluene and the case groups are 101 workers who had been exposed to toluene. This study was conducted to examine the correlation between the concentration of toluene in work environment and workers' hippuric acid concentration, to investigate the complaint difference of subjective symptoms by means of questionaire, between the case groups and control groups. The results and conclusions are as fellows: 1) The highest mean concentration of toluene in the air is $544.13{\pm}7.75ppm$ in the Printing Department of mixing organic solvents. The concentrations of the others are $463.27{\pm}5.24ppm$ in Department of mixture for organic solvents, and $393.56{\pm}45.69ppm$ in the Printing Department(1), and $248.38{\pm}45.16ppm$ in the Printing Department(2), and $159.38{\pm}18.51ppm$ in the Printing Department(3). 2) The highest mean concentration of hippuric acid in urine is $6034.84{\pm}1298.35mg/l$ in the Printing Department with mixing organic solvents. The concentrations of the others are $4798.44{\pm}784.53mg/l$ in Department of mixture of organic solvents, and $2883.06{\pm}701.90mg/l$ in the Printing Department(1), and $1449.98{\pm}905.19mg/l$ in the Printing Department(2), and $598.52{\pm}299.43mg/l$ in the Printing Department(3). 3) As the toluene concentration in the work environment becomes higher, the concentration of hippuric acid in urine also becomes higher. It is found that the coefficient of correlation between toluene concentration and the concentration of hippuric acid are positive (r=0.868, Y=7.18X+349.57) 4) It is found that the case groups complain of much more subjective symptoms than the control groups.
