Alcohol Use Disorder, Stress, Mental Health and Suicide among Seoul Citizens (서울시 지역주민의 알코올장애와 스트레스, 정신건강 및 자살)

  • Objectives: This study furthers the currents understanding of alcohol use disorders, stress, mental health status and suicide among Seoul citizens. This study is to identify differences according to socio-demographic variables and variables which affect stress, mental status and suicide for developing mental health programs. Methods: The study subjects using cluster-stratified sampling method were 1234 adults over 19 years old from 17 dong, S-Ku in Seoul City. A cross-sectional study with face-to-face interview was used to collect data. A questionnaire measuring socio-demographic variables, Symptom Checklist-90-Revised (SCL-90-R) and psychosocial wellbeing index-short form (PWI-SF) was utilized. Results: Three findings in this study were discussed: (1) The prevalence of problem drinking and alcohol dependence and AUDIT score were significantly higher among male, younger age, and high level of education and income; (2) AUDIT score were related with stress, all 9 symptoms of SCL-R and suicide plan; (3) Problem drinkers were more likely to have 2.5 times of anxiety, 3.0 times of hostility, and 2.4 times psychoticism than non alcohol problem drinkers. Alcohol dependence drinkers were more likely to have 1.9 times of high risk group of stress, 4.1 times of obsessive-compulsive, 4.7 times of anxiety, 5.5 times of hostility, and 2.3 times psychoticism than non alcohol dependence drinkers. Conclusion: The findings suggest that the future intervention of Mental Health programs should be carefully designed and tailored by socio-demographic variables.

Practical Use of Cancer Control Promoters in Municipalities in Japan

  • The Cancer Control Act in Japan became effective in 2006. In Ibaraki, Toyama, and Hyogo prefectures, the Cancer Control Promoter (CCP) plan was created to strengthen partnerships for cancer prevention. This study aimed to examine the curre nt status of CCP utilization and analyze relationships with intersectoral collaboration, both within the government and with outside partners. In 2008, we mailed questionnaires to 100 administrators responsible for disease prevention and health promotion in municipal governments of the three prefectures. Ninety-one administrators responded (response rate, 91.0%). We analyzed responses to questions regarding whether or not the municipalities had used CCPs. Items assessing intersectoral collaboration examined municipality characteristics and relationships with outside partners and sectors specializing in areas other than community health. Among 90 administrators with valid data, 33 municipalities (36.7%) used CCPs while 57 (63.3%) did not. The Fisher's exact test revealed that intersectoral collaboration for using CCPs was associated with communication with all of the municipal government sectors not related to health. The present study indicated that CCPs were not consistently used in municipalities. However, we found that intersectoral collaborations, especially within the local government, may be related to the practical use of CCPs. This, in turn, may result in effective cancer control and prevention, as well as improvement in community health.

Risk associated with Adverse Events of Folk Medicine Reported in the Internet News Articles (인터넷 신문기사로 본 민간요법 유해사례의 위험성)

  • Folk medicine is traditionally passed down to cure disease, and adverse events (AEs) of folk medicine are any unfavorable and unintended discomforts temporally associated with the use of folk medicine. The aim of this study was to analyze AEs types and risks of folk medicine through the internet news articles. Included in this analysis are all articles on the topic of folk medicine and AE reported in the top 3 online news websites (NAVER, DAUM and NATE) determined by InternetTrend$^{TM}$(www.internettrend.co.kr). It was searched in the last five years (between 1 January 2009 and 28 February 2014). In total, 18 AEs articles of 973 news articles met our inclusion criteria. A total of 27 people were experienced AEs associated with use of folk medicine. Age was from 4 months to 76 years old, and it was occurred in both men and women. Folk medicine that caused AEs in twice or more was therapy that patient taking the dictamnus or aconitum of toxic herbal medicines, vinegar therapy of external use to topical skin, and cupping or bee sting therapy by practitioners. Death as a kind of serious AEs was 11 people, and 10 people were died after treatment by unqualified practitioner. Folk medicine that is popular and widely used in Korea is actively interacted with information on the internet, so it apt to misuse and abuse without guidance of health professionals. Aspects of health care system, we point out that the need for government and medical society establish not only correct health information plan and promotion of risk but also system as reporting and monitoring of AEs by folk medicine.

A study on the current status of oriental medical care services in health centers and implications for its expansion (보건소장의 한방진료에 대한 태도와 관련 요인 분석)

  • Currently the oriental medical care services in the health centers is getting popularity because of their unique aspects which western medicine can not cover. This study was conducted to speculate the current status of oriental medical care services in health centers and possibility of how to effectively provide the oriental medical care services(or traditional medical care services) in health centers. For the study, the survey questionnaires were distributed to all 269 health centers and 138(51.3%) of them were collected. Eight of the collected were inadequate for the analysis and 130 of 269 questionnaires were finally chosen for the study. The SPSS/PC WIN 8.0 was used for the statistical analysis. The results are as follows. First, 91 out of 130 sample health centers(70%) are providing oriental medical care services. The results show that the willingness of the directors and the public awareness in community are the most important factors to provide oriental medical care services in health centers. In contrast, the lack of cooperative efforts between western and oriental medical doctors and the lack of government supports are considered as factors that intervene the oriental medical care services in health centers. About 80% of the sample health centers respond that the government supports is needed for oriental medical care services in health centers. Second, it was asked to the directors of 39 health centers which do not provide oriental medical care services regarding their future plan to include the services. About 70% of health centers respond that no plan is available now. They acknowledge that the demand on the services is the most important factor to consider the oriental services as their future medical services. Third, 69.2% of the oriental medical doctors in health centers are public health doctor. 95.6% of the sample health centers have space for the oriental medical care services in their facilities and some health centers provide the services through the private clinics nearby facilities. Finally, the surveyors consider both budgetary constraints and difficulty in recruiting doctors as barriers for the effective oriental medical care services in health centers. Finally, t-test and LSD were employed to find out the difference among several groups. The analysis shows statistically significant difference among groups about their recognition on health care policy, health care system, and effectiveness of oriental medical care services. To be conclude, the study shows the necessity of oriental medical care services in health centers. To do so, the government supports, cooperative efforts between oriental and western medical doctors, and providing job security for doctors should proceed to provide effective oriental medical care services in health centers.

Evaluation of Healthy City Project Using SPIRIT Checklist: Wonju City Case (SPIRIT 체크리스트를 활용한 건강도시평가: 원주시 사례)

  • Objectives: The objectives of this study was to evaluate Wonju Healthy City project and identify its problems, and seeking a way for its improvement based on the Healthy City project philosophy and strategies. Methods: We used the SPIRIT Checklist that was a process evaluation tool and developed by Alliance for Healthy Cities for the study. We analyzed 39 related materials and gathered opinions on the evaluation result with Healthy City Team staffs, related department staffs and the advisory committee. Finally, a joint meeting with AFHC SPIRIT evaluation expert verified the result of the analysis. Results: The evaluation of Wonju Healthy City project confirmed that Wonju city is equipped with the resources, such as mid-term plan, infrastructure, cooperative organizations, and the Healthy City network to enable the consistent implementation of the Healthy City project based on strong political commitment. However, the necessity of additional complementary processes as well as the application of further improvements to assist health promotion strategies was evident. Conclusion: It is required to improve Wonju Healthy City project that activation of health promotion programs based on the political support and cooperation with public health center and Healthy City project departments in city hall.

The Study on the Contents and Satisfaction of Oriental Medicine Examination Program (한방건강검진 프로그램의 내용과 참가자들의 만족도에 관한 연구)

As a point at issue of occidental medicine examination, followings have been indicated, 1) Occidental medical examination is performed by mainly test and instrument and therefore, role of doctors could be excluded and it could be diagnosed only for target disease and 2) in Korea, it could not be conducted in public medical system and therefore improvement effect of public health promotion could not be made due to increase of total medical expenses and infirmity of post management. These points are substantial limit of paradigm resided in occidental medicine and also problem caused by unique characteristics of medical system of Korea. In Korea, result of occidental medical examination show increased health risk from aged thirties and major diagnosed diseases are circulatory disease or chronic disease such as hepatic, hyperlipemic, hypertensional disease and diabetes, etc. Accounting that those chronic disease make a role as major public health risk, it is difficult that medical examination by only occidental medicine make effect on public health promotion. 2. Characteristics of oriental medicine examination could be summarized as diagnosis (information acquisition based on the facts) and demonstration (speculation based on acquired information) and in addition, quadruple diagnosis, as a medical examination method, include test as well as basic examination. Accounting on oriental medical examination, principally it is performed by independent herbalist and therefore, herbalist could acquire systemic result during first medical examination. Based on the theory of inner-outer examination, oriental medical examination has a principle of universe theory, systemic analysis of quadruple diagnosis, demonstration & reasoning. In addition, root of oriental medical examination could be found in pre-disease theory, a principal theory of oriental medicine. Pre-disease service could prove the advantage of oriental medicine in medical examination activities and therefore, it is needed that content of oriental medical examination should be actualized in current medical system. 3. In this study, oriental medical examination program, comprised of pulse-diagnosis, contrast muscular taking and medical consultation of herbalist is progressed communally with occidental medical examination. As pre-examination, questionary was given of general characteristics, health promoting life style, physical constitution and subjective symptom of musculoskeletal system. In addition, post-examination notification was given to subjects about health promoting control, physical constitution regimen and management of musculoskeletal system. During this study program, verification was conducted for input of acquired information and difference of each information after analysis and in addition, performed was analysis of factor influencing health promoting life style and musculoskeletal subjective symptom and evaluation of relationship of physical constitution and health promoting life study. In addition, it was verified of difference between musculoskeletal subjective symptom and result of muscle contrast picture evaluation. 4. Evaluation of oriental medical examination model is divided into 2 categories of oriental medical examination-consultation and result evaluation -post management. Oriental medical examination-consultation demands establishment of examination system, standardization of examination and establishment of examination form and in addition, it should be admitted as enlarged examination assists systemic quadruple diagnosis of herbalist not a key of oriental medical examination. In addition, information acquisition for research purpose should be performed according to the systemic research plan based on the separation of questionary for examination purpose and research purpose. For evaluation of the result, it was concluded that needed are result evaluation meets oriental medical system and post-notification system, informing health management information, based on examination result. 5. Accounting on satisfaction for oriental medical examination model, affirmative reply was much higher (66%) than negative (8.64%). Satisfaction of each area was in order of consultation of herbalist, systemic muscle contrast taking, pulse examination, post-notification and questionary fill-up and dissatisfaction was in order of post-notification, questionary fill-up, consultation of herbalist, systemic muscle contrast taking and pulse measurement. Satisfaction for collaborative examination of occidental and oriental medicine was over 60% and over 75% hope oriental examination would be included in later medical examination program. Based on this result, collaborative examination including occidental and oriental medicine could increase satisfaction of subjects for medical examination program.

Survey on the Industrial Safety & Hygiene Consultant System and It's Operational Issues (산업안전·위생지도사 제도 및 운영에 대한 인식도 조사연구)

  • The Korean government had introduced the industrial safety & hygiene consultant(ISHC) system to improve worksite health and safety level by providing specialized industrial safety & hygiene(S&H) services in 1996. Despite of initial hopeful expectation it has been said that this plan might fail to settled down and maintain its purpose. This study was designed to investigate operational issues and problems concerning the ISHC system, that would direct the way of change and future role. Questionnaire were sent to ISHC, professional engineer, agencies for occupational injury prevention, S&H specialist in the corporation nationwide and totally 256 people responded to in having 62.7% of response rate. The result of this study were as follows; 1. One hundred and forty two cases of safety assessment and 27 cases of process safety reports were the six years' results since 1996 the ISHC program had first been introduced. This results showed that injury prevention activities of ISHC had been stagnant. 2. The 78.8% of respondents had negative opinion about injury prevention activities of ISHC program. 3. The operational problems of ISHC were lack of understanding of company about ISHC, low systematization of work items, duty overlapping with agencies for occupational injury prevention, low support by government. in order. 4. It was suggested that abolition of ISHC and promotion of professional engineer system should be considered. 5. To promote professional engineer system it was suggested that the professional engineer could conduct consulting service and ISHC should be integrated with the expert system.

Subjective Perception of Drinking among New College Students of Nursing (간호대학 신입생의 음주 유형)

  • Purpose : Although quantitative research on alcohol consumption among nursing students is important, qualitative research is needed to determine the subjective views of individual students, such as their feelings and thoughts, and ensure the implementation of a targeted alcohol intervention program. Q-methodology is a systematic approach that examines the subjective perspectives of individuals, including their views, beliefs, and attitudes, enabling understanding of the types and characteristics according to the individual's subjectivity structure. This study examined the subjective perceptions of drinking among freshmen in nursing college using Q methodology. Methods : Q-sorting was conducted, collecting 38 P samples and 40 statements. The data were analyzed using the PC QUANL program. The principal component factor analysis method was used for Q-factor analysis. Results : The results identified four types of drinking perceptions among freshmen in nursing college. Type 1 was "safety and health pursuit," type 2 was "friendship pursuit," type 3 was "'norm-oriented," and type 4 was "sound manners." When looking at the subjective perceptions of drinking among freshmen in nursing college, there was a common opinion that drinking should not be forced and that it is an individual choice. However, the difference in views (positive and negative) of drinking shows the need for customized educational programs and interventions suitable for each type. Conclusion : Nursing freshmen should be prepared to play an important role in health care as an educational role and model in preventing damage from drinking and maintaining health promotion throughout their life by habituating proper drinking behavior during college life. In addition, it is necessary to develop a plan to increase positive awareness of drinking among nursing students through various strategic programs that can participate in sobriety prevention programs within the university.

The Comparison of Health Behaviors Between Fishing Area Residents and Agricultural Area Residents (농촌지역과 어촌지역 주민들의 건강행태 비교)

  • Objectives: This study compared the difference of health behaviors between fishing area residents and agricultural area residents. Methods: We carried out interview survey for 503 adult persons of fishing villages, the study area. And surveyed 775 adult persons of agricultural villages as a control area in short distance from the study area, and 1425 adult persons of agricultural villages in long distance. The proportion on health behavior was standardized to the nationwide population of the year of 2000 by sex. Results: In male, The proportion of heavy alcohol drinking in the study area was higher than that in both controls significantly. And the proportion of trial to quit alcohol during the past one year in the study area was lower than that in both controls, but it was not significant difference. And the proportion of regular exercise in study area was lower than that in the long distant control, but it was not significant difference. In female, the proportions of drinking, smoking, and regular exercise in the study area were significantly lower than that in the long distant control respectively. There was no significant difference in trial of low salt diet between the study area and the control area. Conclusions: When we plan the health promotion program in fishing village, we must consider alcohol drinking behavior of men and exercise behavior of both sex in addtition to smoking behavior.

The Meaning and Challenges of Health Care Workforce Support Act (보건의료인력지원법의 의미와 과제)

  • The 「Health Care Workforce Support Act」 was came into effect on 29 October 2019. Health care workforce is the most important national health resource, and the quantity and quality of them is also related to the level of national health care. This study examined the background and major contents of this Act and suggested future improvement by analyzing main issues. Th e Act was enacted to support the health care workforce for their recruitment and better working environment. By stipulating the establishment of the master plan, management of the supply and demand, improvement of working environment for health care workforce, the act contributed to raise the quality of health service and health promotion. Given that the act was developed in consideration to support of health care workers, it has many meanings. However, it is necessary to reexamine the above-mentioned five issues in order to develop the effective law while utilizing the significance of the law for health care workforce support and has to revise as soon as possible.
