Comparison of author key words and Medical Subject Heading terms in the Journal of Korean Society of Dental Hygiene from 2001 to 2015 (한국치위생학회지 게재 논문의 저자 키워드와 MeSH 용어의 비교(창간호~2015년))

  • Objectives: The purpose of this study was to compare the author key words and MeSH (Medical Subject Headings) terms in the Journal of Korean Society of Dental Hygiene (JKSDH). Methods: A total of 3,242 author key words from 974 informative articles published from 2001 to 2015 were compared with MeSH terms, according to the criteria of complete coincidence, incomplete coincidence, and complete non-coincidence. Results: The coincidence rate of 564 author key words with MeSH terms was 17.4%. The author key words that appeared most frequently (in descending order) were oral health (41 times), dental hygienists (30 times), dental caries (29 times), and knowledge (29 times). There was a non-coincidence rate of 70.5% for 2,286 author key words with MeSH terms. Conclusions: Many author key words used in the JKSDH did not coincide with MeSH terms. The use of author key words that coincide with MeSH terms is necessary to accomplish the international journal.

A Study on the Medicinal Application in the Zangfuxushibiaobenyongyaoshi - Based on the Zangfuyaoshibuzheng of Zhang Shanlei - (『장부허실표본용약식(臟腑虛實標本用藥式)』에 나타난 약물(藥物) 운용(運用)에 관한 고찰(考察) - 장산뢰(張山雷)의 『장부약식보정(臟腑藥式補正)』을 중심으로 -)

  • Objectives : Based on the Zangfuyaoshibuzheng of Zhang Shanlei, this paper examines the tip/root division of the Zangfu, features of medicinal application according to deficiency/excess or cold/heat, and erroneous cases in the Zangfuxushibiaobenyongyaoshi of Zhang Jiegu. Methods : Categorization according to tip/root, cold/heat, deficiency/excess and respective medicinal applications in the Zangfuxushibiaobenyongyaoshi were organized, followed by thorough analysis based on the annotations in the Zangfuyaoshibuzheng. Results : First, in the treatment principle of the Zangfu diseases, each of their physiological function and pathological mechanism were analyzed so that the disease patterns are understood accurately and treated accordingly. Second, in categorizing treatment methods, terminology application was modified in overlapping or ambiguous cases so that they could be better distinguished. Third, medicinal categories that were designated to treat each Zangfu disease were explained well based on unique features. In cases where medicinals were falsely included, they were identified and corrected based on sound reasoning. Conclusions : The pattern diagnosis in the Zangfuyaoshibuzheng is clear, and medicinal application analysis is intelligible. As it thoroughly corrects and revises errors in Zhang Jiegu's theories, it could provide valuable assistance in selecting each medicinals when treating disease patterns of the Five Zang.

Treatment outcome of conservative surgery plus postoperative radiotherapy for extremity soft tissue sarcoma

  • Purpose: To evaluate the treatment outcome and prognostic factor of postoperative radiotherapy for extremity soft tissue sarcoma (STS). Materials and Methods: Forty three patients with extremity STS were treated with conservative surgery and postoperative radiotherapy from January 1981 to December 2010 at Korea University Medical Center. Median total 60 Gy (range, 50 to 74.4 Gy) of radiation was delivered and 7 patients were treated with chemotherapy. Results: The median follow-up period was 70 months (range, 5 to 302 months). Twelve patients (27.9%) sustained relapse of their disease. Local recurrence occurred in 3 patients (7.0%) and distant metastases developed in 10 patients (23.3%). The 5-year overall survival (OS) was 69.2% and disease free survival was 67.9%. The 5-year local relapse-free survival was 90.7% and distant relapse-free survival was 73.3%. On univariate analysis, no significant prognostic factors were associated with development of local recurrence. Histologic grade (p = 0.005) and stage (p = 0.02) influenced the development of distant metastases. Histologic grade was unique significant prognostic factor for the OS on univariate and multivariate analysis. Severe acute treatment-related complications, Common Terminology Criteria for Adverse Events (CTCAE) grade 3 or 4, developed in 6 patients (14.0%) and severe late complications in 2 patients (4.7%). Conclusion: Conservative surgery with postoperative radiotherapy achieved a satisfactory rate of local control with acceptable complication rate in extremity STS. Most failures were distant metastases that correlate with tumor grade and stage. The majority of local recurrences developed within the field. Selective dose escalation of radiotherapy or development of effective systemic treatment might be considered.

Analysis of words related with medical concept and context of paragraphs in Suwen of Huang Di Nei Jing based on concreteness and ideality (『황제내경(黃帝內經) · 소문(素問)』의 개념어(槪念語)와 논지(論旨) 분석(分析) - 구체성과 관념성을 중심으로 -)

  • Objective : analysis of words related with medical concept and context of paragraphs in Suwen of Huang Di Nei Jing based on concreteness and ideality. Method : First, I extract words having medical concept out of the whole sentence of Huang Di Nei Jing. and classifies them according to the type of medical concept. Second, I classify all sentence of it on the basis of analysing of the words. Result : 1. There are not an interrelationship between the abstract and concrete words from the perspective of cause and effect. Abstract words group are divided into two different parts according to the fundamental character. There are the concept being formed from pure idea and the concept being formed from material idea. The main words composed most important part of the oriental medicine have a combination mode with these two word groups. 2. We have several data about concreteness and ideality of Huang Di Nei Jing. Subjects and contents in provisions are concrete relatively. But the logical basis of sentences is remarkably ideational. Many kinds of abstract words are used dominantly to compose logics of these basis. It confirms that concrete words can not be used to make theories and concepts in Suwen. Conclusion : I analyzed words related with medical concept and context of paragraphs in Suwen of Huang Di Nei Jing based on concreteness and ideality and I found that the concept and logical system of Huang Di Nei Jing in the objective point of view.

Study on the Meaning of Nasal discharge(涕) in Five fluids (오액(五液) 중(中) '체(涕)'의 의미에 대한 고찰)

  • Objectives : The paper raises an objection to the word '涕' being used to refer to nasal discharge, and proposes a word for nasal discharge upon studying a set of medical books. Methods : The author finds and confirms the dictionary definition of '涕' and studies how they are used differently in medical books. Through this study, the author shows how the word '涕' is used incorrectly and makes deductions for its reason. The author takes a look at the old form of the word '涕', its etymological origin, takes a guess as to the real word that should have been used to refer to nasal discharge, and find examples of instances where this correct word for nasal discharge are more appropriate. Results & Conclusions : In medical books such as Huangdineijing Suwen, '涕' is used to mean nasal discharge, but the word's dictionary definition does not validate such usage. Yugunryeombu (劉君廉夫), in its commentary for Somun, used '?' and '鼻夷' for '涕', and '?' means nasal discharge and used as same as '涕' when its used to mean tear. This is a phenomenon that originated from '弟' and '夷' being used interchangeably which led to the incorrect usage of '?'. If someone were to refer to nasal discharge, he needs to use '?'. '鼻夷' is believed to be the same word as '弟鼻', which is the old form of '?', and it means both tear(pronounced 'Che') and nasal discharge(pronounced 'Je'). However, the pronunciation different between 'Che' and 'Je', and its definition as tear, is divided in later periods into '涕' following the shape of '弟'. Following the shape of '夷', the meaning of nasal discharge remains in '?' while retaining the pronunciation of 'yi'. Therefore, the word '涕' used to mean nasal discharge is an incorrect form of '?', and should all be re-written to '?'.

The Comparative Study between the Effect of Herbal Formula in Schoolbook and the Effect deduced from Compositional Herbal Effects ("방제학"에 기재된 방제 효능과 본초 구성을 기반으로 도출된 효능의 비교 연구)

  • Objective The analysis method based on the herbal formula's effects is a general tool, in traditional medicine. In effective applications of herbal formula, Korean herbal medicines traditionally used the classification methods based on the curative effects through the various compositions of herbal formulas. However' the effects of herbal formulas were not filed systemically in ancient literatures, and the standards to confirm their effects are not clear. Thus, it is not easy to classify herbal formulas according to the curative effects. Furthermore, there are no standards to estimate the effects of prescriptions frequently directed in clinic. In this study, we aimed to provide the methodology of classifying the curative effects of herbal formulas by calculating the combination of the effect of each compositional herb through the DB systems. Results : By comparing effects of herbal formula with those of compositional herbs, we found that about 25-50% of the herbal effects were included in herbal formula's effects. These results showed that the prospective estimation of herbal formula's effects may be possible through the DB systems filing herbal effects. To enhance the accuracy in explaining the herbal formula's effect, more studies are needed by giving prominence to major effects and by subtracting minor effects.

An improvement plan of Curriculum in Departments of Dental Technology (치기공과 교육과정의 개선방안)

  • This research collected the curriculum for Dental Technology from a total of 20 schools --3-year colleges and 4-year colleges-- all in Korea. And we analyzed the average credits of subjects from students. As a result of this analysis, we get the conclusion below: 1. In the arithmetic mean of the major basis subjects which graduates and undergraduates answered about each subjects; Seminar, Dental morphology I II, Dental morphology practice I II, and Dental devices & instruments don't have many credits. And averages of the major application subjects credits which are Implants(especially low), Occlusal anatomy practice I II, Dental ceramics practice I II, and Dental ceramics practice are low, mostly have a converged tendency in high points. 2. In an analysis of the correlation which is based on the major basis subjects: Dental esthetic, oral anatomy I II, Dental materal practice III, Dental casting pracedure, Oral hygiene, Health & medical law, Management administration, and Medical terminology have a meaningful difference. (${\rho}$ < 0.05) 3. In an analysis of the correlation which is based on the major application subjects; Crown and bridge prosthodontics practice IV, Complete denture prosthodontics I II III, Complete denture prosthodontics practice I II III, Dental ceramics I II, Dental ceramics practice I II, Dental ceramics practice IIII, Occlusal anatomy I II, Occlusal anatomy practice I, Operative dentistry laboratory technology I, Operative dentistry laboratory technology practice II, Dental attachment laboratory technology practice, Implants, and Dental laboratory clinical practice have meaningful difference. (${\rho}$ < 0.05) 4. In an analysis of the correlation which is based on the ratio of a theory to an actual training; 40:60(38.57%) is the highest, followed by 30:70(30.04%), 50:50(23.32%), 60:40(5.83%), and 70:30(2.24%). These have meaningful difference. (${\rho}$ < 0.05) 5. In an analysis of the correlation which is based on the distinction of sex: Partial denture prosthodontics practice I II III, Complete denture prosthodontics I II III, Complete denture prosthodontics practice I II III, Occlusal anatomy practice I II, Implants, Medical terminology have meaningful difference. (${\rho}$ < 0.05) For the purpose of training entrepreneurs of middle standing who is required by a future society, Department of Dental Technology's Curriculum need to be managed with planning a curriculum which reflects opinions of graduates, undergraduates and a society, and also are considered not focusing on a supplier but focusing on a user.

The Kampo Medicine Goshajinkigan Prevents Neuropathy in Breast Cancer Patients Treated with Docetaxel

  • Background: Goshajinkigan (GJG) is used for the treatment of several neurological symptoms. We investigated the efficacy of GJG and mecobalamin (B12) against neurotoxicity associated with docetaxel (DOC) in breast cancer patients. Materials and Methods: Sixty breast cancer patients were treated with DOC. Thirty-three patients (GJG group) received oral administration of 7.5 g/day GJG and 27 patients (B12 group) received oral administration of 1500 ${\mu}g/day$ B12. Neuropathy was evaluated according to DEB-NTC (Neurotoxicity Criteria of Debiopharm), Common Terminology Criteria for Adverse Events (NCI-CTC) ver. 3.0, and a visual analogue scale (VAS). This study employed a randomized open design. Results: The incidence of neuropathy was 39.3% in the GJG group, and 88.9% in the B12 group (p<0.01). In the GJG group, grade 1 DEB-NTC was observed in 2 cases, grade 2 in 5 cases and grade 3 in 5 cases. Grade 1 NCI-CTC was observed in 7 cases, grade 2 in 6 cases, and VAS was $2.7{\pm}2.2$. In the B12 group, grades 1, 2 and 3 DEB-NTC were observed in one case, 12 cases and 12 cases, respectively; and grades 1, 2 and 3 NCI-CTC were observed in 11 cases, 12 cases and one case, and VAS was $4.9{\pm}2.4$. Conclusions: Concomitant administration of GJG is useful in preventing neuropathy in breast cancer patients treated with a DOC regimen.

A Study on the English Translations of Shanghanlun (Treatise on Cold Damage) and the Cold Pathogen Chapter of Donguibogam (『상한론(傷寒論)』 영역본과 『동의보감(東醫寶鑑)』 영역본 잡병편 '한(寒)'문의 비교 연구)

  • This study utilized Corpus-based Analysis process to compare the Cold Pathogen chapter in the 'English version of "Donguibogam"' to the 'English version of the "Shanghanlun"' translated by 罗希文 (Luo xi wen). Results of the linguistic analysis indicate that TTR, a ratio of number of types to number of tokens in the English version of "Shanghanlun" was 5.92% while TTR in the Cold pathogen chapter of English version of "Donguibogam" was 6.01%. It was also noted that the types of words frequently appearing in the two publications were the scientific name of medicinal herbs; the method of producing the herbal prescription (including terminology representing weights and measures); and Chinese descriptions of concepts considered important in both Korean and Chinese medicinal practices. Finally, it was possible to find points of comparison in naming of symptoms, diagnosis, prescriptions, and respective names of six meridians. Though the language difference is minimal, the vocabulary found in the Cold Pathogen chapter of "Donguibogam" was more diverse than Luo's translation of "Sanghanlun". In general, literal translation in keeping with the sense of original text was better performed in Luo's translation of the "Sanghanlun" whereas the English version of the Cold Pathogen chapter in the "Donguibogam" was more of a "free" translation.

Computer-based clinical coding activity analysis for neurosurgical terms

  • Background: It is not possible to measure how much activity is required to understand and code a medical data. We introduce an assessment method in clinical coding, and applied this method to neurosurgical terms. Methods: Coding activity consists of two stages. At first, the coders need to understand a presented medical term (informational activity). The second coding stage is about a navigating terminology browser to find a code that matches the concept (code-matching activity). Systematized Nomenclature of Medicine - Clinical Terms (SNOMED CT) was used for the coding system. A new computer application to record the trajectory of the computer mouse and record the usage time was programmed. Using this application, we measured the time that was spent. A senior neurosurgeon who has studied SNOMED CT has analyzed the accuracy of the input coding. This method was tested by five neurosurgical residents (NSRs) and five medical record administrators (MRAs), and 20 neurosurgical terms were used. Results: The mean accuracy of the NSR group was 89.33%, and the mean accuracy of the MRA group was 80% (p=0.024). The mean duration for total coding of the NSR group was 158.47 seconds, and the mean duration for total coding of the MRA group was 271.75 seconds (p=0.003). Conclusion: We proposed a method to analyze the clinical coding process. Through this method, it was possible to accurately calculate the time required for the coding. In neurosurgical terms, NSRs had shorter time to complete the coding and higher accuracy than MRAs.
