Three clustering patterns among metabolic syndrome risk factors and their associations with dietary factors in Korean adolescents: based on the Korea National Health and Nutrition Examination Survey of 2007-2010

  • BACKGROUND/OBJECTIVE: Even though the prevalence of metabolic syndrome in adolescents is increasing, little is presently known about this syndrome in adolescents. This study aimed to cluster metabolic risk factors as well as examine the associations between identified patterns and nutrient intake using data from the Korean National Health Examination and Nutritional Assessment (KNHANES). SUBJECTS/METHODS: A total of 2,958 subjects aged 10 to 18 years along with both biochemical and dietary data information were obtained from KNHANES 2007-2010. Six components of metabolic syndrome were used to identify any patterns via factor analysis. Individuals were categorized into quartile groups according to their pattern score. RESULTS: Three clustering patterns with high loadings were identified and named as follows: 1) high blood pressure, 2) dyslipidemia, and 3) glucose abnormality patterns. The high blood pressure pattern showed high loadings of systolic and diastolic blood pressures, the dyslipidemia pattern showed high loadings of triglyceride and HDL-cholesterol levels, and the glucose abnormality pattern showed high loadings of fasting blood glucose levels. Intakes of fat and riboflavin were significantly decreased, whereas those of sodium and niacin were significantly increased across the quartiles in the dyslipidemia pattern. No nutrient intake except that of thiamin was significantly associated with the high blood pressure or glucose abnormality pattern. CONCLUSION: Our findings show that metabolic syndrome risk factors in the Korean adolescent population are characterized by three distinct patterns, which are differentially associated with dietary factors. Characterizing metabolic risk factors and providing specific dietary guidelines for target groups are important.

남성 근로자의 혈중 항산화 영양소 상태와 대사증후군 인자와의 관계 (The Relationship Between Plasma Antioxidant Levels and Metabolic Syndrome Risk Factors in Male Workers)

  • The principal objective of this study was to evaluate the association between plasma antioxidant levels and metabolic syndrome in male workers, and to provide basic information regarding the control and prevention of metabolic syndrome. We analyzed 163 male workers who had participated in annual medical examinations from January to December 2007. The subjects were classified into normal and metabolic syndrome groups according to the NCEP-ATP III criteria and the Asia-Pacific criteria for waist circumference. Anthropometric parameters, lifestyles, blood lipid profiles, and antioxidant levels were evaluated. As compared to the normal group, the metabolic syndrome group evidenced significantly higher plasma levels of $\alpha$- tocopherol(p<0.05) and retinol(p<0.05), but significantly lower plasma levels of lycopene(p<0.05) and $\beta$-carotene(p<0.05). This tendency was found to be predominantly attributable to increases in the number of metabolic syndrome components. In our simple regression analysis, higher plasma levels of $\alpha$-tocopherol($\beta$=0.001, p<0.01) and retinol($\beta$=0.021, p<0.001) were associated with significantly higher risks of metabolic syndrome, but lycopene($\beta$=-1.499, p<0.01) and $\beta$-carotene($\beta$=-0.048, p<0.01) were associated with significantly lower risks of metabolic syndrome. Retinol($\beta$=0.013, p<0.05) and $\beta$-carotene($beta$=-0.044, p<0.01) were associated significantly with metabolic syndrome, when adjusted for age and BMI. These data indicate that the plasma levels of $\alpha$-tocopherol, retinol, lycopene, and $\beta$-carotene are associated with metabolic syndrome. Specifically, low lycopene and $\beta$-carotene levels in the plasma appear to increase the risk of metabolic syndrome. Therefore, proper nutritional education programs for male workers are required to increase dietary intakes of antioxidant vitamins. Further studies will be necessary to determine whether antioxidant levels can be utilized as a predictive or a preventive factor.

  • Purpose: The purpose of this study was to assess the difference of metabolic syndrome (MS) risk factors according to the MS diagnosis in the age of fifties women. Methods: Forty-two subjects (non metabolic syndrome 30, metabolic syndrome 12) were recruited from the health promotion center of a tertiary care hospital in an urban city. MS was defined by third report of the national cholesterol education program (NCEP) expert panel on detection, evaluation, and treatment of high blood cholesterol in adults (Adult Treatment Panel III) (ATP III), and abdominal obesity was determined by Asia-Pacific criteria in waist circumference. Results: The prevalence of MS by definition of NCEP-ATP III and Asia-Pacific criteria in waist circumference was 28.6% in the age of fifties women. The educational level and the median HDL-cholesterol were significantly lower in the MS patient group than in the Non-MS group. The median of weight, triglyceride, and fasting glucose were significantly higher in the MS patient group than in the Non-MS group. Conclusion: These results indicate that the nursing care should be focused on weight, triglyceride, HDL-cholesterol, and fasting glucose of the MS patients in the fifties women.

소아청소년에서의 대사증후군 역학 (Epidemiology of the metabolic syndrome among Korean children and adolescents)

  • The prevalence of obesity in Korean children is estimated to be around 10%, and has increased significantly over the past 20 years. Metabolic syndrome, which includes central obesity, glucose intolerance, insulin resistance, dyslipidemia, and hypertension, is a well documented risk factor for cardiovascular diseases, but there is no general consensus concerning its definition for children. In this study, ablut 30-40% of overweight or obese children had metabolic syndrome compared to less than 10% in the general population. Dyslipidemia was the most common component and hyperglycemia was the least common component of metabolic syndrome. If the prevalence of childhood obesity continues to increase, it is highly likely that the prevalence of the metabolic syndrome will also increase. These findings emphasize the importance of preventing obesity in addition to the diagnosis and management of metabolic syndrome in Korean adolescents.

Effects of a Self-Care Reinforcement Program for Socially Vulnerable Elderly Women with Metabolic Syndrome in Korea

  • Purpose: This study evaluates the efficacy of a Self-Care Reinforcement Program (SCRP) based on the Selection Optimization Compensation (SOC) model, in socially vulnerable elderly women with metabolic syndrome. Methods: This study adopts a pretest-posttest nonequivalent control group design. The participants were 64 socially vulnerable elderly Korean women with metabolic syndrome (experimental group: 31, control group: 33). Participants' body composition analysis, nutrient intake, risk factors of metabolic syndrome, depressive symptoms, and social network were measured. Data were analyzed with an independent t-test; statistical significance levels were set at p<.05. The SCRP, including metabolic syndrome education, nutritional education, exercise, and social network, was performed three times a week for 8 weeks. Results: There were statistically significant differences between the experimental and control groups in terms of systolic blood pressure, diastolic pressure, fasting blood sugar, triglycerides, sodium intake, depressive symptoms, and social networks. Conclusion: The SCRP is effective and can be recommended as a community health nursing intervention for socially vulnerable elderly women with metabolic syndrome.

중년 남성의 지방 에너지비에 따른 대사증후군 위험도 비교 - 2012~2013년 국민건강영양조사 자료 이용 - (Metabolic Syndrome Risk by Dietary Fat Energy Ratio in Middle-aged Men - Using the 2012~2013 Korean National Health and Nutrition Examination Survey Data -)

  • This study aimed to compare energy nutrient intake, health related factors, physical characteristics, blood biochemical indices, prevalence of metabolic syndrome and odds ratio (OR) of metabolic syndrome based on dietary fat energy ratio. Subjects were 1,205 men aged 40~64 years. The average fat intake was 52.8 g. Subjects were divided into three groups (deficient, normal, excess) based on dietary fat energy ratio. The dietary fat energy rations of the three groups were 36.9%, 42.9% and 20.2%, respectively. Energy and protein intake were increased significantly with dietary fat energy ratio (p<0.001), whereas carbohydrate intake decreased (p<0.001). In health related factors, amount of smoking alone showed increase based on dietary fat energy ratio (p<0.001). In comparing physical characteristics, blood pressure and blood biochemical indices, excepting diastolic blood pressure, increased significantly based on dietary fat energy ratio (p<0.01~p<0.001). The rate that exceeded criteria in risk factors for metabolic syndrome was higher in the serum triglyceride (41.2%) and was lower in the waist circumference (22.2%). Prevalence of metabolic syndrome was 37.9%, and showed significant correlation to dietary fat energy ratio (p<0.05). The OR of metabolic syndrome was higher in deficient and excess group than in normal group, but it had no relationship between fat energy ratio and metabolic syndrome. The results of this study provide basic data to establish fat intake guidelines for prevention of metabolic syndrome in middle-aged men.

중·노년의 대사증후군과 구강질환 관련성 (Relationship between metabolic syndrome and oral diseases in the middle aged and elderly people)

  • Objectives: The purpose of the study was to identify the relationship between metabolic syndrome and oral diseases in the middle aged and elderly in Korea. Methods: The study subjects were 6,390 people over 40 years old from 2010 and 2012 Korea National Health and Nutrition Examination Survey. The survey questionnaire consisted of health, nutrition, and oral examination surveys. The independent variables included general characteristics, health behavior, oral health behavior, and metabolic syndrome. The dependent variables included dental caries experience and periodontal disease. The oral examination was carried out by the dentist based on World Health Organization standard. Results: The average prevalence rate of metabolic syndrome MS was 23.79%, including 54.84% of risk group and 21.37% of normal group. The missing teeth rate was 82.38%, DMFT rate was 90.28% and the periodontal disease rate was 33.15%. Those having abnormal fasting blood glucose had 1.17 fold(95% CI: 1.00~1.37) higher periodontal disease than the normal group. The abnormal HDL cholesterol group had 1.25 times higher odds ratio(95% CI: 1.07~1.46) and the obese group had 1.27 times higher odds ratio(95% CI: 1.07~1.51). The risk group had 1.20 times higher odds ration(95% CI: 1.00~1.44) and that of the metabolic syndrome group was 1.60 times higher(95% CI: 1.29~1.97) in periodontal disease. The high blood pressure group had 1.25 times of missing teeth prevalence rate(95% CI: 1.00~1.37). The metabolic syndrome group had 1.47 times of missing teeth prevalence rate(95% CI: 1.11~1.94). Conclusions: The middle aged and elderly people in Korea had higher rate of metabolic syndrome and oral disease. It is necessary to implement the preventive oral health examination for the control of metabolic syndrome and oral diseases prevalence.

일개 제조업 근로자의 3년간 체중변화와 대사증후군 진단 구성요소의 비교 분석: 국가건강검진(2015~2017년) 자료를 중심으로 (A Study on the Three-Year Weight Changes of Workers at a Manufacturing Company and the Comparison of Metabolic Syndrome Diagnosis Components: Focused on the Data of Korean National Health Screening (2015~2017))

  • Purpose: This study aims to provide preliminary data for weight management, and prevention and management of cardiovascular and cerebrovascular diseases. We examined the effect of changes in the weight of workers at a manufacturing company over three years on their metabolic syndrome and metabolic syndrome diagnosis components. Methods: Necessary data were collected from the questionnaire and the results of the Korean National Health Screening of 2015 and 2017, which included 228 workers at a manufacturing company in G region. The collected data were analyzed using the SPSS/WIN 23.0 program. ANCOVA was used to examine the differences in the metabolic syndrome diagnosis components according to weight change. In addition, multiple logistic regression analysis was used to obtain the odds ratios of metabolic syndrome and metabolic syndrome analysis component, based on the weight changes in the normal weight group and the obesity group. Results: Waist measure, systolic blood pressure, and blood pressure were found to have significant effects based on participants' weight change over three years. These factors increased with a larger increase in weight at a statistically significant level. This study analyzed the weight changes of the normal weight group and the obesity group considering the data from the National Health Screening of 2015, and found that the risk of metabolic syndrome increased at a statistically significant level as body weight increased; thus, the obesity group showed a higher risk in this regard. It was also found that waist measure, fasting blood sugar, and high-density low cholesterol increased at a statistically significant level as body weight increased. Conclusion: Health administrators need to recognize the importance of workers' weight management, select an intensive management group based on a time series analysis of weight changes, and develop and implement programs to manage the metabolic syndrome diagnosis components.

한국인 대학생군 대상의 청.장년층 대사이상증후군 위험성 평가 (Evaluation of the Risk of Metabolic Syndrome for the Young Adults in Korean Students of a University)

  • Metabolic syndrome, defined as the clustering of several metabolic disorders including obesity (waist circumference ${\geq}90$ if male or ${\geq}80$ if female, cm), dyslipidemia ($TG{\geq}150$ or HDL-C<40 if male or <50 if female, mg/dl), hypertension ($BP{\geq}130/85mmHg$) and hyperglycemia (fasting plasma $glucose{\geq}110mg/dl$), increases the cardiovascular risk of the general population. Recently, risk of this syndrome arises in young adults world widely. Therefore, we randomly selected and evaluated the risk of metabolic syndrome of total 43 people (group I-22, group II-21) for 2 years. Group I was 22 peoples (15 males, 7 females) with age of 22 thru 35 year old (average 28 year old) and group II was 21 people (19 male, 2 female) with age of 22 thur 32 years old (average 24 year old) in Cheongju area from March 1st thru 30th of 2008 in Cheongju area from September 1st thru 30th of 2007 in order to find out how serious this phenomenon is in young adult of Korea. 13.95% (n=7) of total people has a metabolic syndrome by NCEP/ATPIII definition among this group (group I-6, group II-1). Those of 6 have 3 or over risk factor for metabolic syndrome such as obesity, hypertension, fasting blood glucose and hypetriglyceridemia at the same time (group I-5, group II-1). Group I have more risk factor because of more higher age than group II. Therefore we need aggressively to monitor and provide them for early diagnosis, educational programs and assistance for lifestyle changes in order to prevent metabolic syndrome among young adults.

Metabolic Syndrome Related with Serum Biochemical Factors Affecting Serum GGT at Transitional Ages in Gumi

  • This study has been done with 1,431 subjects who visited Health Promotion Centers of the hospitals in Gumi for National Health Screening Program for People at Transitional Ages from April to December 2007. Serum biochemical tests related with metabolic syndrome were performed. Among biochemical factors related with metabolic syndrome, the mean values of serum glucose, AST, ALT, triglyceride and HDL cholesterol except LDL cholesterol were significantly higher in males than in females, so a significant difference by sex was observed (P<0.001). AST, ALT, triglyceride and HDL were thought to be significantly affecting serum GGT for males. In contrast, ALT and HDL cholesterol were important factors for females (P<0.001). For both sexes, serum glucose and LDL cholesterol did not produce any meaningful effect on serum GGT. In males AST, ALT and HDL cholesterol were associated with high risk of abnormality of serum GGT and in females AST, ALT and LDL cholesterol were related with high risk of abnormality of serum GGT. Therefore, AST and ALT showed a significant effect on abnormality of serum GGT in both males and females. It was observed that males exhibited significantly high correlation between metabolic syndrome related biochemical factors and serum GGT than females, and their influence on abnormality of serum GGT was also higher in males than in females. Therefore, serum GGT tests performed for health screening are considered to be useful for managements of cardiovascular diseases and metabolic syndrome as well as liver function test.
