대기관리권역법 시행효과 평가를 위한 전북지역 초미세먼지 현황 및 건강영향 사전연구 (A Preliminary Study on the Current Status and Health Effects of PM2.5 for Evaluating the Act on Air Quality Management Regions in Jeollabuk-do Province)

  • Background: In order to evaluate the national effort to improve air quality through the Act on Air Quality Management Regions, an investigation of the current status of air quality in respective regions and the health impact of air pollution should be undertaken. Objectives: This study is a preliminary study on the status of PM2.5 in the central region of South Korea and its health effects. It can be applied to the evaluation of the effectiveness of the Act on Air Quality Management Regions. Methods: Time series variation of PM2.5 concentrations in the air quality management region (hereafter referred to as AQMR) and in the non-AQMR area in the central region of South Korea were created. Odds ratios (OR) were calculated based on the survey results of whether the residents of Jeollabuk-do were feeling anxiety caused by air pollution by sex, region, and regional characteristics. Relative risk (RR) values of lung cancer deaths in each city in the AQMR for the central region of South Korea were calculated. Results: During the period from Dec. 2018 to Dec. 2021, the average concentration of PM2.5 was higher in the AQMR at 26.0 ㎍/m3 compared to 19.5 ㎍/m3 in the non-AQMR. The odds ratio (OR) calculated using data from the Jeonbuk Research Institute (2020) suggested that women were more uncomfortable or anxious about their lives due to air pollutants than were men. The OR evaluation results also showed that in terms of PM2.5 sensitivity, residents in the non-AQMR were relatively more sensitive than those in the AQMR. The relative risk (RR) was calculated to evaluate the relationship between health hazards (PM2.5) and diseases (lung cancer). As a result, for super-aged people over 80 years old, compared to the non-AQMR (Muju-gun and Jangsu-gun), the residents in the AQMR (Jeonju, Gunsan, and Iksan) have about twice the relative risk of lung cancer. Conclusions: It is necessary to make efforts to improve residents' health and extend their healthy lives through efforts to reduce air pollutants such as PM2.5 in areas specially selected as an AQMR.

보건소직원의 조직에 대한 인식과 동기부여요인 및 직무만족요인 (Recognition Level of Organization, Motivation and Job Satisfaction Factors of the Staff of Health Centers)

  • 보건소 조직구조에 대한 인식도 점수는 5 점척도 기준으로 재량권 필요성에 대한 인식도가 3.55 점으로 가장 높았고 보건소 조직구조에 대한 일반적 인식이 3.06점, 업무분장의 적합성 3.05점, 인력ㆍ예산의 적정성이 2.93 점, 의사결정권 소재에 대한 인식이 2.77 점 순이었다. 보건소 조직구조에 대한 직원들의 일반적 인식도 점수는 중소도시에서, 50 대 이상에서, 고졸 이하에서, 6급이상에서, 공무원 근무경력이 20년이상에서, 현부서 근무기간이 2년이하에서, 월평균 보수가 181만원이상에서 각각 타 군보다 높았으며 이들 변수들은 유의한 관련성이 있었다 그리고 의사결정권 소재에 대한 인식도 점수는 대도시에서, 남자에서, 기혼자에서, 6급이상자에서, 보건ㆍ행정직에서, 월평균 보수가 131-180 만원에서 각각 타 군보다 높았으며 이들 변수들은 유의한 관련성이 있었다. 재량권 필요성에 대한 인식도 점수는 20 대에서, 미혼자에서, 대졸이상자에서, 간호직에서, 공무원 근무경력이 5년이하자에서, 현부서 근무기간이 2년 이하자에서, 월평균 보수가 80 만원 이하자에서 각각 타 군보다 높았다. 인력ㆍ예산 적정성에 대한 인식도 점수는 여자에서,30 대에서, 기혼자에서,8 급에서, 보건ㆍ행정직에서, 현부서 근무기간 2-4년인자에서 각각 타 군보다 높았다. 그리고 업무분장의 적합성에 대한 인식도 점수는 중소도시에서, 기혼자에서, 의료기술직에서, 공무원 근무경력이 20 년이상자에서, 현부서 근무기간이 4년이하자에서 각각 타 군보다 높았으며 이틀 변수들은 유의한 관련성이 있었다, 보건소 직원들의 보건소 조직관리에 대한 인식도 조사에서 의사결정시 의견반응에 관한 인식도가 2.92 점으로 가장 높았으며, 목표량 설정방법의 합리성에 관한 인식이 2.88점, 보건소 인사관리에 대한 인식이 2.63점이었다. 보건소 인사관리에 대한 인식도 점수는 중소도시에서, 40대에서, 6급 이상자에서, 의료기술직에서, 공무원 근무경력이 20년 이상자에서, 현부서 근무기간이 2년 이하자에서, 월평균 보수가 181만원 이상자에서 각각 타 군보다 높았으며 이들 변수들은 유의한 관련성이 있었다. 의사결정시 의견반응에 관한 인식도 점수는 중소도시에서, 여자에서, 8급에서, 보건ㆍ행정직에서, 현부서 근무기간 2 년 이하자에서 각각 타 군보다 높았으며, 목표량 설정방법의 합리성에 관한 인식도 점수는 50대 이상에서, 고졸 이하자에서,6 급 이상 자에서, 의무직에서, 공무원 근무경력 15-20 년인 자에서 각각 타 군보다 높았으며 이들 변수들은 유의한 관련성이 있었다. 직무만족도에 유의하게 영향을 미치는 요인은 성, 교육정도, 업무분장의 적합성, 목표량 설정방법의 합리성에 관한 인식, 동기요인, 위생요인 이었으며, 조직문화에 유의하게 영향을 미치는 요인은 연령, 공무원 근무경력, 현부서 근무기간, 보건소 인력ㆍ예산의 적정성에 대한 인식, 업무분장의 적합성, 의사결정시 의견반응에 관한 인식, 목표량 설정방법의 합리성에 관한 인식이었다.

황연해독탕(黃連解毒湯)과 건강부자탕(乾薑附子湯)이 LPS유도에 의한 마우스 혈중 IL-6와 $TNF-{\alpha}$ 변화에 미치는 영향 (Effects of Hwangryunhaedok-Tang and Geongangbuja-Tang on the Change of Interleukin-6 and $TNF-{\alpha}$ Level Induced by LPS I.C.V. Injection in Mice)

  • Objective : This study was conducted to investigate the effects of Hwangryunhaedok-Tang and Geongangbuja-Tang on the change of interleukin-6 (IL-6) and tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) level induced by LPS I.C.V. injection in mice. Method : We devided group into 6 mice and 6 mice were assingned to each group. In the normal group only saline was administered intragastrically, and in the control group LPS was injected intracerebroventricularly 1 hr after intragastric administration of saline. In the experiment groups Hwangryunhaedok-Tang(0.5g/kg, 1.0g/kg, 3.0g/kg) was administered intragastrically to mice 1 hr prior to LPS(100mg/mouse) I.C.V. injection.. Also Geongangbuja-Tang (0.5g/kg, 1.0g/kg, 3.0g/kg) was administered intragastrically to mice 1 hr prior to LPS(100mg/mouse) I.C.V. injection. To measure the plasma IL-6 and $TNF-{\alpha}$ level of mice, their blood samples were collected from retro-orbital plexus, immediately centrifuged at $4^{\circ}C$, and plasma was removed and stored frozen at $-83^{\circ}C$ for later determination of IL-6 and $TNF-{\alpha}$. The level of IL-6 and $TNF-{\alpha}$ production was measured by enzyme-linked immunosorbent assay in the plasma. Result : Regarding IL-6 level, The 0.5g/kg and the 1g/kg groups of Geongangbuja-Tang decreased IL-6 level. Especially the 3g/kg control group decreased IL-6 level significantly than the normal group(p<0.01). Regarding $TNF-{\alpha}$ level, the 3g/kg group of Geongangbuja-Tang decreased it significantly(p<0.05). Conclusion : These data revealed that Hwangryunhaedok-Tang might not have the anti imflammatory effect and Geongangbuja-Tang(3g/kg)might have the anti imflammatory effect by reducing the plasma IL-6 and $TNF-{\alpha}$ level in mice LPS Injection.EIM (Eighteen Incompatible Medicaments) is an important component in Oriental pharmacology and is directly related to clinical prescriptions. Medical practitioners argued that the definite cause and meaning of EIM was ambiguous and therefore debated the issue of clinical application of the EIM. This study conducted an in-depth literary research on the origin, meaning and contents of EIM with the purpose to contribute in its efforts to be used clinically. Even after thousands of years have past since establishment of Oriental medicine, EIM is still tabooed and was an obstacle that hindered ideologies. Modern herbal medicine texts claim that the use of EIM can reduce treatment effects and promote poisoning and side effects. However, since long ago, there has been medical practitioners who reject this as false. Recently, poisoning caused by EIM has been claimed to be from the toxicity of the drug itself, rather than the result of interaction between the drugs, and therefore they suggest that EIM is not a forbidden domain. In addition, EIM showed a difference in number depending on the era. However, this can be understood not as a definite number, but instead as a warning to be careful during combination of drugs for use as clinical medicine. Historically, there were very few cases in which EIM was used for clinical tests and thus, the clinical value is not, while others applied EIM directly to their bodies, which showed signs for the usefulness and potential of EIM for us. A more concrete and in-depth study must be made on EIM.

일 의료원의 통합 고충처리센터 접수 내용과 이에 대한 해결방안 분석 (Analysis of the Issues received by Quality Improvement Department and their Management in a Medical Center)

  • Background : A continuous healthcare quality improvement is needed to provide high quality healthcare service as well as to maintain trust in terms of satisfying the needs of the patients. Recently it also became an essential issue. in hospital management, recognized for it's competitive potentiality among healthcare organization groups. This study was conducted to analyze patient complaints and issues received by the Quality Improvement Department. Its purpose is to improve healthcare qualities within the hospital, as well as establish policies and appropriate strategies in hospital management. Method : From July 1st to September 30th of the year 1999, we analyzed all complaints and issues made by various patients and their families, which were received through 24 hour phone consultation, numerous suggestion boxes, letters and E-mails, The issues were classified into 16 different categories based on a Patient Satisfaction Assessment Tool. All data were segregated according to the departmental frequencies and their contents. To come up with for environmental and patient satisfaction improvement, all complaints or issues were communicated with hospital administrators, medical and nursing staff and employees. Comprehensive customer satisfaction activities including improving phone etiquette were discussed in Customer Satisfaction Team, CQI Team and each Department. All opportunities for improvement were implemented. Feedback actions were discussed. Results : A total of 317 cases were collected. Issues regarding parking and other accommodation facilities were most common complaints that were 14.5% of total. Issues regarding admission rooms (10.7%), admission procedures (10.7%), waiting room environment (8.8%), nurses and nurse assistants (7.6%), physicians (6.6%) and others (23%) followed. Thirteen of 45 departments received more than 8 complaints. The Nursing Department had the most complaint, receiving 9.8% of total complaints. Complaints regarding the Nursing Department were predominantly related to the environment of patient rooms. The Department of Psychiatry for phone etiquette (4.7%), Department of Otolaryngology for the nursing staff's attitude and phone etiquette (4.4%), and the Admission Department followed. As a part of efforts to improve patient satisfaction, a new parking structure was built and reallocation of the parking space was done. Renovation of other accommodation facilities were carried out by hospital administration, Monthly phone call and answering attitude survey was done by QI Department. Based on this survey we made a phone etiquette manual and distributed throughout the hospital. Compare to the last year, Patient Satisfaction Index measured by Korea Productivity Center using National Customer Satisfaction Index was improved 7 points. According to our organization's own study, we confirmed the phone etiquette was improved 11% than last year. Conclusions : Issues related to parking and other accommodation facilities ranked first followed by complaints made regarding the patient care area, the admission and cashier process, and nurses' and doctors' attitude. The Nursing and Psychiatry Departments need improvement regarding phone etiquette. Results were shared and played a vital role in policymaking and strategic planning of the hospital. It is imperative that we keep our database updated by listening to and solving the needs of each patient. The CQI activities can be achieved only by full commitment of the hospital top management supported by related personal.

고페닐알라닌혈증 환아들에게 BH4 부하검사 이전에 시행되는 페닐알라닌 부하의 효과에 대한 고찰 (A Study on Effectiveness of Conventional Phenylalanine Loading before BH4 Loading Test in Children with Hyperphenylalaninemia)

  • 목적: $BH_4$ 부하검사는 PKU와 $BH_4$ 결핍증을 감별하고, PKU 환자 중 $BH_4$ 반응성을 확인할 수 있는 중요한 검사이다. 일반적으로 알려진 프로토콜로 페닐알라닌 부하와 $BH_4$ 부하검사는 환자의 편의 및 입원기간을 단축시킬 수 있는 유용한 검사방법이다. 하지만 페닐알라닌 부하의 용량이나 부하 후 $BH_4$ 투여시간이 임상적으로 적절한지에 대하여는 충분한 연구가 이루어지지 않았다. 본 연구는 순천향대학교 소아청소년과 유전대사 클리닉에 내원한 환자 중 100 mg/kg의 페닐알라닌을 복용 후 $BH_4$ 부하검사를 시행한 6명 환자들의 의무기록을 분석하여 기존의 페닐알라닌 부하 방법의 효용성을 연구하였다. 방법: 2004년 7월부터 2015년 7월까지 순천향대학교 유전대사 클리닉에서 추적관찰하고 있는 총 207명의 환자 중 100 mg/kg의 페닐알라닌을 복용 후 $BH_4$ 부하검사를 시행한 6명 환자들을 대상으로 의무기록을 분석하였다. 이들은 모두 제한식사를 시행하고 있었으며 100 mg/kg의 페닐알라닌을 복용 후 3시간 뒤 20 mg/kg의 $BH_4$를 부하하였다. $BH_4$를 부하한 시점을 Basal로 하여 1, 2, 4, 6, 8, 12, 24시간에 혈중 페닐알라닌 수치를 연속적으로 측정하였다. 결과: 6명의 최고 페닐알라닌 농도의 평균은 $20.0{\pm}11.70mg/dL$ 이었다. 1명은 페닐알라닌 복용 후 7시간 째, 다른 1명은 5시간 째, 나머지 3명은 4시간째에 최고농도에 도달하였다. 3시간 만에 최고 농도에 도달한 환자는 1명이었다. 페닐알라닌 부하 후 페닐알라닌 수치가 $400{\mu}mol/L$ 이상으로 상승한 환자는 6명 중 4명(66%)이었다. 나머지 2명의 환자는 페닐알라닌 수치가 각각 6.1 mg/dL ($366{\mu}mol/L$), 5 mg/dL ($300{\mu}mol/L$)이었다. 결론: 본 연구에서 기존에 알려진 바와 같이 100 mg/kg의 페닐알라닌 복용 후에 3시간째에 최고 농도에 도달한 환자들은 6명 중 1명(16%)이고 $400{\mu}mol/L$ 이상 페닐알라닌 수치가 올라간 환자는 4명(66%)이었다. 일반적으로 알려진 검사 방법으로 페닐알라닌 농도가 $400{\mu}mol/L$ 이상으로 상승하지 않은 환자가 있었고 3시간 보다 더 늦게 최고 농도에 도달하는 단점이 있었다. 따라서 페닐알라닌은 200 mg/kg이상 증량하여 복용하고 적어도 페닐알라닌 복용 4-6시간 후에 $BH_4$ loading을 하는 것이 적절한 진단에 도움이 될 것으로 사료된다.

인체대장암세포주 SW480에 대한 재배 와송의 항암효과 연구 (Anti-cancer Effects of Cultivated Orostachys japonicus on Human Colon Cancer Cell Line SW480)

  • 와송(둥근바위솔, Orostachys japonicus, OJ)은 면역조절, 항노화, 항산화, 독성제거 등의 치료적 효과를 가진 약용식물로 알려져 있는데, 본 연구에서는 인체대장암세포에 대한 재배 와송의 항암효과를 규명하고자 하였다. 인체대장암세포주 SW480에 와송열수추출물을 72시간 동안 처리한 결과, 대장암세포의 생존율은 농도-의존적으로 유의하게 감소하였다. 또한, 와송은 SW480 세포의 증식과 이주를 억제하는 것으로 나타났는데, 대조군의 스크래치 gap은 24시간부터 유의하게 감소하는 반면, 와송열수추출물을 처리한 실험군 I과 II의 스크래치 gap은 48시간부터 감소하기 시작하였다. 특히 실험군 I의 스크래치 gap은 72시간까지 유의하게 유지되었다. 와송이 생체 내에서 종양의 형성에 어떠한 영향을 미치는지 알아보기 위하여 대장암세포 주사 전, 31일 동안 수컷 C57BL/6 마우스(4주령)에게 와송열수추출물을 경구로 자유 섭취하게 하였다. 이후 SW480 세포($1{\times}10^7cells/100{\mu}l$)를 피하로 주입한 후 7일, 14일 그리고 28일에서 종양의 형성을 관찰하였고, 각 실험동물을 희생하여 종양의 무게와 부피($mm^3$)를 측정하여 비교 분석하였다. 와송열수추출물을 섭취하는 동안 실험군 I, II의 체중은 대조군에 비해 지속적으로 유의하게 증가하였다. SW480 세포 주입 이후 모든 실험군의 체중은 감소하였으나, 세포 주입 후 14일부터 와송 섭취 실험군의 체중은 유의하게 증가하는 것으로 나타났다. 대장암세포 주입 후 7일과 14일에서 와송을 섭취한 실험군의 종양 무게와 부피는 대조군보다 높았으나, 28일에서는 대조군보다 낮게 나타났고, 특히 실험군 II에서 종양 무게와 부피는 유의하게 감소하는 것으로 확인되었다. 이상의 결과를 통해 와송이 인체 대장암세포의 성장, 증식 및 이주를 억제하며, 생체 내 종양형성(tumorigenesis)을 예방적으로 억제함을 알 수 있었다. 따라서, 재배 와송은 천연 항암제 소재로서 활용될 가능성을 가지는 것으로 보이며, 이에 대한 심층적인 연구가 필요할 것으로 사료된다.

질적 간호제공을 위한 간호단위 시범 운영 효과에 관한 임상적 연구 (A Clinical Study for Promoting Quality Nusing Care in a University Hospital)

  • The purpose of this study was to develop a new nursing unit which can meet changing health care needs, enhance patients' satisfaction and nurses' job satisfaction, and finally guarantee quality nursing care with present manpower. For this, one medical unit was selected as a unit for quality care. And one medical unit which is similar in staffing and patients' characteristics was selected as a control unit. To assess present problems and identify the remedies to the problems a hospital-wide survey and a workshop were performed. According to the survey results, educational programs and improvement of the facilities and equipment supply system, managereal support for interdepartmental cooperation and intensification of bed-side nursing care were adopted as main principles for operating model unit, This model unit was operated for 3 months from Sep. 1, 1992 to Nov. 30, 1992. To evaluate the effectiveness of the model unit, derect/indirect nursing care hours, patients' satisfaction to nursing care, nurses' job satisfaction, and quality care index were measured. Direct/indirect nursing care hours were compared with that of the control unit, and patients' and nurses' satisfaction and quality care index were measured before and after operating model unit and compared with each other. The results of the study were as follows; 1. In the model unit mean direct nursing care hours per cach shift was 146.88 minutes and indirect nursing care hours was 354.72 minutes. The ratio of the direct nursing care hour to indirect nursing hour was 29.6 ; 70.4 and that of the control unit was 26.9 : 73.1. Direct nursing care hour in model unit was longer than that of the control unit. But, the difference was not significant. In subcategories of direct nursing care, the time spent in mobility and exercise, conservation of body temperature, hygiene, and communication and health education were longer than that of the con" trol unit. 2. Indirect nursing care hour in model unit was shorter than that of the control unit. But, the difference was not significant. In subcategories of indirect nursing care, the time spent in drug management and ward arrangement was shorter than that of the control unit. 3. Patients' satisfaction to nursing care was increased significantly after operating the model unit (T=-3.48, P=-0.002) and satisfaction to subcategories of physical comfort measure, psychological cate, and unit management components were significantly higher than before. 4. In the model unit, nurses' total job satisfaction was increased significantly after operating the model unit(Z=2.1004, P=.0357) and satisfaction to subcategory of satisfaction to administration was significantly higher than before (Z=-2.0732, P=.0382). 5. After operating the model unit, quality care index was increased from 89 to 93. With this results, it can be summarized that all the measures tried for quality care, such as educational programs, managereal support for interdepartmental cooperation, and improvement of the equipment and facility provision resulted in partial increase in direct nursing care hours, nurses satisfaction to their job and patients' satisfaction to nursing care. In can be postulated that managereal support and motivation without proper staff supplementation is not enough for increasing direct nursing care hours. And for the enhancement of the level in clinical nursing, and staff supplement must be considered sincerely and the measures for reducing indirect nursing care hours, such as computerization of nursing care activities, improvement of facilities and equipment and facilities supply system, must be instituted in addition.

하향평가와 상향평가 결과에 영향을 미치는 특성 분석 (Analysis of characteristics affecting the score-groups by supervisor and subordinate rating)

  • This study was designed to compare the differences m results of supervisor and subordinate rating. Data was collected from personnel evaluation and subordinate rating results for middle managers(n=68) in hospital from 3rd January to 20th March in 2004. Supervisor rating consisted of performance, ability and attitude evaluation. Subordinate rating consisted of leadership, ability and attitude evaluation. Collected data included sociodemographic characteristics, work department, work level, years of work, years at present level and whether working in a patient serving department. The difference of standardized supervisor and subordinate rating score was used to define groups as 'higher in supervisor rating group'. Groups were defined in total score, ability score and attitude score. Main results were as follows: 1. In total score, sectional chiefs were apt to be 'higher in subordinate rating group' while chief clerks were apt to be 'similar group' or 'higher in supervisor rating group'. Staffs in patient serving department were likely to be 'higher in supervisor rating group' and staffs in non-patient serving department were likely to be 'higher in subordinate rating group'. All these results were statistically significant. 2. In ability score, there were no statistically significant differences in age, sex, years of education, work department, work level, years of work and whether working in a patient serving department among 'higher in supervisor rating group', 'similar group' and 'higher in subordinate rating group'. 3. In attitude score, staffs in the department of medical affairs and the department of administration were apt to be 'higher in subordinate rating group'. Staffs in the department of nursing were apt to be 'higher in supervisor rating group'. Staffs in a patient serving department were likely to be 'higher in supervisor rating group' and staffs in a non-patient serving department were likely to be 'higher in subordinate rating group'. All these results were statistically significant. 4. Logistic analysis about total score showed that sectional chiefs had higher Odds Ratio(OR) to be in 'higher in subordinate rating group'. Staffs in a non-patient serving department had higher OR to be in 'higher in subordinate rating group'. Both these results were statistically significant. 5. Logistic analysis about ability score showed that sectional chiefs had higher OR to be in 'higher in subordinate rating group'. Staffs in a non-patient serving department had higher OR to be in 'higher in subordinate rating group'. These results were not statistically significant. 6. Logistic analysis about total score showed that sectional chiefs had higher OR to be in 'higher in subordinate rating group', but the difference was not statistically significant. Staffs in a non-patient serving department had significantly higher OR to be in 'higher in subordinate rating group'. In conclusion, there is no clear superiority between supervisor and subordinate rating in personnel evaluation. It would be better to use a mixed model. It's also suggested to use an intervening rate of application or scores considering work levels and work department in personnel evaluation. These results would be helpful for hospitals planning a supervisor and subordinate rating system for personnel evaluation.

Streptozotocin으로 유발한 당뇨쥐에 있어서 차가버섯(Inonotus Obliquus)의 혈당 및 DNA 손상 개선효과 (The Improvement of Chaga Mushroom (Inonotus Obliquus) Extract Supplementation on the Blood Glucose and Cellular DNA Damage in Streptozotocin-Induced Diabetic Rats)

  • 최근 당뇨환자에게서 산화적 손상이 증가됨이 보고되면서 항산화식품의 섭취를 통한 DNA 손상 개선노력이 다각도로 시도되고 있다. 차가버섯을 비롯한 여러 가지 버섯류에 항산화활성 및 혈당강하 효과가 있음이 알려지고 있으므로 본 연구에서는 streptozotocin (STZ) 유발 당뇨쥐를 사용하여 차가버섯에 항산화 효과 및 DNA 손상 억제효과가 있는지, 그리고 이런 항산화효과가 당뇨쥐의 혈당을 감소시키는지를 알아보았다. Sprague-dawley종 수컷 흰쥐(4주령, $110{\pm}6\;g$) 32마리를 대조군, 당뇨군, 저농도 차가버섯투여군 (0.5 g/kg BW) 및 고동도 차가버섯투여군 (5 g/kg BW) 등 4군으로 나누어 동물실험을 진행하였으며, 꼬리 정맥에 STZ을 주사하여 당뇨를 유발시킨 후 4주간 사육하였다. 4주 후에 혈액과 간을 채취하여 혈장 glucose, 혈장 총항산화능 (total radical-trapping antioxidant potential, TRAP) 수준 및 ${\alpha}$-tocopherol 수준, 적혈구 catalase, superoxide dismutase (SOD), 및 glutathione peroxidase (GSH-Px)의 활성도, 그리고 임파구 및 간조직의 DNA 손상 정도를 comet assay로 측정하였다. 실험 결과, 대조군에 비해 당뇨군에서 혈당이 상승하고 (p < 0.05) 혈장 TRAP 수준은 감소하였으며 (p < 0.05) 임파구의 DNA 손상은 증가하였고 (p < 0.05) 나머지 지표들은 차이가 나타나지 않았다. 당뇨군에 비해 저농도 및 고농도 차가버섯 투여군들에서 혈당이 감소하는 경향이 있었고 혈장 TRAP 수준과 ${\alpha}$-tocopherol 수준이 증가하는 경향을 보였으나 모두 유의적인 차이는 아니었으며, 당뇨군에 비해 차가버섯 투여군에서 tail length로 본 임파구 DNA 손상 정도가 유의적으로 감소하였다 (p < 0.05). 나머지 지표들은 당뇨군과 차가버섯 투여군들 간에 차이를 보이지 않았다. 본 연구결과, 차가버섯 열수추출물을 고농도 (5 g/kg BW)로 4주 동안 당뇨유발 흰쥐에게 투여하였을 때 혈당강하 효과는 유의적으로 나타나지 않았으나 당뇨로 인해 유발 된 DNA 손상정도가 정상수준까지 뚜렷하게 감소함을 관찰하였으며 이를 통해 부분적으로 차가버섯의 항산화 효과를 관찰할 수 있었다. 이와 같은 결과는 앞으로 차가버섯을 이용하여 당뇨환자의 DNA손상을 개선시키기 위한 건강기능식품 개발 연구에 활용될 수 있을 것으로 생각된다.

내독소 유도 급성폐손상에서 항히스타민의 역할 (The Effect of Antihistamine on Endotoxin-induced Acute Lung Injury)

  • 연구배경 : 히스타민은 폐 내에 널리 분포하며 강력한 모세혈관 투피성 증가 작용이 있을 뿐만 아니라 내피세포 표면에서 P-selectin의 발현을 증가 시키고 IL-8 분비를 촉진시켜 호중구의 조직 내 이동 및 활성화에 관여한 다고 보고되고 있다. 그러므로 내독소로 유도되는 급성폐손상의 발병기전에 내인성 히스타민이 호중구 의존성 폐손상의 주요 매개물질로 작용할 수 있을 것으로 추정되나 자세한 역할은 아직 잘 알려져 있지 않다. 저자들은 내독소로 유도되는 급성폐손상의 발병기 전에 내인성 히스타민이 관여한다면 항히스타민제를 전처치 시 내독소에 의한 폐손상이 감소될 수 있는지 알아보고 폐손상이 감소된다면 어떤 기전이 연관되는지 알아보고자 하였다. 방 법 : Sprague-Dawley쥐를 이용하여 생리 식염수를 기도 내 투여한 정상군, 내독소를 기도내 투여한 내독소군, $H_1$ 수용체 차단제 (mepyramine) 및 $H_2$수용체 차단제(ranitidine)를 정주 후 내독소를 투여한 $H_1$ 처치군 및 $H_2$ 처치군 등 모두 네군으로 나누어 처치 5 시간 후 급성폐손상의 여러 지표들을 측정 비교하였다. 결 과 : 내독소군은 정상군에 비해 측정한 폐손상지표들이 모두 유의하게 높았다(각 p<0.01). $H_2$처치군에서는 폐단백누출지표, BAL 액내 총단백 및 LDH농도가 모두 내독소군에 비해 유의하게 낮았다(각 p<0.05, p<0.05, p<0.05). $H_1$처치군에서는 내독소군에 비해 폐단백누출지표 만이 유의하게 낮았다(p<0.05). 그러나 $H_1$ 처치군 및 $H_2$처치군에서 측정된 MPO 활성도, 병리학적 손상지수와 BAL액내 호중구수, TNF-${\alpha}$, IL-$l{\beta}$ 및 IL-10 농도는 내독소군과 차이가 없었다. 결 론 : 백서에서의 내독소 유도 급성폐손상 모형에서 $H_2$ 수용체 차단제는 폐포-모세혈관 막의 증가된 투과성을 유의하게 감소시키나 호중구의 폐내 침윤은 감소시키지 못하였다.
