A Research on Relation between Measles Occurrence and Related Medical Text during Joseon Dynasty - Based on 18th and 19th Century - (조선 홍역발생과 관련의서 편찬관계 고찰 - 18C, 19C를 중심으로 -)

  • In this study, we research the tendency of medical text publication by examining the period of the great measles outbreak and the period of the publication of specialized smallpox texts. Using the National Institute of Korean History database for the Annals of Joseon Dynasty, the Daily Records of Royal Secretariat of Joseon Dynasty and Bibyeonsadeunglok, we will compare all records of measles occurrence. Measles and smallpox (Majin in Korean) have similar symptoms and treatment methods. In East Asia, when measles occurred it spread to Joseon and Japan, which are verified by records of the Annals of Joseon Dynasty, the Daily Records of Royal Secretariat of Joseon Dynasty and Bibyeonsadeunglok. The medical books related to measles are; Ryuhasinbang, Majinpyeon, Geupyubang, Yimsinyeokbang, Eulmisinjeon, Majingbang, Jinyeokbang, Magwahoetong, Majingibang, Susengsingam, Hongjinsinbang. Measles and Majin are the same disease. During the period of measles occurrence, measles-related medical books were published, and this relation of measles occurrence and measles-related medical text publication is verified by several national records.

  • Following the Five Year Measles Elimination Program, measles has been declared eliminated from the Republic of Korea since 2006. However, there remain challenges related to the surveillance of measles in the postelimination phase. Even though the routine surveillance system has revealed a gradual decrease in the number of reported cases since 2002, 4 resurgences have occurred, notably due to outbreaks. Because vaccine-modified measles is becoming widespread due to high vaccination coverage, conducting laboratory confirmation in each case becomes important. Moreover, susceptible individuals with measles have been identified through seroprevalence studies. Lastly, the efforts to improve the timeliness of measles reporting have led to the establishment of an active laboratory-based surveillance network, which has shortened the interval between diagnosis and notification. In these circumstances, searching for more sensitive and effective surveillance measures is important for maintaining the elimination status and preventing future outbreaks of measles in Korea.

  • Korea was verified as a measles-free country by the World Health Organization in 2014; however, some imported measles cases and related outbreaks were reported. As the incidence of wild-type measles decreases in measles-free countries, the proportion of vaccine-associated measles is increasing. Wild-type measles is highly contagious and requires follow-up and management of the contacts, whereas vaccine-related measles is not contagious and does not require follow-up intervention. Therefore, measles genotyping should be performed to reliably distinguish between them in suspected patients. Two patients with vaccine-associated measles were confirmed through genotyping at Daegu Fatima Hospital. In Korea, there are only a few reports of vaccine-associated measles confirmed through genotyping; thus, we report recent vaccine-associated measles cases, emphasizing the need for genotyping.

Measles Infections and Measles Vaccinations Rates for the Past 10 Years in Kang Wha -A Cohort Observation- (강화 지역주민의 과거 10년간 홍역이환율 및 예방접종율의 변화 -출생년도별 코호트 분석-)

  • The objective of the study is to measure the changes in measles infection and measles vaccination rates for the past 10 years in a rural area, Kang Wha. The study population were the entire children who were born between 1971 and 1950 in three townships (Sunwon, Naegae, Buleun) in Kangwha County. Two interview surveys were carried out during the 10 years of period, one in 1977 and the other in 1981. The data were collected by Family Health Workers through interview with structured questionnaires. The diagnosis of measles was mainly based on histories, symptoms and sighs of the disease. If a mother had reported measles history of her child, a public physician reviewed and decided the final diagnosis of the reported case. A retrospective cohort observation was done in order to see the trends of measles infections and measles vaccinations. The major findings were as follows; 1. The 5 year prevalence rate of measles vaccinations was 51.3% between 1971 and 1975 and 71.9% between 1976 and 1980 respectively. The difference between two periods was statistically significant (P<0.05). The secular trend of measles vaccinations showed increasing tendency from 1971 to 1978 and since then kept maintained. 2. In the birth cohort analysis of measles vaccinations, the vaccination rates, in general, were higher in the later cohort groups than that of earlier cohort groups. 9. The 5-year experience rates for measles infections were 24.3% between 1971 and 1975 and 17.2% between 1976 and 1980 respectively. This difference was statistically significant(P<0.05). The secular trend of experience rates for measles infections showed decreasing tendency from 1971 to 1980 except an outbreak in 1976. 4. The birth cohort analysis of experience rates for measles infections showed that the rate was higher in the later cohort groups than that of the earlier cohort groups. This decreasing tendency was prominent between $1973{\sim}1974$ and $1976{\sim}1977$. 5. The distribution of age specific incidence rates for measles infections showed unimodal curve with the peak at the age of 12 to 18 months. This findings were same in both two surveys. 6. Seasonal variations of the measles infections showed two peaks, one major peak in March through May and the another minor peak in September through December. 7. The 5-year reduction rate for measles infections among those vaccinated was 90.4% between 1971 and 1975 and 88% between 1976 and 1980.

Research on the measles experience of A primary school children in the Epidemic area (홍역이환지역 일 초등학교 학생의 홍역경험 조사연구)

  • This study is a investigation research to examine total 123 children's thought, perception and coping method about measles, with 62 children who had experienced the measles and 61 good health children in the school, which had spread to a primary school in kyoungsang-bukdo province. The data collection was done from December 6 to December 8. The analysis of data was SPSS(10.0) for descriptive statistics. The data were collected from total 123 children, 62 of them who had diagnosed measles on October, 2000, when the measles had ended, were used a purposive sampling, the others who had a good health were used a convenience sampling. From three to six grade students made out directly the questionaire in the way of self-questionaire, and from one to two grade students, made out answering the question which the researcher had read the items. The researchers call back the students to complement the contents about the items inadequate answers, so they raised the reliability. The results were as follows. 1. The results of observe carefully the measles infection degree by a sex, the boys(27.4%) were sick severely compared with girls(9.7%)were sick weakly compared with the boys(27.4%)(p〈0.01). 2. The 62.9 percents of infected children by measles thought that they were infected at school, regarded the doctors as a first judge, and had caught the measles because of their fault. The results of observe carefully, 33.8 percents children of infected by measles had have a secondary vaccination. To the question whether they go to institute or not while suffering from the measles, 35.5% children didn't go there since they had heard the fact they infected the measles. 3. To a question of asking if close mend caught the measles, what will they do, 65.1% children of caught the measles answer ‘I can't meet my friend'(p〈0.001). It was significant that infected children have more interests than didn't infected children. The 22% children of didn't catch the measles take great interested in health compare with the 12.2% children of infected the measles(p〈0.05). To the question if they listen the preservation of health broadcasting, 63.4% children answer ‘I listen well’. It was significant between the infected group and didn't infect group(p〈0.05). To the question if they delivery to the written information at home, it was significant between the infected group and didn't infect group(p〈0.05). 4. The infected group of 69.1% children think the health is most important in their life school than any other they like study.

Changes of Measles Specific IgG in the Sera of Children after Birth (출생 후 혈청내 홍역 특히 IgG의 변동)

  • Purpose : The incidence of measles was gradually decreased since 1963 when measles vaccination was firstly developed and liscenced in the world. But, recently the outbreaks of measles in infants and school children have been reported despite of wide spread use of measles vaccination. This study was performed to evaluate the efficacy of measles vaccination and the necessity of revaccination in Korean infants and children. Methods : 168 subjects of mothers and neonates, infants and children were enrolled in this study during the periods of 10 months from March to December in 1995. Measles specific IgG in the sera of mothers and children was measured using EIA kit (Sigma Co., MO, USA). Antibody titer of over or equal to 110 AU/ml was considered positive. Results : The results obtained were as follows. 1) Values of measles specific IgG in the sera of mother and neonate were 82.9 AU/ml and 89.3 AU/ml respectively and were rapidly decreased within 6 month after birth. Positive antibody levels (${\geq}$ 110 AU/ml) were observed in only 25 % of neonates. 2) In vaccinated children, values of measles specific IgG were 117.4 AU/ml in 9~15 month group, 76.9 AU/ml in 3~6 year group and 79.5 AU/ml in 10~15 year group after either one or two times of measles vaccination. Positive antibody levels in vaccinated children were observed in 57.7% of 9~15 month group, 38.4% of 3~6 year group and 34.7% of 10~15 year group. Conclusion : These results suggest that primary measles vaccination before 6 months of age can be considered and revaccination of measles should be recommended before 3~6 years of age. Further studies will be needed to clarify the reasons of high proportion of primary measles vaccination failure and to established the appropriate schedule of measles vaccination in korean infants and children.

Hospital-Acquired Measles: A Systematic Review Using the Outbreak Reports and Intervention Studies of Nosocomial Infection (ORION) Statement

  • Purpose: Despite the recent increased number of nosocomial measles, the outbreak investigation reports are not usually standardized, thus posing unclear understanding of magnitude of its public health burden. We used the Outbreak Reports and Intervention Studies of Nosocomial Infection (ORION) statement, to compare nosocomial outbreaks and synthesize evidence to prevent hospital transmission of measles. Methods: A PubMed, Web of Science, Embase, Scopus, and Cochrane search in English, using the medical subject headings "measles," "nosocomial," "hospital," and "healthcare," was performed. We evaluated the quality of outbreak reports of nosocomial measles infection worldwide using the ORION statement findings and recommendations. Results: We reviewed 24 studies in accordance to the ORION statement. Measles transmission in healthcare settings is a significant burden on the morbidity, mortality, and economy of measles. The healthcare workers' booster vaccination guidelines should be monitored and enhanced during the post-elimination period of measles. The outcomes of infections must be explicit for outbreak reports. Conclusions: This study identified the epidemiological and clinical characteristics of nosocomial measles infections and provided strong evidence for infection control policies in hospitals.

Reemerging Old Infectious Diseases: Diagnosis of Measles, Mumps, Rubella, and Pertussis (다시 출현하는 감염병: 홍역, 볼거리, 풍진, 백일해의 진단을 중심으로)

  • Reemerging infectious diseases are infections that had decreased in incidence in the global population and were brought under control through effective health care policy such as vaccination, but more recently, began to resurge as a health problem due to many reasons. Measles, rubella, mumps and pertussis are the examples. Immunization with MMR (measles, mumps, rubella) and pertussis vaccine has contributed to marked decrease in measles, mumps, rubella and pertussis incidence worldwide. In Korea, measles and rubella almost disappeared after the introduction of 2 doses of MMR immunization schedule. Recently, these infections have been reemerging in many countries with low vaccination rates and can be introduced again in Korea. However mumps and pertussis outbreaks are reported among fully vaccinated populations. Declining vaccine effectiveness, an increased awareness and surveillance of the disease or improved laboratory diagnostic tools had been suggested as possible causes. For the clinicians, it is difficult to diagnose these reemerging infectious diseases partly because of few experience of typical cases of measles and rubella or partly because of modification of clinical symptoms and signs of infectious diseases in immunized population. In this article, the diagnosis of measles, mumps, rubella and pertussis will be reviewed in the aspects of clinical characteristics, serologic methods, virus isolation, and polymerase chain reaction.

Response of Measles-specific Antibody in Children with Measles During Measles Epidemic in Seongnam, 1993 (1993년도 성남지역에서 유행한 홍역 환아에서의 홍역 특이 항체 반응)

  • Measles outbreak in the world was decreased since measles vaccine had been introduced. Although vaccination rate is high, measles was not eradicated and measles reappeared among vaccinated children. We measured measles-specific antibody from the vaccinated and unvaccinated groups who had experienced apparent measles in the Seongnam city in 1993. The results were as follows. 1) The data included total 126 children (M:F=1 : 1). Age distribution of measles outbreak revealed 6 children in 5yr, 11 in 6yr, 20 in 7yr, 39 in 8yr, 22 in 9yr, 11 in 10yr, 11 in 11yr, and 6 in 12yr. 2) MMR vaccination rate was 78.6%(99/126) in the children who had experienced measles. Positive rate of measles-specific IgM Ab was 80.8% (80/99) among the vaccinated group and among 9E.6.% (25/27) the unvaccinated. 3) Positive rate of measles-specific IgG Ab was 90.9% (90/99) among MMR-vaccinated group, and 85.2% (23/27) in unvaccinated group. In conclusion, measles-specific IgM antibody have been detected more than 1 month in most patients. The relatively high proportion of measles-specific IgM positivity may mean primary vaccine failure. To booster the antibody titers and to prevent measles epidemic in school-aged children, revaccination of measles should be considered.

Molecular epidemiological study of measles virus throughout an imported epidemic outbreak in Gyeonggi-do in 2014 (2014년 경기지역에서 발생한 해외유입 홍역 바이러스의 분자역학적 특성 분석)

  • Measles virus is a highly contagious, exanthematic virus, preventable by the use of an effective live-attenuated vaccine. However, measles virus remains endemic in many area of the world causing nearly 200,000 deaths per year and still a major cause of child mortality, mostly in developing countries. In March 2014, Republic of Korea was certified as a 'national measles elimination' by the WHO as a result of a high-quality case-based surveillance system and population immunity, which was achieved by a high vaccination rate (>95.0% since 1996). But, since the beginning of 2014, the Gyeonggi province has experienced a resurgence of measles cases. In this study, we investigated the characteristics of measles viruses isolated from confirmed measles in Gyeonggi province during January 1, 2014 ~ July 31, 2014, 60 isolates were obtained from 72 confirmed measles specimens. Genotypic distributions and genetic diversities of isolated measles virus were analyzed by sequencing of nucleoprotein (N) gene. 58 (96.7%) imported cases were identified. The predominant genotype was B3, which reflects the circulating measles virus in adjacent countries. The sequences of nucleoprotein (N) gene of isolated MeV were showed that the strains characterized showed the highest degree of identity (99%) with the Philippine related strains in 2013-2014. Therefore, infected traveler returning from the Philippines transmitted secondary infection in Korea.
