  • Article 10 of the Constitution All citizens have dignity and values as humans and have the right to pursue happiness. There is a need for support measures such as a means to realize the respect of dignity and values as humans, and how to efficiently maintain policies on welfare for the elderly classified as the socially disadvantaged. It was considered necessary to develop an elderly-friendly city for economic, physical, and social life. Dasan Jeong Yak-yong's respect for adults was to practice the ideology of Confucianism, so this was the most important thing in the past Joseon Dynasty. In particular, it was considered important to think of adults first in practicing filial piety.This study focuses on the long-term care insurance system for the elderly, one of the welfare policy measures for the elderly. The significance of implementing the long-term care insurance system for the elderly is to benefit not only the elderly, but also all generations, including the middle-aged and children who were in charge of long-term care. To this end, the government should properly manage the factors of the social insurance financial crisis caused by the low birth rate and aging population.In addition, concerns about health insurance are high, so it is necessary to secure an appropriate level of government budget for health insurance financial management and minimize unnecessary non-benefit. In addition, it is necessary to induce appropriate medical use through connection with construction medical insurance.

한국 국민의료비 관리의 문제점 분석 : 건강보험, 산재보험, 자동차보험을 중심으로 (Problems of National Medical Expenses Management in Korea)

  • 본 연구는 국내외 문헌고찰을 통하여 건강보험과 산재보험, 자동차보험을 중심으로 선진국의 국민의료비 관리제도를 살펴보고, 우리나라 국민의료비 관리제도의 문제점을 파악하여 올바른 국민의료비 관리방안을 제안하는데 목적이 있다. 분석결과 첫째, 선진국의 경우 국민의료비를 단일기관에서 통합해 운영하면서, 동일한 수가와 급여기준을 가지고 운영하고 있었다. 즉, 의료비 발생원인에 관계없이 동일한 기관에서 동일한 기준으로 통합관리하고 있었다. 둘째, 우리나라의 경우 국민의료비를 보험제도별로 다른 기관에서 분리운영하면서, 서로 다른 의료비 심사·평가, 수가를 적용하고 있었다. 이로 인해 동일증상과 상병임에도 불구하고 관리주체에 따라 의료이용량의 차이가 매우 컸다. 이러한 문제를 근본적으로 해결하기 위해서는 의료비발생의 원인이 무엇이든지 하나의 기관에서 동일한 수가체계와 기준에 근거해 통합관리해야 한다.

  • In order to provide the National Dental Health Program with dasic data, and to implove the laborartory managent style, the author studied the operational situations of dental lads through questionaire. The results are as follows, 1. Unlicensured personnel should be replaced by the licensured(from present rate lf 49.2% licensured to more than80%) 2. Minimum wage guarantee system as well as well as oage raise should be established. 3. Monthly balancing accounts system and bepreciation reserve accounts system should be established for the reasonable management. 4. Manufacturing period should be elongated from presnt 2.8 days to 5-7 days for quality betterment of the processes. 5. Medical insurance system should be applied to the dental processes. 6. Manufacturing cost should be increased by 35%. 7. Retirement grants lr retirement annuity system, improvement of occupational environ-ment, and holidays incentives, and participation in medical insurance system should be adopted for the wellfare of the dental technicians. 8. Laboratory managers should operate the lads with pride and valuation. 9. Business Management course should be included in the curriculum of the dental labo-ratory school. 10. Good relationship between Korean Association of Dental Laboratory Technicians and local laboratories shorld be improved.

  • 한국(韓國)의 의료보험제도(醫療保險制度)는 일본제도(日本制度)를 거울삼아 서둘러 적용대상자(適用對象者)를 확대하여 1989년 전국민의료보험(全國民醫療保險)을 달성하였으나 제도(制度)와 효율적(效率的) 운영(運營)을 위한 장치가 결여되어 있고 직장(職場), 공교(公敎), 지역의보간(地域醫保間)에 형평문제(衡平問題) 제기되는 등 구조적인 과제를 안고 있다. 앞으로 제도(制度)의 개혁추진(改革推進) 노력으로 이 분야의 정책수립(政策樹立)과 집행(執行) 에 있어서 경직성을 줄이고 비용효과적(費用效果的)인 의료이용(醫療利用)을 도모해 나가지 못할 경우 낭비요인(浪費要因)이 점점 더 크게 불어나 국민의료비(國民醫療費)가 계속적으로 급속히 증대될 전망이다. 이와 같은 제도(制度)의 질적개선(質的改善)의 필요성에 비추어 최근 네덜란드가 시도하고 있는 제도개혁(制度改革)은 우리에게 여러가지 귀중한 교훈을 던져주고 있다고 판단된다. 네덜란드의 개혁안은 시장경쟁원리(市場競爭原理)를 폭넓게 수용하여 제도(制度)의 능률(能率)을 높이고 의료비(醫療費)를 절감하려는 취지를 담고 있다. 특히 의보가입자(醫保加入者)가 보험자(保險者)를 선택할 수 있고 또한 보험자(保險者)가 의료기관(醫療機關)을 선택할 수 있도록 관계(關係) 집단간(集團間) 상호경쟁(相互競爭)을 유도함으로써 효율(效率)을 증진시켜 나가겠다는 정책방향이 뚜렷히 제시되고 있어 우리의 주목을 끌고 있다.

입원환자 의료비 부담에 영향을 미치는 요인 (Factors Affecting the Burden of Medical Costs for Inpatients)

  • This study analyzes Korea health panel data (2008) (beta version 1.2) of Korea Institute for Health and Social Affairs, and National Health Insurance Corporation to figure out determinants of healthcare expenditure. In result of Multiple Logistic Analysis, in-patents felt burden on the medical expenditure were 70.0%. As to the patients' payment of medical expenditure, patients over 65 years old had 4.765 times higher than those under 14 years, disabled patients 2.778 than non-disabled patients, chronic patients 1.632 times than non-chronic patients, patients belonging to 12 million won ~ 46 million won and under 12 million won in family income had 1.680 times and 2.168 times respectively than patients with over 46 million won, patients in professional recuperation facility 1.546 times than patients in hospital, patients in private medical institutions 1.700 times than patients in national and public medical institutions, patients using upper grade rooms 1.701 times than patients in non-upper grade rooms. As a health care safety net mechanism to protect people from medical expenditure burden, there is the patients' payment ceiling in the National Health Insurance System. Thus, in order to facilitate the patient's payment ceiling, it is required that the level of ceiling is to be specified according to the income level, and self-payment items is to be included.

의료서비스 선택과 비급여 의료비 부담: 일본 혼합진료금지제도 고찰 (Choices of Medical Services and Burden of Health Care Costs: Japanese Prohibition of Mixed Treatment in Health Care)

  • With the introduction of national health insurance, the burden of health care costs decreased and choices of medical services widened. However, because of the rapid expansion of non-covered medical services by health insurance, financial security for health care expenditure is still low. This gives patients barriers to choose medical services especially for non-covered medical services, and it becomes narrower. Compared to Korea, Japan has high financial protection in health care utilization, but there exists a limitation using covered and non-covered medical services both together. This is called a prohibition of mixed treatment in health care. This study reviews the Japanese health care system that limits choosing medical services and the burden of health care costs. The prohibition of mixed treatment can alleviate the out-of-pocket burden in the non-benefit sector, but it can be found that it has a huge limitation in that it places restrictions on choices for both healthcare professionals and patients.

우리나라 청소년 음주의 사회경제적 비용 추계 (Estimating the Socioeconomic Costs of Alcohol Drinking Among Adolescents in Korea)

  • The purpose of this study was to estimate the socioeconomic costs resulting from alcohol drinking among adolescents as of 2006 from a societal perspective. Methods: The costs were classified into direct costs, indirect costs, and other costs. The direct costs consisted of direct medical costs and direct non-medical costs. The indirect costs were computed by future income losses from premature death, productivity losses from using medical services and reduction of productivity from drinking and hangover. The other costs consisted of property damage, public administrative expenses, and traffic accident compensation. Results: The socioeconomic costs of alcohol drinking among adolescents as of 2006 were estimated to be 387.5 billion won (0.05% of GDP). In the case of the former, the amount included 48.25% for reduction of productivity from drinking and hangover, 39.38% for future income losses from premature death, and 6.71% for hangover costs. Conclusions: The results showed that the socioeconomic costs of alcohol drinking among adolescents in Korea were a serious as compared with that of the United States. Therefore, the active interventions such as a surveillance system and a prevention program to control adolescents drinking by government and preventive medicine specialist are needed.

  • A review team has been set up in Yong Nahm central Bureau Dong Bang Life Insurance Company to study on yearly treands and Mortalities of life insureds according to the causes of deaths. The subjects included in this study were the Life insureds in these area offices and one Who died within 2 years after the admission during 17 months period between 1984-4-1 and 1985-8-31. In cases of the insurance for educational fund, the subjects were the applicants who pay the dues of insurances instead of the insured youngsters. 1) The most frequent cause of death was malignant newgrowth, the next was heart disease, the third was central nervous vascular system diseases, and the fourth was Hepatitis and Liver cirrhosis. 2) The previous Leading cause of deaths were infectious diseases, Parasitic disease or Respiratory diseases, Such as Pneumonia bronchitis, Tuberculosis, Since remarkable prolongation of the longevity of Korean population in recent years, this has been showed marked changes after 1980, this phenomenon would be in accordance with Bogue's theory. 3) The authors hoped to extend this study to get more clear views, because it covered only a short period and very limitted areas of subjects to make a difinite conclusion on this time.

  • 국민건강보험공단은 의사가 요양급여기준을 위반하여 처방을 함으로써 국민건강보험공단이 약국에 약제비를 지급하게 된 것은 의사가 국민건강보험공단에 대해 민사상 불법행위를 한 것이라고 주장하며 손해배상청구를 하였다. 대법원이 이를 인정하는 판결이 잇따르자, 국민건강보험공단은 의사들이 의료법을 위반하여 진료를 하여 약국에 지급하게 된 약제비나 해당 의료기관에 지급하게 된 요양급여비용에 대해서도 모두 국민건강보험공단에 대한 민법상 불법행위라고 주장하며 손해배상을 청구하였으며, 대법원은 이를 모두 인정하였다. 그러나 우리나라 국민건강보험제도에서 보험급여의 주체는 국민건강보험공단이며, 환자가 의료기관에 진료를 요청하여 의료기관이 행하는 요양급여 역시 보험급여이므로 이러한 요양급여를 행하는 주체 역시 국민건강보험공단이다. 그리고 국민건강보험법은 의사의 의료법위반행위를 규제하기 위해 만든 법이 아니므로 의사의 의료법 위반행위를 국민건강보험공단에 대한 민법상 불법행위책임에서의 위법성으로 포섭할 수 없다. 그렇다면 환자가 보건복지부장관으로부터 부여 받은 면허를 가진 의사에 의해 요양급여기준에 맞는 진료를 받은 경우에는 국민건강보험공단은 민법상 환자에게 요양급여를 해 줄 의무를 면하는 이득을 얻었으므로 설사 해당 의사가 진료를 하는 과정에 의료법을 위반한 행위를 하였더라도 국민건강보험공단으로서는 손해가 없다. 대법원이 의료법위반으로 진료를 한 행위를 모두 국민건강보험공단에 대한 민법상 불법행위책임으로 인정하는 것은 국민건강보험공단이 보험급여의 주체로서 보험급여를 해 줄 의무를 면하였다는 이득은 고려하지 않고, 오로지 의사에게 요양급여비용을 지급하였다는 측면만을 고려한 것으로서 민법 제750조의 법리에 어긋난다. 의사가 의료법을 위반하였다면 의료법에 따라 제재를 받을 일이지, 국민건강보험법으로 규율할 일이 아니며, 대법원이 위와 같이 판결한 것은 국민건강보험법의 법리와 민법의 법리를 혼동한 탓으로 생각된다.

한방의료의 이용행태 및 이용결정요인 분석 - 일부 대도시 지역주민을 중심으로 - (Utilization Patterns and Determinants of Oriental Medical Services : Focused on the Residents of Taegu City)

  • This study was conducted to examine utilization patterns and determinants of oriental medical services. Data were collected from 545 residents in Taegu city The results of this study are summarized as follows 1) 37.8% of subjects used oriental medical services in the past year. Especially, the female, the ages of 50 and 60 over, the single. low-educated. high-income class, white-collar class, medical insured tended to use more oriental medical services than another groups. 2) 46.7% of users of oriental medical services reported that number of visits in the past year was 2 - 5 times 37 1% of them was 1 times, and 4.5% over 10 times. 3) According to the reasons to choose the oriental medical facilities, most was 'on their own judgement'(48.8%) and 'by the advice of relatives and friends'(42.0%) Regarding to the objectives of using oriental medicine, 68.3% was 'treatment', 31.7% 'health counselling and promotion'. And among diseases of users, diseases of musculo-skeletal system was the highest(54.5%). 4) 57.9% of oriental medical services users had experience of utilizing western medicine on the same diseases. Among peoples with experiencing western medicine on the same diseases. 54.4% received oriental medical services 'in addition to western medicine', 45.6%'in place of western medicine 'And 41.2% of using both services reported that they had difficulty in deciding to choose the type of services -oriental medical services or western medicine-for their diseases. 37.3% of them answered that 'providing relevant information' was the most desirable measure to solve this problem, 27.3% 'establishment of effective referral system between oriental and western medical facilities '23.6% 'cooperative medical treatment systems in the same facilities', 11.8%'integration of oriental medicine into western medicine 5) According to the satisfaction level with each items of oriental medical services, the respondents had positive views on efficacy, kindness, and side-effects. They, however, had negative view on the cost of oriental medical services. 6) In regarding to the priority of improvement of oriental medical system,'expansion of insurance benefit package 'ranked first. 'expansion of insurance benefit Package 'ranked second, 'improvement of scientific methods and diagnostic technique 'third, and 'safety of herbal medicine' fourth in order. 7) The significant factors influencing the utilization of oriental medical services were kindness of oriental medical practitioners, efficacy , travel time, age To be brief, utilization rate of oriental medical services in urban area generally tends to be high. There, however, have been various barriers to limit oriental medical services, such as incomplete benefit package of oriental medical insurance and lack of coordination and referral system between oriental and western medical services, lack of scientific diagnostic procedures, high price etc . For the development of oriental medical services, Much attention to remove these limiting factors should be placed. In addition, kindness of oriental medical practitioners , which is expected to be more important factor in the consumer - focused health care environment than ever, should be kept high consistently. Since this study was conducted for specific residents of an urban city. further research including more sampling in different urban areas should be required to generalize the results of the study.
