Comparative Effect of Interventions for Fall Prevention in Hospitals: Network Meta-analysis (병원 입원 환자를 위한 낙상예방중재 효과의 비교우위: 네트워크 메타분석)

  • Purpose: This study aimed to assess and compare the effectiveness of various fall prevention interventions in hospitals through a network meta-analysis. Methods: A network meta-analysis was conducted using the "netmeta" package in R software (v4.1), employing a frequency method. Odds ratios of fall rates and injurious fall rates were utilized to confirm the effects of interventions for fall prevention. Comparative rankings of these interventions were determined using cumulative probability (P-score). Results: Comparative rankings via cumulative probability (P-scores) revealed individualized education as the most effective intervention for fall incidence (P-Score 87.8%). Followed by fall-preventing sensors (60.9%), multicomponent interventions (47.4%), usual care (33.2%), and environmental modification (20.7%). For fall-related injuries, individualized education ranked highest (P-Score 97.1%), followed by multicomponent interventions (76.0%), usual care (47.6%), environmental modification (24.2%), and fall-preventing sensors (5.1%). Conclusion: This study provides valuable insights into the relative effectiveness of diverse interventions in preventing fall incidence through network meta-analysis. The findings aim to support nurses in making informed decisions when implementing fall prevention strategies in clinical practice.

Vitamin D and fibromyalgia: a meta-analysis

  • Vitamin D is a cofactor responsible for autoimmune disorders. There is no agreement in the studies investigating the association between vitamin D and fibromyalgia. This study aims to combine the conflicting results of the primary studies which compared these patients with control groups regarding the serum concentration of vitamin D. This meta-analysis has been designed based on PRISMA guidelines. Relevant keywords were searched in PubMed, Science direct, Scopus, Cochrane, and Google scholar and primary studies were selected. After screening the eligible studies according to inclusion/exclusion criteria, we investigated the risk of bias in the selected studies and also the heterogeneity between the primary results using Cochrane (Q) and I-squared ($I^2$) indices. The primary results were combined using inverse variance method and Cohen statistics as well as a random effects model. Publication bias was assessed using Egger test. Sensitivity analysis was applied to investigate the influence of each primary study on the final result of the meta-analysis. Suspected factors in the heterogeneity were assessed using meta-regression models. We entered 12 eligible studies in the meta-analysis including 851 cases compared with 862 controls. The standardized mean difference of Vitamin D between the two groups was -0.56 (95% confidence interval: -1.05, -0.08). Our meta-analysis showed that vitamin D serum levels of patients with fibromyalgia was significantly lower than that of control group.

A Systematic Review and Meta-Analysis of Randomized Controlled Trials on Acupuncture for Improving Post-Operative Cognitive Dysfunction (POCD) and Chemotherapy-Related Cognitive Impairment (CRCI)

  • Objectives: This study was conducted to evaluate the efficacy of Korean Medicine Therapy (KMT) for the treatment of Chemotherapy-induced Cognitive Impairment (CICI) through systematic review and meta-analysis of randomized controlled trials (RCTs) as proceeding research to further research the efficacy of KMT for CICI patients. Methods: We conducted a systematic review and meta-analysis according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines (PRISMA). The RCTs on the efficacy of KMT for treatment of CICI were searched by structured search strategies in MEDLINE, EMBASE, Cochrane library, CAJD, KISS, NDSL, KoreaMed, and OASIS. The searched RCTs were screened by inclusion and exclusion criteria. We evaluated the quality of the method in the included studies using the Jadad score and Cochran ROB tool. The efficacy outcomes were the Visual Analogue Scale (VAS) and the Clinical total Effective Rate (CER) of CICI. They were analyzed using mean difference for continuous variable or Relative Risk (RR) for Dichotomous variable in the random-effect model. The RevMan 5.3 program was used for meta-analysis. Results: We included 19 RCTs, including 653 participants, in the systematic review and meta-analysis. There were favorable results for the KMT group after the intervention compared with the pharmacotherapy group, physiotherapy group, and combined treatment group. KMT group showed improvement using CER and VAS compared with exercises, but their heterogeneities were slightly significant. KMT was more effective compared to the Rehabilitation program in CER and the subgroup analysis results showed that KMT had a significant difference compared to other therapies in VAS, to Medication therapy in CER. Conclusions: KMT presented reasonable evidence on improving the Clinical total Effective Rate and Visual Analogue Scale in CICI patients. However, further evaluation in future research is required.

A Meta-Analysis on the Effects of Academic Achievement in Web-Based Instruction (웹 기반 교수-학습이 학업성취에 미치는 영향에 대한 메타 분석)

  • The purpose of this study has been found to be effective using web-based instruction than traditional teaching-learning method on academic achievement applying the meta-analysis method. The results of this study were as follows: First, The 85% subject of analysis of web-based instruction selected in this study turned out to be clear effective than traditional teaching-learning method in academic achievement of students. Second, Web-based instruction is more effective for academic achievement of elementary school students and university students than for middle school students and high school students relatively. Third, Web-based instruction is a most effective method in social subject and physical education but less effective in language subject. The overall results of this study concluded more powerful and big decisions which have integrated each different effects on academic achievement of studies web-based instruction method applying meta-analysis. Through this study, make better results were obtained and suggested the base line data and direction for follow up studies.

Meta-Analysis on the Effectiveness of Interventions Applied to Preventing Endotracheal Suction-Induced Hypoxemia (기관내 흡인으로 인한 저산소증 예방을 위한 중재 효과에 대한 메타분석)

  • Purpose: This study was conducted to investigate the effects of the suction-induced hypoxemia interventions. Method: 30 suction-induced hypoxemia interventions were reviewed for the purpose of meta-analysis. Result: The study showed that both preoxygenation and insufflation were the most frequently examined oxygenation time periods, and hyperoxygenation combined with hyperinflation was the most commonly applied oxygenation method in order to prevent suction-induced hypoxemia. The greatest effect was obtained by providing oxygenation before and after suctioning, whereas negative effect(the contrary results from the study hypotheses) was frequently obtained by applying insufflation only. Applying hyperoxygenation combined with hyperinflation had the greatest effect over that of applying hyperoxygenation only, even though the difference between effect sizes of both methods were statistically significant. Conclusion: The results of meta-analysis showed that the occurrence rate of hypoxemia after suctioning was significantly reduced with the overall interventions for hypoxemia (decreasing 40% of occurrence rate), independent with time periods or methods for providing oxygenation.

Importance of Meta-Analysis and Practical Obstacles in Oncological and Epidemiological Studies: Statistics Very Close but Also Far!

  • Studies of epidemiological and prognostic factors are very important for oncology practice. There is a rapidly increasing amount of research and resultant knowledge in the scientific literature. This means that health professionals have major challenges in accessing relevant information and they increasingly require best available evidence to make their clinical decisions. Meta-analyses of prognostic and other epidemiological factors are very practical statistical approaches to define clinically important parameters. However, they also feature many obstacles in terms of data collection, standardization of results from multiple centers, bias, and commentary for intepretation. In this paper, the obstacles of meta-analysis are briefly reviewed, and potential problems with this statistical method are discussed.

The Effects of Hardiness : A Meta-Analysis of Korean Nursing Research Findings (국내 강인성 효과 연구결과에 대한 메타분석)

  • Purpose: This study was conducted to meta-analyze the effects of hardiness on health-related variables. Method: After review of 19 studies performed by Korean nurses, research variables, statistical data(r or F), and other methodological data were extracted and coded. Research variables were categorized under 5 groups such as health-related behavior, well-being, adaptation, stress, and support according to conceptual similarity. Using SAS program, 20 research variables and 34 effect sizes were calculated after eliminating heterogeneous data by Q-test, Results: Effects of hardiness on whole research variables was .512 and ranged from .322 to .643 by categories. The greatest effect was obtained from well-being category, whereas the smallest effect from stress category. All effect sizes were statistically significant. But fail-safe numbers were small and failed to achieve reasonable tolerance level. Conclusion: Results of meta-analysis indicated that hardiness has a moderate effect on health-related variables. But for improving the reliability of the results by minimizing publication bias, the more hardiness studies should be done.

Meta Analysis of Usability Experimental Research Using New Bi-Clustering Algorithm

  • Usability evaluation(UE) experiments are conducted to provide UE practitioners with guidelines for better outcomes. In UE research, significant quantities of empirical results have been accumulated in the past decades. While those results have been anticipated to integrate for producing generalized guidelines, traditional meta-analysis has limitations to combine UE empirical results that often show considerable heterogeneity. In this study, a new data mining method called weighted bi-clustering(WBC) was proposed to partition heterogeneous studies into homogeneous subsets. We applied the WBC to UE empirical results and identified two homogeneous subsets, each of which can be meta-analyzed. In addition, interactions between experimental conditions and UE methods were hypothesized based on the resulting partition and some interactions were confirmed via statistical tests.

The evaluation of methodological quality of meta-analysis studies in speech language pathology using AMSTAR (AMSTAR에 기반한 국내 언어치료 분야 메타분석 논문의 방법론적 질평가)

  • Although research using meta-analysis is increasing in the field of rehabilitation science, not all meta-analytical papers are of the same quality. In particular, although meta-analysis is a research method with the highest level of evidence, it may be possible to derive distorted conclusions or alternatives by simply integrating representative values without considering heterogeneity among individual studies. This study analyzed the current status of meta-analysis papers on the subject of language arbitration published in Korea from January 2010 to June 2019, and used A Measurement Tool to Assess the Methodological Quality of Systematic Review (AMSTAR). As a result of evaluating the methodological quality of the final five papers, the average of 7.4 points out of 11 points was found above average. In order to raise the qualitative level of Meta-analysis in speech-language pathology in the future, it is necessary to include verification of publication bias and specification of conflicts of interest.

A systematic review and meta-analysis of flipped learning among university students in Korea: Self-directed learning, learning motivation, efficacy, and learning achievement (국내 대학생에게 적용한 플립러닝의 체계적 고찰 및 메타분석 - 자기주도학습, 학습동기, 효능감, 학업성취도를 중심으로 -)

  • Purpose: This study aimed to provide a systematic review and meta-analysis of research on flipped learning effects applied to university students. Methods: The random effect model was applied to 21 papers to calculate the effect size. To verify the moderation effect, a meta regression analysis and meta ANOVA were performed. Publication bias was verified through a funnel plot, and then an Egger's regression test was conducted. Results: The overall average effect size was .69 (95% CI: .51-.87), showing a median effect size, which was statistically significant. The outcome variables were in the order of learning motivation (Hedges' g=.83), self-directed learning (Hedges' g=.78), learning achievement (Hedges' g=.66), and efficacy (Hedges' g=.50), which were statistically significant. Conclusion: Flipped learning was found to be statistically significant in improving self-directed learning, learning motivation, efficacy, and learning achievement amng university students. It is suggestd that this method be actively applied in university education.
