교육훈련비와 병원의 경영성과 간의 관계에 관한 연구 - 의료법인 병원을 중심으로 - (A Study on the Relationship between the Hospital's Management Performance and Training Expense)

  • 본 연구는 병원의 교육훈련비 지출이 병원의 경영성과와 어떠한 관계를 가지는지에 대해 우리나라의 전체 의료법인의 5개년 재무제표 자료를 이용하여 분석하였으며, 분석결과는 다음과 같이 요약할 수 있다. 의료수익률 기준 회귀분석의 경우 5개 연도 중 3개 연도에서 교육비와 경영성과 간의 관계가 부정적인 것으로 나타났고, 순이익률 기준 회귀분석의 경우는 단지 1개 연도만 교육훈련비가 경영성과에 부정적인 관계를 가지는 것으로 제시되었다. 그리고 대도시와 지방으로 구분하여 분석한 결과를 보면, 대도시의 경우 순이익률 기준에 있어 1개 연도가 교육훈련비와 경영성과 간의 관계가 긍정적인 것으로 제시되어 전체 분석과는 상반된 결과를 보여주었다. 본 연구는 전국규모의 의료법인 전체의 가장 최근의 5개년 재무자료를 이용하여 교육훈련비와 경영성과 간의 관계를 분석한 최초의 논문이라는 점에서 그 의의를 가진다.

응급구조학전공 학생의 병원 임상실습 만족도에 영향을 미치는 요인 (Influencing Factors on the Satisfaction of the Paramedic Students in Clinical Training)

  • Purpose: This study aims to assess the influencing factors on the level of satisfaction with clinical training and to provide basic data for more efficient clinical training. Method: The study was conducted on 402 paramedic students who have experienced clinical training from September 6 until October 12 in 2011. The questionnaires consisted of 40 questions. We used SPSS 18.0 frequency analysis, technical statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Result: 1. The level of satisfaction with clinical training showed significant difference between the frequency (F=8.837, p=.000) and clinical training managers (F=5.418, p=.001). 2. The level of satisfaction with clinical training showed the strongest positive correlation with the satisfaction of clinical training hospitals (r=.694, p=.000). 3. Multiple regression analysis revealed the most powerful predictor for satisfaction with clinical training was the satisfaction level of clinical training hospitals(48.2%) and the frequency of clinical training experiences(.8%), the preparation before the clinical training(5.4%), the total duration of clinical training(.7%), and the satisfaction of emergency department education(1.0%). These five variables accounted for 56.1% of the satisfaction of clinical training among paramedic students. Conclusion: As student's satisfaction of the clinical training increases, the quality of paramedics is expected to improve in the future. As a result, the future paramedics can be nurtured to be highly skilled in on-the-scene emergency situations after graduation.

2급 응급구조사 병원임상실습교육 효과 비교 (Comparison of Effectiveness on Hospital-based Skill Training in Level 2 Emergency Medical Technician)

  • 본 연구는 2급 응급구조사 병원임상실습교육의 효과와 만족도를 조사한 연구로써 2급 응급구조사 전문화 교육의 만족도를 높이고, 효과적으로 운영하기 위한 기초자료를 제공하는데 목적이 있다. 조사대상은 2013년 7월 29일부터 8월 22일까지 2급 응급구조사 병원임상실습교육에 참여한 70명의 2급 응급구조사로 하였으며 구조화된 무기명 자기기입식 설문지를 이용하여 조사하였다. 연구결과 대상자가 자가평가한 업무능력은 교육 전이 3.09점, 교육 후 3.73점으로 유의한 차이를 보였고, 교육만족도는 3.80점으로 나타났다. 대상자들이 생각하는 교육적정기간은 1주~2주가 64.3%였고, 2급 응급구조사 자격과정을 오래전에 수료했을수록 교육만족도가 높았다. 향후 2급 응급구조사 병원임상실습교육의 요구도, 문제점 등을 파악하기 위한 다각적인 연구가 필요하며, 수련을 담당하는 의료기관의 실습교육 모니터링을 포함하는 프로그램의 개발이 필요하다.

Risk Factors for Stage IV Breast Cancer at the Time of Presentation in Turkey

  • Background: Breast cancer (BC) is the one of the most common cancers in women. It is also a leading cause of death. Unfortunately, some patients initially present with distant metastases and are diagnosed with stage IV disease that is nearly always, by then, incurable. This retrospective analysis investigated the risk factors for stage IV BC that may underlie such late presentation. Materials and Methods: In all, 916 patients with BC who visited the medical oncology polyclinic of eight different centres in Turkeybetween December 2011 and January 2013 were analysed. Results: A total of 115 patients (12.6%) presented with stage IV disease. In univariate analysis; to comparing these with patients at other stages, no statistical difference was found for median diagnosis age or age at menarche (p=0.611 and p=0.820), whereas age at menopause and age at first live birth were significant (p=0.018 and p=0.003). No difference was detected in terms of accompanying diseases, use of oral contraceptives and hormone replacement therapy, smoking, alcohol consumption and the rate of family history of BC between the patients (p=0.655, p=0.389, p=0.762, p=0.813, p=0.229, p=0.737). However, screening methods were employed less often, the rate of illiteracy was higher, and the rate of other cancers was higher in patients with stage IV BC (p=0.022, p=0.022, p=0.018). No statistical difference was observed between the patients in terms of tumour histopathology, and status of oestrogen receptor, progesterone receptor, or human epidermal growth factor-2 receptor (p=0.389, p=0.326, p=0.949, p=0.326). Grade 3 tumours were more frequent in patients with stage IV disease (p<0.001). On multivariate analysis, risk factors for stage IV breast cancer at the time of presentation were found to be age at first live birth and educational level (p=0.003 and p=0.047). Conclusions: Efforts should be made to perform mammography scans, in particular, at regular intervals through national training programs for all women, particularly those with family histories of breast and other types of cancer, and to establish early diagnosis of BC long before it proceeds to stage IV. Additionally, women's education had better be upgraded. In order to make women aware of BC, national education-programmes must be organised.

하지 로봇재활의료기기의 안전성 및 필수성능 평가 기준 개발 (The Development of Safety and Essential Performance Criteria for Lower Extremity Robotic Assisted Gait Training System)

  • The purpose of this study is to provide basic data to ensure the safety and essential performance of a Lower Extremity robotic assisted gait training system and to provide advanced technology and technical basis to the industry handling the system. Based on IEC 60601-1:2012/AMD2:2020 (Medical Electrical Equipment - General requirements for basic safety and essential performance of medical electrical equipment), IEC 62366-1:2015/AMD1:2020 (Medical devices - Part 1: Application of usability engineering to medical devices) and EN ISO 14971:2019 (Medical devices - Application of risk management to medical devices), the requirements for ensuring the safety and essential performance of the Lower Extremity robotic assisted gait training system were derived. Through the Delphi survey method and scenario analysis, which reflects the opinions and knowledge of experts in the fields of development, testing and review of technical documents, and quality assurance of medical devices, validity and reliability were conducted and obtained results with adequate content validity ratio (CVR; 0.7≤) and excellent reliability (Cronbach's α; 0.9≤). As a result, it was confirmed that the reliability and validity of the risk management process to ensure the safety and essential performance of the Lower Extremity robotic assisted gait training system are required a model can be established to provide measures to reduce risks according to the level of risk exposure caused by usage.

Utility of Peripheral Blood Parameters in Predicting Breast Cancer Risk

  • Purpose: We aimed to study the inflammatory parameters of complete blood count in breast cancer cases. Materials and Methods: This retrospective study covered 178 breast cancer patients and 107 age and body mass index matched healthy women. Complete blood count parameters, neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR) and MPV/platelet were analyzed. Results: The leukocyte, neutrophil and neutrophil/lymphocyte ratio were higher in the patient group (p values 0.001, 0.0001 and 0.0001, respectively) while haemoglobin and hematocrit were higher in the control group (p=0.0001 for both). Logistic regression analysis showed that elevated neutrophils and platelet distribution width (PDW) (OR: 0.627, 95%CI: 0.508-0.774, p=0.001 and OR: 1.191 95%CI: 1.057-1.342 p=0.003) were independent variables for predicting breast cancer. The cut-off value for the neutrophil/lymphocyte ratio was 2.56. Conclusions: According to our study results, neutrophil levels as part of complete blood count may be used as an independent predictor of breast cancer risk.

일반인에게 실시한 심폐소생술 교육효과 (CPR Training Effect for Civilian)

  • Purpose: This study was focused on the training effects of the cardiopulmonary resuscitation(CPR) which is based on the AHA guideline 2010 for lay person. The purpose of this study was to investigate the effectiveness of the CPR training for civilian by checking the performance ability of the subjects before and after the CPR education. Method: This study was a single primary experimental study designed with pre and post test. Study subjects were 55 apartment managers who participated in all test on December 5 and December 11, 2011. The tools of this study were CPR performing evaluation sheet and CPR teaching program(AHA). Data were analysed by descriptive statistics, t-test, repeated ANOVA and Bonferroni with SPSS 18.0 program. Result: The CPR practice performing ability was significantly increased by test. The CPR practice performing ability of the learning experience group had significantly higher score than that of non-experience group. The self-confidence on CPR practice when faced emergency situation was highly increased after practice training. Conclusion: Practical performance evaluation score has improved step by step. The primary and secondary practical evaluation showed a significant difference in all items and practice-oriented education was found to be effective in CPR training.

응급구조학과 학생들의 구급차 동승실습 중 감염관리에 대한 인지도 및 수행도 (Paramedic students' awareness and performance of infection control on ambulance attendant training)

  • Purpose: This study aims to investigate awareness and performance of infection control during ambulance attendant training, and to provide basic data for infection control. Methods: The subjects were 235 paramedic students who completed ambulance attendant training. There were 51 questions. The infection control dimension was divided into hand washing, personal protective equipment use, and environmental management, for each sub-dimension, awareness and performance were measured by a 4-point scale. The collected data were analyzed using SPSS statistics ver. 22.0. Results: A total of 95.3%, of the subjects completed an orientation for ambulance attendant training and 71.7% received education on infection. In all three sub-dimensions, hand- washing (p<.001), personal protective equipment use (p<.001), and environmental management (p<.001), awareness scored higher than performance. The awareness of infection control showed a significantly positive correlation (r=.394) with performance. Conclusion: In order to improve performance of infection control, education to improve awareness should be provided, and paramedics with higher performance levels in hand washing, and use of gloves and masks wearing should be assigned as training advisors.

Mean Platelet Volume as a Prognostic Marker in Metastatic Colorectal Cancer Patients Treated with Bevacizumab-Combined Chemotherapy

  • Background: Recent studies have revealed a prognostic impact of the MPV (mean platelet volume)/platelet count ratio in terms of survival in advanced non-small cell lung cancer. However, there has been no direct analysis of the survival impact of MPV in patients with mCRC. The aim of the study is to evaluate the pretreatment MPV of patients with metastatic and non-metastatic colorectal cancer (non-mCRC) and also the prognostic significance of pretreatment MPV to progression in mCRC patients treated with bevacizumab-combined chemotherapy. Materials and Methods: Fifty-three metastatic and ninety-five non-metastatic colorectal cancer patients were included into the study. Data on sex, age, lymph node status, MPV, platelet and platecrit (PCT) levels were obtained retrospectively from the patient medical records. Results: The MPV was significantly higher in the patients with mCRC compared to those with non-mCRC ($7.895{\pm}1.060$ versus $7.322{\pm}1.136$, p=0.013). The benefit of bevacizumab on PFS was significantly greater among the patients with low MPV than those with high MPV. The hazard ratio (HR) of disease progression was 0.41 (95%CI, 0.174-0.986; p=0.04). In conclusion, despite the retrospective design and small sample size, MPV can be considered a prognostic factor for mCRC patients treated with bevacizumab-combined chemotherapy.

Regular Exercise Training Increases the Number of Endothelial Progenitor Cells and Decreases Homocysteine Levels in Healthy Peripheral Blood

  • Endothelial progenitor cells (EPCs) are known to play an important role in the repair of damaged blood vessels. We used an endothelial progenitor cell colony-forming assay (EPC-CFA) to determine whether EPC numbers could be increased in healthy individuals through regular exercise training. The number of functional EPCs obtained from human peripheral blood-derived AC133 stem cells was measured after a 28-day regular exercise training program. The number of total endothelial progenitor cell colony-forming units (EPC-CFU) was significantly increased compared to that in the control group (p=0.02, n=5). In addition, we observed a significant decrease in homocysteine levels followed by an increase in the number of EPC-CFUs (p=0.04, n=5), indicating that the 28-day regular exercise training could increase the number of EPC colonies and decrease homocysteine levels. Moreover, an inverse correlation was observed between small-endothelial progenitor cell colony-forming units (small-EPC-CFUs) and plasma homocysteine levels in healthy men (r=-0.8125, p=0.047). We found that regular exercise training could increase the number of EPC-CFUs and decrease homocysteine levels, thus decreasing the cardiovascular disease risk in men.
