중환자실에서의 인간중심 간호와 가족 만족도 관계 (The Relationship between Person-Centered Nursing and Family Satisfaction in ICUs)

  • Purpose : This study aims to identify the relationship between person-centered critical care nursing (PCCN) and family satisfaction for intensive care unit (ICU) nurses. Methods : This research used a cross-sectional survey. The participants were 142 nurses and 121 family members from 8 ICUs in 4 hospitals in B city. The questionnaire survey was conducted between December 2018 and March 2019. The eight ICUs were divided into those that scored high and those that scored low for person-centered care relative to the average score for PCCN, and the difference in family satisfaction between the ICUs with high and low scores was compared using t-test. Results : The mean score of PCCN was $3.68{\pm}0.40$ out of 5. In the subcategories of PCCN, "comfort"was the highest at $3.95{\pm}0.49$, followed by "respect" at $3.73{\pm}0.57$, "compassion" at $3.59{\pm}0.57$, and "individuality" at $3.47{\pm}0.54$. The average score of family satisfaction with the ICUs was $3.45{\pm}0.67$ out of 5. In its subcategories, "emotion" was the highest at $3.69{\pm}0.72$, followed by "information" at $3.61{\pm}0.75$, "participation" at $3.30{\pm}0.79$, and "resources" at $3.20{\pm}0.80$. The family satisfaction ($3.75{\pm}0.54$) with the ICU of the highest-scoring PCCN was significantly higher than the family satisfaction ($3.25{\pm}0.53$) with the ICU with the lowest-scoring PCCN (t=4.97, p<.001). Conclusion: The results suggest that the PCCN scores of ICU nurses should be promoted to improve family satisfaction. It is necessary to pursue a variety of ways to secure the provision of person-centered nursing practice in ICUs.

현 환경미화원용 안전작업복의 착용감 평가 -공적으로 공급된 환경미화원복을 중심으로- (Evaluation on Wear Sensation of Safety Clothing for Current Street Cleaners -Focusing on Publicly Supplied Safety Clothing for Street Cleaners-)

  • This study evaluates the comfort and suitability of safety clothing. We made three safety clothes 'y', 'lg', 'lgH'. Only 'y' made with fluorescence fabric has passed International Standards (ISO 20471). A wearing test investigated the physiological response and subjective sensations at 30℃ and 50% RH, six men participated. Mean skin temperature and tympanic temperature showed significant difference with experimental course (p<.05). The micro-temperature (Tm) showed significant differences between three garments on the thigh (p<.01). When wearing 'lg', Tm was the highest. Micro-humidity indicated the interaction between clothing and the experimental course on the chest with no significant difference; however, there was a low tendency when wearing 'lgH' after the second half of exercise. The sweat rate indicated a significant difference between 'lg' and 'lgH' at 20 minutes of exercise (p<.05). Three sensations showed significant differences with the time (p<.01), there was also a tendency to feel hotter and more humid when wearing 'lg'. Skin sensation showed significant differences between pre-experiment and post-experiment (p<.05), 'y' was the most preferred. The 'lg' with long-sleeved currently worn in Korea tended to feel hotter, more humid and more uncomfortable than 'y'. It indicates that there is room for improvement in Korean safety clothing.

Community Care for Cancer Patients in Rural Areas: An Integrated Regional Cancer Center and Public Health Center Partnership Model

  • Purpose: The accessibility of medical facilities for cancer patients affects both their comfort and survival. Patients in rural areas have a higher socioeconomic burden and are more vulnerable to emergency situations than urban dwellers. This study examined the feasibility and effectiveness of a cancer care model integrating a regional cancer center (RCC) and public health center (PHC). Methods: This study analyzed the construction of a safety care network for cancer patients that integrated an RCC and PHC. Two public health institutions (an RCC in Gyeongnam and a PHC in Geochang County) collaborated on the development of the community care model. The study lasted 13 months beginning in February 2019 to February 2020. Results: The RCC developed the protocol for evaluating and measuring 27 cancer-related symptoms, conducted education for PHC nurses, and administered case counseling. The staff at the PHC registered, evaluated, and routinely monitored patients through home visits. A smartphone application and regular video conferences were incorporated to facilitate mutual communication. In total, 177 patients (mean age: 70.9 years; men: 59%) were enrolled from February 2019 to February 2020. Patients' greatest unmet need was the presence of a nearby cancer treatment hospital (83%). In total, 28 (33%) and 44 (52%) participants answered that the care model was very helpful or helpful, respectively. Conclusion: We confirmed that a combined RCC-PHC program for cancer patients in rural areas is feasible and can bring satisfaction to patients as a safety care network. This program could mitigate health inequalities caused by accessibility issues.

VSimulators: A New UK-based Immersive Experimental Facility for Studying Occupant Response to Wind-induced Motion of Tall Buildings

  • Current vibration serviceability assessment criteria for wind-induced vibrations in tall buildings are based largely on human 'perception' thresholds which are shown not to be directly translatable to human 'acceptability' of vibrations. There is also a considerable debate about both the metrics and criteria for vibration acceptability, such as frequency of occurrence or peak vs mean vibration, and how these might vary with the nature of the vibration. Furthermore, the design criteria are necessarily simplified for ease of application so cannot account for a range of environmental, situational and human factors that may enhance or diminish the impact of vibrations on serviceability. The dual-site VSimulators facility was created specifically to provide an experimental platform to address gaps in understanding of human response to building vibration. This paper considers how VSimulators can be used to inform general design guidance and support design of specific buildings for habitability, in terms of vibration, which allow engineers and clients to make informed decisions with regard to sustainable design, in terms of energy and financial cost. This paper first provides a brief overview of current vibration serviceability assessment guidelines, and the current understanding and limitations of occupants' acceptability of wind-induced motion in tall buildings. It then describes how the dual-site VSimulators facility at the Universities of Bath and Exeter can be used to assess the effects of motion and environment on human comfort, wellbeing and productivity with examples of how the facility capabilities have been used to provide new, human experience based experimental research approaches.

Guidance and rationale for the immediate implant placement in the maxillary molar

  • Objectives: While the reliability of immediate implant placement in the maxillary molar has been discussed, its significance is questionable. There have been no guidelines for case selection and surgical technique for successful treatment outcomes of immediate maxillary molar implants. Therefore, in this study, we classified alveolar bone height and socket morphology of the maxillary molar to establish guidelines for immediate implant placement. Materials and Methods: From 2011 to 2019, we retrospectively analyzed 106 patients with 148 immediate implants at the Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital. Inclusion and exclusion criteria were applied, and patient characteristics and treatment results were evaluated clinically and radiologically. Results: A total of 29 tapered, sand-blasted, large-grit, and acid-etched (SLA) surfaces of implants were placed in 26 patients. The mean patient age was 64.88 years. Two implants failed and were reinstalled, resulting in a 93.10% survival rate. Fluctuating marginal bone level changes indicating bone regeneration and bone loss were observed in the first year following installation and remained stable after one year of prosthesis loading, with an average bone loss of 0.01±0.01 mm on the distal side and 0.03±0.03 mm on the mesial side. Conclusion: This clinical study demonstrated the significance of immediate implant placement in maxillary molars as a reliable treatment with a high survival rate using tapered SLA implants. With an accurate approach to immediate implantation, surgical intervention and treatment time can be reduced, resulting in patient satisfaction and comfort.

A Systematic Review of Kinesiology Taping in Patients With Lymphedema

  • Background: The compression therapy, which is the standard treatment for lymphedema patients, may be difficult to implement and contraindicated to some patients depending on their health condition. Objects: The purpose of this study is to investigate whether kinesiology taping (KT) can be used effectively and safely in the management of lymphedema as an alternative treatment through systematic review and meta-analysis. Methods: In February 2023, the literature was systematically collected through eight search engines with a combination of terms, 'lymphedema' and 'kinesiology taping.' We qualitatively analyzed the differences and safety of KT methods, and quantitatively meta-analyzed the effects of volume reduction in edema, range of motion (ROM), and pain improvement using Review Manager ver. 5. 4. To assess the risk of bias in the randomized controlled trial (RCT) studies, Risk of Bias was used. Results: A total of 616 articles searched and 20 studies were selected, including 12 RCTs and eight case studies. KT intervention could not replace multilayer compression bandage (MLB), but it demonstrated similar or better results compared to compression garment (CG), with reduced pain and improved intervention comfort. Studies reported skin adverse events ranging from 2.5% to 20.68%, with a total adverse event incidence of 7.7%. There was no significant difference in the application method of KT. As a result of the meta-analysis from the 8 RCTs, the KT intervention showed a mean difference (MD) of -7.18 with a 95% confidence interval (CI) [-12.64 to -1.72] in the volume change of lymphedema, while the pain difference was MD 0.82 with CI 95% [0.50 to 1.15], in comparison to the MLB and CG intervention. Conclusion: KT therapy led to a reduction in edema size, volume, pain, and improved ROM and quality of life. KT may be a viable option for lymphedema patients who have trouble applying traditional compression therapies.

비만아동의 온도적응성에 대한 착의훈련 효과 (Effect of Wear Training on Temperature Adaptability of the Obese Children)

  • 본 연구에서는 비만아동의 온도적응능력을 향상시키기 위해 초등학교 4학년$\∼$6학년에 재학 중인 비만아동 13명(남자 6명, 여자 7명)을 대상으로 하여, 10주간의 단기 의복착용훈련 프로그램을 시행한 후 프로그램의 효과를 알아보았다. 프로그램 시행기간 중 아동이 매일 직접 측정한 주택 내 실내온도는 평균 25.1$^{circ}C$였고, 아동이 착용한 체표면적당 의복의 무게(착의량)는 평균 300g/$m^{2}$였으며, 실내온도와 착의량 간에는 유의한 상관이 있었다(p < .01). 프로그램의 시행효과를 구체적으로 알아보기 위해, 23.0$\pm$0.5$^{circ}C$, 50$\pm$5$\%$RH로 조절된 인공기후실에서 실험을 실시하여, 반소매 면 티셔츠(0.13clo)와 T/C 반바지(0.09c1o)를 착용하고 안정상태에 있는 아동의 체온, 피부온, 혈압 및 맥박 등의 생리반응과 쾌적감 및 온랭감의 주관적 감각반응을 프로그램 시행 전후에 측정한 후 얻은 주요 결과는 다음과 같다. 평균피부온은 프로그램 시행전후에 유의한 차이가 없었으나, 전반적으로 체온과 평균혈압은 프로그램 시행 후에 내려갔다(p < .01). 주관적 감각반응에서는 프로그램 시행 후에 여아가 남아에 비해 약간 더 따뜻하게 느꼈고(p < .05), 이에 따라 아동자신이 선택한 쾌적온도는 더 낮은 경향을 나타내었다(p < .1). 이와 같은 연구결과는 비만아동의 행동성 및 자율성 체온조절연구에 유익할 것이나, 보다 명확한 자료를 얻기 위해서는 종합적이고 장기적인 훈련프로그램의 시행이 요구된다.

학교운동장 피복물질 간의 온열효과 비교 - 율전초등학교를 대상으로 - (Comparison of Thermal Effects of Different School Ground Surface Materials - A Case of Yooljeon Elementary School-)

  • 마사토가 학교 운동장 피복재료로 많이 사용되어 왔고 일부에서는 천연잔디를 사용해 왔으나 최근에는 인조잔디가 많이 사용되기 시작하고 있으며 서울의 경우 174개교의 운동장에 인조잔디가 설치되었고, 미국에서는 인조잔디를 시공한 곳이 운동장뿐 아니라 공원에 이르기까지 3,500곳 이상이 된다. 이러한 인조잔디 사용의 증가로 인해 인조잔디가 주변 환경에 미치는 영향은 많이 연구된 것에 비해 인조잔디가 주변 미기후에 미치는 영향은 한국에서는 많이 연구되지 않고 있다. 그러므로 본 연구의 목적은 학교 운동장에 시공되는 세가지 포장재료가 - 인조잔디, 천연 잔디 및 마사토 - 각각 주변에 미치는 기온저감 및 열환경 영향을 조사하여 학교운동장 계획에 유용한 정보를 제공하는 데에 있다. 본 연구에서는 전산유체역학기법(Computational Fluid Dynamics, CFD) 시뮬레이션을 이용하여 세 가지 포장재료에서의 기온 및 열쾌적지수(Predicted Mean Vote, PMV) 시뮬레이션 결과를 도출하여 현장 관측 기온 값과 비교하였다. 2011년 7월 20일 14시 30분 주간의 기온 저감효과는 천연잔디가 인조잔디와 마사토포장과 비교하여 가장 높게 나타났다. 야간에도 23시 30분에 기온 저감 효과가 나타났지만 주간보다는 크지 않았다. PMV효과도 역시 천연잔디가 인조잔디와 마사토포장보다 주간보다 크게 나타났으나 야간에는 별 차이가 없었다. 본 연구결과 인조잔디가 기온 저감효과 및 열쾌적성 효과가 가장 낮게 나타나 학교운동장 계획 시 이와 같은 열환경 효과를 고려할 필요가 있다고 판단되었다.

병원환경내 소음과 입원환자의 반응에 관한 연구 -일 종합병원을 중심으로- (A Study on the Noise and Reaction to Noise of Inpatient)

  • This descriptive-correlational study was conducted to exam if there were relation between noise level and reaction to noise of inpatient. The purpose of this study was to provide a basic data for comfort of patient hospitalized. The hypotheses of the study are : 1. The higher perceived noise level of inpatient, the higher reaction level to noise. 2. The higher perceived noise level of inpatient, physiological reaction level to noise. 3. The higher perceived noise level of inpatient, the higher emotional reaction level to noise. The participant were 153 patients hospitalized in one general hospital. The research instruments used for this study were noise scale and reaction of patient scale developed by the author. Data was collected over a period of 10 days from the 9th of July to the 18th of July, 1994. Statistical analysis of the data included percentage, t-test, ANOVA and Scheffe test. Examination of the hypotheses was done by use of pearson correlation coefficient. The results are summarized as follows ; 1. The mean score of noise level was 2.24. Among noise factors reported by the subjects, that which ranked highest was 'Conversation of Visitors'(2.82). Next were 'noise of handling receptacle'(2.73), 'the others noise from outside'(2.73) and 'Conversation of supporter'(2.71). 2. The mean score of reaction level to noise was 2.19, physiological reaction level 2.04 and emotional reaction level 2.37. Among Physiological reaction to noise reported by the subjects, that which ranked highest was 'tired'(2.39). Next were 'sweating'(2.22) and 'headache'(2.20). Among emotional reaction to noise reported by the subjects, that which ranked highest was 'to irritate nerve'(2.53). Next were 'disturbing rest'(2.51) and 'to disturb sleep'(2.46). 3. The relationship between perceived noise level of inpatient and reaction to noise was statically significant (r=0.599, p=.0001). The relationship between perceived noise level of inpatient and physiological reaction to noise was statically significant (r=0.554, p=.0001). The relationship between perceived noise level of inpatient and emotional reaction to noise was statically significant(r=0.535, p=.0001). Thus hypothese 1, 2, 3 were supported. 4. There were significant differences between noise level of inpatient, admission periods, mobility of physical condition and exposure level to noise. 5. There were significant differences between physiological reaction level to noise, admission periods, mobility of physical condition and operation Yes or No. 6. There were significant difference between emotional reaction level to noise, admission periods, mobility of physical condition and exposure level to noise.

간호사의 정맥주사 관리에 대한 인식과 수행에 관한 연구 (A Study on the Nurse's Recognition and Performance in Intravenous Therapy Management)

  • The purposes of this study were to identify nurse's recognition and performance and to represent the factors of hindrance in the IV therapy management. The subjects were 420 nurses who worked at five general hospitals in Pusan. Tha data were collected using a questionnaire and the period of data collection was from January 1 to January 31, 1998. The instrument for this study was made by author oneself on the basis of guidelines Simmons et al', CDC' Stanley' and Kurdi' guideline, Cllinical Nurse's Association' that consist of 68 items for 5 fields ; pre-injection, just before-injection, needle-injection, during injection, post-injection field. Cron-bach Alpha coefficient of recognition and performance in the IV therapy management was .93 and .87. The datas were analized by a SPSS program using frequency, percent, paired t-test, t-test and oneway ANOVA. The results obtained were as follows : 1. The mean score of recognition in IV therapy management was significantly higher than that of performance(t=5.86, P<.001). 2. The items of lower than mean score of each fields in performance were the identification of drugs, hands washing, patient teaching about medication, disinfectional methods of the injection site and the rubber stopper in bottle, the use of disposable gloves, mask and eye goggles at the chemotherapy preparation, use of tape and armboard, changing the IV tubing, labeling the dressing over the injection site, observation and recordings of patient's condition after medication and confirmation of the needle length at the needle removal. 3. The factors of hindrance in IV therapy were 'having no time', 'insufficiency of goods', 'unknowing of methods', 'no disadvantage', and 'factors of doctor's doing'. The most important factor was 'have no time', especially item of hands washing. The other factors of hindrance showed high frequency in the following items ; 'insufficiency of goods' in the use of disposable gloves, mask and eye goggles at the chemotherapy preparation, 'unknowing of methods' in the certification of drugs compatibility, 'no disadvantage' in the labeling the dressing over the injection site, and 'factors of doctor's doing' in the changing the subclavian catheter dressing and checking the glucose level during the TPN infusion. In conclusion, there is necessity of educational program which can improve the nurse's knowledge of drugs, disinfection methods, comfort of patient and recordings in IV therapy management and alternative plan which are political and financial aids such as setting up the sink, giving of paper towels and necessary goods in the IV therapy for reducing the factors of hindrance for IV therapy management.
