Efficacy Evaluation of Alpha/Beta Radioactivity Screening in Urine Samples using Liquid Scintillation Counting

  • Rapid screening for internal contamination by alpha- and beta-emitting radionuclides is essential in situations involving radiation workers or radiation accidents. This study focused on the use of urine samples and liquid scintillation counting to quickly and accurately assess contamination. Calibration of the alpha and beta detection areas ensured precise measurement results. The major radionuclides recommended for surveillance during accidents were also considered. This study evaluated the effectiveness of the method by examining various parameters, including the limit of detection, linearity, sensitivity, selectivity, accuracy, ruggedness, and blind test sample analysis. The liquid scintillation counting method is an effective tool for screening urinary samples to detect alpha- and beta-emitting radionuclides, particularly during radiation emergencies, despite some limitations in precision.

  • Diagnostic reference level (DRL) is employed to optimize the radiation doses of patients. The objective of this study is to review the DRLs for interventional procedures in Korea and abroad. Literature review was performed to investigate radiation dose index and measurement methodology commonly used in DRL determination. Dose area product (DAP) and fluoroscopy time within each major procedure category were systematically abstracted and analyzed. A wide variation was found in the radiation dose. The DAP values and fluoroscopy times ranged 0.01-3,081 Gy·㎠ and 2-16,878 seconds for all the interventional procedures, 8.5-1,679 Gy·㎠ and 32-5,775 seconds for the transcatheter arterial chemoembolization (TACE), and 0.1-686 Gy·㎠ and 16-6,636 seconds for the transfemoral cerebral angiography (TFCA), respectively. The DRL values of the DAP and fluoroscopy time were 238 Gy·㎠ and 1,224 seconds for the TACE and 189 Gy·㎠ and 686 seconds for the TFCA, respectively. Generally, the DRLs of Korea were lower than those of other developed countries, except for the percutaneous transluminal angioplasty with stent in arteries of the lower extremity (LE PTA and stent), aneurysm coil embolization, and Hickman insertion procedures. The wide variation in the radiation doses of the different procedures suggests that more attention must be paid to reduce unnecessary radiation exposure from medical imaging. Furthermore, periodic nationwide survey of medical radiation exposures is necessary to optimize the patient dose for radiation protection, which will ultimately contribute to patient dose reduction and radiological safety.

의료기관 방사선 종사자의 직무별 개인피폭선량에 관한 연구 (Medical Radiation Exposure Dose of Workers in the Private Study of the Job Function)

  • 본 연구는 방사성동위원소의 의학적 이용도가 증가함에 따라 의료기관 핵의학과 방사선관계종사자의 직무별 방사선 이용에 대한 개인 방사선피폭선량의 실태를 파악하여, 방사선 위험에 대해 경각심을 고취시키고, 방사선 관계종사자들에게 안전관리와 합리적인 피폭선량 관리에 도움을 주고자 분석하였다. 2010년 1월 1일부터 2010년 12월 31일까지 의료기관에서 근무하는 방사선종사자로 분류되어 개인 방사선피폭선량 측정을 정기적, 연속적으로 1년간 조사 관리된 540명의 종사자를 대상으로 부서별, 선량영역구간별, 근무기간별, 직무별 관련업무를 파악하여 심부선량에 대하여 연간평균피폭선량을 각각 분석하였다. 분석법으로는 빈도분석과 ANOVA를 시행하였다. 의료기관 방사선종사자의 부서별 연간피폭선량은 핵의학과 4.57 mSv로 가장 높았으며, 심장혈관중재술실 2.09 mSv, 마취통증의학과 1.42 mSv, 영상의학과 1.10 mSv, 구강악안면 방사선과 0.59 mSv, 방사선종양학과 0.50 mSv 순으로 높게 나타났다. 선량영역별 분포는 핵의학과, 심장혈관중재술실에서 5.01~19.05 mSv의 높은 선량영역분포를 보였으며, 부서별 방사선사의 연간피폭선량은 핵의학과 7.14 mSv로 가장 높은 피폭선량을 보이고 있으며, 심장혈관중재술실 1.46 mSv로 높았고, 영상의학과 0.97 mSv, 구강악안면방사선과 0.66 mSv, 방사선종양학과 0.54 mSv 순으로 나타났다. 세부업무에 따른 직무별 연간평균피폭선량은 싸이크로트론 관련 합성 업무 17.47 mSv로 가장 높은 피폭선량을 보였으며, Gamma camera 영상실 7.24 mSv, PET/CT 영상실 업무가 7.60 mSv로 높게 나타났고, 인터벤션 2.04 mSv, 심혈관중재술실 1.46 mSv, 일반촬영 1.21 mSv, Primart 치료실 0.90 mSv, 구강악안면방사선과 일반촬영 0.66 mSv 순으로 나타났다. 근무기간별, 선량영역별에 따른 연간평균피폭선량은 구강악안면방사선과에서는 10~14년 종사자가 1.01~3.00 mSv로 높은 평균선량을 보였고, 방사선종양학과는 모든 근무기간에 따라 0.00~1.00 mSv 의 낮은 선량영역구간에서 분포를 보였으며, 심혈관중재술실은 10~14년, 15~19년 근무에 따라 각각 1.01~3.00 mSv 선량영역구간에서 분포하였으며, 영상의학과에서는 1~4년, 5~9년 종사자가 각각 1.01~8.00 mSv의 가장 높은 선량영역구간에서 분포를 보였고, 핵의학과에서는 1~4년, 5~9년 종사자가 각각 3.01~19.05 mSv 의 가장 높은 선량영역구간에서 분포를 보였으며, 10~14년, 15~19년 종사자에서도 각각 3.01~15.00 mSv의 높은 선량영역구간에서 분포를 보였다. 이와 같은 결과로 볼 때 의료기관에서 근무하는 방사선관계종사자의 대부분이 현재의 방사선 안전관리가 실효성 있게 이루어지고 있었으며, 직무특성에 따라 많은 차이가 있는 것을 알게 되었다. 그러나 방사선 피폭을 최소화시키는 노력이 필요하며, 이를 위해서 체계적 교육과 합리적인 피폭량 관리를 위한 체계가 필요하다고 사료된다.

An Off-site Screening Process for the Public in Radiation Emergencies and Disasters

  • Background: A contamination screening process for the local population in radiation emergencies is discussed. Materials and Methods: We present an overview of the relevant Korean governmental regulations that underpin the development of an effective response system. Moreover, case studies of foreign countries responding to mass casualties are presented, and indicate that responses should be able to handle a large demand for contamination screening of the local public as well as screening of the immediate victims of the incident. Results and Discussion: We propose operating procedures for an off-site contamination screening post operated by the local government for members of the public who have not been directly harmed in the accident. In order to devise screening categories, sorting strategies assessing contamination and exposure are discussed, as well as a psychological response system. Conclusion: This study will lead to the effective operation of contamination screening clinics if an accident occurs. Furthermore, the role of contamination screening clinics in the overall context of the radiation emergency treatment system should be clearly established.

중재적 방사선시술에서 부가 차폐체 사용 시 종사자의 산란선 피폭 감소효과 (Effect of Reducing Scattering Radiation Exposure of Medical Staffs When Additional Shielding is Used in Interventional Radiology)

  • This article is designed to look into the radiation exposure dose to each body part and the shielding effect for workers using an additional shielding to reduce their radiation exposured by scattering radiation which is generated in a space between the operating table and lead curtain during interventional radiology(IR) procedures. After placing a human phantom on the table of SIEMENS' angiography machine, the following measurements were taken, depending on the presence of an additional shield of lead equivalent of 0.25 mmPb, manufactured for this purpose: dose to gonad, dose to an area where the personal dosimeter is placed, and dose to an area of eye lens is located. An ion chamber(chamber volume 1,800 cc) was utilized to measure scattering radiation. The two imaging tests were carried out as follows: fluoroscopy of the abdomen (66 kV, 100 mA, 60 seconds) and of the head (70 kV, 65 mA, 60 seconds); and digital subtraction angiography(DSA) of the abdomen (67 kV, 264 mA, 20 seconds) and of the head (79 kV, 300 mA, 20 seconds). In all the experiments, the shielding efficiency of the gonad position was the largest at 59.8%. In case an additional shielding was used as protection against scattering radiation that came through the operating table and the lead curtain during an IR, the radiation shielding efficiency was estimated to be up to 59.8%, leading to a conclusion that its presence may effectively reduce the radiation exposure dose of medical staffs.

X-ray 저선량 피폭방지를 위한 기능성 차폐크림의 방어 효과 검증 (Verification of the Protective Effect of Functional Shielding Cream for the Prevention of X-ray Low-dose Exposure)

  • 의료기관의 방사선 업무 종사자의 경우 환자 보호와 정확한 시술을 위해 방사선 노출이 이루어져 저선량 피폭의 문제점을 안고 있다. 저선량 방사선 피폭은 주로 Apron 영역 이외에 신체 부분에서 발생되며, 가장 많은 곳이 손 등 피부이다. 특히, 의료인의 손은 시술 중 감각과 미세한 동작을 필요로 하기에 방사선 노출영역에서는 무방비 상태로 피폭의 위험성을 안고 있다. 납장갑 등 차폐체로 문제를 해결할 수 있지만, 이는 시술 중 손의 활동성을 제안하여 사용상의 어려움이 있다. 이러한 문제점을 해결하고자 기능성 방사선 보호효과를 얻을 수 있는 차폐크림을 개발하여 납당량 0.1 mmPb와 차폐성능을 비교하였다. 차폐크림 제조과정에서 크림의 밀도를 높이기 위해 기공을 줄이는 탈포과정을 추가하여 차폐성능을 개선하였다. 따라서 황산바륨을 주 재료로 사용한 차폐크림은 납 plate보다 차폐율이 낮은 실효에너지 영역에서는 59%, 높은 실효에너지에서는 37% 정도의 차이는 보여 기능성 방사선 방어효과가 있는 것으로 나타났다. 따라서 피부에 직접 사용할 수 있는 장점으로 상시 방어효과와 수술용 장갑 착용전에 사용이 가능한 것으로 사료된다.

원전 코호트 연구의 적정 대상규모와 검정력 추정 (Power Estimation and Follow-Up Period Evaluation in Korea Radiation Effect and Epidemiology Cohort Study)

  • Objectives: The objective of this study was to calculate sample size and power in an ongoing cohort, Korea radiation effect and epidemiology cohort (KREEC). Method: Sample size calculation was performed using PASS 2002 based on Cox regression and Poisson regression models. Person-year was calculated by using data from '1993-1997 Total cancer incidence by sex and age, Seoul' and Korean statistical informative service. Results: With the assumption of relative risk=1.3, exposure:non-exposure=1:2 and power=0.8, sample size calculation was 405 events based on a Cox regression model. When the relative risk was assumed to be 1.5 then number of events was 170. Based on a Poisson regression model, relative risk=1.3, exposure:non-exposure=1:2 and power=0.8 rendered 385 events. Relative risk of 1.5 resulted in a total of 157 events. We calculated person-years (PY) with event numbers and cancer incidence rate in the nonexposure group. Based on a Cox regression model, with relative risk=1.3, exposure:non-exposure=1:2 and power=0.8, 136 245PY was needed to secure the power. In a Poisson regression model, with relative risk=1.3, exposure:non-exposure=1:2 and power=0.8, person-year needed was 129517PY. A total of 1939 cases were identified in KREEC until December 2007. Conclusions: A retrospective power calculation in an ongoing study might be biased by the data. Prospective power calculation should be carried out based on various assumptions prior to the study.

  • Astronauts are constantly exposed to space radiation at a low-dose rate during long-tenn stays in space. Therefore, it is important to determine correctly the biological effects of space radiation on human health. Space radiations contain various kinds of different energy particles, especially high linear energy transfer (LET) particles. Therefore, we have to study the relative biological effectiveness (RBE) of space radiation under microgravity environment which may change RBE from a stress for cells. Furthermore, the research about space radiation might give us useful information about birth and evolution of life on the earth. We also can realize the importance of preventing the ozone layer from depletion by use of exposure equipment to sunlight at International Space Station (ISS).

  • The purpose of this report is recommending a standard indicator which reflects the radiation exposure that is incident on a detector after every exposure event and that reflects the noise levels present in the image data. The experiment was performed with mobile digital X-ray unit and used a acrylic phantom for exposure index measurement. Exposure modality was kVp, mAs, SID. After every exposure, make a data sheet for characteristic curve of detector response. The equipment performed Mobile digital X-ray unit provide the user with values ralated to the incident exposure(air kerma)to the digital detector. They are showed as a logarithmic function shaped. As a result, DEI means a relative measure of exposure to the detector, as compared to the expected exposure for a particular anatomical view. Radiographic technique is the combination of factors used to exposure an anatomical part to produce a high quality radiography and technique charts used most commonly by radiographers to produce consistently exposure level which patient dose can be kept acceptably low.

Personal Dosimeters Worn by Radiation Workers in Korea: Actual Condition and Consideration of Their Proper Application for Radiation Protection

  • Background: Assessment of the radiation doses to which workers are exposed can differ depending on the placement of dosimeters on the body. In addition, it is affected by whether the placement is under or over a shielding apron. This study aimed to evaluate the actual positioning of personal dosimeters on the body, with or without shielding aprons, among radiation workers in Korea. Materials and Methods: We analyzed the survey data, which included demographic characteristics, such as sex, age, occupation, work history, and placement of the personal dosimeter being worn, from a cohort study of Korean radiation workers. We assessed the use of personal dosimeters among workers, stratified by sex, age, working period, starting year of work, and occupation. Results and Discussion: Overall, high compliance (89.1% to 99.0%) with the wearing of dosimeters on the chest was observed regardless of workers' characteristics, such as age, sex, occupation, and work history. However, the placement of dosimeters, either under or over the shielding aprons, was inconsistent. Overall, 40.1% of workers wore dosimeters under their aprons, while the others wore dosimeters over their aprons. This inconsistency indicates that radiation doses are possibly measured differently under the same exposure conditions solely owing to variations in the placement of worn dosimeters. Conclusion: Although a lack of uniformity in dosimeter placement when wearing a shielding apron may not cause serious harm in radiation dose management for workers, the development of detailed guidelines for dosimeter placement may improve the accuracy of dose assessment.
