The Development of Microwave Tissue Coagulator (마이크로파를 이용한 수술기의 개발)

  • In surgery the hemostatic control is very important for the parenchymatous organs. These organs consist of the fragile and the blood content tissues such as liver, spleen and kidney, etc.. One of the control methods to solve this problem is to insert the mono polar typed needle electrode, which gathers the thermal effect of microwave, directly into tissues so as to coagulate and stop the hemorrhage. This method has same advantages: First, the range of the heat energy is limited. Second, the coagulation, the hemostatic characteristic, and stability are excellent. Third, more convenient operation is possible. This paper is aimed to manufacture the microwave tissue coagulation system and to suggest the new direction for development, hereafter.

A Study on the Space Design for Nosocomial Infection Control in Intensive Care Unit (병원 감염관리를 위한 중환자부 공간계획에 관한 연구)

  • Purpose: Recently an experience in the MERS crisis focused on the importance of infection control in hospitals. According to Korean National healthcare-associated Infection Surveillance System (KONIS) of the KSICP, a great number of 498 people, 841 people, and 1021 people were infected by pneumonia, urinary tract infection, and bacteremia respectively from 94 hospital ICUs during the year of 2014. Therefore, the purpose of this study is to investigate the configuration and design guidelines for the ICU rooms to minimize the nosocominal infections. Methods: Based on the several infection control guidelines and revised Medical Law, consequent analyses which classified the planning and operational behavior in the ICUs of seven hospitals, were performed to reduce the cross-infection. Results: The results of this study are offering a space, configuration and design guidelines for effective infection control in the intensive care units through the unit-bed area, the bed-to-bed distance, the isolation room, etc. Implications: It is expected that this study propose the direction of architectural planning and guideline for the ICU room in order to realize the intension of revised Medical Law.

A Study on the Evacuation Procedure Analysis Model of General Hospital Considering Patients Types (환자의 유형을 고려한 종합 병원의 피난 절차 분석 모델에 관한 연구)

  • Purpose: This study aimed to present an analysis model evaluating evacuation performance considering patient types and procedural evacuation in the medical facility. The user group of the medical facility, including users challenged in evacuation behavior, entails the risk of many casualties. Therefore, it is necessary to plan an evacuation procedure that considers the evacuation characteristics of users. Methods: Through the review of precedent studies, the evacuation procedure of the medical facility, the classification of patient types, and the evacuation procedure was set as conditions and variables for the analysis. The result caused by a variety of conditions and variables were explored. Results: 1) The total evacuation completion time and congestion time were shortened at the procedural evacuation. Moreover, it derived many users from evacuating at the initial phase. 2) The proposed model can provide a basis for proposing a space planning direction that considers the possibility of not carrying out the evacuation plan. 3) It supports safe evacuation by identifying variables that reduce overcrowding by comparing the congestion time of overcrowded spaces. 4) The analysis model can identify the overcrowded space through the evacuation route and suggest the basis for architectural improvements that reduce overcrowding. Implications: The study results can be used to analyze the performance of evacuation procedures and support the establishment of evacuation procedures and building plans for safe evacuation for medical facilities.

A Study on the Change of General Hospitals Size in Seoul during 15 years - Focused on General Hospitals bed and Gross Area in Seoul since 2005 (대도시 종합병원의 15년간 규모 변화 - 2005년 이후 서울특별시 소재 종합병원의 병상수와 연면적 변화)

  • Purpose: As the medical environment changes, the size of medical facilities continues to change. Of course, it is not easy to analyze the correlation between changes in the medical environment and changes in facility size in a rigorous scientific method. However, the results of investigating and analyzing changes in the size that occurred for any cause be able to serve as important references for future medical facility architectural plans. Methods: This study surveyed 68 general hospitals in Seoul for 15 years from 2005 to 2020. The change in the number of beds was investigated with data from the Ministry and Welfare. The gross area change was confirmed by analyzing the of all hospitals. Results & Implications: As a result of the survey, general hospitals in Seoul show a common phenomenon in which all hospitals except one case have expanded their size in the direction of increasing the gross area per bed over the past 15 years. However, in the method, a relatively small hospital with less than 500 beds mainly chose to reduce the number of beds. Large hospitals increased both the number of beds and the gross area, however showed a significant increase in the gross area.

Community Integration Study through Rehabilitation Medical Support for People with Disabilities

  • Purpose: This study is to propose the establishment and direction of a public health-medical cooperation system for rehabilitation medical services for people with physical and brain disabilities in Gangneung, Korea. Research design, data and methodology: The study focused on 30 individuals with these disabilities registered. Data was collected from December 20, 2021, to December 31, 2021, through structured surveys administered by researchers visiting disability-related facilities, utilizing convenience and random sampling methods. Descriptive statistics and cross-analysis were applied for analysis. Results: Specifically, among respondents with physical disabilities, a total of 20 needs were identified, with 'Visiting health services' (25.0%) and 'Oral health services' (20.0%) ranking highest. The survey results regarding visit-based rehabilitation services for disability support showed a high demand, emphasizing the necessity of service provision tailored to the needs of recipients, focusing on disability prevention, health management, and motor function recovery, rather than solely medical or therapeutic concepts. Conclusions: Gangwon National University Hospital, as the regional referral hospital in Gangwon, should collaborate with Gangwon Province Rehabilitation Hospital to provide prompt acute rehabilitation services. Moreover, cooperation and collaboration with Gangneung Asan Hospital, the tertiary hospital in the region, are essential to ensure continued acute and recovery phase rehabilitation therapy for a certain period in the Gangneung area.

Dosimetric Analysis on the Effect of Target Motion in the Delivery of Conventional IMRT, RapidArc and Tomotherapy

  • One of the methods to consider the effect of respiratory motion of a tumor target in radiotherapy is to establish a treatment plan with the internal target volume (ITV) created based on an accurate analysis of the target motion displacement. When this method is applied to intensity modulated radiotherapy (IMRT), it is expected to yield a different treatment dose distribution under the motion condition according to the IMRT method. In this study, we prepared ITV-based IMRT plans with conventional IMRT using fixed gantry angle beams, RapidArc using volumetric modulated arc therapy, and tomotherapy using helical therapy. Then, the variation in dose distribution caused by the target motion was analyzed by the dose measurement in the actual motion condition. A delivery quality assurance plan was prepared for the established IMRT plan and the dose distribution in the actual motion condition was measured and analyzed using a two-dimensional diode detector placed on a moving phantom capable of simulating breathing movements. The dose measurement was performed considering only a uniform target shape and motion in the superior-inferior (SI) direction. In this condition, it was confirmed that the error of the dose distribution due to the target motion is minimum in tomotherapy. This is thought to be due to the characteristic of tomotherapy that treats the target sequentially by dividing it into several slices. When the target shape is uniform and the main target motion direction is SI, it is considered that tomotherapy for the ITV-based IMRT method has a characteristic which can reduce the dose difference compared with the plan dose under the target motion condition.

A Case Study and Implications on Improvements in Environmental Design of Outpatient Department and Health Examination Center in Mental Health Hospital - For Health Promotion Department of National Mental Health Center (정신의료시설 내 외래부 및 건강검진센터의 환경디자인 개선 사례 및 시사점 연구 - 국립정신건강센터 건강증진과를 대상으로)

  • Purpose: This study is a case study in which the space was improved by applying the design direction derived through the theoretical basis and service design process to the outpatient department and health examination center in mental health facilities used by various stakeholders. And it aims to present implications through this. Methods: The research method is based on the analysis of the service design process with a focus on literature review. Results: As a result of deriving the design direction, it was organized into 1) improvement of spatial arrangement, 2) improvement of wayfinding system, and 3) creation of comfortable environment. The design improvement plan suggested division of areas, change of nurse station location, creation of a pleasant waiting space for the outpatient department, reinforcement of access, improvement of room relocation and flow, and increased comfort of common spaces for the health examination center. Implications: First, it is necessary to expand research and application of spatial planning and environmental design reflecting the characteristics of patients and environments of mental health institutions. Second, in the medical environment, the divided territoriality should be reviewed for various stakeholders as well as the coexistence. Third, it is necessary to promote medical service and environmental improvement through the service design process.

Assessment of Nutrient Intake and Dietary Quality of Korean Adults in Metabolic Syndrome Patients According to Taking Medical Care: Based on the 2017 Korea National Health and Nutrition Examination Survey (한국 성인의 대사증후군 여부와 치료 유무에 따른 식생활 평가 : 2017년 국민건강영양조사 자료 이용)

  • Objectives: This study established a practical direction for the prevention and management of metabolic syndrome by evaluating the health status, nutrition intake level, and diet quality according to metabolic syndrome and related drug treatment in Korean adults. Methods: The data from the 2017 KNHANES (Korea National Health and Nutrition Examination Survey) was analyzed. The analysis included 2,978 adults, classified into the normal, metabolic syndrome (MetS), metabolic syndrome with medicines (MetS-M), and without medicines (MetS-noM) groups. The nutrient intake, NAR (nutrient adequacy ratio), INQ (index of nutritional quality), and DDS (dietary diversity score) were analyzed. Results: The mean BMI was significantly higher in the MetS group than in the normal group for all subjects. Subjects of the MetS group tended to consume less energy and major nutrients, while males aged 50 ~ 64 and all females showed less intake of nutrients in the MetS-M group. The energy intake ratio was within 55 ~ 65 : 7~ 20 : 15 ~ 30 of KDRI (Korean Dietary Recommended Intake), but the carbohydrate energy ratio of all subjects aged 50 to 64 was over 65%. The NAR of the major nutrients was lower in the MetS-M group, the average INQ was around 0.8, especially the INQ of calcium and vitamin A was less than 1, and the total DDS score was less than 4 points. Conclusions: This study confirmed that the nutrient intake and diet quality differed among subjects diagnosed with metabolic syndrome and managed with medical care. The intakes of energy and many nutrients, the quality of diets, and the diversity of food groups in the MetS-M group were lower than in the normal group. Therefore, these will be an important basis for establishing a specific direction of diet education for preventing and managing metabolic syndrome according to gender, age, metabolic syndrome, and drug treatment.

Study on the development of convergence subjects for departments of emergency medical technology in preparation for the fourth industry revolution (4차 산업혁명 시대 응급구조(학)과 융복합 교과목 개발을 위한 연구)

  • Purpose: This study aimed to examine the development and improvement direction of university education according to job type and investigate the types of knowledge required for employment after graduation for departments of emergency medical technology in preparation for the era of the fourth industrial revolution in a converged society. Methods: From June 3 to June 10, 2019, data were collected through structured questionnaires from 90 paramedics working in firefighting, hospitals, different industries, and emergency patient transportation. Data were analyzed using the software SPSS version 18.0. Results: For employment after graduation in departments of emergency medical technology, character and sincerity education should be strengthened. To prepare for the fourth industrial revolution, educational innovations such as improving the field practice system and devising a convergence curriculum are needed. Preparation for the fourth industrial revolution should be accomplished through the development of convergence subjects such as forensic science, information and communications technology, and safety engineering. Ninety percent of the study respondents indicated their need for convergence subjects. Conclusion: The demands of future society should be responded to flexibly by holding and conducting convergence subjects. Furthermore, preparations should be made in anticipation of a shortage of 120,000 college freshmen for the 2024-2025 school year by increasing the competitiveness of departments of emergency medical technology.

Development of Tomotherapy couch device capable of yaw-directional correction (Yaw방향의 보정이 가능한 Tomotherapy couch device의 개발)

  • Objectives : A self-made "Tomotherapy couch device" capable of correcting the Yaw direction was fabricated and evaluated for its usefulness. Materials and Methods : "Tomotherapy couch device" capable of correcting the Yaw direction is made of rigid fibreboard with a flexural strength of $200kg/cm^2$. CBCT Image from Novalis Tx and Iso-Align Phantom from MED-TEC were used to evaluate the physical accuracy. The treatment plan was designed using Accuray $Precision^{TM}$ and In House Head and Phantom. Accuray $PrecisionART^{TM}$ and $Precision^{TM}$ was used to evaluate dose. Results : Evaluation results, the self-fabricated device accurately corrected the setup error, Target dose was within 95 %~107 % of all. In order to directly evaluate the OAR dose according to the Yaw change, the absolute dose was measured. As a result, when the error in the Yaw direction was $3^{\circ}$, the specific OAR showed a maximum difference of 18.4 %. Conclusion : "Tomotherapy couch device" capable of correcting the Yaw direction can be manufactured at a lower cost compared to the effect, and it can prevent the patient's MVCT image dose for re-imaging. Accurate radiation therapy without errors can be performed.
