Accuracy of maximal expiratory flow-volume curve curvilinearity and fractional exhaled nitric oxide for detection of children with atopic asthma

  • Purpose: Airway pathology in children with atopic asthma can be reflected by the concave shape of the maximal expiratory flow-volume (MEFV) curve and high fractional exhaled nitric oxide (FeNO) values. We evaluated the capacity of the curvilinearity of the MEFV curve, FeNO, and their combination to distinguish subjects with atopic asthma from healthy individuals. Methods: FeNO and angle ${\beta}$, which characterizes the general configuration of the MEFV curve, were determined in 119 steroid-naïve individuals with atopic asthma aged 8 to 16 years, and in 92 age-matched healthy controls. Receiver operating characteristic (ROC) curve analyses were performed to determine the cutoff points of FeNO and angle ${\beta}$ that provided the best combination of sensitivity and specificity for asthma detection. Results: Asthmatic patients had a significantly smaller angle ${\beta}$ and higher FeNO compared with healthy controls (both, P<0.001). For asthma detection, the best cutoff values of angle ${\beta}$ and FeNO were observed at $189.3^{\circ}$ and 22 parts per billion, respectively. The area under the ROC curve for the combination of angle ${\beta}$ and FeNO improved to 0.91 (95% confidence interval [CI], 0.87-0.95) from 0.80 (95% CI, 0.75-0.86; P<0.001) for angle ${\beta}$ alone and 0.86 (95% CI, 0.82-0.91; P=0.002) for FeNO alone. In addition, the combination enhanced sensitivity with no significant decrease in specificity. Conclusion: These data suggest that the combined use of the curvilinearity of the MEFV curve and FeNO is a useful tool to differentiate between children with and without atopic asthma.

Exocrine Secretory Responsiveness of Dispersed Pancreatic Acini to Secretagogues in Camostat-treated Rats (Camostat 투여 흰쥐 이자 외분비선의 분비자극물질에 대한 반응성)

  • It is well known that chronic stimulation with CCK gives rise to growth of exocrine pancreas and to increased content of enzyme proteins in pancreas. However, littls Is known about changes of the secretory function of exocrine pancreas which has been chronically stimulated with CCK, especially about the responsiveness to secretagogues such as CCK, caerulein and carbachol. The present study was performed to investigate the effect of camostat on secretory profiles and the responsiveness to secretagogues of exocrine pancreas by observing in vitro amylase release stimulated by cholecystokinin-octapeptide(CCK-8) and carbachol in dispersed isolated pancreatic acini from camostat-treated rats for 4 or 10 days. The results summarized as follows : 1) The maximal effective concentration of CCK-8 in amylase release in the camostat treated group was greater than control group, but that of carbachol was not different between groups. 2) Analysis of the stimulated amylase release as the percentage of the maximal response revealed that camostat treatment caused right-shift of the dose-response curve of CCK-8. Camostat did not cause significant changes in the dose-response curve of carbachol. 3) There were considerable increases in the amylase release in the camostat-treated group, compared to the control when acini were stimulated with CCK-8 $10^{-9}\;M$ and carbaochol $10^{-6}\;M$, and higher concentrations. 4) There was a reverse correlation between the tissue content and the maximal release(percent of the total content) of amylase. These results suggest that chronic exposure of exocrine pancreas to increased endogenous CCK can enhance the responsiveness of exocrine enzyme secretion to secretagogues, especially at higher concentrations of CCK and carbachol.

  • In this paper, we present the bit error performances of 4PSK and 8PSK signal transmission schemes using Maximal Ratio Combining diversity reception for m-distribution and Rician fading channels. The suitability of modeling a Rician fading environment by a properly chosen m-distribution model is examined. Using the error performance of the derived equation has been evaluated and shown in figures as a function of fading index (m), Rician factor (K), diversity branches number (L) and E($E_b/N_o$). It is found that MRC (Maximal Ratio Combining) diversity technique is very efficient for reducing the effects of fading, And then, diversity benifit much large as depth of fading becomes deeper but more decreases as many diversity branchs. Also the results show that the error performance corresponds as much as fading becomes weak and increases with the number diversty branches.

The effects of pulsed ultrasound and continued ultrasound intervention before an exercise on maximal voluntary isometric contraction and range of motion of exercise-induced muscle damage (운동 전 적용된 지속초음파와 맥동초음파가 운동 유발성 근육 손상의 최대등척성근력과 관절가동범위에 미치는 효과)

  • Background: The purpose of this study was to investigate the effects of pulsed-ultrasound intervention and continued-ultrasound on the MVIC (maximal voluntary isometric contraction) and active ROM (range of motion) recovery of before EIMD (exercise-induced muscle damage). Design: Randomized controlled trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I (n=10) and experiment group II (n=10). The subjects in experimental group were intervened by pulsed-ultrasound and continued-ultrasound, while ones on control group weren't by any intervention after induced EIMD. Results: First, In comparison of the MVIC, in the among group comparison, the MVIC of continued-ultrasound group was significantly larger than those of other groups (p<.005). Second, In the among group comparison, the active extension angle of continued-ultrasound group was significantly smaller than those of other groups (p<.005). Third, In the among group comparison, the active flexion angle of continued-ultrasound group was significantly lager than those of other groups (p<.05). Conclusion: The above results revealed that the continued-ultrasound intervention before an exercise had a significantly improve of muscle function after EIMD. Therefore we can consider the continued ultrasound as a considerable intervention method to prevent or reduce an exercise injury.

The Effects of Eye Health and Dynamic Visual Activity on Eye Movement in University Students (안구운동이 대학생의 눈 건강과 동체시력에 미치는 영향)

  • Younger generations use a variety of visual media, such as smartphones, televisions, and computers. Due to the development of visual media, the health of eye was weakening. Therefore, this study aimed to investigate the effects of eye health and dynamic visual activity on eye muscle stimulation with eye movement and therapeutic massage in university students. 38 university students in their 20s were divided into a control group(18) and an experimental group(20). The experimental group performed eye movement and therapeutic massage every 30 minutes for 5 times a week for 4 weeks. Ocular fatigue, maximal blinking interval, ocular surface temperature, and dynamic visual activity were measured before and after 4 weeks. Ocular fatigue was decreased significantly after 4 weeks (p<.05), and maximal blinking interval was increased significantly after 4 weeks (p<.05). Ocular surface temperature was not showed a significant difference after 4 weeks (p>.05), and dynamic visual activity was increased significantly after 4 weeks (p<.05). Therefore, eye movement and therapeutic massage are recommended for the improvement of eye function and eye health. It is expected to be used as research data to restore and prevent for eye health.

Practical and Provable Security against Differential and Linear Cryptanalysis for Substitution-Permutation Networks

  • We examine the diffusion layers of some block ciphers referred to as substitution-permutation networks. We investigate the practical and provable security of these diffusion layers against differential and linear cryptanalysis. First, in terms of practical security, we show that the minimum number of differentially active S-boxes and that of linearly active S-boxes are generally not identical and propose some special conditions in which those are identical. We also study the optimal diffusion effect for some diffusion layers according to their constraints. Second, we obtain the results that the consecutive two rounds of SPN structure provide provable security against differential and linear cryptanalysis, i.e., we prove that the probability of each differential (resp. linear hull) of the consecutive two rounds of SPN structure with a maximal diffusion layer is bounded by $p^n(resp.q^n)$ and that of each differential (resp. linear hull) of the SDS function with a semi-maximal diffusion layer is bounded by $p^{n-1}(resp. q^{n-1})$, where p and q are maximum differential and linear probabilities of the substitution layer, respectively.

A Study on the Early Diagnosis of Pneumoconiosis (진폐증의 조기진단에 관한 연구)

  • In order to assess the method which is more sensitive one to detect the early change of lung tissue by the inhaled dust, we have performed the various medical examinations such as chest radiography, pulmonary function test, high resolution chest CT, brnchoalveolar lavage and lung biopsy used bronchoscope and ultrathin bronchoscopy examination to 48 persons. The control group were 8 persons who did not exposed to dust, 40 cases of the experimental group have professionally exposed to the mineral dust. The results were as follows 1. The total number of cells in bronchoalveolar lavage was significantly increased in all of the pneumoconiosis group classified by chest and high resolution chest CT. 2. The composition rate of macrophage to the total number of cells in bronchoalveolar lavage fluid was significantly decreased in all of the pneumoconiosis group compared with the control group. 3. The composition rate of neutophils and lymphocytes to the total number of cells in bronchoalveolar lavage fluid was significantly increased in all of the pneumoconiosis group compared with the control group. 4. The forced expiratory volume in one second ($FEV_{1-0}$), maximal mid-expiratory flow (MMF), and maximal voluntary ventilation (MVV) were significantly increased only in the group of the progressed pneumoconiosis relatively. 5. We observed submucosal edema, anthracotic pigmentation and granuloma formation in transbronchial lung biopsy of the suspected pneumoconiosis (category 0/1) case which is thought to the early change of coal workers' pneumoconiosis.

The Maximal Covering Location Problem with Cost Restrictions (비용 제약 하에서 서비스 수준을 최대화화는 설비입지선정에 관한 연구)

  • This paper studied a maximal covering location problem with cost restrictions, to maximize level of service within predetermined cost. It is assumed that all demand have to be met. If the demand node is located within a given range, then its demand is assumed to be covered, but if it is not, then its demand is assumed to be uncovered. An uncovered demand is received a service but at an unsatisfactory level. The objective function is to maximize the sum of covered demand, Two heuristics based on the Lagrangean relaxation of allocation and decoupling are presented and tested. Upper bounds are found through a subgradient optimization and lower bounds are by a cutting algorithm suggested in this paper. The cutting algorithm enables the Lagrangean relaxation to be proceeded continually by allowing infeasible solution temporarily when the feasible solution is not easy to find through iterations. The performances are evaluated through computational experiments. It is shown that both heuristics are able to find the optimal solution in a relatively short computational time for the most instances, and that decoupling relaxation outperformed allocation relaxation.

The Effect of Singing Therapy on Classical Singers with Voice Problems (음성장애가 있는 성악인에 대한 성악치료의 효과)

  • Background and Objectives : Voice therapy has been used as a viable adjuvant to surgery and pharmacological therapy fir the management of voice disorders. The singing voice features a special brand of emotion, intensity, and energy so successful therapeutical approach requires the doctor is able to involve himself into the physical and psychic condition and artistic usage of the voice. The purpose of this study was to evaluate the effect and utility of singing therapy as an initial treatment for classically trained singers with voice disorders. Material and method : Twenty-one male and fifty-five female classic singers with voice disorders were treated with singing therapy. At first, abdominal breathing, resonant phonation, and relaxation method was trained, then after accessing each patient's singing abilities, treatment methods for each specific problems was applied. The results were compared according to age, sex, treatment duration, part, laryngeal pathology, patient subjective evaluation, perceptual evaluation of voice, and maximal phonation time. Results : Patients subjective evaluation, perceptual evaluation, pathologic findings of larynx, maximal phonation time showed superior results after singing therapy. Conclusion : Singing therapy changes the mode of respiration and phonation and enhances the vocal function and improves the laryngeal pathology, The result of this study indicate that singing therapy is an effective treatment method that laryngologists can use for classical singers with voice disorders.

Some Special Cases of a Continuous Time-Cost Tradeoff Problem with Multiple Milestones under a Chain Precedence Graph

  • We consider a time-cost tradeoff problem with multiple milestones under a chain precedence graph. In the problem, some penalty occurs unless a milestone is completed before its appointed date. This can be avoided through compressing the processing time of the jobs with additional costs. We describe the compression cost as the convex or the concave function. The objective is to minimize the sum of the total penalty cost and the total compression cost. It has been known that the problems with the concave and the convex cost functions for the compression are NP-hard and polynomially solvable, respectively. Thus, we consider the special cases such that the cost functions or maximal compression amounts of each job are identical. When the cost functions are convex, we show that the problem with the identical costs functions can be solved in strongly polynomial time. When the cost functions are concave, we show that the problem remains NP-hard even if the cost functions are identical, and develop the strongly polynomial approach for the case with the identical maximal compression amounts.
